Fluid resuscitation in multiple trauma patients.

PURPOSE OF REVIEW: Fluid resuscitation in trauma patients with hemorrhagic shock is controversially discussed in the literature. The coincidence of brain injury complicates management of these patients. This article summarizes the current knowledge on nonblood component fluid resuscitation and choice of fluids in patients with multiple trauma. RECENT FINDINGS: Whereas current evidence suggests the efficacy of fluid therapy in hemorrhagic shock without active bleeding, experimental and clinical data demonstrate that aggressive volume challenge may be futile or even deleterious in the setting o

Effect of Hypertonic Saline in Intra-Articular Hydraulic Distension for Adhesive Capsulitis.

BACKGROUND: Maintaining the integrity of the capsule along with infusing a sufficient amount of fluid is 1 of the therapeutic concepts in intra-articular hydraulic distension (IHD) for adhesive capsulitis. It has been known that hypertonic saline solution decreases tissue edema and increases the fluid volume within the epidural space, causing microdissection, in epidural adhesiolysis. OBJECTIVE: To investigate the effect of hypertonic saline solution in capsule-preserving intra-articular hydraulic distension (CPIHD) for adhesive capsulitis. DESIGN: Prospective randomized controlled trial. S

Rebound swelling of astroglial cells exposed to hypertonic mannitol.

BACKGROUND: Mannitol is widely used in anesthesia and critical care medicine. Although its clinical effects were originally attributed to osmotic dehydration of brain cells, other mechanisms have also been proposed. Osmotic dehydration of astroglial cells is opposed by powerful volume-regulating mechanisms that involve inward transport of electrolytes. These mechanisms have been studied previously by exposing cells to hypertonic saline gradients. Because of its potential clinical relevance, the volume response of astroglial cells exposed to hypertonic mannitol was investigated. METHODS: Rat C

Hypovolemic shock resuscitation with hyperosmotic 7.5% NaCl: effects on respiratory system mechanics.

The infusion of the hypertonic 7.5% NaCl solution stimulates the afferent vagal innervation of the lungs. In order to verify if this reflex elicits any respiratory mechanical alterations, normovolemic (n = 18) and hypovolemic (n = 18) (20 ml of shed blood/kg) guinea pigs were studied. The animals were assigned to groups of nine receiving either isotonic 0.9% NaCl or hypertonic 7.5% NaCl solutions (4 ml/kg body weight). Passive respiratory system elastance (Ers) and the maximal value of the resistance (Rrsmax) were measured. The latter was further partitioned into its minimal value (Rrsu) using

[Osmolarity of solutions used in nebulization].

Inhaled medications are widely used in patients suffering from bronchial diseases. Beside their pharmacological properties, nebulised solutions have physico-chemical characteristics that can alter bronchial reactivity. Non-isotonic solutions can induce a bronchial hyperresponsiveness and/or a severe bronchonconstriction. Nevertheless, multiple drugs are used for nebulisation despite their unknown osmolarity. The aim of this study was to measure the tonicity of drug solutions commonly used for nebulisation in patients suffering from bronchial disease. Drug solutions were prepared either accordi

Therapeutic effects of compound hypertonic saline on rats with sepsis.

Sepsis is one of the major causes of death and is the biggest obstacle preventing improvement of the success rate in curing critical illnesses. Currently, isotonic solutions are used in fluid resuscitation technique. Several studies have shown that hypertonic saline applied in hemorrhagic shock can rapidly increase the plasma osmotic pressure, facilitate the rapid return of interstitial fluid into the blood vessels, and restore the effective circulating blood volume. Here, we established a rat model of sepsis by using the cecal ligation and puncture approach. We found that intravenous injectio

Differential responses in adrenal and renal nerves to CNS osmotic stimulation.

Hypertonic solutions act in the central nervous system (CNS) to increase mean arterial blood pressure (MAP) by activation of the sympathoadrenal axis. However, adrenal nerve activity (pre- and postganglionic nerve fibers) has not been determined during central osmotic stimulation. Therefore, these experiments evaluated adrenal (AdSNA) and renal (RSNA) sympathetic nerve activity, MAP, and heart rate (HR) following CNS administration of isotonic, hypertonic, and hypotonic sodium chloride solutions in chloralose-anesthetized rats. Injection of isotonic saline (5 microliters) did not alter MAP, HR

A hypertonic infusion in the treatment of experimental shock in calves and clinical shock in dogs and cats.

A novel composite hypertonic solution for intravenous use was tested in two experimental models, one of endotoxic shock and one of shock linked with dehydration, both in anaesthetised calves. Endotoxic shock was induced with Escherichia coli lipopolysaccharide and was characterised by a low cardiac output, hypoxaemia, acidosis and anuria. Treatment with a small volume of the solution increased cardiac output, improved oxygen carriage, corrected acidosis and stimulated renal function. Experimental dehydration in calves was induced by intraperitoneal mannitol and frusemide diuresis, and was char

Responses of lateral preoptic neurons and behavioral reactions to angiotensin II and hypertonic NaCl and sucrose administered into the rat cerebral ventricle.

In 18 acutely prepared rats and in 3 chronically implanted rats, multiple-unit responses of lateral preoptic area (LPOA) neurons to angiotensin II (A II) and hypertonic NaCl and sucrose injected into the lateral ventricle were recorded. In addition, behavioral reactions were observed in the chronically prepared rats. Results showed that i.vt. injections of hypertonic solutions or A II affect neural activity in the LPOA, indicating that the LPOA has connections with periventricular structures that are sensitive to these substances. Experiments with chronically implanted animals demonstrated the

Severe nasal hemorrhage in an anesthetized horse.

An 8-year-old Thoroughbred gelding with colic was anesthetized for surgical correction of right dorsal displacement of the ascending colon. Removal of the nasogastric tube at the end of surgery resulted in hemorrhage from the nares and loss of 24 L of blood. Treatment included administration of acetated Ringer's solution, hypertonic saline solution, and dobutamine. A blood transfusion was started after hemorrhage was controlled, and arterial pressure was restored to the prehemorrhage value, but was stopped after infusion of 2.7 L of blood because of a suspected adverse reaction. This case indi

Hypertonic saline dextran resuscitation during the initial phase of acute endotoxemia: effect on regional blood flow.

BACKGROUND AND METHODS: Small-volume resuscitation by means of bolus application of hypertonic saline solutions has been demonstrated to restore central hemodynamics and regional blood flow in severe hemorrhagic and traumatic shock. The aim of this study was to elucidate the potential of this new concept for treatment of profound hypovolemia and microcirculatory deterioration associated with sepsis and endotoxic shock. In a porcine model of acute hyperdynamic endotoxemia (elicited by continuous iv infusion of Salmonella abortus equi endotoxin for 3.5 hrs), small-volume resuscitation applying h

Hypertonic saline hydroxyethylstarch restores right ventricular-arterial coupling after normovolemic hemodilution in piglets.

BACKGROUND: Normovolemic hemodilution is known to inhibit hypoxic pulmonary vasoconstriction. How the coupling between the pulmonary arterial (PA) circulation and the right ventricle (RV) is affected by normovolemic hemodilution and by the composition of replacement solutions remains unknown. Therefore, the effects of isotonic and hypertonic saline hydroxyethylstarch solutions on the pulmonary circulation and RV, in control and hypoxic conditions, were compared. METHODS: Anesthetized piglets (n = 14) were equipped with manometer-tipped catheters in the RV and main PA and an ultrasonic flow pr

Shock and resuscitation. III. Accurate refractometric COP determinations in hypovolemia treated with HALFD.

In 38 critically burned patients with symptomatic hypovolemia being treated by intravenous fluids, the accuracy of colloid oncotic pressure (COP) calculated from the refractometrically-determined serum total protein (TPRI) was compared with COP values determined by a commercially-available clinical oncometer. Sera were obtained randomly from seven patients receiving Ringer's lactate solution, five receiving a hypertonic solution (240 mOsm Na+) and 26 receiving a hypertonic solution containing albumin (12.5 gm/liter, HALFD method). There was poor correlation between COP measure and that calcula

A case of hyperglycemic hyperosmolar non-ketotic coma during anesthesia: a possible cause of failed re-awakening.

The case of a non diabetic 6-year-old boy affected by Down's syndrome, who developed hyperosmolar hyperglycemic non-ketotic coma following the infusion of hypertonic dextrose solution during general anesthesia for a surgical procedure for cryptorchidism is reported. Following surgery, the patient remained deeply comatose and generalized seizures occurred. Hyperosmolarity due to hyperglycemia and acidosis were reduced by administration of insulin at low rate, hypotonic saline and sodium-bicarbonate solutions. The patient's clinical conditions promptly improved following normalization of blood g

Acute volume regulation of brain cells in response to hypertonic challenge.

BACKGROUND: Hypertonic dehydration of the brain through administration of osmotic agents, either alone or in combination with "loop" diuretics, has been a mainstay in the treatment of increased intracranial pressure for decades. Controversy exists, however, as to the mechanism and long-term value of such therapy. Although many cell types possess volume regulatory mechanisms capable of opposing hypertonic dehydration, such behavior in the brain is poorly understood. METHODS: As a model for the mammalian central nervous system, the real-time volume behavior of rat C6 glioma cells was observed b
