Selective arterial embolization in the treatment of an aneurysmal bone cyst of the pelvis.

This case report deals with an aggressive aneurysmal bone cyst of the pelvis in a 20-year-old man causing considerable destruction of the left ilium and the anterior column of the acetabulum, with extension into the pelvis. The treatment of large pelvic aneurysmal bone cysts is challenging because of local destruction of adjacent structures and because of the risk of severe intraoperative bleeding. Therefore selective preoperative embolization may be a valuable tool in the management of aneurysmal bone cysts.

Surgical resection with neoadjuvant chemotherapy for iliac Ewing's sarcoma in adult females: A rare case report highlighting multidisciplinary approaches and promising outcomes.

INTRODUCTION ET IMPORTANCE: The iliac origin Ewing's sarcoma is a highly malignant primitive bone tumor. Its occurrence in adults is very rare. The prognosis for this tumor in adult patients is unfavorable and has a high rate of local recurrence. The main treatment goals include preventing local recurrences and distant metastases. A secondary objective is to maintain the quality of life by avoiding major amputative surgery. The primary aim of this report is to underscore the rarity and significance of the combination of surgery with neoadjuvant chemotherapy for better outcomes. CASE PRESENTAT

Type 3 internal hemipelvectomy: a report of two cases.

Type 3 internal hemipelvectomy involves resection of the pubis. We report on 2 patients who underwent type 3 internal hemipelvectomy. One patient developed a bladder hernia, tumour recurrence, and a pathological fracture of the proximal femur. These were resolved with external hemipelvectomy 7 months later. Another patient underwent additional fascia lata transplantation to prevent development of hernia. Both patients had a stress fracture in the contralateral posterior ilium mimicking bone metastasis.

SPECT/CT imaging in 99mTc-PMT hepatobiliary scintigraphy to detect bone metastases from hepatocellular carcinoma.

We report a 62-year-old man who presented with pain on the right side of his hip. CT revealed destructive masses in the right femur and left ilium. Histological examination indicated metastases from hepatocellular carcinoma, and further investigations revealed the primary tumor in the liver. Hepatobiliary scintigraphy using 99mTc N-pyrydoxyl-5-methyltryptophan and fused SPECT/CT clearly showed abnormal accumulation in these bone metastases from hepatocellular carcinoma.

Assessing Bone Mineral Density in Weight-Bearing Regions of the Body through Texture Analysis of Abdomen and Pelvis CT Hounsfield Unit.

Objective: This study aimed to develop a novel method for opportunistically screening osteoporosis by measuring bone mineral density (BMD) from CT images. We addressed the limitations of commercially available software and introduced texture analysis using Hounsfield units (HU) as an alternative approach. Methods: A total of 458 samples (296 patients) were selected from a dataset of 1320 cases (782 patients) between 1 March 2013, and 30 August 2022. BMD measurements were obtained from the ilium, femoral neck, intertrochanteric region of both femurs, and L1-L5 and sacrum spine body. The region

[Chondrosarcoma arising in solitary osteochondroma: a case study].

Chondrosarcoma is a rare malignant bone tumor. It can arise de novo or secondary to a malignant transformation of a benign underlying cartilage tumor. Secondary chondrosarcoma arising from solitary benign osteochondroma is extremely rare and data show that the reported incidence of osteochondroma of the pelvis is very low. We here report the case of a 20-year old patient with chondrosarcoma secondary to malignant transformation of an osteochondroma of the right wing of ilium, adjacent to the sacroiliac joint.

[Free bone and skin-bone autoplasty on a vascular pedicle in patients with defects and pseudarthroses].

Radical sequestr- and necrectomy followed by skin, bone or skin-bone autoplasty on the vascular pedicle with microvascular anastomoses between the autotransplant vessels and those of the extremity was successfully fulfilled in 16 patients with defects of tubular bones and false joints complicated by chronic osteomyelitis. The pedicled autografts were taken from the upper flaring portion of the ilium (13), from the fibula (2) and from the external surface of the femur (1). The two-step method of autoplasty is described.

ISOLATED HYDATID DISEASE OF THE ILIAC BONE.

Hydatid disease of the bone usually is asymptomatic and is found as an accidental finding during unrelated imaging. However, they can become symptomatic due to enlargement and pressure effect or being infected by bacteria. Hydatid disease usually involves multiple organs (such as liver, lungs and brain). In our case, hydatid disease had involved bone and the patient presented only with a chronic hip pain without other symptom or sign. Here, a case of isolated ilium hydatidosis is reported.

Superior gluteal artery laceration, a complication of iliac bone graft surgery.

In the 2 cases cited, laceration of the superior gluteal artery occurred when removing bone from the posterior ilium. A review of the anatomy shows how the superior gluteal artery, as it exists the sciatic notch, can be compressed locally and exposed for clipping or ligation. The internal iliac artery can be occluded by embolization of a Fogerty catheter. The bony origin of the gluteus maximus is a good landmark to help avoid entering the sciatic notch and a special retractor provides the necessary exposure.

Intraosseous hibernoma: a potential mimic of metastatic carcinoma.

Intraosseous hibernoma is a rare benign bone tumor, with only 3 cases reported in English literature. In this report, we describe a 50-year-old woman with a history of stage IIB breast cancer and posterolateral right hip pain. Imaging studies showed a sclerotic lesion in the right ilium, which was biopsied and showed mildly thickened bone trabeculae and multivacuolated brown fat cells replacing the normal white fat and hematopoietic elements, diagnostic of intraosseous hibernoma.

Incidence and risk factors of adjacent segment degeneration in lumbosacral or lumbo-iliac fusion with L2 as the uppermost instrumented vertebra.

OBJECTIVE: Lumbosacral or lumbo-iliac fusion may be warranted in patients with symptomatic diffuse lumbar spondylosis or mild to moderate lumbar spinal deformity. Depending on the extent of pathology, the L2 vertebra may be chosen as the uppermost instrumented vertebra (UIV) to avoid fusion crossing the thoracolumbar junction. However, the incidence of adjacent segment degeneration (ASD) following L2 to sacrum or ilium fusion has not been well studied. The authors aimed to determine the incidence of ASD following instrumented fusion in adult patients undergoing L2 to sacrum or ilium fusion and

Percutaneous reduction of LC-2 pelvic ring fracture.

Minimally invasive reduction of lateral compression type 2 (LC-2) pelvic ring fractures is not well described [1]. While the gold standard remains open reduction and internal fixation with plates and screws, this technique is associated with notable risks, including significant blood loss, nerve injury, wound dehiscence, and postoperative infection [1-3]. A minimally invasive technique of fracture reduction and fixation for LC-2 fractures is presented, with emphasis on imaging to visualize realignment of the reduced ilium.

A rare case of monomorphic epitheliotropic intestinal T-cell lymphoma mimicking Crohn's disease.

A 70-years-old male was referred to us due to bloody diarrhea and involuntary weight loss during the last 2 months. Laboratory workup revealed anemia, hypoalbuminemia, and elevated reactive-C-protein. Computed tomography documented a wall thickening of sigmoid colon. Colonoscopy revealed pleomorphic ulceration of the sigmoid and descending colon. Aphthous ulcers were seen in ilium. Histology revealed mucosal infiltration with small sized atypical lymphoid cells in the epithelium and lamina propria, with extensive epitheliotropism.

Cystic echinococcosis of the pelvic bone with recurrences: a case report.

Hydatid cysts commonly affect the liver and the lung. However, they rarely involve bones with vertebral column. We hereby report a case of a female patient with cystic echinococcosis of the hip bone and ilium. She presented with a long history of frequent recurrences highlighting the dismal prognosis at this rare site. Resection of the hydatid cyst from the sacroiliac region was done with allograft and autograft (rib graft) with lumbosacroiliac fixation. Follow-up of the patient at 6 months showed no detectable abnormality on radiology and the patient was doing well.

Hydatid disease of bones and joints. 8 cases followed for 4-16 years.

Hydatid disease is a rare parasitic disease that seldom involves the skeleton. Treatment is difficult because of problems with the preoperative diagnosis, the invasive nature of the bony involvement and the variable anaphylactic reaction to the cyst fluid antigen. We present 8 cases with osseous hydatidosis who were treated over a period of 11 years. The spine was involved in 2 cases, the ilium in 2, the hip in 2, the tibia in 1 and the humerus in 1. We point out that diagnosis is difficult and the prognosis is often poor.

B19 parvovirus infection and transient aplastic crisis in a child with sickle cell anemia.

Transient aplastic crisis is reported in a young child with sickle cell anemia with acute B19 parvovirus infection. She also developed acute chest syndrome and bone marrow/bone infarction involving the right ilium. The clinically unsuspected bone marrow infarction in this patient may have contributed to acute chest syndrome secondary to pulmonary fat embolism. Transient cessation of erythropoiesis as a result of B19 parvovirus infection, and not the localized bone marrow infarction, was the probable cause of reticulocytopenia and worsening of anemia in this child.

Intraosseous lipoma: role and limitations of modern imaging techniques.

We report a case of intraosseous lipoma involving the ilium in a 37-year-old male. There were no clinical symptoms. Plain films suggested the diagnosis, which was confirmed by computed tomography and magnetic resonance imaging with fat-cancelling sequences. However, we were unable to rule out fatty involution of an old aneurysmal bone cyst. Modern imaging techniques are very reliable for the diagnosis of intraosseous lipoma. Computed tomography is usually sufficient to establish the diagnosis and to guide therapeutic decisions.

Technique for achievement and maintenance of reduction for severe spondylolisthesis using spinous process traction wiring and external fixation of the pelvis.

A technique is described for achievement and maintenance of reduction for severe spondylolisthesis in conjunction with reconstructive surgery. Spinous-process traction wires exert a posterior and cephalad force on the lumbar spine, while Hoffman pins anchored to the ilium rotate the sacrum in an anterior and caudad direction. Once maximum reduction is achieved, the patient is kept supine for 3 months with pins and traction wires incorporated in a plaster cast. At follow-up, two patients demonstrated excellent correction of slip angle and a solid fusion.
