It has been observed that controlled lung ventilation during surgery in children is accompanied by an increase in intrapulmonary blood shunting, which averages 7.3% in urological surgery, 12% in abdominal surgery and 16.8% in thoracic surgery. The shunting is caused by ventilation-perfusion disturbances. The use of high-frequency jet ventilation, i.e. application of 16.7 Hertz oscillatory modulations together with conventional controlled lung ventilation enables one to decrease intrapulmonary shunting, reduce the signs of circulatory hyperdynamia and the load on the heart ventricles and leads

OBJECTIVES/HYPOTHESIS: To assess the feasibility of jet ventilation in obese patients and to compare complications of jet ventilation in obese and nonobese patients.
STUDY DESIGN: Retrospective review of medical records.
METHODS: We reviewed 46 patient charts (70 procedures) with the diagnosis of tracheal or subglottic stenosis who underwent endoscopic surgery with jet ventilation between March 2014 and January 2017. Adequacy of jet ventilation was assessed by chest rise, avoidance of endotracheal intubation, and length of case and ventilation. Records were reviewed for demographic details,

Six children conventionally ventilated for acute pulmonary parenchymal failure developed severe hypoxemia (mean PaO2 48 +/- 7 mmHg at an FiO2 of 0.95 +/- 0.08) persisting for more than 6 h despite a progressive increase in positive end expiratory pressure (PEEP) to 14.7 +/- 1.5 cmH2O. Combined high-frequency jet ventilation (HFJV, mean rate 225 b/min superimposed on small tidal volume conventional ventilation) resulted in a sustained increase in PaO2 to 93 +/- 21 mmHg, p less than 0.05 while peak inspiratory pressure decreased from 47 +/- 8 to 35 +/- 6 cmH2O and positive end expiratory pressur

Pleomorphic adenoma (PA) is primarily found in the salivary glands, and is extremely rare in the subglottic region. Here we present a subglottic PA that presented with symptoms of dry cough and dyspnea. A submucosal mass was found in the subglottic region under laryngoscopy, occluding approximately 40% of the lumen. The patient underwent the transoral endoscopic CO2 laser microsurgery under high-frequency jet ventilation for mass resection, and the pathology report supported the diagnosis of PA. At the 2-year follow-up, there was no evidence of recurrence, and the patient is currently under re

BACKGROUND: High-frequency ventilation can be delivered with either oscillatory ventilation (HFOV) or jet ventilation (HFJV). Traditional clinician biases may limit the range of function of these important ventilation modes. We hypothesized that (1) the jet ventilator can be an accurate monitor of mean airway pressure (P (aw)) during HFOV, and (2) a mathematical relationship can be used to determine the positive end-expiratory pressure (PEEP) setting required for HFJV to reproduce the P (aw) of HFOV.
METHODS: In phase 1 of our experiment, we used a differential pressure pneumotachometer and a

Invasive airway management techniques are still challenging even for skilled anesthesiologists, intensivists and emergency physicians. All current percutaneous tracheostomy techniques are based on Seldinger's principle. Using the recent introduced Blue Dolphin Kit is feasible and safe, but without clear advantage compared to other kits. There is no data available to generally support performing early (<8 days) tracheostomy in intensive care patients requiring mechanical ventilation. Experimental data show promising results of supporting expiration through a small bore catheter using ejectors b

High-frequency subglottic jet ventilation was used in 352 cases of adults treated with suspension microlaryngoscopy. Of these, 89 had difficult airways, and these are the subject of this manuscript. All cases were managed by the senior anaesthesiologist (AP) and the senior surgeon (JR), between 2000 and 2004. There were minimal complications associated with this technique in this series. The subglottic catheter never proved impossible to insert, and was used on one occasion in a rescue mode after supraglottic jetting led to oxygen desaturation. Body habitus did not prevent use of the technique

UNLABELLED: The problem of respiratory support in tracheal surgery is still discussed in recent days. Anaesthesiologist must choose the most effective and safe respiratory technique during tracheal resection and reconstruction. The article deals with a case of comparison of apneic oxygenation (AO) and high frequency jet ventilation (HFJV) during double level tracheal resection and reconstruction in patient with multifocal tracheal stenosis and underlying cerebral trauma.
MATERIALS AND METHODS: AO and HFJV were used due to surgical need for 20 min each technique. PvO2, PvCO2, hematocrit, blood

This case report describes the use of an endotracheal ventilation catheter (ETVC) to provide prolonged intraoperative jet ventilation, reintubation and the maintenance of tracheal access following extubation. It emphasizes that excellent oxygenation and ventilation can be achieved but such management can be complicated by a pneumothorax even when the risks are minimized. A 43-yr-old man presented for possible pulmonary sleeve resection. Placement of a double lumen endotracheal tube (DLT) by direct laryngoscopy was unsuccessful due to the inability to visualize the glottis. A 7.5 mm endotrachea

High-frequency subglottic jet ventilation was used in 142 consecutive adults with grade I airways treated with suspension microlaryngoscopy. All cases were managed by the senior anesthesiologist (AP) and the large majority by the senior surgeon (JR), between 2000 and 2002. The laser was used in 30% of cases. There were few complications, none serious, associated with this technique in this series. The subglottic catheter never proved impossible to insert and only needed to be replaced by the supraglottic catheter in two patients because of limitation of visualization of the posterior glottis.

OBJECTIVE: Test the hypothesis that the center of ventilation, a measure of ventro-dorsal atelectasis, is posterior during supraglottic ventilation indicating better dependent-lung ventilation. Secondarily, we tested the hypothesis that supraglottic ventilation improves oxygenation and carbon dioxide elimination.
BACKGROUND: Supraglottic and subglottic jet ventilation are both used during laryngotracheal surgery. Supraglottic jet ventilation may better prevent atelectasis and provide superior ventilation.
DESIGN: Randomized, cross-over trial.
SETTING: Operating rooms.
PATIENTS: Patients ha

An 81-year-old woman with acute respiratory distress was referred to our hospital. Computed tomography showed a large mass in the upper mediastinum with severe tracheal stenosis. Endotracheal intubation was performed under the preparation of extracorporeal membrane oxygenation and high-frequency jet ventilation, and the tumor was completely removed. The pathologic diagnosis was a goiter. Post-operatively, respiratory distress disappeared. No tumor recurrences have been noted for more than 2 years after surgery.

BACKGROUND: Supraglottic jet ventilation (SJV) via the "jet endotracheal tube" (JET) designed by Wei (WEI JET; Wei Medical LLC, Cherry Hill, NJ) provides adequate oxygenation and ventilation during direct laryngoscopy and tracheal intubation in animals. It has facilitated intubation in apneic pigs with a simulated difficult airway.
OBJECTIVE: To report on the first clinical study to examine the efficacy of using SJV via the WEI JET, in combination with end-tidal CO(2) pressure (PetCO(2)) monitoring during SJV, in maintaining oxygenation during direct laryngoscopy, and in facilitating placemen

Mediastinitis is a bacterial or non-bacterial infection of the mediastinum. Descending necrotizing mediastinitis (DNM) is a rare but life-threatening complication. The mostly feared form of mediastinitis very often has a fatal outcome. We report on a patient with tracheobronchial necrosis following DNM and emphasize the paramount importance of early and complex surgical therapy combined with immediate intensive care treatment. This case describes the intensive medicine and anaesthetic management, which was complicated by the tracheobronchial-pleural fistula and in particular by the respiratory

OBJECTIVE: To assess the effect of high-frequency ventilation (HFV) in children and adolescents with acute respiratory distress syndrome (ARDS) through estimates of survival rate and time of ventilation. To verify whether HFV can reduce the indication for extracorporeal membrane oxygenation (ECMO) in children and adolescents with ARDS.
METHODS: a systematic review of medical literature on the use of HFV and ECMO in children and adolescents with ARDS was carried out. Medline, Lilacs and Embase databases were searched for the following terms: adult respiratory distress syndrome, ARDS, acute res

Advancements in the fields of obstetrics and neonatal medicine, notably the use of antenatal corticosteroids and exogenous surfactant, have significantly improved the chances of survival for babies born prematurely. Extremely premature infants, born before 28 weeks of gestation, have underdeveloped lungs and pulmonary vasculature, necessitating a carefully tailored respiratory management strategy that incorporates both noninvasive and invasive methods throughout their NICU stay. The primary goal of respiratory management in this cohort is to provide the least amount of support necessary while

Two cases are reported of tracheal fracture occurring during blunt cervico-thoracic trauma. The first case emphasized the impossibility of passing a tracheal tube below the fracture preoperatively, without the fibreoptic or rigid tracheoscope which has the added advantage of making an accurate diagnosis of the fracture. The clinical and paraclinical signs could only lead to a suspicion of the diagnosis, but not its confirmation. The second case showed the use of different ventilatory means during and after surgery (separate lung ventilation, jet ventilation, high frequency ventilation) in case
