Transcranial direct current stimulation's effect on exercise capacity, respiratory muscle strength, and physical performance in patients with chronic obstructive pulmonary disease: Randomized controlled trial.

BackgroundChronic obstructive pulmonary disease (COPD) is associated with alterations in brain function that contribute to reduced respiratory and skeletal muscle strength, ultimately limiting exercise capacity. Transcranial direct current stimulation (tDCS), a non-invasive neuromodulation technique, has demonstrated efficacy in enhancing lower extremity function in healthy older adults.ObjectiveTo evaluate the effects of a single session of anodal tDCS on exercise capacity, respiratory muscle strength, and physical performance in individuals with COPD.MethodsIn a randomized, double-blind, sha

Cardiopulmonary Rehabilitation Improves Respiratory Muscle Function and Functional Capacity in Children with Congenital Heart Disease. A Prospective Cohort Study.

Critical surgical and medical advances have shifted the focus of congenital heart disease (CHD) patients from survival to achievement of a greater health-related quality of life (HRQoL). HRQoL is influenced, amongst other factors, by aerobic capacity and respiratory muscle strength, both of which are reduced in CHD patients. This study evaluates the influence of a cardiopulmonary rehabilitation program (CPRP) on respiratory muscle strength and functional capacity. Fifteen CHD patients, ages 12 to 16, with reduced aerobic capacity in cardiopulmonary exercise testing (CPET) were enrolled in a CP

Mechanical constraints on exercise hyperpnea in endurance athletes.

We determined how close highly trained athletes [n = 8; maximal oxygen consumption (VO2max) = 73 +/- 1 ml.kg-1.min-1] came to their mechanical limits for generating expiratory airflow and inspiratory pleural pressure during maximal short-term exercise. Mechanical limits to expiratory flow were assessed at rest by measuring, over a range of lung volumes, the pleural pressures beyond which no further increases in flow rate are observed (Pmaxe). The capacity to generate inspiratory pressure (Pcapi) was also measured at rest over a range of lung volumes and flow rates. During progressive exercise,

Effect of 2-5 years of tobacco smoking on ventilatory function tests.

Smoking is a pernicious scourge of the world today. There is paucity of literature on the effect of acute smoking on ventilatory functions. The present work is undertaken to study the effect of 2-5 years of tobacco smoking on ventilatory functions. The study group consisted of 30 male, young, healthy subjects, free from cardiopulmonary diseases and with history of smoking of 2-5 years duration, on an average of 10 cigarettes per day. The control group consisted of 30 age-and sex-matched healthy individuals who had naver smoked tobacco. The ventilatory function tests were carried out using elec

Inspiratory muscle training for asthma.

BACKGROUND: In some people with asthma, expiratory airflow limitation, premature closure of small airways, activity of inspiratory muscles at the end of expiration and reduced pulmonary compliance may lead to lung hyperinflation. With the increase in lung volume, chest wall geometry is modified, shortening the inspiratory muscles and leaving them at a sub-optimal position in their length-tension relationship. Thus, the capacity of these muscles to generate tension is reduced. An increase in cross-sectional area of the inspiratory muscles caused by hypertrophy could offset the functional weaken

Load dependence of secondary diaphragm inflammation and injury after acute inspiratory loading.

Chronic or prolonged low-intensity loading of the inspiratory muscles has recently been shown to produce diaphragm injury. The present study was designed to examine whether an acute episode of inspiratory resistive loading (IRL) could produce secondary diaphragm inflammation and injury. On Day 1, three groups of anesthetized and intubated New Zealand White rabbits were subjected to moderate IRL (Pao of approximately 30 cm H2O), high IRL (Pao of approximately 45 cm H2O), or no load for 1.5 h. On Day 3, costal and crural diaphragms, parasternals, and gastrocnemius muscles were taken to assess in

P 0.1/PIMax: an index for assessing respiratory capacity in acute respiratory failure.

We studied airway occlusion pressure (P 0.1) and maximal inspiratory pressure (PIMax) in 10 healthy volunteers (Group A), 10 early postsurgical cardiac patients on spontaneous breathing (Group B), 10 patients mechanically ventilated for ARF (Group C), 10 patients weaning from mechanical ventilation after ARF (Group D) and 10 patients extubated after post-ARF (Group E). We calculated the index P 0.1/PIMax in an attempt to link the ventilatory demands and muscle ventilatory reserve. We found that the sensitivity and specificity in diagnosing the need for either full (C), partial (D) or no ventil

In normal subjects bracing impairs the function of the inspiratory muscles.

Normal subjects can increase their capacity to sustain hyperpnoea by bracing their arms on fixed objects, a procedure which is also known to reduce dyspnoea in patients with chronic obstructive pulmonary disease (COPD). In the present study, it was tested whether bracing per se could improve the function of the diaphragm. The effect of bracing on diaphragm function was studied in six normal subjects by recording changes in oesophageal (delta Poes) and transdiaphragmatic (delta Pdi) pressure during inspiratory capacity (IC) manoeuvres in the seated and upright postures, and in the seated postur

Dose-dependent effects of inspiratory muscle training in neuromuscular disorders.

The goal of this study was to show whether a correlation exists between the intensity of specific respiratory muscle training and the improvement of strength and endurance in inspiratory musculature in patients with neuromuscular disorders (NMD). Sixteen patients with NMD (13 with Duchenne muscular dystrophy and 3 with spinal muscular atrophy) performed inspiratory muscle training (IMT) at home with a special training apparatus for 9 months. Maximal inspiratory mouth pressure (PI(MAX)) and 12s-maximum voluntary ventilation (12s-MVV) test served as parameters for inspiratory muscle strength and

Optimal Maximal Forced Inspiratory Flow for Dry Powder Inhalers in Reducing Exacerbations in COPD.

BACKGROUND: The optimal management of chronic obstructive pulmonary disease (COPD) requires effective drug delivery through dry powder inhalers (DPIs). Maximum forced inspiratory flow (FIFmax), easily obtained from routine spirometry, can reflect inspiratory capacity and serve as a physiological marker associated with exacerbation risk. However, its clinical implications have not been fully established. METHODS: This retrospective cohort study analyzed COPD patients treated with DPIs over a 5-year observational period. Baseline and follow-up FIFmax values were measured through repeated spirom

Respiratory muscle dysfunction in COPD: from muscles to cell.

Respiratory muscle dysfunction is a cardinal feature of acute and chronic respiratory failure in COPD. Diaphragm and accessory inspiratory muscles face increased load due to increased lung resistance and elastance, as well as increased ventilatory demands. Concomitantly, the capacity of the inspiratory muscles to generate pressure is decreased due to mechanical disadvantage imposed by hyperinflation. Additionally, inflammation and oxidative stress impair muscle fiber specific force generation and increase diaphragm susceptibility to sarcomer disruption during acute inspiratory loading. In resp

Retinoic acid receptor alpha activation is necessary and sufficient for plasticity induced by recurrent central apnea.

Reductions in respiratory-related synaptic inputs to inspiratory motor neurons initiate a form of plasticity that proportionally enhances inspiratory motor output, even in the absence of changing blood gases. This form of plasticity is known as inactivity-induced inspiratory motor facilitation (iMF). iMF triggered by brief, recurrent reductions in respiratory neural activity requires local retinoic acid (RA) synthesis, but receptor subtypes activated by RA are unknown. To test the hypothesis that retinoic acid receptor alpha (RARα) is necessary for iMF, RAR subtype-specific inhibitors were del

Effect of sacrum-backrest distance on respiratory function during wheelchair sitting: implications for seating assessment.

PURPOSE: Wheelchair seating configuration may influence respiratory mechanics by altering pelvic alignment, trunk posture, and diaphragmatic excursion. Deep sitting is generally recommended to maintain pelvic neutrality; however, the quantitative impact of sacrum-backrest distance on ventilatory performance remains unclear. This study aimed to investigate the effect of sacrum-backrest distance on respiratory function and thoracic expansion during wheelchair sitting. MATERIALS AND METHODS: Nineteen healthy young females were assessed under three wheelchair sitting conditions defined by sacrum-
