Clinical and Molecular Characterization of Osteogenesis Imperfecta Type V.

Osteogenesis imperfecta type V (OI-V) has a wide clinical variability, with distinct clinical/radiological features, such as calcification of the interosseous membrane (CIM) between the radius-ulna and/or tibia-fibula, hyperplastic callus (HPC) formation, dislocation of the radial head (DRH), and absence of dentinogenesis imperfecta (DI). Recently, a single heterozygous mutation (c.-14C>T) in the 5'UTR of the IFITM5 gene was identified to be causative for OI-V. Here, we describe 7 individuals from 5 unrelated families that carry the c.-14C>T IFITM5 mutation. The clinical findings in these case

Changes in length of the radioulnar ligament and distal oblique bundle after Colles' fracture.

The purpose of this study was to investigate changes in length of the radioulnar ligament and distal oblique bundle (DOB) within the distal interosseous membrane after Colles' fracture and correlate the magnitude of the changes in length with clinical features. This study investigated 10 patients with malunion of a Colles' fracture. In three-dimensional computed tomography, the paths of the four limbs of the radioulnar ligament (superficial and deep, dorsal, and palmar limbs) and DOB were modelled and each path length was computed. Differences in length between the affected and contralateral u

Distal radioulnar joint injuries.

Distal radioulnar joint is a trochoid joint relatively new in evolution. Along with proximal radioulnar joint, forearm bones and interosseous membrane, it allows pronosupination and load transmission across the wrist. Injuries around distal radioulnar joint are not uncommon, and are usually associated with distal radius fractures, fractures of the ulnar styloid and with the eponymous Galeazzi or Essex_Lopresti fractures. The injury can be purely involving the soft tissue especially the triangular fibrocartilage or the radioulnar ligaments. The patients usually present with ulnar sided wrist pai

[Secondary treatment for undetected Essex-Lopresti lesion].

AIM: Essex-Lopresti lesion (ELL) consists of a radial head fracture in combination with a rupture of the interosseous membrane and a dislocation of the distal radioulnar joint. In the case of radial head resection proximal migration of the radius occurs. The aim of this retrospective study was to analyse the treatment and outcome for chronic ELL. METHOD: Altogether 6 patients with secondary ELL were detected and clinically reviewed. Therapy consisted of implantation of a silastic radial head prosthesis in 3 cases and ulna shortening in 2 other cases. One patient received both procedures. In t

Athletic injuries of the wrist.

The wrist is at risk for injury in many athletic endeavors, often in acute falls or dorsiflexion injuries and at other times from repetitive stress involved with fitness routines or specific motions called for in the sport. Dorsiflexion accounts for most wrist problems giving rise to compression injuries dorsally such as scaphoid impaction, triquetrohamate impaction, and occult ganglions. These are seen most often in weight lifters, gymnasts, and push-up enthusiasts. Tension injuries occur on the palmar aspect of the wrist with disruption or attenuation of the supporting ligaments or interosse

A series of tibial fractures treated conservatively.

Two hundred and two fractures of the tibia have been reviewed. They were treated by a standard, conservative method; with a Phemister graft when union was delayed and a pseudarthrosis threatened. The classifications of Charnley (1968) and Nicoll (1964) have proved to be useful in assessing results and these complement each other. The factors that influence bony union include damage to the interosseous membrane, the degree of the displacement and comminution of the fracture, and injury to soft tissues. Infection has a very unfavourable influence on bone union and leads to the majority of bad se

Proximal radioulnar synostosis following Monteggia fracture-dislocation: a case report.

INTRODUCTION AND IMPORTANCE: Radioulnar synostosis is a rare complication of a forearm fracture that restricts pronation-supination. This study presents a case of proximal radioulnar synostosis in an adult male after Monteggia fracture-dislocation who had a loss of pronation and supination movements. CASE PRESENTATION: Herein, we report a case of proximal radioulnar synostosis in a 43-year-old man who presented with loss of pronation and supination of the right forearm that restricted his daily activities. He had a history of Monteggia fracture-dislocation 9 months back, which was managed wit

Ultrastructure of neuromuscular junction in vacor-induced diabetic rats.

OBJECTIVES: Rodenticide Vacor causes a severe peripheral neuropathy in humans. Electrophysiologic studies on a peripheral motor nerve-skeletal system of Vacor-treated rat showed decreased amplitude of muscle action potential without conduction velocity abnormalities. The ultrastructural studies of the neuromuscular junction were performed to clarify the anatomic site of the Vacor-induced peripheral neuropathy in male Wistar rats. METHODS: After oral administration of a single dose of Vacor, 80 mg/kg of body weight, to the experimental animals, neuromuscular junctions within the interosseous m

Anatomical variations of the abductor pollicis longus: a pilot study.

BACKGROUND: The abductor pollicis longus (APL) originates from the lateral part of the dorsal surface of the body of the ulna below the insertion of the anconeus muscle, from the interosseous membrane, and from the middle third of the dorsal surface of the body of the radius. However, the number of its accessory bands and their insertion vary considerably. MATERIALS AND METHODS: Fifty upper limbs (2 paired, 31 male, 19 female) were obtained from adult Caucasian cadavers, and fixed in 10% formalin solution before examination. RESULTS: The APL muscle was present in all specimens. The muscles w

Danger zones associated with fibular osteotomy.

Anatomic dissections were performed on five cadaveric lower extremities. Measurements of important neurovascular structures in relation to the fibula were obtained at 1-cm intervals along the length of the fibula with the tip of the fibular head as the reference point. Neurovascular structures adjacent to the fibula in danger of injury with fibular osteotomy were identified. At the proximal one-third of the fibula, the peroneal nerves and their muscular branches are at primary risk. The anterior tibial artery is vulnerable where it penetrates the interosseous membrane and where it runs adjacen

Acute Capitellum Fracture with Distal Radio-ulnar Joint Disruption: An Atypical Variant of an Essex Lopresti Injury - Case Report.

INTRODUCTION: The Essex-Lopresti injury, as defined in the literature, is characterized by a distinct triad: Radial head (RH) fracture, tearing of the interosseous membrane and disruption of the distal radioulnar joint (DRUJ). We present an atypical case of DRUJ disruption resulting from a displaced capitellum fracture rather than an RH fracture. CASE REPORT: A 35-year-old female sustained a distal humerus capitellum fracture with associated DRUJ instability following a high-energy mechanical fall. Surgical management involved open reduction and internal fixation (ORIF) of the capitellum frac

Treatment of sequelae of radial head fractures with a bipolar radial head prosthesis: good outcome after 1-4 years follow-up in 11 patients.

BACKGROUND: In the presence of additional disruption of the distal radioulnar ligaments, the interosseous membrane, or the lateral- and/or medial collateral ligament, radial head fractures treated by resection will result in valgus elbow instability, proximal radial migration and/or posterolateral rotatory instability. Radial head replacement has been used to treat or prevent this. We report our experience with the Judet CRF II radial head prosthesis. PATIENTS AND METHODS: We treated 11 patients with a bipolar radial head prosthesis because of elbow instability after previous treatment for Ma

Central bone grafting for nonunion of fractures of the tibia: a retrospective series.

Nonunion of the tibia associated with bone loss, previous infection, obliteration of the intramedullary canal or located in the distal metaphysis poses a challenge to the surgeon and significant morbidity to patients. We retrospectively reviewed the records of 24 patients who were treated by central bone grafting and compared them to those of 20 who were treated with a traditional posterolateral graft. Central bone grafting entails a lateral approach, anterior to the fibula and interosseous membrane which is used to create a central space filled with cancellous iliac crest autograft. Upon cons

Restoring rotational balance of the paediatric forearm: lessons from post-traumatic malunion.

INTRODUCTION: Post-traumatic malunion of paediatric forearm fractures may lead to impairment of pronation and supination. Treatment decisions are traditionally based on radiographic alignment, although functional limitations do not always correlate with deformity on plain radiographs. Understanding the interaction between bony deformity, soft tissue contracture and growth related remodelling is essential for restoring rotational balance of the forearm.Current concepts:Impaired forearm rotation may result from angular or rotational malalignment, loss of the radial bow or soft tissue contracture

Posttraumatic torsional injury as an indirect cause of fibular intraneural ganglion cysts: case illustrations and potential mechanisms.

Trauma has been loosely associated with the development of fibular intraneural ganglion cysts. Sporadic reports of direct trauma to the proximal lateral leg in the region of the superior tibiofibular joint (STFJ) capsule support the development of a capsular rent and the subsequent egress of joint fluid from the articular joint. This report provides evidence to suggest that indirect trauma from torsion can link concomitant ankle injury and fibular nerve palsy (foot drop) and fibular intraneural ganglion cysts. We present two cases to illustrate different potential mechanisms. One patient susta

Naringin increases osteoprotegerin expression in fibroblasts from periprosthetic membrane by the Wnt/β-catenin signaling pathway.

BACKGROUND: The osteoclast bone resorption is critical in aseptic loosening after joint replacement. The balance between activator of nuclear factor kappa B ligand (RANKL) and osteoprotegerin (OPG) is considered to play a central role in osteoclast maturation. Fibroblasts from the periprosthetic membrane express RANKL and promote osteoclast formation. Studies have demonstrated that naringin inhibited osteoclastogenesis and wear particle-induced osteolysis. In this study, the naringin-induced OPG/RANKL effects and its underlying mechanism were studied in fibroblasts from periprosthetic membrane

Ipsilateral proximal and distal radius fractures without elbow dislocation: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Ipsilateral proximal and distal radius fractures without elbow dislocation are extremely rare injuries and a few cases have been reported. Currently, there is no well-established consensus regarding their optimal management. CASE PRESENTATION: We report the case of a 57-year-old woman who had a right bipolar radius fracture after falling from a standing height. The radiological investigation showed a comminuted distal radius fracture and a fracture of the radial head without elbow dislocation. The patient underwent surgical treatment and followed a rehabilitation
