Laparoscopic hepatectomy by curettage and aspiration. Experiences of 62 cases.

BACKGROUND: This article introduces a new technique for hepatectomy. Its purpose is to describe the details of laparoscopic hepatectomy by curettage and aspiration (LHCA) and develop a new instrument for this technique. METHODS: We have performed laparoscopic hepatectomy by curettage and aspiration (LHCA) in 62 patients in our institute between 1998 and 2005: 34 men and 28 women, mean age 47.8 years (range: 26-71 years). Their diagnoses included 18 primary hepatic carcinoma, 2 metastatic carcinoma, 19 intrahepatic duct calculus, and 23 benign entities. RESULTS: The LHCA operation was complet

Significance of the effective remnant liver volume in major hepatectomies.

The aim of this study is to identify the minimum safe amount of effective remnant liver volume (ERLV) in patients undergoing a major hepatectomy. Thirty-eight consecutive major hepatectomies (resection of > or = 3 Couinaud segments) performed between July 1999 and March 2004 in which a frozen section liver biopsy was obtained were included. No patient had chronic viral hepatitis, cirrhosis, or cholestasis. The total liver volume (TLV) was calculated using the Vauthey formula, and the postsurgical liver volume (PSLV) was derived by subtracting the estimated volume of liver resected from the TLV

Safe and permissible limits of hepatectomy in obstructive jaundice patients.

The safe and permissible limits of hepatectomy in obstructive jaundice patients and the usefulness of preoperative portal embolization (PE) for increasing the limit for safe hepatectomy were examined. We classified 416 patients with hepatectomy performed over 9 years under the following headings: normal liver function (n = 242); chronic hepatitis (n = 71); liver cirrhosis (n = 64); and liver after relief of obstructive jaundice (n = 39). Hepatectomy was done after the total bilirubin level was reduced below 3 mg/dl by preoperative biliary drainage. Factors influencing the maximum total bilirub

Hepatectomy following transarterial chemotherapy for a metastasis in the caudate lobe.

We describe a solitary liver metastasis in the caudate lobe from a colon cancer treated with a hepatic resection following transarterial chemotherapy. A 73-year-old male was admitted with a complaint of melena. The findings from endoscopic examination of the colon showed a type 3 cancer at the hepatic flexure. Computed tomography revealed a metastatic lesion in the caudate lobe of the liver, which was three centimeters in diameter and located between the roots of the middle and the left hepatic vein. A right hemicolectomy was performed and the surgical findings revealed extended lymph node met

[PGE1 and dopamine administration in a pediatric patient during hepatectomy].

A 2-year-old male underwent medial inferior hepatectomy for the treatment of metastatic tumors. Due to congenital hepatoblastoma, at 6-months of age, right lobectomy of his liver had been performed. To protect liver functions, PGE1 and dopamine were administered during the surgery. Although half of his circulating blood volume was lost, his perioperative hemodynamics was stable, with no development of postoperative liver dysfunction. PGE1 and dopamine may thus be beneficial in pediatric hepatectomy.

Parenchyma-preserving hepatectomy in the surgical treatment of hilar cholangiocarcinoma.

BACKGROUND: Although extended hepatic resection has been shown to improve prognosis by increasing the surgical curability rate in hilar cholangiocarcinoma, high surgical morbidity and mortality rates have been reported in patients with obstructive jaundice. Postoperative liver failure after hepatic resection in patients with obstructive jaundice has been shown to depend on the volume of the resected hepatic mass. The aim of this study was to evaluate the results of parenchyma-preserving hepatectomy in a surgical treatment for hilar cholangiocarcinoma. STUDY DESIGN: Ninety-three resected patie

[A facilitated technique for donor hepatectomy: experimental study in the pig].

A facilitated technique of porcine donor hepatectomy is reported underlying the possibility to make an easy procedure without in situ portal perfusion, so other abdominal organs supplied by superior mesenteric system can be harvested at the same time. The viability of those grafts procured without in situ portal perfusion has been compared with an other group of livers harvested with in situ aortic and portal flushing. The evaluation of the histology and early graft function two hours after riperfusion has shown no differences between both groups.

Outcome of major hepatectomy with pancreatoduodenectomy for advanced biliary malignancies.

In patients with advanced biliary malignancies a chance of curability is obtained by performing only major hepatectomy with concomitant pancreatoduodenectomy. This aggressive procedure carries two major risks: hepatic failure and pancreatic anastomotic leakage. Ten patients with advanced biliary malignancies were treated by major hepatectomy with pancreatoduodenectomy. Nine patients underwent right portal venous embolization before hepatectomy. Complete external drainage of pancreatic juice followed by second-stage pancreatojejunostomy was performed in five patients. Three of these five underw

[Hepatectomy in repeated operations on patients with parasitic diseases of the liver].

The results of resection of the liver in repeated operations on patients with parasitic disease of the liver are discussed. Operations were conducted on 22 patients with alveococcosis and 7 patients with echinococcosis. By means of the method of atypical liver resection radical operations were performed on 20 patients (69%) and palliative resections of the liver in 9 patients (31%). The palliative resections were combined with cryodestruction of the remaining parasitic tissue, which improved the therapeutic effect significantly and prolonged the patients' survival. The volume of the resection

Partial hepatectomy on cirrhotic liver with a right lateral tumor.

A total of 24 patients with cirrhotic liver and solitary, small hepatocellular carcinoma (HCC) located at the lateral part of the right lobe underwent surgery with our technique of hepatic clamping and finger dissection. There were no operative mortality or acute or chronic hepatic failure. Total operating time was 129 +/- 20 minutes; actual resection time was only 22.7 +/- 4.9 minutes. The average amount of blood transfused during this procedure was 1552 +/- 909 ml. The preoperative serum bromsulphalein retention rate proportionately reflected the postoperative peak serum conjugated bilirubin

[The rationale of treatment after hepatectomy for primary cancer of the liver].

During a period of ten years ranging from January 1973 to February 1983, a total of 51 patients with primary cancer of the liver underwent hepatic resection at the Keio University Hospital. Thirty five of 51 patients were cirrhotic and two were jaundiced. Six cirrhotic patients died of liver failure within one month following hepatic resection. In order to minimize the postoperative death, it is mandatory to maintain the integrity of circulatory dynamics and microcirculation. Monitoring of circulatory dynamics with the use of Swan-Ganz catheter is very useful for managing the patients with mas

The mechanism of impaired coagulation after partial hepatectomy in the dog.

Coagulation mechanisms were examined in the dog after a 70 per cent hepatectomy and the additional effect of varying periods of ischemia on the liver remnant. Dogs were submitted to a 70 per cent partial hepatectomy, and the liver remnant was rendered ischemic by occluding the vascular inflow. Portal decompression during ischemia was accomplished by allowing portal venous flow through the lobes subsequently resected. Dogs in the control group, those undergoing hepatectomy alone and those undergoing hepatectomy together with 60 minutes of ischemia time exhibited a fall in hemoglobin and hematoc

Decreased fresh gas flow cannot compensate for an increased operating room temperature in maintaining body temperature during donor hepatectomy for living liver donor hepatectomy.

BACKGROUND: The purpose of this study was to compare the effect of various combinations of fresh gas flow (FGF) of anesthesia and different ambient operation room temperatures (ORT) on changes in nasopharyngeal temperature (NT) among living donors undergoing partial hepatectomy. METHODS: The anesthesia records of 167 patients were reviewed retrospectively. The patients were allocated into 4 groups: GI (n=37): isoflurane in 2 L FGF and at typical ambient ORT (19 degrees C-21 degrees C); GII (n=11) isoflurane in 1 L FGF and 1 L air at typical ORT; GIII (n=31) isoflurane in 0.5 L FGF at typical

Hepatectomy with Hepatic Vein Resection and Reconstruction Under Total Vascular Exclusion and Venous Drainage via a Venovenous Bypass: An Additional Approach for Complex Hepatectomies.

BACKGROUND: Total vascular exclusion (TVE) with liver hypothermic perfusion under venovenous bypass (VVB) is usually needed to perform hepatectomy with Inferior vena cava and hepatic veins resection-reconstruction.1-4 An alternative technique is represented by liver resection under intermittent pedicular clamping, IVC total clamping and VVB, without cold perfusion and liver outflow drainage through the VVB.5 PATIENTS AND METHODS: The patient is a 60-year-old woman with past medical history of right hepatectomy for leiomyosarcoma 14 years previously. She presented with a single liver recurrence

Randomized Phase II/III Trial Comparing Hepatectomy, Followed by mFOLFOX6 With Hepatectomy Alone for Liver Metastasis From Colorectal Cancer: Long-Term Results of JCOG0603.

JCOG0603 demonstrated improved disease-free survival (DFS) with adjuvant mFOLFOX6 after hepatectomy compared with hepatectomy alone in colorectal liver-only metastasis (CRLM), but the overall survival (OS) data were immature. Here, we report OS after long-term follow-up. Eligible patients with colorectal adenocarcinoma and an unlimited number of liver metastases were randomized to adjuvant mFOLFOX6 or hepatectomy alone. DFS was the primary endpoint, and OS was a secondary endpoint. Between March 2007 and January 2019, 151 and 149 patients were randomized to adjuvant chemotherapy and hepatectom

Minimally Invasive Approach of Bile Duct Stricture after Laparoscopic Hepatectomy.

INTRODUCTION: Extragonadal germ cell tumors in pediatric patients are rare, accounting for approximately 10 to 15% of all germ cell tumors. High-risk chemotherapy followed by surgical resection represents the standard of care. Post-hepatectomy biliary complications, such as hepatic duct stenosis, constitute a relevant clinical challenge. CASE PRESENTATION: We present the case of a 10-year-old pediatric patient with a retroperitoneal and mediastinal extragonadal germ cell tumor treated with high-risk chemotherapy according to the GALOP 2017 protocol and multiple surgical procedures, including

Robotic-Modified Taj Mahal Hepatectomy for Type IV Hilar Cholangiocarcinoma.

INTRODUCTION AND OBJECTIVES: Curative resection of type IV hilar cholangiocarcinoma typically requires trisectionectomy, a procedure associated with high morbidity. The Taj Mahal hepatectomy, involving resection of segment IVb, V, and the caudate lobe, offers a parenchyma-preserving alternative.1 Since complete resection of segment IV facilitates caudate lobectomy, and inclusion of segment V provides no added benefit, a modified approach-removing only segment IV (IVa and IVb) with the caudate lobe-has been developed.2 The robotic performance of this modified procedure has not been reported pre

Laparoscopic Hepatectomy for Hepatic Inflammatory Myofibroblastic Tumor: A Case Report.

INTRODUCTION: Inflammatory myofibroblastic tumors (IMTs) are neoplasms composed of spindle-shaped myofibroblasts along with inflammatory infiltrate, and possess malignant potential. Primary hepatic IMTs (HIMTs) are relatively uncommon and difficult to distinguish from liver malignancies such as intrahepatic cholangiocarcinoma (ICC) and hepatocellular carcinoma due to the lack of characteristic radiological findings. CASE PRESENTATION: We present the case of a 76-year-old man with a medical history of renal cell carcinoma and gastric cancer. During routine follow-up, a CT revealed an irregular

Bona Fide Intersegmental Plane Hepatectomy Along the Glissonean Branches (BIPHG).

This review traces the author's personal journey over three decades in the evolution of liver resection techniques, culminating in the proposal of the Bona Fide Intersegmental Plane Hepatectomy Along Glissonean Branches (BIPHG). Beginning with pioneering experiences in laparoscopic and robotic liver surgeries, the author highlights key anatomical insights-particularly Laennec's capsule and intersegmental planes-that have advanced surgical precision. Transitioning from laparoscopy-assisted to pure laparoscopic donor hepatectomy illustrated the importance of safety, visualization, and anatomy-gu
