How Does Integrated Care Do With, Not for, Neighbourhoods & Communities?

A notable development in recent years has been the emphasis on structuring integrated care around 'neighbourhoods' or 'places' and engaging 'communities' in the planning, design and delivery of support. Across the world, related principles are being used to inform national and regional policies and to develop local practice innovations. Despite such interest, community and neighbourhood working can feel a radical endeavour which clashes with established structures, relationships, investments and skills. A group of international experts from policy, practice, communities and research came toget

Latent tuberculosis prevalence in healthcare workers in Laos: a cross-sectional study.

BACKGROUND: Individuals with latent tuberculosis infection (LTBI) have a high risk of active infection, morbidity and mortality. Healthcare workers are a group who have increased risk of infection and onward transmission to their patients and other susceptible individuals; however, LTBI is often undiagnosed, and individuals are asymptomatic. Interferon gamma release assays (IGRA) can detect evidence of TB infection in otherwise asymptomatic individuals and are a good indication of LTBI. Laos, a resource limited country in southeast Asia, has limited data on TB prevalence in the general populat

Use of intravenous immunoglobulins in clinical practice: data from three French university hospitals.

Since several years, the use of intravenous immunoglobulins (IVIg) has increased. This growth has encouraged some countries to publish guidelines. In parallel, some countries have conducted audits to know how IVIg are used in clinical practice in the light of the available guidelines. The objective of this study was to assess IVIg use in three French university hospitals in 2006. All IVIg administrations were evaluated during 6 months (12 September 2005-12 March 2006) in French university hospitals of Marseille. Different data were recorded for each administration: patient characteristics, ind

Efficiency and safety gains from bedside electronic transfusion checks in four hospitals in England.

Bedside electronic transfusion check (BETC) systems support safer practice by aiding staff in patient identification during group and screen (G&S) sample labelling and blood component administration, steps where most transfusion errors occur. While the safety rationale is clear, the significant costs necessitate robust cost-benefit evaluation. During phased BETC implementation in four London hospitals, we mapped in detail transfusion workflow, quantifying staff time saved under three modes of technology adoption which enable three corresponding states of organisational use: Mode-A (baseline: m

Students sitting medical finals--ready to be house officers?

An initial survey of students approaching qualification and the preregistration house officer year revealed anxiety about competence in several important clinical skills. A questionnaire study was then undertaken to assess, first, the extent to which students had attained the skills required for the preregistration year and, second, the amount of training in these skills provided during the preregistration year. 122 medical students taking their final examinations were asked about training and practice in eight core clinical skills, and 84 graduates from the same school, approaching the end of

The Relationship Between Physician Organization and Health Care Use in Traditional Medicare and Medicare Advantage.

Publicly funded insurance programs increasingly rely on private health plans to manage care, and physicians have organized into larger groups, many of which are owned by hospitals or health care systems. We examine the relationship between physician organization and health care resource use and whether this relationship differs between traditional Medicare and Medicare Advantage. We find that physician group practice is associated with less use of hospital inpatient and outpatient services, but that this relationship is attenuated for patients enrolled in Medicare Advantage. We also find that

The implementation of diverse skin-toned female manikins in hospital provider basic life support training.

BACKGROUND: Women are less likely than men to receive bystander cardiopulmonary resuscitation (CPR). CPR training has traditionally relied on manikins depicting a lean, white male chest, which may reinforce disparities. This quality improvement initiative evaluated healthcare staff perceptions of sex/gender differences in CPR and the impact of diverse skin-toned female manikins on provider confidence. METHODS: Six female CPR manikins representing diverse skin tones were introduced into basic life support training at two Covenant Health hospitals. Participants completed anonymous pre- and post

Barriers and Priorities for Immunotherapy Implementation in Advanced Gastric Cancer: Findings from a Two-Phase Qualitative Study.

BACKGROUND: Immunotherapy has changed the therapeutic approach to advanced gastric cancer (AGC) and has shown survival benefits compared with standard chemotherapy. However, in clinical practice, its use remains heterogeneous. Differences in diagnostic pathways, organisational aspects, and regional settings may lead to delays in treatment access. This study aimed to analyse these issues and to describe possible shared strategies to improve the use of immunotherapy in AGC in Italy. MATERIALS AND METHODS: The analysis was performed in two steps. First, a focus group comprising 13 oncologists an

National hospital antibiotic timing measures for pneumonia and antibiotic overuse.

The development of drug-resistant bacteria from the overuse of antibiotics is a serious problem, with overutilization threatening to disarm caregivers and their patients even as together they face increasingly virulent strains of microbes. On the other hand, the speedy treatment of pneumonia with antibiotics is a firmly established, evidence-based practice, enshrined in Joint Commission on Accreditation for Healthcare Organizations Core Measures used in hospital accrediting and public reporting, and in Centers for Medicare and Medicaid Services (CMS) public-reporting and pay-for-performance ho

Breast-feeding counselling in a diarrhoeal disease hospital.

Lactation counsellors were trained to advise mothers of partially breast-fed infants who were admitted to hospital because of diarrhoea, so that they could start exclusive breast-feeding during their hospital stay. Infants (n = 250) up to 12 weeks of age were randomized to intervention and control groups. Mothers in the intervention group were individually advised by the counsellors while mothers in the control group received only routine group health education. During follow-up at home by the counsellors a week later, only the mothers in the intervention group were counselled. All the mothers

The effect of resident peer-to-peer education on compliance with urinary catheter placement indications in the emergency department.

OBJECTIVE This study aims to evaluate the effect of resident peer-to-peer education on knowledge of appropriate urinary catheter (UC) placement in the emergency department (ED) and to determine if this translates into further reduction in UC utilisation. Background Instituting guidelines for appropriate UC placement reduces UC utilisation in the ED. No study has explored if resident education in a teaching hospital would further reduce UC utilisation. METHODS An educational intervention implemented in February 2009 consisted of a lecture, distribution of pocket cards and a peer-administered we
