Comparison of the long-term results between surgical and conservative treatment in patients with intermittent claudication.

BACKGROUND: The optional therapeutic strategy for patients with intermittent claudication remains controversial. In this study, we investigated the influence of surgical and conservative therapies on improving the quality of life in patients with intermittent claudication. METHODS: We analyzed 427 patients who were admitted to our hospital with intermittent claudication in their legs during a 15-year period from January 1984 to December 1999. We separated them into 2 groups; 259 patients (362 legs) were treated surgically and 168 patients were treated conservatively. RESULTS: At the supraing

[Evaluation of intermittent claudication using a treadmill with measurement of systolic pressure at the ankle following exercise].

Intermittent claudication (IC) and maximum walking distance (MWD) were studied in 173 patients and compared to post-stress Doppler claudicant ankle pressure (PSCAP) and to ankle index (PSCAI) recorded 2 minutes after exercise. Self-evaluation of MWD by the patient was inaccurate in 60% of cases. The treadmill testing (at 12% of rate and 2 miles/hour) exhibited a clinically overt IC in 93 cases with MWD of 166 m +/- 76, a PSCAP of 24 mm Hg +/- 24 and a PSCAI of 0.17 +/- 0.17. A clinically questionable IC occurred in 50 cases with significantly higher values (p less than 0.001) for MWD (278 m +/

Feasibility Randomized Controlled Trial of Remotely Supervised Exercise versus Self-Directed Exercise for Intermittent Claudication.

BACKGROUND: This trial examined the feasibility of a future trial of remotely supervised exercise (RSE) versus self-directed exercise (SDE) for stable claudication. METHODS: Randomized, single center, assessor blind, feasibility trial. In the intervention arm patients received fortnightly video or telephone calls from a physiotherapist with a tailored exercise prescription and review of progress with the aim of a minimum of 120 min of leg exercise per week. In the comparator arm, patients were asked to do this themselves. Both groups had an electronic walking log which was inspected at 3 and

Long-term patient-reported outcomes among patients undergoing revascularization vs medical therapy for intermittent claudication.

OBJECTIVE: Society for Vascular Surgery guidelines recommend revascularization for patients with intermittent claudication (IC) if it can improve patient function and quality of life. However, it is still unclear if patients with IC achieve a significant functional benefit from surgery compared with medical management alone. This study examines the relationship between IC treatment modality (operative vs nonoperative optimal medical management) and patient-reported outcomes for physical function (PROMIS-PF) and satisfaction in social roles and activities (PROMIS-SA). METHODS: We identified pa

Detection of Intermittent Claudication from Smartphone Inertial Data in Community Walks Using Machine Learning Classifiers.

Peripheral arterial disease (PAD) causes blockage of the arteries, altering the blood flow to the lower limbs. This blockage can cause the individual with PAD to feel severe pain in the lower limbs. The main contribution of this research is the discovery of a solution that allows the automatic detection of the onset of claudication based on data analysis from patients' smartphones. For the data-collection procedure, 40 patients were asked to walk with a smartphone on a thirty-meter path, back and forth, for six minutes. Each patient conducted the test twice on two different days. Several machi

Short interval or continuous training programs to improve walking distance for intermittent claudication: Pilot study.

OBJECTIVE: Supervised exercise training is part of first-line therapies for intermittent claudication. Short periods of intensive treadmill training have been found efficient; however, the optimal modalities remain to be determined, especially interval training with active recovery (ITAR). In this prospective assessor-blinded single-centre pilot study, we assessed the feasibility of a randomised controlled trial comparing parallel 4-week intensive rehabilitation programs comprising treadmill training performed as ITAR or conventional training with constant slope and speed interspersed with res

Metal-on-metal total hip arthroplasty adverse local tissue reaction with intermittent unilateral vascular claudication.

Osteolysis and adverse local soft-tissue reactions are well-documented complications of metal-on-metal prosthetic implants. This case report describes a 68-year-old man who presented to the clinic 10 years after staged bilateral metal-on-metal total hip arthroplasty revisions with the primary complaint of groin pain, intermittent right leg pain, swelling, and muscle cramping while ambulating that resolved with rest. A complete workup was negative for deep venous thrombosis and infection. His symptoms were found to be secondary to an iliopsoas bursal mass externally compressing the femoral vasc

Behaviour change interventions to promote physical activity in people with intermittent claudication: the OPTIMA systematic review.

BACKGROUND: People with intermittent claudication are significantly less active compared to their peers without intermittent claudication, worsening future health outcomes. Supervised exercise therapy is not commonly available, but behaviour change techniques in unsupervised interventions can improve physical activity. Specific behaviour change techniques, theoretical mechanisms and contextual features linked to effectiveness remain unclear. OBJECTIVES: To conduct an integrative synthesis of: effectiveness of behaviour change technique-based interventions on daily physical activity and clinic

Outcomes after infrainguinal interventions for intermittent claudication in patients with end-stage renal disease are poor.

BACKGROUND: Intermittent claudication (IC) interventions are predicated on safety, durability, and long-term expected benefit. Patients with end-stage renal disease (ESRD) have a higher risk of complications after many surgical procedures and have lower overall survival. Our goal was to assess perioperative and 1-year outcomes of IC interventions in patients with ESRD. METHODS: The Vascular Quality Initiative was queried from 2010 to 2020 for peripheral vascular interventions (PVI) and infrainguinal bypasses (IIBs) for IC. Demographics, comorbidities, procedural details, and outcomes were ana

Intermittent claudication-like syndrome secondary to atherosclerosis in a yellow-naped Amazon parrot (Amazona ochrocephala auropalliata).

A 25-year-old yellow-naped Amazon parrot (Amazona ochrocephala auropalliata) was presented for nasal discharge and sneezing. Physical examination revealed poor feather quality, a mild serous nasal discharge, and a mass on the dorsal surface of the oral cavity. Cytologic examination of a mass aspirate as well as results of a choanal culture revealed squamous metaplasia of the salivary glands and bacterial rhinitis, respectively. Following resolution of the presenting conditions, the patient was presented for hind limb weakness and ataxia. The clinical signs were transient and generally resolved

[Cystic adventitial degeneration of the popliteal artery. Rare cause of intermittent claudication in middle-aged adults].

We report on a 36-year old male patient with clinically prolonged intermittent claudication of the right leg. An operative specimen measuring 8.0 cm of the A. poplitea was sent for examination, which was sheathed by an aneurysmal cyst having a maximal diameter of 1.5 cm. Examination under the light microscope showed an incomplete septated ganglion-like cystic formation with flat connective tissue-like cells or even larger histiocytoid cell elements as inner lining. Immunohistochemically, the cells on the inner lining of the cyst showed to be strongly positive to macrophage marker PG-M1, KP1 an

[The effect of sulodexide on the quality of life of patients with intermittent claudication: results of a multicenter observational study of ANDANTE].

RELEVANCE: Arterial disease of the lower extremities is a common form of atherosclerosis associated with an increased risk of cardiovascular disease and mortality. Arterial disease of the lower extremities tends to progress from asymptomatic forms to intermittent claudication and critical ischemia. Sulodexide therapy in patients with PH allows for increased pain-free walking distance. At the same time, the scientific literature has not fully documented how long-term sulodexide therapy affects the quality of life of patients with arterial disease of the lower extremities. The aim of the study w
