Association between preterm delivery and subsequent maternal risk of hypertension and type 2 diabetes mellitus in a UK population-based retrospective cohort study.

OBJECTIVES: Women with a history of preterm delivery (PTD) are at higher risk of developing cardiovascular diseases (CVD) later in life. However, it is not well established whether PTD is associated with CVD risk factors, hypertension and type 2 diabetes mellitus (T2DM). Therefore, in this study, we examined the associations between PTD compared with term delivery and subsequent risk of hypertension and T2DM. DESIGN: Retrospective matched population-based open cohort study. SETTING: Clinical Practice Research Datalink GOLD data in the UK. PARTICIPANTS: A total of 3335 18-49-year-old women w

Antenatal counselling for congenital anomaly tests: an exploratory video-observational study about client-midwife communication.

OBJECTIVE: antenatal counselling for congenital anomaly tests is conceptualised as having both Health Education (HE) and Decision-Making Support (DMS) functions. Building and maintaining a client-midwife relation (CMR) is seen as a necessary condition for enabling these two counselling functions. However, little is known about how these functions are fulfilled in daily practice. This study aims to describe the relative expression of the antenatal counselling functions; to describe the ratio of client versus midwife conversational contribution and to get insight into clients' characteristics, w

High-normal and mildly elevated bilirubin levels in prefibrotic primary myelofibrosis patients are associated with lower thrombotic risk.

High-normal or mildly elevated bilirubin concentrations without apparent significance are commonly observed in clinical practice. Several observations in various chronic diseases associate them with reduced risk of metabolic and cardiovascular diseases, likely due to their antioxidant and anti-inflammatory activity. We aimed to investigate potential clinical and prognostic associations of high-normal to mildly elevated bilirubin in patients with prefibrotic primary myelofibrosis (pre-PMF), a chronic myeloproliferative neoplasm with high inflammatory and thrombotic burden. We retrospectively an

Ability of patient-reported outcomes to predict progression-free survival in patients with mild to moderate fibrosing interstitial lung disease.

BACKGROUND: The prognostic utility of patient-reported outcomes (PROs) in mild to moderate fibrosing interstitial lung disease (FILD) remains insufficiently characterized. METHODS: In this retrospective study, we examined the associations between PROs-Short-Form 36 (SF-36), Nagasaki University Respiratory Activities of Daily Living Questionnaire (NRADL), Hospital Anxiety and Depression Scale-Depression (HADS-D) and HADS-Anxiety (HADS-A)-and progression-free survival (PFS) in patients with FILD and a gender-age-physiology (GAP) score ≤5. We employed principal component analysis (PCA), hierarch

Cannabis Assessment in Oncology Patients - Evaluation Survey: Understanding Usage Patterns in a Rural Radiation Cohort in Kentucky.

BACKGROUND: Research on self-administration of unprescribed cannabis remains limited because of its evolving legal status in the United States. We conducted, to our knowledge, the first fully independent tablet application-executed oncology survey without research staff oversight in urban and rural radiation oncology practices. Objectives were to: (1) characterize symptoms motivating cannabis use, (2) compare urban and rural patterns of use, (3) assess concomitant use with opioids and gabapentin in an opioid-endemic region, and (4) evaluate associations with patient-reported quality of life.

Cancer prevention in context: interplay of perceived discrimination, neighborhood deprivation, and cancer-related risk and protective factors.

BACKGROUND: Socioenvironmental deprivation and discrimination are associated with poorer cancer outcomes, yet their combined impact remains understudied. This study examines the independent and combined impact of these factors on cancer-related outcomes. METHODS: Survey data (2018-2020) from 7977 participants across 6 National Cancer Institute-designated cancer centers assessed perceived discrimination (PD), area-level deprivation, lifestyle factors (smoking and obesity), and self-reported breast and colorectal cancer screening behaviors. Multivariate and multilevel logistic regression models

Total versus Partial Workplace Tobacco Use Bans in Texas Behavioral Health Centers and their Associations with Employees' Report of Clear Signage, Policy Enforcement, Policy Awareness, and Their Beliefs, Training, and Practices Conducive to Addressing Clients' Tobacco Use.

OBJECTIVES: Adults with behavioral health needs exhibit elevated tobacco use rates. Tobacco-free workplace policies (TFWPs) at behavioral health treatment centers can effectively curb clients' tobacco use and secondhand smoke/vape exposure. However, there is little extant observational research about how total versus partial workplace tobacco use bans are associated with employee's perceptions of signage clarity, consistency of enforcement, and stakeholders' policy awareness in behavioral health centers. Additionally, little is understood about the relations of total or partial TFWPs with othe

Poor sleep quality, depression and hope before breast cancer surgery.

AIM: This study aims to identify the predictors of poor sleep quality and the associations between depression, hope and sleep in a sample of 156 women with breast cancer prior to surgery. BACKGROUND: The care and treatment of breast cancer is among the most important scientific challenges in public health. Sleep disturbances and depression are the important complaint of cancer patients. However, they are often neglected. Patients diagnosed with breast cancer also experience prejudice, fear of death, suffering, mutilation, fear of lymphedema, as well as feelings of social devaluation leading t

AF is associated with self-reported syncope and falls in a general population cohort.

BACKGROUND: syncope is an important, but underestimated clinical problem in older persons. It is often overlooked in clinical practice or mistaken for falls. Atrial fibrillation (AF) is the most common cardiac arrhythmia, but little evidence exists regarding the association between AF, falls and syncope in the general population. METHODS: cross-sectional analyses within a population sample of people aged 50+, taken from The Irish Longitudinal Study on Ageing. Ten-minute electrocardiogram recordings (n = 4,885) were analysed to detect AF. Syncope (self-reported faints or blackouts) and falls i

Variation in restarting antithrombotic drugs at hospital discharge after intracerebral hemorrhage.

BACKGROUND AND PURPOSE: Whether intracerebral hemorrhage (ICH) survivors should restart antithrombotic drugs is unknown. We analyzed the frequency of restarting antithrombotic drugs in ICH survivors who had taken prophylactic antithrombotic drugs in atrial fibrillation or after thromboembolic disease in 5 cohorts and explored factors associated with doing so. METHODS: We compared the characteristics and proportions of patients taking antithrombotic drugs at ICH onset and discharge in 4 hospital-based cohorts (Lille, France, n=542; Utrecht, The Netherlands, n=389; multicenter Clinical Relevanc

Perineal Care Strategies and Perineal Outcomes in Primiparous Women: A Prospective Observational Pilot Study.

BACKGROUND: Pregnancy and childbirth expose the pelvic floor and perineal tissues to substantial biomechanical stress, increasing the risk of obstetric perineal trauma and subsequent maternal morbidity. OBJECTIVES: This prospective observational pilot study aimed to explore the association between antenatal perineal massage and perineal outcomes at birth, as well as postpartum pelvic floor findings, in primiparous women. METHODS: Women attending an antenatal education programme at the Family Counselling Centre of Francavilla al Mare (Italy) between May and November 2024 were recruited and fo

Adverse effects of red blood cell transfusions in neonates: a systematic review and meta-analysis.

BACKGROUND: Controversy exists regarding the contribution of blood transfusions to a range of adverse clinical outcomes in neonates. The aim of our systematic review was to identify the broader literature on harmful effects and associations potentially attributable to red blood cell (RBC) transfusions. STUDY DESIGN AND METHODS: A comprehensive search of MEDLINE (PubMed) and EMBASE was undertaken. Eligible studies included both randomized controlled trials (RCTs) and nonrandomized studies examining the effects of small volume (10-20 mL/kg) RBC transfusions on neonates. Primary outcomes of inte

Healthcare-associated infections and Shanghai clinicians: a multicenter cross-sectional study.

Literature about healthcare-associated infection (HCAI) in China is scarce. A cross-sectional anonymous survey was conducted on 647 clinicians (199 physicians and 448 nurses) from six Shanghai hospitals (grades A-C) to investigate their cognizance, knowledge, attitude, self-reported practice, and risks regarding HCAI with emphasis on precautions. The mean overall score of HCAI knowledge was 40.89±11.4 (mean±SD; range, 13∼72) out of 100 for physicians and 43.48±9.9 (10∼70) for nurses. The respondents generally received high scores in hand hygiene, HCAI core concept, and healthcare worker safety

Cross-sectional survey of antimicrobial prescribing patterns in UK small animal veterinary practice.

The increase in the prevalence of antimicrobial resistance has resulted in both human and veterinary antimicrobial use coming under increased scrutiny. The aim of this study was to characterise antimicrobial prescribing patterns in small-animal veterinary practices in the UK. A cross-sectional survey of UK small animal veterinarians was undertaken. A postal questionnaire to evaluate antimicrobial prescribing habits was sent to 900 clinicians. Data were collected on the clinicians, their practices and their sources of information regarding antimicrobials and their use. Respondents were asked if

Vaccinations, infections and antibacterials in the first grass pollen season of life and risk of later hayfever.

BACKGROUND: It has been hypothesized that early-life exposure to vaccinations, infections or antibacterials influence allergic disease development. Concurrent exposure to grass pollens may alter any effect. OBJECTIVE: To test the hypothesis that exposure to antibacterials, vaccinations (DTP or MMR) or specific infections during the first grass pollen seasons of life influences the risk of hayfever more than at any other time of the year. METHODS: Nested case-control studies were based on birth cohorts within two large databases of computerized patient records from UK general practices: the G

Practice Variation Among Surgeons Treating Lumbar Spinal Stenosis in a Single Institution.

STUDY DESIGN: A retrospective study. OBJECTIVE: The aim of this study was to examine practice variation in the treatment of lumbar spinal stenosis and identify targets for reducing variation. SUMMARY OF BACKGROUND DATA: Lumbar spinal stenosis is a degenerative condition susceptible to practice variation. Reducing variation aims to improve quality, increase safety, and lower costs. Establishing differences in surgeons' practices from a single institution can help identify personalized variation. METHODS: We identified adult patients first diagnosed with lumbar spinal stenosis between 2003 an

Caregiver Weight Misperception and Feeding Practices in U.S. Preschool-Aged Children: A Theory-Based Cross-Sectional Study.

BACKGROUND/OBJECTIVES: Caregiver perception of child weight may be a cognitive antecedent of feeding practices, yet evidence linking misperception to feeding practices remains inconsistent. Guided by Social Cognitive Theory, this study examined whether caregiver weight perception (underestimation, overestimation, or accurate) is independently associated with feeding practices among caregivers of U.S. preschool-aged children. METHODS: Primary caregivers of children aged 3-5 years (analytic n = 139) were recruited across the Midwestern United States from April 2022 to March 2023. Weight percept

Associations between injecting illicit drugs into the femoral vein and deep vein thrombosis: A case control study.

INTRODUCTION AND AIMS: Injecting drug use is a risk factor for deep vein thrombosis (DVT), and people who inject drugs commonly report injecting into the femoral vein. However, it is unclear whether the act of inserting a needle into the femoral vein or the pharmacodynamic properties of the injected drug increases DVT risk. We aimed to quantify the strength of association between injecting illicit drugs into the femoral vein and the odds of acquiring ileo-femoral DVT. DESIGN AND METHODS: We used case control methodology. The study took place in Leeds, UK. A total of 313 people who inject drug

A Cohort Analysis of Statin Treatment Patterns Among Small-Sized Primary Care Practices.

BACKGROUND: Small-sized primary care practices, defined as practices with fewer than 10 clinicians, delivered the majority of outpatient visits in the USA. Statin therapy in high-risk individuals reduces atherosclerotic cardiovascular disease (ASCVD) events, but prescribing patterns in small primary care practices are not well known. This study describes statin treatment patterns in small-sized primary care practices and examines patient- and practice-level factors associated with lack of statin treatment. METHODS: We conducted a retrospective cohort analysis of statin-eligible patients from
