BACKGROUND: Neospora is a newly recognized Toxoplasma-like protozoan that causes spontaneous abortion and/or neonatal disease in a wide range of animals. The purpose of this study was to determine the susceptibility of primates to Neospora infection.
EXPERIMENTAL DESIGN: In experiment 1, two rhesus macaque fetuses were inoculated in utero at gestational day 65 with 1 x 10(6) culture-derived Neospora tachyzoites. A control fetus was given uninfected vehicle. The fetuses were removed by hysterotomy between 13 and 22 days postinoculation. In experiment 2, two pregnant macaques were inoculated in

Management of intrauterine fetal death is a common medial problem. The risks of time-related development of consumptive coagulopathy and intrauterine infection with speticemia force the clinician to terminate pregnancy as soon as possible while still taking into account the mental distress of the patients. Rapid surgical intervention (e.g. hysterotomy) is associated with a considerable risk of maternal morbidity and mortality. The following reports from international experts present a topical survey of current methods for the management of fetal demise. This background of clinical experience f

Myelomeningocele (MMC) is a severe neural tube defect resulting from incomplete closure of the spinal column during early embryogenesis, often leading to lifelong neurological, urological, and orthopedic impairments. The "two-hit hypothesis" describes both the primary embryologic malformation and progressive in-utero neural injury as contributors to clinical severity. While postnatal repair prevents further exposure of neural tissue, it does not reverse the intrauterine damage already sustained. This manuscript provides a comprehensive review of the historical evolution, surgical techniques, a

Normal uterine function depends on cyclical regeneration and the capacity to sustain pregnancy. A cesarean incision represents an injury to this remarkable organ. Although the uterus possesses exceptional healing potential, cesarean delivery increases the risk of secondary infertility, pelvic pain, uterine rupture, and abnormal placentation in subsequent pregnancies. The two most important determinants of successful hysterotomy healing after cesarean delivery are the location of the incision and the surgical technique used for closure. The anatomic site of entry-whether the corpus, lower uteri

In the treatment of uterine lesions associated with infertility, such as septate uterus and synechiae, hysteroscopic surgery has replaced conventional abdominal surgery. Hysteroscopic metroplasty improves obstetrical prognosis in patients with septate uterus. Advantages of hysteroscopy are low morbidity rate, decreased hospitalization time, and absence of hysterotomy. Hysteroscopic surgery of synechiae allows to achieve good anatomical results in most of the cases. The first risk of this surgery is perforation. However, obstetrical results are uneven; pregnancy rates are low in cases of comple

Clinical changes, and their implication, in medically induced abortion within South Australia from 1970-1984 were assessed by analysis of legally required reports provided by operating practitioners. In the past 15 years vacuum aspiration has largely replaced dilatation and curettage for early abortions, and intrauterine chemical methods have greatly decreased the use of hysterotomy for late abortions. A marked improvement in gestational pattern has also occurred, and recorded complication rates have been halved for all gestational ages. Sterilization rates at the time of abortions increased w

Preterm birth remains a major challenge in modern obstetrics and is increasing, even among low-risk primiparous women. Very few interventions have made a positive impact on outcome although cervical cerclage appears to benefit some women. Transabdominal cervical cerclage can be highly successful and should be considered in women with previous failed transvaginal cerclage, but requires operative abdominal delivery as it cannot be removed. The authors report the first case they are aware of, where a spontaneous abortion occurred with a transabdominal cerclage in situ and a mid-trimester dilation

OBJECTIVES: To report the neurodevelopmental outcomes after a fetoscopic myelomeningocele repair and to compare them with children who had an open hysterotomy repair or a postnatal repair.
STUDY DESIGN: We included 132 infants (prenatal repair, 93 [69 fetoscopic and 24 open hysterotomy]; postnatal repair, 39). Neurodevelopmental outcomes at or beyond 18 months were evaluated by a developmental pediatrician using the Capute scales (Clinical Adaptive Test [CAT]/Clinical Linguistic & Auditory Milestone Scale [CLAMS]) and/or during parental interview using Developmental Profile 3 test. OBJECTIVE: To assess whether Pfannenstiel skin incision compared with vertical skin incision was associated with postcesarean delivery wound complications in morbidly obese women.
METHODS: We assembled a retrospective cohort of patients with body mass index (BMI) of 40 or higher who delivered by cesarean between July 2012 and May 2019. The primary outcome was a composite wound morbidity (until 42 days postpartum) including wound separation, infection, and dehiscence. Secondary outcomes included individual composite components plus select maternal and neonatal outcomes. Comparisons of demograp

OBJECTIVE: Cesarean delivery between 23 and 27 weeks' gestation is a risk factor for performing classical and inverted T uterine incisions. When attempting cesarean delivery via a low transverse incision at a very preterm gestational age, having difficulty in delivery of the fetus may require conversion to an inverted T-incision. We sought to examine maternal short-term outcomes according to the type of attempted uterine incisions in preterm deliveries.
STUDY DESIGN: This was a multihospital retrospective cohort study of women undergoing cesarean delivery between 23 0/7 and 27 6/7 week' gesta

Pre-clinical evaluation of surgical procedures aimed to correct craniosynostosis is ideally performed in species of small animals characterized by perinatal brain development, early skeletal maturation, and genuine synostosis in all newborns. It would be nearly impossible to breed such a colony to homozygosity, so most researchers have resorted to artificial postnatal suture immobilization. Our aim was to test the hypothesis that artificial immobilization of a unicoronal suture in the fetal rabbit (25 days of gestation) would result in neurocranial growth alterations similar to those seen in t

A wide range of fetal interventions are being performed worldwide to save the fetus's life, prevent permanent fetal organ damage, and allow a successful transition to extrauterine life. However, these are invasive procedures and can be associated with serious complications. This article focuses on promoting a culture of safety by highlighting five common error traps while anesthetizing patients for fetal interventions. They include failure to preserve uteroplacental perfusion and gas exchange, failure to achieve adequate uterine relaxation prior to hysterotomy, failure to monitor the fetus and

Cesarean scar ectopic pregnancy is one of the rarest of all ectopic pregnancies. Cesarean scar ectopic can occur after previous uterine manipulation, in vitro fertilization, hysterotomy, etc. With the increasing number of cesarean sections, the incidence of cesarean scar ectopics has increased worldwide. A high degree of suspicion over the occurrence of ectopic pregnancy after a cesarean section should be maintained by all healthcare workers. Timely diagnosis and treatment according to the presentation of an individual is of utmost importance. Here, we present a case of a 24-year-old second gr

BACKGROUND: Recurrent uterine sacculation is very rare and associated only with pregnancy. It commonly complicates delivery with a retained placenta.
CASE: A 24-year-old woman, gravida 4, para 0, with three previous elective abortions, presented in preterm labor at 32 6/7 weeks' gestation. Her second and third abortions were complicated by incomplete evacuation of uterine contents, and uterine sacculation was diagnosed. Preterm delivery and a retained placenta complicated the present pregnancy. At laparotomy, recurrence of uterine sacculation was noted in the left fundal region of a bicornuat

INTRODUCTION: Spina bifida is the most common non-lethal congenital birth defect of the central nervous system that causes chronic disability due to the combined effects of local nerve damage and the sequelae of non-communicating hydrocephalus. This abnormality can be identified early in gestation and the damage can be progressive over the course of pregnancy. Advances in fetal treatment have made minimally invasive prenatal surgery a realistic consideration for spina bifida in order to improve the outcome for children affected this condition.

Triamcinolone-induced alteration of the notochordal-basichondrocranial relationship in the rhesus monkey. Timed-mated pregnant rhesus monkeys received triamcinolone acetonide (10 mg/kg, IM) during early organogenesis. Treated and control embryos and fetuses were removed by hysterotomy on days 35, 42, 50, 60, and 70. All treated cases 42 days or older exhibited craniofacial and brain defects. Microscopic examination showed abnormal notochordal folding and a shortened cranial base in the 35-day embryos prior to any observed CNS involvement. Bends, deviations, focal proliferation, and persistence

Placental adhesive disorders involve the growth of placental tissue into or through the uterine wall. Among these disorders, placenta percreta is the rarest one. However, it may cause significant complications. This report aimed to report a neglected patient with placenta percreta who developed uterine rupture with life-threatening late postpartum intra-abdominal hemorrhage. On admission, the patient had acute abdomen with moderate abdominal distention and was subjected to emergency laparotomy. A full-thickness defect of the anterior uterine wall involving the hysterotomy site was seen. Placen

Surgical training has always been an essential element of obstetrics. Skills acquisition for cesarean delivery relies heavily on apprenticeship-style training, with a notable paucity of formal simulation training before taking an active role in surgery. Patient safety is compromised when surgeons possess poor surgical skills and inadequate knowledge to handle acute emergencies. Experience comes with "real-world" exposure or can be obtained in a simulated setting, where scenarios can be practiced without the risk of patient harm. As part of a larger Intensive Course in Obstetric Emergencies, a

BACKGROUND: Symptomatic uterine diverticula are rare and, to our knowledge, have never been reported as a cause of postpartum hemorrhage.
CASE: A 35-year-old woman presented with severe vaginal bleeding on the 47th day after a cesarean section. She had received endometrial curettage due to postpartum hemorrhage a few days earlier in another local hospital. Transvaginal ultrasound revealed a thickened heteroechoic endometrium and an isolated isthmic heteroechoic cystic lesion. Repeat endometriaI curettage was performed initially. However, recurrent vaginal bleeding and hypovolemic shock occurr
