THE EFFECTS OF LOSS OF GASTRIC AND PANCREATIC SECRETIONS AND THE METHODS FOR RESTORATION OF NORMAL CONDITIONS IN THE BODY.

The composition of gastric and pancreatic juices and the effects of their loss on the composition of the body fluids were studied. Loss of gastric juice by removing water and chloride ions only partly neutralized by fixed base results in dehydration and alkalosis. Loss of pancreatic juice by removing water and a relative excess of fixed base results in dehydration and acidosis. Normal conditions in the body may be restored after the loss of either gastric or pancreatic juice by the administration of a combined solution, which provides (1) water in abundance because of its hypotonicity (2) an a

P-glycoprotein regulates a volume-activated chloride current in bovine non-pigmented ciliary epithelial cells.

1. The whole-cell patch clamp technique was used to investigate the swelling-activated currents in bovine non-pigmented ciliary epithelial (NPCE) cells. 2. Exposure to hypotonic solution activated a current that was blocked by 5-nitro-2-(3-phenylpropylamino)benzoic acid (NPPB). The I-V relationship was shifted in the direction expected for a Cl- current when the external Cl- was replaced by gluconate (permeability ratio P(gluconate)/PCl = 0.17). The inhibition of the current evoked by voltage clamp steps of +80 mV yielded an IC50 for NPPB of 13.4 microM. 3. The current was found to be dependen

Nutritional management of young infants presenting with acute bronchiolitis in Belgium, France and Switzerland: survey of current practices and documentary search of national guidelines worldwide.

Feeding difficulties are common in young infants presenting with acute bronchiolitis, but limited data is available to guide clinicians adapting nutritional management. We aimed to assess paediatricians' nutritional practices among Western Europe French speaking countries. A survey was disseminated to describe advice given to parents for at home nutritional support, in hospital nutritional management, and preferred methods for enteral nutrition and for intravenous fluid management. A documentary search of international guidelines was concomitantly conducted. Ninety-three (66%) contacted physic

Sodium-Based Osmotherapy in Continuous Renal Replacement Therapy: a Mathematical Approach.

Cerebral edema, in a variety of circumstances, may be accompanied by states of hyponatremia. The threat of brain injury from hypotonic stress-induced astrocyte demyelination is more common when vulnerable patients with hyponatremia who have end stage liver disease, traumatic brain injury, heart failure, or other conditions undergo overly rapid correction of hyponatremia. These scenarios, in the context of declining urinary output from CKD and/or AKI, may require controlled elevations of plasma tonicity vis-à-vis increases of the plasma sodium concentration. We offer a strategic solution to thi

Homeostasis of intracellular Ca2+ in equine chondrocytes: response to hypotonic shock.

REASONS FOR PERFORMING STUDY: Ca2+ homeostasis in articular chondrocytes affects synthesis and degradation of the cartilage matrix, as well as other cellular functions, thereby contributing to joint integrity. Although it will be affected by mechanical loading, the sensitivity of intracellular Ca2+ concentration ([Ca2+]i) in equine articular chondrocytes to many stimuli remains unknown. HYPOTHESIS: An improved understanding of Ca2+ homeostasis in equine articular chondrocytes, and how it is altered during joint loading and pathology, will be important in understanding how joints respond to me

Mycoplasma orale infection affects K+ and Cl- currents in the HSG salivary gland cell line.

The relations between K+ channel and Cl- channel currents and mycoplasma infection status were studied longitudinally in HSG cells, a human submandibular gland cell line. The K+ channel currents were disrupted by the occurrence of mycoplasma infection: muscarinic activation of K+ channels and K+ channel expression as estimated by ionomycin- or hypotonically induced K+ current responses were all decreased. Similar decreases in ionomycin- and hypotonically induced responses were observed for Cl- channels, but only the latter decrease was statistically significant. Also, Cl- currents could be eli

Mechanisms of regulatory volume decrease in UC-11MG human astrocytoma cells.

Swelling of astrocytes commonly occurs after cerebral ischemia and other brain injuries. Because these cells constitute 20-25% of human brain volume, their swelling is a major factor in the morbidity and mortality associated with cerebral edema. Many cells, including astrocytes, resist or reverse the tendency to swell by activating transport pathways that lead to a regulatory volume decrease. Here we report the results of studies designed to elucidate the mechanisms of the regulatory volume decrease that occurs after astrocytes are swollen by exposure to hypotonic medium. Using UC-11MG cells,

Accumulation of cAMP and calcium in S49 mouse lymphoma cells following hyposmotic swelling.

Swelling of S49 "wild type" mouse lymphoma cells in hyposmolar medium was used to examine the effects of cellular deformation on cAMP metabolism. In S49 wild type mouse lymphoma cells incubated in a defined medium, progressive reductions in medium osmolarity of 5-50% resulted in proportionate expansion of cell volume. Increases in cell volume were accompanied by incremental increases in intracellular cAMP and calcium. These responses in S49 cells occurred rapidly, with increases in calcium concentration and cAMP content occurring within 1-2 min. Swelling of S49 cells in the absence of ions (hy

The pathogenesis of Bartter's syndrome. Functional and histologic studies.

We describe a patient with Bartter's snyndrome. In addition to the well-known characteristic findings by light microscopy, electron micrograms confirmed the presence of juxtaglomerular cell hyperplasia with polymorphous renin secretory granules and dense multivesicular bodies. Volume expansion by albumin infusion decreased plasma renin activity and aldosterone excretion, and improved the pressor response to exogenous angiotensin, suggesting that the renin-angiotensin-aldosterone system was not autonomous but that a decreased extracellular volume might be a major defect in this patient. During

Renal excretion of water in infants with acute gastroenteritis.

Infants with severe gastroenteritis were given water loads of 20 ml/kg body weight during their recovery. The renal responses were correlated with changes in body weight and osmolality. A water load was not excreted until the body weight had reached a peak. It is concluded that while volume depletion persists administration of hypotonic fluids leads to a fall in osmolality which will not be corrected by renal excretion of water. The relevance of this observation to the high incidence of neurological disturbances during the treatment of hyperosmolar states is discussed.

Acute hyponatremia after cardioplegia by histidine-tryptophane-ketoglutarate--a retrospective study.

BACKGROUND: Hyponatremia is the most common electrolyte disorder in hospitalized patients and is known to be associated with increased mortality. The administration of antegrade single-shot, up to two liters, histidine-tryptophane-ketoglutarate (HTK) solution for adequate electromechanical cardiac arrest and myocardial preservation during minimally invasive aortic valve replacement (MIAVR) is a standard procedure. We aimed to determine the impact of HTK infusion on electrolyte and acid-base balance. METHODS: In this retrospective analysis we reviewed data on patient characteristics, type of s

[Therapy of the severely burned child from the pediatric intensive care viewpoint].

All burn injuries involving more than 10% of the total body surfaces in children necessitate immediate fluid replacement. Such patients should be admitted to a hospital with an intensive care unit specialized in dealing with such accidents. Fluid replacement should be started, with administration of an isotonic electrolyte solution, such as lactated Ringer's, to avoid severe burn shock. Several other fluid replacement protocols have been proposed. Controversy exists as to whether a hypertonic or hypotonic solution should be used and whether or not colloid should be added to these solutions. Th

Deep freezing of concentrated boar semen for intra-uterine insemination: effects on sperm viability.

The use of deep-frozen boar semen for artificial insemination (AI) is constrained by the need for high sperm numbers per dose, yielding few doses per ejaculate. With the advancement of new, intra-uterine insemination strategies, there is an opportunity for freezing small volumes containing high sperm numbers, provided the spermatozoa properly sustain cryopreservation. The present study aimed to concentrate (2 x 10(9) spz/mL) and freeze boar spermatozoa packed in a 0.5 mL volume plastic medium straw (MS) or a multiple FlatPack (MFP) (four 0.7 mL volume segments of a single FlatPack [SFP]) inten

Role of hypo-osmotic sperm swelling test in assisted reproduction.

Hypo-osmotic sperm swelling (HOS) profile is found to be a better predictor of the fertilisability in those husbands possessing subnormal semen parameters. The group where husband's HOS gradings were less than 60%, no one could conceive his fertile wife even after intra-uterine insemination (IUI) for 8 times. However, in the other group where the HOS values were greater than 60%, conception were registered in 6 oligospermic and 2 asthenospermic husbands after 2 to 8 IUI amongst 22 married couples. No conception occurred in any normal coital cycle. All the pregnancies were delivered by elective

The effect of intestinal solute load on water secretion in infants with acquired monosaccharide intolerance.

The osmotic effect of unabsorbed glucose on intestinal secretion of water and electrolytes was investigated by jejunal perfusion in eight infants ranging from 2 to 17 months of age. All infants had or were recovering from acquired monosaccharide intolerance. Isocaloric solutions of glucose and glucose polymer containing 1% polyethylene glycol as a nonabsorbable marker were tested. The osmolalities of the glucose polymer solutions were 1/3 to 1/6 that of the glucose. No major differences were noted in carbohydrate absorption whereas mean water flux changed from secretion of 0.37 ml/min to absor

[The gynecological TURP syndrome. Severe hyponatremia and pulmonary edema during hysteroscopy].

Fluid overload and dilutional hyponatremia are rare but typical complications of endoscopic transurethral resection of the prostate gland (TURP syndrome). Less commonly, this complication caused by excessive absorption of hypotonic, electrolyte-free non-conductive distention solution, may also occur during endoscopic surgery of the uterus, e.g. operative hysteroscopy. A case report is presented of a 44-year-old woman scheduled for operative hysteroscopy for intracavital myoma under general anaesthesia, suffering from severe absorption syndrome leading to hyponatremia of 106 mEq/l and pulmonary
