Application of the Endomodel rotating hinge knee prosthesis for knee osteoarthritis.

One hundred thirty-six knees were treated with the Endomodel rotating hinge knee prosthesis as primary total knee arthroplasty (TKA). The indications for surgery included osteoarthritis (110 knees), rheumatoid arthritis (18 knees), and osteonecrosis (8 knees). Patients were divided into four study groups according to follow-up duration. Group A was followed up from 10 to 15 years, group B from 8 to 10 years, group C from 5 to 8 years, and group D from 2 to 5 years. The Hospital for Special Surgery knee score, as well as each parameter individually, showed statistically significant improvement

Total knee replacement for osteoarthritis of the knee with congenital dislocation of the patella.

We carried out three total knee replacements with proximal realignment in two patients with severe osteoarthritis of the knee and congenital dislocation of the patella. During the operation, the femur and the tibia were cut according to the recommendations of the manufacturer of the implant. The femoral component was placed in external rotation and the centre of the tibial component aligned in relation to the tibial tuberosity with regard to rotation and translation. After making the bone cuts, the iliotibial tract was detached from Gerdy's tubercle, the popliteus tendon divided, and the bicep

Vascular malformations in muscles around the knee presenting as knee pain.

Vascular malformations involving the muscles around the knee can be a cause of knee pain. We studied ten such cases. All underwent magnetic resonance (MR) imaging which confirmed the diagnosis. Eight cases underwent wide surgical excision of the lesion, histology of which showed a vascular malformation with predominantly venous element. There was no recurrence in any of the operated cases at the last follow-up. We believe that most of these lesions can be treated successfully by careful preoperative planning aided by MR imaging and a wide-open excision of the lesion.

Total knee arthroplasty for arthritis of the knee with extra-articular deformity.

BACKGROUND: Simultaneous corrective osteotomy of angular deformity and total knee arthroplasty has been considered the treatment of choice for patients with arthritis of the knee associated with ipsilateral extra-articular deformity. However, this procedure is technically demanding, and the functional outcome of the total knee arthroplasty may be jeopardized if the osteotomy fails. This retrospective study was performed to evaluate the clinical results of total knee arthroplasty combined with intra-articular bone resection, without osteotomy, in patients with extra-articular deformity and arth

A locked knee in extension: a complication of a degenerate knee with patella alta.

We present a case of superior dislocation of the patella trapped by interlocked osteophytes. Unlike previous reports, in which the mechanism resulted from a blow to the inferior pole, it is postulated that increased load on the extensor mechanism, combined with patella alta and patellofemoral osteophytes, caused locking of the knee in extension.

Selective ligament release in total knee arthroplasty of the knee in valgus.

An approach to the valgus knee based on anatomic function of ligaments in flexion and extension consistently yields a knee that is balanced in flexion and extension when the implants have been positioned correctly. Two hundred thirty-one knees had a valgus deformity (range, 12 degrees-45 degrees) and were corrected with valgus alignment to 5 degrees by resecting the intact joint surfaces to match implant thickness. Femoral joint surfaces were aligned in 5 degrees valgus to the long axis of the femur and parallel to the epicondylar axis of the femur in flexion and extension. The tibial surfaces

Ceramic unicompartmental knee arthroplasty for spontaneous osteonecrosis of the knee joint.

Unicompartmental replacement with a ceramic knee prosthesis was performed in 10 patients with spontaneous osteonecrosis. The follow-up period averaged 42 months, with a range of 24 to 68 months. All of the knees received a Kyocera unicompartmental knee arthroplasty and all patients were evaluated using The Knee Society Clinical Rating System. The mean knee score increased from 50.6 to 90.8 at the last follow-up, showing no deterioration with time. All of the patients showed marked improvement of pain and excellent or good results were obtained in all cases. None of the patients had progressive

Total knee arthroplasty for corticosteroid associated avascular necrosis of the knee.

Despite their age, patients younger than 50 years who have collapse of their femoral condyles caused by steroid associated avascular necrosis have few options except total knee arthroplasty. There have been no specific reports of the results of total knee replacements for this disease. Between 1980 and 1993, 31 porous coated anatomic total knee replacements were performed in 21 patients younger than 50 years of age with avascular necrosis of the femoral condyles and tibial plateaus. There were 17 women and 4 men, with an average age of 36 years (range, 22-48 years). Seventeen of 21 patients ha

Reactivation of rheumatoid arthritis in knees following total knee replacement.

Between May 1985 and December 1990, 20 patients with rheumatoid arthritis had 34 total knee replacements performed. The mean follow-up period was 4.1 years and the range was 2.3 to 7.8 years. Five knees with total knee replacements (74.7%) in 4 patients were affected by activation of the rheumatoid process at an average postoperative interval of 28.6 months while the other 29 knees with total knee replacements (85.3%) remained in remission. Factors that appeared to have an influence on post total knee replacement rheumatoid activation included the activity of the disease preoperatively, and wh

A planar model of the knee joint to characterize the knee extensor mechanism.

A simple planar static model of the knee joint was developed to calculate effective moment arms for the quadriceps muscle. A pathway for the instantaneous center of rotation was chosen that gives realistic orientations of the femur relative to the tibia. Using the model, nonlinear force and moment equilibrium equations were solved at one degree increments for knee flexion angles from 0 (full extension) to 90 degrees, yielding patellar orientation, patellofemoral contact force and patellar ligament force and direction with respect to both the tibial insertion point and the tibiofemoral contact

Robotic-Assisted Conversion of Unicompartmental Knee Arthroplasty to Total Knee Arthroplasty.

BACKGROUND: Unicompartmental knee arthroplasty (UKA) procedures have become much more common in the United States in recent years, with >40,000 UKAs performed annually1. However, it is estimated that 10% to 40% of UKAs fail and thus require conversion to total knee arthroplasty (TKA)2-5. In the field of total joint arthroplasty, robotic-assisted surgeries have demonstrated advantages such as better accuracy and precision of implant positioning and improved restoration of a neutral mechanical axis6-9. These advantages may be useful in UKA to TKA conversion surgeries, as the use of robotic assis

Floating Knee Arthrodesis After Periprosthetic Knee Infection: A Multi-Center Study.

BACKGROUND: Knee arthrodesis is a means of avoiding above-knee amputation after a periprosthetic joint infection (PJI). The objective of this study was to analyze the results of floating knee arthrodesis in patients who had a history of a periprosthetic knee infection and to perform an external validation of the Baeza-Ortega scale. The analysis consisted of determining reinfection rates, functional results, and the survival of arthrodesis. METHODS: There were 86 patients who had undergone floating knee arthrodesis in cases of PJI who were retrospectively included in the study. The operations

Functional Outcome of Above-Knee Amputation After Infected Total Knee Arthroplasty.

BACKGROUND: Periprosthetic joint infection is a serious complication of total knee arthroplasty (TKA). Management usually involves irrigation and debridement, polyethylene liner exchange, one-stage revision, two-stage revision, knee arthrodesis, or ultimately above-knee amputation (AKA). We present our experience with 21 patients who underwent AKA as a sequela of unresolved infected TKA, highlighting their etiology and functional outcomes. METHODS: A retrospective chart review was performed for all cases of infected TKA treated with AKA at 1 institute from January 2007 to December 2020, with

A national questionnaire survey on knee manipulation following total knee arthroplasty.

BACKGROUND: Adequate range of knee motion is critical for successful total knee arthroplasty. While aggressive physical therapy is an important component, manipulation may be a necessary supplement. There seems to be a lack of consensus with variable practices existing in managing stiff postoperative knees following arthroplasty. Hence we aim to determine the current practice and trend among knee surgeons throughout the United Kingdom. MATERIALS AND METHODS: Postal questionnaires were sent out to 100 knee surgeons registered with British Association of Knee Surgeons, ensuring that the whole o

Primary total knee arthroplasty in infection sequelae about the native knee.

The aim of this study is to assess the results of primary total knee arthroplasty (TKA) after bone or joint sepsis about the native knee and to analyze the risk factors of periprosthetic infection. Sixty-two patients (62 knees), considered to have prior sepsis history to be resolved, underwent primary TKA and were followed during a mean of 6.1years (range, 2-10.4years). Of the 62 patients, periprosthetic infection occurred in 6 after primary TKA (9.7%). Five of 6 patients grew the same organism as prior infection. The number of prior surgeries undertaken for deep infection was found to be an i

Soft-tissue balancing during total knee arthroplasty in the varus knee.

Soft-tissue balancing during total knee arthroplasty is an important step in optimizing the mechanical balance of the knee joint. Soft-tissue contractures that result from varus coronal plane deformity can pose a difficult problem, and the surgeon should have a standard procedure for managing such situations in the operating room. Balance may be assessed intraoperatively with the use of spacer blocks, laminar spreaders, and tensioning devices as well as by placement of trial components. Techniques used to balance the varus knee during primary total knee arthroplasty include femoral component r

Knee joint laxity affects muscle activation patterns in the healthy knee.

This study investigated the effects of anterior knee joint laxity on muscle activation patterns prior to and following a lower extremity perturbation. Participants were subjected to a forward and either internal (IR) or external (ER) rotation perturbation of the trunk and thigh on the weight-bearing shank. Pre-activity (%MVIC) before the perturbation, and reflex time (ms) and mean reflex amplitude (%MVIC) following the perturbation were recorded via surface electromyography (sEMG) in the medial and lateral gastrocnemius, hamstring and quadriceps muscles. Twenty-one NCAA DI intercollegiate fema

Objective knee evaluation using the Genucom Knee Analysis System. Clinical implications.

The Genucom Knee Analysis System was used in a clinical setting to study the clinical practicality of the Genucom instrument, and the degree of correlation between Genucom and clinical knee examinations. The Genucom evaluation performed by an experienced examiner required 25 to 30 minutes for a 14 test bilateral examination. The instrument provided detailed graphical and tabular data of the test results. One hundred "normal knees" were examined in a series of seven Genucom tests. Normal "laxity bands" for both males and females were generated for a range of load applications for the seven test

Evaluation of the Coronal Plane Alignment of the Knee (CPAK) classification in arthritic knees following robotic total knee arthroplasty.

BACKGROUND: The Coronal Plane Alignment of the Knee (CPAK) classification describes knee morphology using arithmetic hip-knee-ankle angle (aHKA) and joint line obliquity (JLO). Currently there is uncertainty about the relevance and restoration of coronal alignment following total knee arthroplasty (TKA), particularly among Asian populations. This study examined changes in coronal alignment following robotic TKA using functional alignment, along with concordance between CPAK JLO and knee joint line obliquity (KJLO) – this would help develop strategies to optimize outcomes of TKA among Asian pat
