Alternative treatment for varus instability of the hallux interphalangeal joint: A case report.

Forefoot injuries are commonly caused by sport-related activities and are often around the metatarsophalangeal joints. The hallux interphalangeal (IP) joint is anatomically stable; therefore, injuries to this joint are considered to be rare compared with those to the metatarsophalangeal joint. Instability of the hallux IP joint has rarely been reported in barefoot contact sports, and its treatment has not been sufficiently explored. This study investigated chronic varus instability of the hallux IP joint. We performed a surgical reconstruction owing to conservative treatment failure. A good sur

Plantar plate radiofrequency and Weil osteotomy for subtle metatarsophalangeal joint instablity.

BACKGROUND: To the present day, literature has only discussed how to treat extensive plantar plate and collateral ligament lesions, with gross joint subluxation and obvious clinical instability. The treatment options for early stages of the disease with minor injuries and subtle instabilities have not been described. The main purpose of this prospective study is to evaluate the efficacy of the combination of the arthroscopic radiofrequency shrinkage and distal Weil osteotomy in the treatment of subtle metatarsophalangeal joint instability. METHOD: Prospective data (clinical, radiological, and

Lumbar facet joint effusion in MRI: a sign of instability in degenerative spondylolisthesis?

PURPOSE: The term "segmental instability" of the lumbar spine is not clearly defined, especially as it relates to degenerative spondylolisthesis (DS) and rotational translation (RT). We investigated whether facet joint effusion on conventional supine MRI indicated increased abnormal motion in DS and RT. METHODS: 160 patients (119 female, 41 male, mean age 68.8 years, range 38.8-89.3 years) who had undergone decompression only or decompression with instrumented fusion for degenerative spondylolisthesis with different degrees of narrowing of the spinal canal were identified retrospectively from

Instability of the distal radio-ulnar joint: stabilization by a Gore-Tex ligament.

Diagnosis and treatment of problems caused by instability of the distal radio-ulnar joint are complex. In this retrospective study, the results of a uniform procedure by using a Gore-Tex ligament for stabilization of the distal radio-ulnar joint are presented. In eight patients, nine wrists were operated on. The mean age of the patients was 35 years. The dominant side was involved in six patients. The mean follow-up was 3.4 years. Three of nine wrist problems were of spontaneous onset. In these wrists, the final results were excellent. The other six wrists were operated on for problems after t

Increased interleukin-1beta production in the synovium of glenohumeral joints with anterior instability.

Macroscopic synovitis of the glenohumeral joint is frequently seen during arthroscopy in patients with anterior instability. Interleukin-1beta is known to be expressed in inflamed tissue, to correlate with the magnitude of inflammation, and to affect articular cartilage in the joint. We hypothesized that chronic synovitis may occur in the glenohumeral joint in patients with anterior instability. The purpose of this study was to examine the expression of interleukin-1beta in the synovium of the glenohumeral joint with anterior instability and to discuss its clinicopathologic significance. Surgical treatment of chronic lateral instability of the ankle joint. A new procedure.

Sixty patients with chronic lateral functional and mechanical instability of the ankle joint were treated with shortening and reinsertion of the lateral ankle ligaments. All patients were followed prospectively for 2 to 5 years (mean, 3 years 6 months). We found the functional results to be excellent or good in 53 patients (88%). Patients with unsatisfactory results had either generalized joint hypermobility or long-standing ligament insufficiency. Anterior talar translation (ATT) and talar tilt (TT) were measured radiologically on standardized radiographs. Patients with excellent and good fun

[Personal experiences in surgical treatment of chronic instability of the anterior knee joint].

The results achieved with three different operative methods for the treatment of chronic anterior instabilities of the knee joint are presented. In 23 patients Trillat's modification of the O'Donoghue procedure was performed to reconstruct the medial collateral ligaments and the posterior capsule. Thirteen patients underwent reconstruction of the cruciate ligament with a free graft of the patellar ligament according to the Brückner method. In 37 patients the Brückner method was used for cruciate ligament replacement combined with lateral repair according to Ellison, and in some of these patien

Outcomes after anatomic reconstruction of the radioulnar ligaments for distal radioulnar joint instability.

This retrospective study investigated the clinical outcomes of patients treated for chronic distal radioulnar joint instability with open anatomic reconstruction of the palmar and dorsal radioulnar ligaments. After the midpoint of a tendon graft is anchored at the ulnar fovea, the two graft limbs traverse the distal radioulnar joint. One limb is woven into the palmar wrist capsule and the other is secured to the dorsal wrist capsule and retinaculum to stabilize the joint. Of 30 patients (31 wrists) treated with this technique, 19 were followed longitudinally for a mean of 10 years (range 3-21)

Loss of Frzb and Sfrp1 differentially affects joint homeostasis in instability-induced osteoarthritis.

OBJECTIVE: To investigate the specific role of Frizzled-related protein (FRZB) and Secreted frizzled-related protein 1 (SFRP1) in the onset and progression of Osteoarthritis (OA) using Frzb(-/-) and Sfrp1(-/-) mice in the destabilization of medial meniscus model (DMM), a slowly progressing model of OA. Secreted frizzled-related proteins (SFRPs) were identified as secreted Wingless-type (Wnt) antagonists. The Wnt signaling cascade is a major regulator in cartilage development, homeostasis and degeneration. METHODS: The DMM model was surgically induced in eight-week-old male C57/Bl6 Frzb(-/-),

Traumatic dislocation of hip joint following low-velocity trauma, similarities to glenohumeral instability.

Acute posterior hip dislocation following low velocity trauma in a young healthy adult is a rare injury. Almost 30 cases previously have been described in the literature. They are usually associated with prior trauma to the hip joint and will lead to recurrent instability after relocation. This injury is similar to shoulder instability: lesions and anomalies present in the hip joint with posterior instability are identical to the ones in the shoulder with instability. These are Bankart-like lesion of the posterior labrum, defect in ischiofemoral ligament similar to inferior glenohumeral ligame

[Post-traumatic palmar instability of the thumb metacarpophalangeal joint. The "other skier's thumb"].

"Passive" and "active" stabilizers give the thumb metacarpophalangeal (MP) joint the necessary palmar stability. It is possible to temporarily stabilize the joint by means of the flexor muscles in cases where there is an injury to the palmar ligaments due to hyperextension trauma. However, sudden severe or prolonged strain may lead to palmar instability and/or stress-related pain. In an experimental study, the MP joints of 40 specimen thumbs were hyperextended to varying degrees. The extent of tissue damage was evaluated during dissection. Between 1984 and 1992, in 72 cases with acute hyperext

[Arthroscopic semitendinosus-plasty with lateral extra-articular stabilization in anterior knee joint instability].

Within the framework of a prospective study, 40 patients were examined postoperatively 12 months after their anterior cruciate ligament had been replaced by a doubled semitendinosus tendon arthroscopically combined with a lateral extra-articular procedure. "Second look" arthroscopy was possible in four cases. Evaluation was conducted according to the system of Müller and the Lysholm scoring system. According to the Müller system very good or good results were achieved in 97.5%; 95% of the patients achieved 91-100 points in the Lysholm scoring system. No substantial restrictions in the knee joi

Cervical Spine Involvement in Rheumatoid Arthritis: Does Joint Erosion Severity Correlate with Instability?

BACKGROUND/AIMS: Although numerous risk factors associated with cervical spine involvement (CSI) have been reported in the literature, the relationship between the severity of peripheral joint erosions and CSI has not yet been investigated. In this study, we aimed to investigate this relationship. MATERIALS AND METHODS: Adult rheumatoid arthritis (RA) patients with a disease duration of more than 5 years were enrolled from 2 tertiary rheumatology clinics. Medical records were retrospectively reviewed. Neutral, open-mouth anteroposterior, full flexion, and full extension cervical spine radiogr

Patient-specific gait pattern in individuals with patellofemoral instability reduces knee joint loads.

Patellofemoral instability is influenced by morphological factors and associated with compensational alterations in gait pattern. Recent simulation studies investigated the impact of knee morphology on the stability and loading of the patellofemoral joint but neglected the patient-specific gait pattern. The aim of this study was to investigate the impact of patient-specific gait pattern on muscle forces and joint loading in individuals with patellofemoral instability. Musculoskeletal simulations with a model including a twelve degrees of freedom knee joint were performed based on three-dimensi
