Null IFNAR1 and IFNAR2 alleles are surprisingly common in the Pacific and Arctic.

In this issue of JEM, Bastard et al. (2022. J. Exp. Med.https://doi.org/10.1084/jem.20220028) show that a loss-of-function IFNAR1 allele is common in western Polynesians, while Duncan et al. (2022. J. Exp. Med.https://doi.org/10.1084/jem.20212427) report that a loss-of-function IFNAR2 allele is common in Inuits. Homozygotes lack type I IFN immunity but are selectively vulnerable to influenza, COVID-19 pneumonia, and complications of live-attenuated viral vaccines.

Developmental exposure to legacy environmental contaminants, medial temporal lobe volumes and spatial navigation memory in late adolescents.

Exposure to lead, mercury, and polychlorinated biphenyls (PCBs) has been causally linked to spatial memory deficits and hippocampal changes in animal models. The Inuit community in Northern Canada is exposed to higher concentrations of these contaminants compared to the general population. This study aimed to 1) investigate associations between prenatal and current contaminant exposures and medial temporal brain volumes in Inuit late adolescents; 2) examine the relationship between these brain structures and spatial memory; and 3) assess the mediating role of brain structures in the associatio

Maternal Mortality Among American Indian/Alaska Native Women: A Scoping Review.

Background: Maternal mortality decreased globally by about 38% between 2000 and 2017, yet, it continues to climb in the United States. Gaping disparities exist in U.S. maternal mortality between white (referent group) and minority women. Despite important and appropriate attention to disparities for black women, almost no attention has been given to American Indian/Alaska Native (AI/AN) women. The purpose of this scoping review is to synthesize available literature concerning AI/AN maternal mortality. Methods: Databases were searched using the terms maternal mortality and pregnancy-related dea

Prenatal and 5-year p,p'-DDE exposures are associated with altered sensory processing in school-aged children in Nunavik: a visual evoked potential study.

Due to their geographic location and traditional diet, rich in seafood and marine mammals, the Inuit living in Arctic Quebec are exposed to high amounts of pollutants, including organochlorine pesticides (OCPs). While the adverse developmental effects of these pesticides on child cognitive functions are well known, the effects of developmental exposure to OP on sensory processes have not been investigated. The aim of this longitudinal study was to assess the effects of prenatal and childhood exposure to 1,1,1-trichloro-2,2-bis(p-chlorophenyl)ethane (p,p'-DDT) and its major metabolite 1,1,-dich

Examining Tympanostomy Tube Outcomes in Indigenous vs. Non-Indigenous Children of Northern Quebec.

OBJECTIVES: Chronic otitis media with effusion (COME) and recurrent acute otitis media (rAOM) pose significant burdens on pediatric patients and their families. These conditions disproportionately affect marginalized populations, such as Indigenous communities in Canada, where higher incidence rates and reduced access to care are well documented. This study aims to better understand the sequelae of rAOM/COME following tympanostomy tube (TT) insertion in Indigenous pediatric patients living in northern Quebec. METHODS: A retrospective matched cohort study was conducted on children who underwen

Genetic diversity of hepatitis B virus genotypes B6, D and F among circumpolar indigenous individuals.

Hepatitis B virus (HBV) infection is highly prevalent in circumpolar indigenous peoples. However, the clinical outcome is extremely variable, such that while hepatocellular carcinoma (HCC) is uncommon in Canadian Inuit, the incidence of HCC is slightly higher in Greenlanders than in Danes, and it is especially high in Alaskan Native people infected with HBV genotypes F (HBV/F) and C (HBV/C). These differences may be associated with the genomic variability of the predominant HBV genotype in each group. The purpose of this study was to determine the rate, nature and regional susceptibility of HB

Community-Centred Health Reform in Northern Canada: Workforce and Service Delivery Reforms in Nunavut between 2021 and 2028.

BACKGROUND: Nunavut faces longstanding health system challenges shaped by geographic remoteness, workforce instability, and historical inequities. In response, the territory is advancing a series of reforms that place community knowledge and Inuit priorities at the centre of policy design, with the aims of stabilizing the health workforce, strengthening cultural safety, and expanding access to care through strength-based approaches. REFORM CONTEXT: This analysis examines a set of converging policy initiatives implemented between 2017 and 2028, including the Canada-Nunavut Health Partnership A

Congenital rickets caused by maternal vitamin D deficiency.

The cases of four newborn infants with congenital rickets are reported. All infants were native Canadian: three were Cree and one was Inuit. One had a narrow chest and pulmonary hypoplasia, two had clinical and radiological signs of rickets with craniotabes, thickened wrists, and prominent costochondral junctions, and one had perinatal asphyxia and hydrops. All had hypocalcemia, hypophosphatemia and secondary hyperparathyroidism. Serum 25-hydroxyvitamin D levels were low in three of the infants. The four mothers had evidence of vitamin D deficiency. All infants recovered following treatment wi

The management of ear disease: guidelines for Aboriginal health care programs.

Middle ear disease represents a continuing burden of illness for children of circumpolar regions. Uncertainty in management has been shown to create a barrier to timely and adequate treatment. Consistent intervention strategies were sought for the management of middle ear disease in children of the central Canadian Arctic. The current literature was reviewed using MEDLINE. A consensus document was established in consultation with specialists in community medicine, infectious disease, otolaryngology, pediatrics, and community-based care providers. Definitive recommendations and supportive algor

Lessons from management of syphilis in Nunavut, Canada, 2012-2020.

BACKGROUND: Nunavut, part of Inuit Nunangat, is a geographically vast territory in northern Canada, with a population of over 38,000 people. Most (85%) of the population identify as Inuit. Nunavut has experienced a significant rise in heterosexual infectious syphilis cases since 2012. Management of communicable diseases, including syphilis, is challenging due to high staff turnover and long delays in specimen transport times. Social determinants of health are also an important contributor. The aim of this study is to describe the epidemiology and program elements for infectious syphilis from 2

A pain in the buttock.

Spondylolysis, a fracture of the pars interarticularis, is a common source back pain in children and adolescents. While the incidence is significantly higher in Asian and Inuit populations, it is never seen in nonambulatory children and is most commonly associated with athletic activities that involve extension or rotational deformity about the spine suggesting a functional component. Given that the associated pain is typically insidious in onset, lacks preceding trauma, and is accompanied by muscular spasm, prompt diagnosis requires a high index of suspicion, familiarity with provocative test

The Nutrition Canada Survey: a review. Statement by the Nutrition Committee of the Canadian Paediatric Society.

The data obtained through the Nutrition Canada national nutrition survey must be interpreted cautiously because of limitations in both the design of the survey and our present knowledge of the implications of the findings. The data suggest that, at present, nutritional status is suboptimal among members of the general population. However, because there is no evidence of clinically apparent malnutrition it is difficult to judge the immediate health significance of this finding. Among the Inuit, however, the data suggest that intake of vitamin C may be sufficiently low to approach the level caus

The synergy of therapeutic vaccination and timely diagnosis for TB control in high-burden settings: Nunavut as a case study.

BACKGROUND: Although the overall prevalence of Tuberculosis (TB) in Canada is relatively low, Indigenous communities are disproportionately burdened, with active TB rates exceeding those of Canadian-born non-Indigenous populations by more than 55-fold. In response, the Government of Canada and Inuit Tapiriit Kanatami have launched an initiative to eliminate TB among Inuit by 2030. We evaluated strategies underpinning this initiative, including the potential use of therapeutic TB vaccines that are currently undergoing clinical trials. METHODS: We developed an agent-based model of TB transmissi

Dietary patterns in Greenland and their relationship with type 2 diabetes mellitus and glucose intolerance.

OBJECTIVE: Traditional Inuit dietary patterns have been found to be beneficial for CVD but have not been investigated in relation to glucose intolerance. We examined the association between dietary patterns and type 2 diabetes mellitus (T2DM), impaired glucose tolerance (IGT) and impaired fasting glucose (IFG). DESIGN: Cross-sectional design with a priori derived dietary patterns from an FFQ resulted in five patterns: imported meat (n 196), traditional food (n 601), balanced diet (n 126), unhealthy diet (n 652) and standard diet (n 799). SETTING: Associations between dietary patterns and glu

Environmental contaminants and human health in the Arctic.

Monitoring of human exposure to persistent organic pollutants POPs and heavy metals have taken place since 1994 in a circumpolar study conducted under the Arctic monitoring and assessment programme (AMAP). The study has confirmed that Arctic populations relying on marine food (Inuit) have an exposure level of POPs and methyl mercury related to the amount of traditional food eaten and in some areas at a level where health effects cannot be excluded. Weighing the benefits of traditional food against the risks, AMAP have so far recommended that consumption of traditional food continues. There is

Lymphoepithelial carcinoma of the parotid gland.

Primary lymphoepithelial carcinoma of the salivary glands is an exceptional oncological condition that predominantly emerges within the parotid gland. A significant prevalence of lymphoepithelial carcinoma has been reported in the Inuit population, along with an associated positive serology for Epstein-Barr virus in these endemic regions. In this paper, we present a case of primary lymphoepithelial carcinoma of the parotid gland in a 68-year-old female patient with a history of diabetes and hypertension residing in a non-endemic area. Histology plays a critical role in the definitive diagnosis
