Beyond BRUE (Brief Resolved Unexplained Event): Recurrent Unexplained Events Revealing Congenital Hyperinsulinism in Infancy.

Brief resolved unexplained events (BRUEs) are uncommon yet clinically significant presentations in infancy that require careful risk stratification, as recurrent episodes may herald serious occult pathology. We report a four-month-old term female infant with a notable family history of sudden unexplained sibling deaths who was admitted to and later discharged from our institution twice for recurrent BRUE episodes following an initially unrevealing workup. She subsequently presented again with vomiting, diarrhea, dehydration, metabolic acidosis, and persistent hypoglycemia, necessitating admiss

Functional evaluation of pancreatic islets from patients with Beckwith-Wiedemann syndrome and congenital hyperinsulinism.

CONTEXT: Beckwith-Wiedemann Syndrome (BWS) is an overgrowth syndrome caused by various genetic or epigenetic abnormalities in a cluster of imprinted genes on chromosome 11p15. Congenital hyperinsulinism (HI) is one of the cardinal features of BWS, but the pathophysiology of HI in BWS has not been clearly defined. OBJECTIVE: We describe the islet phenotype of a series of infants with severe HI and comorbid BWS who required pancreatectomy for intractable hypoglycemia. METHODS: The cases are a subset of HI patients who required pancreatectomy and had Beckwith-Wiedemann Syndrome. Molecular testi

Long-Term Results of Long-Acting Somatostatin Analog Therapy in Children with Congenital Hyperinsulinism.

BACKGROUND AND OBJECTIVES: Congenital hyperinsulinism (CHI) is a rare condition caused by dysregulation of pancreatic insulin secretion, leading to severe hypoglycemia in children. In diazoxide-unresponsive patients, somatostatin analogs are recommended as a treatment option. The aim of the present study was to evaluate the clinical characteristics, growth patterns, side effect profiles, and long-term follow-up of CHI patients who received octreotide long-acting release (octreotide-LAR) therapy. METHODS: In the study, data from 23 CHI patients (F/M: 9/14) who received octreotide-LAR therapy b

Proposed Screening for Congenital Hyperinsulinism in Newborns: Perspective from a Neonatal-Perinatal Medicine Group.

This perspective work by academic neonatal providers is written specifically for the audience of newborn care providers and neonatologists involved in neonatal hypoglycemia screening. Herein, we propose adding a screen for congenital hyperinsulinism (CHI) by measuring glucose and ketone (i.e., β-hydroxybutyrate (BOHB)) concentrations just prior to newborn hospital discharge and as close to 48 h after birth as possible, at the same time that the mandated state Newborn Dried Blood Spot Screen is obtained. In the proposed protocol, we do not recommend specific metabolite cutoffs, as our primary o

Novel Hypoglycemia Phenotype in Congenital Hyperinsulinism Due to Dominant Mutations of Uncoupling Protein 2.

CONTEXT: The rarest genetic form of congenital hyperinsulinism (HI) has been associated with dominant inactivating mutations in uncoupling protein 2 (UCP2), a mitochondrial inner membrane carrier that modulates oxidation of glucose vs amino acids. OBJECTIVE: To evaluate the frequency of UCP2 mutations in children with HI and phenotypic features of this form of HI. DESIGN: We examined 211 children with diazoxide-responsive HI seen at The Children's Hospital of Philadelphia (CHOP) between 1997 and October 2016. SETTING: CHOP Clinical and Translational Research Center. RESULTS: Of 211 cases o

Accuracy of [18F]fluorodopa positron emission tomography for diagnosing and localizing focal congenital hyperinsulinism.

OBJECTIVES: Focal lesions in infants with congenital hyperinsulinism (HI) represent areas of adenomatosis that express a paternally derived ATP-sensitive potassium channel mutation due to embryonic loss of heterozygosity for the maternal 11p region. This study evaluated the accuracy of 18F-fluoro-l-dihydroxyphenylalanine ([18F]DOPA) positron emission tomography (PET) scans in diagnosing focal vs. diffuse disease and identifying the location of focal lesions. DESIGN: A total of 50 infants with HI unresponsive to medical therapy were studied. Patients were injected iv with [18F]DOPA, and PET sc

From congenital hyperinsulinism to diabetes mellitus: the role of pancreatic beta-cell KATP channels.

Pancreatic beta-cell adenosine triphosphate (ATP)-sensitive potassium (K(ATP)) channels play a pivotal role in linking glucose metabolism to regulated insulin secretion. K(ATP) channels are hetero-octameric complexes comprising two subunits Kir6.2 and sulfonylurea receptor 1 (SUR1). Changes in the intracellular concentration of nucleotides (ATP) cause alterations in the resting and opening state of the K(ATP) channels. Loss-of-function mutations in the genes encoding the two subunits of K(ATP) channels lead to the most common form of congenital hyperinsulinism (CHI). This causes persistent and

Congenital hyperinsulinism as part of a multisystem disease caused by a compound heterozygous variant in the YARS1 gene.

Tyrosyl-transfer RNA (tRNA) synthetase, encoded by YARS1, is essential for protein synthesis, catalyzing the attachment of tyrosine to its cognate tRNA during translation. Pathogenic biallelic YARS1 variants cause infantile-onset multisystem neurological, endocrine, and pancreatic disease type 2 (IMNEPD2; OMIM #619418), an orphan disorder. Hypoglycemia in IMNEPD2 remains poorly characterized, with few reported cases. We describe an infant with a compound heterozygous YARS1 genotype (c.176T>C and c.1261G>A) who presented with severe hyperinsulinemic hypoglycemia, notable for a discordance betwe

A Novel ABCC8 Variant in an Adult with Hyperinsulinemic Hypoglycemia Suggestive of Congenital Hyperinsulinism: A Case Report.

Background and Clinical Significance: Congenital hyperinsulinism (CHI) is a disorder involving excessive insulin secretion due to mutations in the insulin regulation pathways. Patients are typically diagnosed during infancy following recurrent hypoglycemic events, which may lead to irreversible neurologic damage if left untreated. Adult presentation and diagnosis are rare. Case Presentation: Here, we report the unusual case of a 41-year-old patient who presented with recurrent episodes of hyperinsulinemic hypoglycemia. An extensive diagnostic evaluation yielded no evidence of an acquired cause

Congenital Hyperinsulinism in Neonates: Diagnostic Challenges and Management in Two Cases With KCNJ11 and ABCC8 Mutation.

We report two cases of persistent neonatal hypoglycemia associated with mutations in the KCNJ11 and ABCC8 genes, encoding the Kir6.2 and SUR1 subunits of the adenosine triphosphate-sensitive potassium channel. In both cases, symptoms were non-specific and initially attributed to other conditions, including suspected infection. Standard treatment with enteral and parenteral glucose infusions failed to restore euglycemia. Diazoxide was administered without a clinical response. Both patients were then treated with octreotide, resulting in stabilization of glycemic levels. Genetic testing confirme

Real-world experience with the use of diazoxide among people living with congenital hyperinsulinism and their caregivers.

INTRODUCTION: Congenital hyperinsulinism (HI) is a rare disease that causes severe hypoglycemia. Diazoxide is the first-line treatment; however, many individuals using diazoxide continue to experience hypoglycemia. Diazoxide is associated with side effects that impact life and well-being. METHODS: The study utilized a mixed-methods approach combining structured, survey-based cross-sectional quantitative data from the HI Global Registry (HIGR) (n=165, 89% were caregivers), of whom 75% reported current diazoxide use, with qualitative interviews with caregivers (n=12) and individuals with HI (n=

Multidisciplinary approach in medicine: successful pregnancy in a patient with hyperinsulinism/hyperammonaemia (HI/HA) syndrome.

This case illustrates the importance of multidisciplinary counselling and management of pregnancies in women with complex medical conditions, especially concerning women with cognitive impairment. We present a woman with hyperinsulinism/hyperammonaemia (HI/HA) syndrome. This syndrome is characterised by recurrent episodes of hypoglycaemia and elevated ammonia levels, which are potentially harmful to both the patient and a developing fetus. We describe a successful multidisciplinary approach during the pregnancy of a mentally challenged patient with HI/HA syndrome. This case illustrates the imp

Treatment of hirsutism, hyperandrogenism, oligomenorrhea, dyslipidemia, and hyperinsulinism in nonobese, adolescent girls: effect of flutamide.

Functional ovarian hyperandrogenism, a variant of polycystic ovary syndrome, is often associated with hyperinsulinism and dyslipidemia. The mechanisms interlinking this triad are poorly understood; both hyperandrogenism and hyperinsulinism have been proposed as factors involved in the pathogenesis of the dyslipidemia. Precocious pubarche (PP) in girls is a risk factor for subsequent anovulation, ovarian and adrenal hyperandrogenism, hyperinsulinism and dyslipidemia. Flutamide, a nonsteroidal antiandrogen, is known to be effective in reducing hirsutism in patients with ovarian hyperandrogenism.

Loss of imprinted genes and paternal SUR1 mutations lead to hyperinsulinism in focal adenomatous hyperplasia.

Two types of histopathological lesions, a focal adenomatous hyperplasia of islet cells of the pancreas in about 30% of operated sporadic cases, and a diffuse form can be observed in congenital hyperinsulinism, or Persistent Hyperinsulinemic Hypoglycemia of Infancy (PHHI). In sporadic focal forms, specific losses of maternal alleles (LOH) of the imprinted chromosomal region 11p15, restricted to the hyperplastic area of the pancreas, were observed. Similar mechanisms are observed in embryonal tumors and in the Beckwith-Wiedemann syndrome which is also associated with neonatal but transient hyper

Dual phenotype in a patient with adult-onset hyperinsulinism and diabetes caused by an activating MAFA variant.

CONTEXT: The coexistence of diabetes and hyperinsulinemic hypoglycemia (HI) within a family is rare. Activating MAFA variants have been described in 3 families with this dual phenotype and are associated with sex-dependent clinical patterns. METHODS: We studied a family where members were affected by HI (n = 5), diabetes (n = 5), or both conditions (n = 1). Clinical, biochemical, and imaging data were collected. Genetic testing for variants in known monogenic diabetes and HI genes was performed in the proband, followed by cascade testing in available relatives. Pancreatic tissue was examined

A New Case of Lethal Congenital Contracture Syndrome Type 3 With Hyperinsulinism and Optic Atrophy.

Lethal congenital contracture syndrome 3 (LCCS3, MIM #611369) is a rare autosomal recessive neuromuscular disorder caused by biallelic loss-of-function (LOF) variants in PIP5K1C, reported in only two families to date. It typically presents with severe fetal akinesia, arthrogryposis multiplex congenita, and perinatal lethality due to respiratory insufficiency case. Herein, we report a new case with survival beyond birth. Prenatal findings included clubfeet with preserved amniotic fluid volume and fetal movements. The infant was delivered by cesarean section at 37 + 7 weeks following breech pres
