Evaluation of spinopelvic parameters in lumbar prolapsed intervertebral disc.

BACKGROUND: Sacro-pelvic morphology and orientation are usually described in terms of pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Orientation and morphology of pelvis can affect degenerative changes in the lumbar spine. Thus, here we investigated the relationships between various sagittal spinopelvic parameters and the degree of disc degeneration in young adults. MATERIAL AND METHODS: A hospital-based cross-sectional study with a total of 60 cases was done. Patients presenting with back or leg pain having prolapsed disc on magnetic resonance imaging (MRI) were included in

Developmental mechanisms of intervertebral disc and vertebral column formation.

The vertebral column consists of repeating units of ossified vertebrae that are adjoined by fibrocartilagenous intervertebral discs. These structures form from the embryonic notochord and somitic mesoderm. In humans, congenital malformations of the vertebral column include scoliosis, kyphosis, spina bifida, and Klippel Feil syndrome. In adulthood, a common malady affecting the vertebral column includes disc degeneration and associated back pain. Indeed, recent reports estimate that low back pain is the number one cause of disability worldwide. Our review provides an overview of the molecular m

[Case study of two children with intervertebral disc calcifications].

Disc calcifications are usually revealed in children by pain crises that affect the spine. The most common location for disc calcifications is the cervical spine. However, other locations are less symptomatic and they may well go unnoticed. We here report two cases of children hospitalized with symptomatic cervical disc calcifications. The first case involves a girl aged four and a half suffering from torticollis revealing the migration of a previously asymptomatic disc calcification ; the second case, instead, involves a 5-year old boy presenting for acute neck pain and in whom calcification

Influence of diabetes mellitus on cervical intervertebral disc herniation.

OBJECTIVES: To establish if patients treated surgically for cervical disc disease have an increased incidence of diabetes mellitus. PATIENTS AND METHODS: We prospectively studied 66 consecutive patients who were treated surgically for cervical disc disease and compared them with 100 age-matched patients at the same time period who underwent surgery for other reasons. RESULTS: There was a significantly increased incidence of diabetes mellitus in the first group. This increased incidence was even higher than that in historical controls treated surgically for lumbar disc disease. Short-term pos

Advances in susceptibility genetics of intervertebral degenerative disc disease.

The traditional view that the etiology of lumbar disc herniation is primarily due to age, gender, occupation, smoking and exposure to vehicular vibration dominated much of the last century. Recent research indicates that heredity may be largely responsible for the degeneration as well as herniation of intervertebral discs. Since 1998, genetic influences have been confirmed by the identification of several genes forms associated with disc degeneration. These researches are paving the way for a better understanding of the biologic mechanisms. Now, many researchers unanimously agree that lumbar d

Optimizing nonviral-mediated transfection of human intervertebral disc chondrocytes.

BACKGROUND CONTEXT: The use of viral vectors for transfection of human disc chondrocytes has been well documented. However, because of immunological and cell toxicity concerns, nonviral reagents may provide gene delivery to intervertebral disc (IVD) chondrocytes without these associated obstacles. Several studies have been done using nonviral delivery systems with varying degrees of success. PURPOSE: The purpose of the study was to determine the efficiency, toxicity, and optimal conditions for gene delivery into human degenerative IVD cells via nonviral reagents in vitro. Matrix metalloproteinase-3 production by human degenerated intervertebral disc.

To assess its possible role in the pathophysiology of intervertebral disc degeneration, we investigated the production of matrix metalloproteinase-3 (MMP-3) using human intervertebral disc explant culture. Five normal and 10 degenerated disc specimens were used. The levels of MMP-3 released in the medium were measured with use of an enzyme immunoassay. The results showed that the level of MMP-3 in the degenerated group (0.57 microg/ml/mg wet weight; n = 10) was significantly higher than that of the control group (0.29 microg/ml/mg wet weight; n = 5) (p < 0.05). Immunostaining of MMP-3 revealed

Nonoperative management of herniated cervical intervertebral disc with radiculopathy.

STUDY DESIGN: A longitudinal cohort study design was used. All patients underwent a systematically and uniformly applied treatment program with increasing intervention as further pain control was needed. All patients were followed up by questionnaire evaluating function and symptoms. OBJECTIVES: The role of surgical versus nonsurgical treatment of patients with cervical disc herniation has not been adequately studied. The majority of published data reflects surgical outcomes, with little available data regarding the outcome of nonoperatively treated patients. Frequently, these patients are tr

Biomechanics of the SB Charité lumbar intervertebral disc endoprosthesis.

The SB Charité intervertebral disc prostheses consist of two metal end-plates and an interposed polyethylene slide core. These were subjected to static and dynamic testing in a servohydraulic test rig. When embedded in polypropylene, the prostheses were found to function adequately under a high static load, and under a lighter dynamic load for a long time. When tested in a cadaveric vertebral segment, a large end-plate surface area proved to be of critical importance in preventing collapse of the prosthesis into the vertebral body.

[Sciatic pain and intervertebral disc prolapse after gynecologic operations].

Among 65 patients operated by lumbar disc in 1977 we encountered 5 women after gynecological tumour operations. 4 of those patients not became symptom free. The relation of mechanical factors, obstruction and circulation changes seemed to be of importance for the evaluation of ischialgia in those cases and led to spinal root and root sheath damages.

Prevalence, MRI findings, and clinical features of lumbosacral intervertebral disc protrusion in French Bulldogs diagnosed with acute thoracic or lumbar intervertebral disc extrusion.

INTRODUCTION: Intervertebral disc protrusion (IVDP) is a neurological disorder commonly observed at the lumbosacral junction of old, medium-to-large breeds, non-chondrodystrophic dogs. Although uncommon, lumbosacral IVDP can also be seen in chondrodystrophic dogs, among them French Bulldogs (FBs) and could be associated with congenital vertebral malformations in this breed. This study aims to evaluate the prevalence, clinical features, and MRI characteristics of lumbosacral IVDP and congenital vertebral malformations in FBs diagnosed with thoracic or lumbar intervertebral disc extrusion (IVDE)

The role of sleep and circadian rhythms in intervertebral disc degeneration.

PURPOSE: Low back pain is a globally prevalent disabling condition, with intervertebral disc degeneration (IDD) as a key inducer. Sleep abnormalities and circadian rhythm disorders are widespread, but their bidirectional associations with IDD and underlying pathophysiological mechanisms remain unclear. This narrative review synthesizes existing studies to clarify the mutual regulatory relationship and corresponding intervention strategies. METHODS: A structured literature search was conducted across PubMed, Embase and Web of Science from database inception through November 2025. Peer-reviewed

The role of NOTCH signaling pathway in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is a prevalent degenerative condition and is recognized as the primary cause of low back pain among the elderly population. IDD can be caused by a variety of factors, including aging, inflammatory stimulation, and oxidative stress. These factors collectively contribute to a reduction in nucleus pulposus (NP) cells and an excessive degradation of the extracellular matrix (ECM), thereby facilitating the progression of IDD. The NOTCH signaling pathway is a highly conserved pathway that conducts downstream signals through interactions between cells, thereby r

Neuroimmune Activation in a Goat Model of Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IVDD) initiates a cascade of structural and biological changes that compromise mechanical function, often leading to chronic pain. While small animal models have provided insight into inflammatory and nociceptive mechanisms of IVDD, translational studies require large animal models that more closely replicate human spine anatomy and physiology. This study induced cervical disc degeneration via intradiscal chondroitinase ABC (ChABC) injection in a large animal model and evaluated the associated disc pathology and neuroinflammatory responses across IVDs and with

[Mechanism of extracellular vesicles in the repair of intervertebral disc degeneration].

Extracellular vesicles (EVs), defined as cell-secreted nanoscale vesicles that carry bioactive molecules, have emerged as a promising therapeutic strategy in tumor and tissue regeneration. Their potential in repairing intervertebral disc degeneration (IDD) through multidimensional regulatory mechanisms is a rapidly advancing field of research. This paper provided an overview of the mechanisms of EVs in IDD repair, thoroughly reviewed recent literature on EVs for IDD, domestically and internationally, and summarized their therapeutic mechanisms. In IDD repair, EVs could act through different me

Intervertebral disc impairments in a mouse model of Alzheimer's Disease.

Chronic low back pain, frequently associated with intervertebral disc (IVD) degeneration, is highly prevalent in individuals with Alzheimer's disease (AD), and the pain intensity is highly correlated with the degree of cognitive impairment. While the incidences of both afflictions increase dramatically in the elderly population, it is unknown whether AD exacerbates the health of the IVD. Utilizing one-year-old male and female 5xFAD mice that constitutively express human APP and PSEN1 transgenes with five AD-linked mutations, we measured the lumbar IVD's extracellular matrix composition, the th

An overview of mechanical microenvironment and mechanotransduction in intervertebral disc degeneration.

Cellular mechanotransduction, essential for many biological functions, involves the conversion of mechanical signals into biochemical signals related to cell activities and metabolism. Physical factors in the local cellular microenvironment include external mechanical forces, mechanical stimulation generated by the extracellular matrix and intercellular mechanical interactions mediated through cell-cell adhesions. Intervertebral disc degeneration (IDD) is a complex pathological process involving diverse etiological contributors, such as mechanical wear, oxidative damage and nutritional deficie
