Microvascular autologous submandibular gland transfer in severe cases of keratoconjunctivitis sicca.

The objective is to evaluate the technique of microvascular autologous submandibular gland transfer for the treatment of severe keratoconjunctivitis sicca. From August 1999 to April 2002, 38 patients with severe keratoconjunctivitis sicca were treated by autologous submandibular gland transfer to the temporal region of the skull. The related vessels were anastomosed to the superficial temporal artery and vein. When the vein was too small, venous bridging was applied. Prior to cutting off the gland, the facial artery was preserved and infused with heparin in normal saline after the gland had be

Ocular surface lesions due to keratoconjunctivitis sicca: in primary Sjögren's syndrome.

Keratoconjunctivitis sicca is usually part of Sjögren's syndrome (SS) which is systemic disease. Ocular surface lesions are ascribed either to the reduction, or to to the low quality of tears. To address this question, ocular surface lesions were evaluated in 20 SS patients by the rose bengal test (RBT). The results were compared to those obtained in 20 normal volunteers and collated with the quality (estimated by the break-up-time) and the quantity of tears (estimated by the shirmer's I test and the level of lacrimal lactoferrin, LF). The RBT scores did not correlate with the shirmer's test r

Conjunctival surface damage distribution in keratoconjunctivitis sicca. An impression cytology study.

Impression cytology was used to study the ocular surface of 32 eyes with keratoconjunctivitis sicca. Among these eyes, on the basis of the results of the Schirmer I test, two groups of eyes have been identified. The first group with Schirmer test less than 5.5 mm/5 min showed decreased concentration of goblet cells and increased cytoplasm/nucleus ratio on all the areas of the bulbar conjunctiva. The second group, with Schirmer test greater than 5.5 mm/5 min, showed similar changes to be localized in the interpalpebral bulbar conjunctiva only, while the eye surface protected by the lids was alm

Topical cyclosporine inhibits conjunctival epithelial apoptosis in experimental murine keratoconjunctivitis sicca.

PURPOSE: Increased apoptosis in the conjunctival epithelium has been observed in experimental murine keratoconjunctivitis sicca (KCS). Topical cyclosporine (CsA) has been noted to reduce conjunctival epithelial apoptosis in chronic canine and human KCS. The purpose of this study is to determine if topical CsA treatment inhibits conjunctival epithelial apoptosis in a murine model of KCS. METHODS: Dry eye was induced in 3 groups of C57BL6 mice by subcutaneous injection of scopolamine TID and exposure to an air draft and low-humidity environment for 16 hours per day for 12 days. The dry eye cont

Sclerosing peritonitis associated with keratoconjunctivitis sicca, pleurisy, and secretory otitis media.

A male case of sclerosing peritonitis of unknown cause, associated with keratoconjunctivitis sicca, pleurisy, and secretory otitis media, is presented. These unusual complications are very similar to the adverse reactions to the beta-adrenergic blocking agent, practolol, which is known to cause sclerosing peritonitis. This similarity suggests an etiological association between the sclerosing peritonitis of this case and that caused by practolol. The sclerosing peritonitis of this case may be considered to occur as part of some systemic disorder, and thus casts doubt on the hitherto accepted hy

Is the lacrimal gland dispensable? Keratoconjunctivitis sicca after lacrimal gland removal.

A persistent, unilateral keratoconjunctivitis sicca developed immediately after palpebral dacryoadenectomy in a healthy 43-year-old woman with normal eyes and tear function before the operation. The unoperated on fellow eye remained normal during a ten-year period. Eight similar cases are collected from the literature. On the basis of the presented cases, we believe that the main lacrimal gland is an indispensable element of the tear system, and the accessory lacrimal glands are unable normally to prevent tear insufficiency.

The burden of keratoconjunctivitis sicca in Nigeria: a scoping review of prevalence and risk factors.

BACKGROUND: Keratoconjunctivitis sicca, commonly known as Dry Eye Disease (DED), is a multifactorial disorder of the ocular surface characterised by tear film instability, ocular discomfort, and visual disturbance. The burden of DED has increased globally and is associated with several environmental, demographic, and ocular risk factors. Despite its growing public health importance, a comprehensive, nationally representative estimate of the prevalence and risk factors of DED in Nigeria is lacking, with existing evidence characterised by significant heterogeneity in study design, diagnostic cri

Differentially Expressed Gene Pathways in the Conjunctiva of Sjögren Syndrome Keratoconjunctivitis Sicca.

Sjögren syndrome (SS) is an autoimmune condition that targets the salivary and lacrimal glands, with cardinal clinical signs of dry eye (keratoconjunctivitis sicca, KCS) and dry mouth. The conjunctiva of SS patients is often infiltrated by immune cells that participate in the induction and maintenance of local inflammation. The purpose of this study was to investigate immune-related molecular pathways activated in the conjunctiva of SS patients. Female SS patients (n=7) and controls (n=19) completed a series of oral, ocular surface exams. Symptom severity scores were evaluated using validated

A Review of Topical Cyclosporine A Formulations-A Disease-Modifying Agent for Keratoconjunctivitis Sicca.

Keratoconjunctivitis sicca (KCS) is a multifactorial disease characterized by tear hyperosmolarity, inflammation, and ocular surface damage. Cyclosporine A (CsA) is used as an effective disease-modifying agent to improve the signs and symptoms of KCS by reducing inflammation, which interferes with tear production. This review provides an overview of efficacy, safety, and limitations of currently marketed topical CsA formulations-including CsA ophthalmic emulsion, cationic nanoemulsion, and aqueous nanomicelles-and highlights newer technologies for controlled ocular delivery of CsA and their cl

Prognostic factors for the clinical severity of keratoconjunctivitis sicca in patients with Sjogren's syndrome.

AIMS: To determine whether prognostic immunological profiles predict the severity of keratoconjunctivitis sicca (KCS) in patients with Sjögren's syndrome (SS). METHODS: 121 patients diagnosed with SS and followed for at least 1 year were enrolled in this study. Patients were allocated to either a mild KCS group (Mi-KCS; n=65) or to a moderate to severe KCS group (MS-KCS; n=56) based on the Oxford scheme and response to treatment. These groups were each sub-divided into two groups based on the clinical severity of KCS and the presence of associated rheumatic disease (primary SS vs secondary SS

Alginate inserts loaded with epidermal growth factor for the treatment of keratoconjunctivitis sicca.

We developed and tested ocular inserts containing epidermal growth factor (EGF) for a causal treatment of keratoconjunctivitis sicca (KCS). The inserts, consisting of different alginates with hydroxyethylcellulose (HEC) as a lubricant and release modifier, released EGF over time periods ranging from a few hours up to several days. The stability of EGF was high, having a protein half-life of approximately 548 days. A clinical pilot study suggests an amelioration of both the main symptoms and the objective criteria: tear film break-up (BUT) time and lissamine green score. Our results show that E
