The frequency of peripheral artery aneurysms in the upper extremities is less than in the lower extremities. Diagnosis and surgical treatment are important because upper extremity aneurysms can severely compromise the function of a limb and possibly lead to the loss of an arm or fingers. Very rarely, posttraumatic upper extremity pseudoaneurysms show symptoms after a long period of time. Diagnosis can be made on review of the patient's history and a physical examination. Surgical reconstruction is the preferred treatment for such patients. We present a case of a brachial artery pseudoaneurysm

The use of elastic intramedullary nails for the treatment of diaphyseal fractures of the humerus has been associated with few complications. We report a fracture of an elastic Marchetti-Vicenzi nail used to treat a pathological fracture of the humerus. The failure of the intramedullary nail followed low-energy trauma and was presumably the end result of metal fatigue, as the original fracture had only healed after 13 months.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2002eng
Articles describing the treatment of proximal humerus malunion are limited. Although in most of the cases, shoulder arthroplasty is the treatment of choice, when the articular surface of the humeral head is intact, other techniques can be considered and successfully used as well. Using arthroscopic techniques for proximal humerus malunion treatment is rarely reported in the literature. We could find only a few cases in which arthroscopic subacromial decompression was used to treat greater tuberosity malunion. Arthroscopic debridement and capsulotomy are also considered in the treatment of prox

BACKGROUND: Nonoperatively treated fractures of the humeral diaphysis have a high rate of union with good functional results. However, there are clinical situations in which operative treatment is more appropriate, and, though interest in plate osteosynthesis has decreased, intramedullary nailing has gained popularity in recent years. We report the results of treating fractures of the humeral diaphysis with a prefabricated brace that permits full motion of all joints and progressive use of the injured extremity.
METHODS: Between 1978 and 1990, 922 patients who had a fracture of the humeral di

Between 1985 and 1991, 22 patients (average age 52 years, range 26 to 65 years) with severely impacted humeral head fractures were operated on with the aim of preserving the humeral head. All 22 cases showed no significant lateral displacement. Thus it was assumed that the periosteum leading medially to the humeral head was intact and that the vessels passing through the periosteum would ensure survival of the humeral head segment. All patients underwent treatment with open reduction. The impacted segment of the humeral head was raised, the tuberosities were relocated, and the void was filled

Dislocation of the shoulder with ipsilateral shaft fracture is an uncommon, but serious injury. I present one case report and the guidelines used for treatment. The method employed was Hackethal's pins through open reduction. The dislocation was reduced during surgery. The fracture and dislocation healed without any problems, and there was no damage to the radial nerve. After a recent follow-up, the patient was allowed to return to work without restrictions, and he has regained normal mobility.

Most humeral shaft fractures are treated by early functional mobilization (Sarmiento-Bracing). In some situations (short transverse fractures, old age of patients living alone, extreme obesity, social circumstances) operative fixation may be warranted in individual patients. There are, however, several absolute and urgent indications for operative fracture stabilization: fractures involving nerve or vessel injury, open fractures, multiple fractures on the ipsilateral arm, and delayed fracture healing. Osteosythesis with a dorsally placed femoral DC plate gives the best results.

The management of ten patients with fracture of the humeral diaphysis and concomitant injury to the brachial artery was analyzed retrospectively for examination of the factors contributing to preservation of limb viability and function. Of ten treated fractures, eight were rigidly stabilized with either a plate or an external fixation device. Seven of the vascular injuries required an interpositional graft. Three arteries were repaired with an end-to-end anastomosis. Two of these repairs failed in patients whose fractures were not rigidly stabilized, and one of these patients required amputati

The case presented illustrates a previously unreported form of valgus angulation of a proximal humeral epiphyseal fracture. For reduction this fracture requires traction and adduction, not abduction, of the arm.

Juvenile supracondylar fracture of the humerus is described briefly and divided into two groups: 1. Those cases amenable to safe closed treatment; 2. Those in which occlusion of the brachial artery associated with severe displacement renders closed treatment unsafe. A specific mechanism of entrapment of the brachial artery, which has not been described previously, is reported, with the nature and the solution of the problem found at primary open operation. A discussion of the surgical anatomy shows how recognition of the mode of occlusion of the brachial artery may contribute to safe and relia

The extra-articular distal humerus locking plate (EADHP) is widely used for distal extra-articular diaphyseal humeral fracture fixation. However, it occasionally causes skin prominence and discomfort. The upside-down use of a proximal humerus internal locking system (PHILOS) plate is suggested as an alternative option, but it lacks biomechanical evidence. The purpose of this study was to compare the biomechanical performance between two different fixation methods: the modified use of the PHILOS plate on the anterior cortex versus conventional use of an EADHP on the posterior cortex. Twelve pai

Our aim was to determine the effect of the initial pattern of fracture and the displacement of fragments on the outcome of proximal humeral fractures treated conservatively. We followed 93 consecutive patients prospectively for one year. Final movement and strength were compared with those of the contralateral side. The final American Shoulder and Elbow Society score and the Disabilities of Arm, Shoulder and Hand and Short-Form 36 questionnaires were compared with those provided by the patient on the day of the injury. Radiographs and CT scans with three-dimensional reconstruction were obtaine

Reverse shoulder arthroplasty (RSA) is being increasingly employed for complex proximal humeral fractures (PHFs) in older patients, particularly in cases with severe osteoporosis. However, bilateral PHFs and subsequent periprosthetic fractures remain rare and challenging. We report a case of a 60-year-old woman with alcoholic liver disease who sustained bilateral PHFs after a fall. Due to her compromised condition, staged bilateral RSA was performed. The initial postoperative course was uneventful. Eight months after the first surgery, she sustained a displaced periprosthetic fracture on the r

Due to demographic changes, proximal humeral fractures (PHF) are becoming increasingly more frequent in routine clinical practice. At the same time, surgical treatment is more frequently indicated and more demanding due to increasingly more complex fracture morphology and declining bone quality. Osteosynthesis using locking plate fixation remains the mainstay of surgical treatment alongside reverse fracture arthroplasty. The treatment concept is individually dependent on the characteristics of the fracture morphology, the patient-specific risk profile and the expertise of the surgeon. In young

BACKGROUND: This is the first systematic review of the relationship between humeral shaft fractures and radial nerve palsy in children. The present comprehensive review is aimed at identifying important clinical findings between humeral diaphysis fractures and radial nerve injuries and assessing the effects of treatment.
METHODS: We searched electronic bibliographic databases, including PubMed, the Cochrane Library, Scopus, and Web of Knowledge, until March 2022. This systematic review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and the pa

BACKGROUND: Humeral shaft fractures occur frequently in older patients. The more proximal end of the shaft is prone to non-union when compared with more distal fractures. We provide an overview of several key features of this significant patient group.
METHOD: Our institutional fracture database was searched for all cases of proximal third humeral shaft fractures in patients over 60 years of age. Retrospective evaluation of patient demographics, management, length of stay and incidence of non-union was performed.
RESULTS: 75 patients (61 female) were analysed. 33 patients were 'frail' (Clini

This study was undertaken to determine the incidence, need for intervention, and time to resolution of pseudosubluxation of the shoulder in pediatric proximal humerus fractures. One hundred and ninety-nine radiographs (199 x-rays) were analyzed for pseudosubluxation of the shoulder following pediatric proximal humeral fractures. Pseudosubluxation occurs when the center of the humeral head aligns with the inferior one-fourth of the glenoid. Fourteen patients met the inclusion criteria for pseudosubluxation. The nonoperative cohort consisted of 100 females and 93 males and the operative cohort c

BACKGROUND: Osteosynthesis of distal diaphyseal humeral fractures is challenging, especially if there is a metaphyseal extension of the fracture line with a butterfly third fragment or if the fracture is bifocal. These fractures put the radial nerve at risk at the time of the fracture and during surgery. We hypothesize that ORIF with a long dorsal Y-plate matching the dorsal aspects of the humeral diaphysis and of the two metaphyseal columns would provide a reliable method of fixation for distal diaphyseal humeral fractures even when there is a metaphyseal extension or bifocal component.
