The authors report on a case where complete blockade of 21-hydroxylase was discovered in a 40-year-old subject. They emphasize the usefulness of urinary 17-ketosteroid and 17-OH progesterone assays in the diagnosis of pseudo-hermaphroditism. In contrast to major hyperandrogenism, the HY antigen test is negative, since the percentage of fluorescent lymphocytes is even lower than in the normal female population.

A 15-year-old girl with primary amenorrhea, hirsutism, and enlargement of the clitoris was found to have an increased titer of the urinary neutral 17-ketosteroids which failed to decrease after the administration of dexamethasone and an adrenal tumor was demonstrable on venography. Analysis of the adrenal venous effluent revealed gradients on the side of the tumor for testosterone, androstenedione, estradiol, and dehydroepiandrosterone sulfate. Following removal of the tumor, the hirsutism improved markedly, the menses returned, and sexual maturation progressed.

A study is presented of the effect of nisoral on the hypothalamo-pituitary-ovarian system in 25 patients with hyperandrogenism (ovarian in 11, suprarenal in 14). It was established that most patients with oligomenorrhea and anovulation showed a restoration of the menstrual cycle after the 2-3 treatment courses and also absence of progression and reduction of the rate of pathological hair growth. After nisoral treatment the testosterone level decreased while estradiol and progesterone increased, gonadotropins remained unchanged, urinary excretion of 17-ketosteroids reduced.

This is a report of a case of a case of amenorrhoea-galactorrhoea syndrome with hyperprolactinaemia associated with increase of plasma androstenedione and urinary 17-ketosteroids, gradually developed in a sterile subject. Plasma LH and testosterone levels and the results of adrenal suppression and ovarian stimulation tests seem to prove, point towards, the adrenal as the likely source of androgens. Treatment with bromocryptin for 20 weeks brought about a reduction to normal of both PRL and androgens. Resumption of ovulatory cycles followed thereafter and a pregnancy eventually occurred.

Steroid profiles of women suffering from idiopathic hirsutism show in more than 50% of the cases of 10--100 fold increase in the excretion of dehydroepiandrosterone (DHEA) compared with normal values. The excretion of DHEA was reduced much more than that of other 17-ketosteroids if the adrenals (NNR) were suppressed by dexamethasone (DXM). Within one week they reached values at the compound noise level of the gas chromatograms. If the ovaries were stimulated with human chorionic gonadotropin during continued suppression of the NNR with DXM no increase of DHEA could be detected.

BACKGROUND: Congenital adrenal hyperplasia is a group of disorders caused by defects in the adrenal steroidogenic pathways. In its most common form, 21-hydroxylase deficiency, patients develop varying degrees of glucocorticoid and mineralocorticoid deficiency as well as androgen excess. Therapy is guided by monitoring clinical parameters as well as adrenal hormone and metabolite concentrations.

Cytochrome P450IIC5 (rabbit liver 21-hydroxylase) is unusual among hepatic forms of cytochromes P450 because it catalyzes the conversion of one active steroid hormone (progesterone) to another active hormone (deoxycorticosterone). In 36 young males with encephaloophthalmopathy, resultant of thyrotoxicosis, androgen and estrogen concentrations were studied using radioimmunoassay (for the blood determination) and physico-chemical methods (for the urine detection). The blood plasma and urine androgen levels decreased in the patients examined. The total 17-ketosteroid excretion remained unchanged, whereas the fraction ratio was disordered. The blood plasma and urine estrogen content increased. A tendency of normalizing the changed sex hormone levels was observed in patients with encephaloophthalmopathy after recovery.

Virilizing adrenal adenomas are rare, and Leydig cell adenomas of the adrenal rarer still. Our patient, a 60-year-old virilized woman, was found to have a Leydig cell adenoma. Virilization associated with normal 17-ketosteroid and elevated testosterone levels necessitates excluding an adrenal cause. Although one might expect high testosterone levels to suppress gonadotropins, this is not the case. In addition to other studies recommended for assessing the incidentally discovered adrenal mass, we would add a testosterone assay.
