Efficacy of a gastric tube insertion guide for insertion of a nasogastric tube: a prospective, randomized controlled clinical study.

PURPOSE: We studied whether a newly available gastric tube insertion guide (Fuji Medical, Tokyo, Japan) was effective in insertion of a nasogastric tube. METHODS: Sixty anesthetized patients were randomly allocated to three groups, and we attempted to insert a nasogastric tube with a blind technique (Group B), with the lateral neck pressure (Group L), or with the tube guide (Group G). If the insertion failed in Group B or L, its insertion was attempted with the tube guide. Primary outcome measure was the insertion success rate within 300 s. RESULTS: The success rate in Group G (19 of 20 pati

Underlying disease for percutaneous endoscopic gastrostomy tube placement predicts short- and long-term mortality.

BACKGROUND: PEG (percutaneous endoscopic gastrostomy) is a well established endoscopic procedure for enteral feeding. However, patients with a shorter life expectancy will not benefit from PEG tube placement. Furthermore, some specific evolving diseases will never benefit from PEG. The aim of the study focuses on short and long term mortality rates after PEG tube placement in a referral gastroenterology centre (Geneva University Hospital). 219 patients were enrolled in this study. PATIENTS AND METHODS: All patients scheduled for a PEG procedure between January 2011 and December 2014 were incl

Safety and effectiveness of dual guidance with video-laryngoscopy and ultrasound in jejunal tube placement in patients undergoing invasive mechanical ventilation.

BACKGROUND: Appropriate and effective post-pyloric enteral nutrition (EN) is valuable for patients undergoing invasive ventilation, but blind bedside insertion of jejunal tubes is often difficult during mechanical ventilation. Here, we first report on the safety and effectiveness of inserting the jejunal tubes under the dual guidance (DG) of video-laryngoscopy and ultrasound in patients in whom jejunal tube placement during invasive mechanical ventilation is difficult. METHODS: Twenty patients in the emergency department-based intensive care unit (ICU) who were receiving invasive mechanical v

Pharyngo-esophageal complications of Ryle tube insertion in neonates: management and fate.

INTRODUCTION: The following provides clinical reporting of seven neonates with iatrogenic pharyngeal trauma due to forceful untrained use of nasogastric feeding tube. A range of symptoms were observed beginning with excessive frothy secretions culminating in more severe pneumothorax in some. These cases are presented in the context of an exhaustive literature review producing only 50 similar cases worldwide. Special attention is paid toward accurate diagnosis, prognosis, and guidance on most effective modes of treatment. PATIENTS AND METHODS: Using Medtronic flexible nasopharyngolaryngeal end

The nasogastric tube is stuck - and causes coughing and dyspnoea: A rare complication.

For critically ill patients, it is very important that enteral nutrition is established as early as possible. Nasogastric tubes are commonly used for transient enteral nutrition and are generally considered safe to use and handle. However, in unconscious patients special care is needed to detect - possibly life threatening - complications. Here we report the case of a nasogastric tube, which inadvertently looped around the epiglottis. The symptoms and treatment of this rare complication are described and possible measures to prevent this situation are discussed.

Nasointestinal tubes versus nasogastric tubes in the management of small-bowel obstruction: A meta-analysis.

BACKGROUND: There is no consensus regarding the therapeutic effect of nasointestinal tubes (NITs) versus nasogastric tubes (NGTs) in the management of small-bowel obstruction (SBO). This study aimed to compare the clinical outcomes between the use of NITs and NGTs in the management of SBO. METHODS: Published studies on comparing NITs with NGTs in the treatment of SBO were searched from electronic databases. Two investigators independently extracted the data; any discrepancies were adjudicated by a third investigator. Pooled odds ratio (OR) and 95% confidence interval (CI) were calculated usin

How to insert a nasogastric tube and check gastric position at the bedside.

Rationale and key points A nasogastric tube (NG tube) can be used to aspirate stomach contents or to administer feed, medication or fluid into the stomach. ▶ A blind technique is used to insert the NG tube through the nostril, along the nasopharynx, through the oesophagus and into the stomach. ▶ It is important for nurses to be able to recognise problems that may arise when inserting a NG tube blindly, and to know what actions to take if it is suspected that the distal tip of the NG tube is not sitting in the stomach, or they are unable to identify its location. ▶ Misplacement and subsequent u

Medication absorption considerations in patients with postpyloric enteral feeding tubes.

PURPOSE: The gastrointestinal absorption sites of medications administered via postpyloric enteral feeding tubes were examined. SUMMARY: Many issues must be considered when administering medications via the postpyloric route, including interactions with enteral feeds, additional toxicities, and the concern of whether the medication will be absorbed. Despite the potential clinical significance of this information, data regarding the gastrointestinal site of absorption for most medications are lacking. Colonic injury following percutaneous endoscopic-guided gastrostomy insertion.

Percutaneous endoscopic gastrostomy (PEG) is a common practice usually offered to patients who are unable to tolerate or swallow oral feed and require long-term nutrition. We present a case of early pneumoperitoneum after a PEG placement due to colonic perforation. The patient was severely malnourished and had a medical history of brain injury, cerebrovascular accident cerebrovascular accident (CVA) and bilateral below knee amputations from a bomb blast 13 years ago. The PEG tube was placed under sedation. On the first postoperative day, the patient had a subtle pneumoperitoneum that was consi

A case of intramural oesophageal dissection secondary to nasogastric tube insertion.

Intramural oesophageal dissection is a rare disorder, caused by the interposition of a divisive force between the mucosal and muscular layers of the oesophagus, leading to their separation. We present a case of intramural oesophageal dissection, secondary to the accidental iatrogenic intramural insertion of a nasogastric tube. We discuss the aetiologies, presentation, investigation and treatment of intramural oesophageal dissection, and make recommendations on the management of suspected oesophageal perforation with prophylactic nasogastric tube insertion. We also discuss other complications a

The role of endoscopically placed feeding or decompression tubes.

The minimally invasive nature of endoscopically placed gastrostomy tubes makes them a viable consideration in palliative care. Complications related to the procedure appear to correlate with age and underlying comorbidities. However, in many instances, the scientific basis for establishing benefit or harm from tube placement is methodologically inadequate. Decisions must be preceded by a discussion of the value and potential risk of artificial nutrition in a particular setting, respecting the wishes and beliefs of each patient and his or her family. The decision to use PEG placement for any rea

[The Bengmark tube in surgical practice and in the critically ill patient].

Enteral nutrition (EN) is increasingly used to minimize the rate of septic complications related to bacterial translocation, due to its effectiveness and low cost. Bengmark's self-propelling auto-positioning feeding tube (SPT) absorbs and uses gut motility for rapid transport to the upper small intestine, thereby allowing uninterrupted EN both in surgical and critically ill patients. We report on our experience with 175 SPTs applied over the period from December 1996 to February 2000, and analyse the safety, compliance, and indications of SPT in surgical and ICU practice. [Study of the length of the installed gastric tube for children].

The purpose of this study was to find the proper measurement of the length of gastric tube for children preoperatively. The author found that the tube can only reach the cardia of stomach by routine measurement and can not achieve the purpose of gastrointestinal decompression. This study was done to 34 patients. The length of the inserted gastric tube was measured by the distance from the hair border of the middle forehead to the umbilicus. The tube was confirmed to reach the body of stomach during surgery and can reduce gastro-intestinal pressure effectively after operation.

Bedside postpyloric placement of weighted feeding tubes.

A pilot study to evaluate a technique for the bedside placement of small-bore, weighted feeding tubes past the pylorus in critically ill patients is described. The technique involves the use of a #8 or #10 French small-bore, tungsten-weighted feeding tube, specific patient positioning, and defined insertion techniques, including gastric distention with air. The tubes were placed at the bedside in the intensive care unit by the nutrition support team nurse. Successful postpyloric placement was achieved in 85 (83%) of 103 patients. Approximate time for successful insertion was 30 minutes. A foll

Pancreatic polypeptide release by intraluminal fatty acids.

The question of whether the response of pancreatic polypeptide to intestinal fatty acids is influenced by the site of intestinal perfusion or the chain length of the fatty acid was investigated. Six dogs with chronic gastric, pancreatic, and intestinal fistulas were studied. Proximal perfusates were administered at the pylorus and diverted via a Foley catheter in the orad stoma of an intestinal fistula placed 45 cm beyond the pancreatic cannula. Distal perfusates were administered into the caudal stoma of the intestinal cannula. Three experimental protocols were used: proximal fatty acid perfu
