Common peroneal nerve palsy complicating knee dislocation and bicruciate ligaments tears.

INTRODUCTION: The occurrence rate of common peroneal nerve (CPN) palsy associated with knee dislocation or bicruciate ligament injury ranges from 10 to 40%. The present study sought first to describe the anatomic lesions encountered and their associated prognoses and second to recommend adequate treatment strategy based on a prospective multicenter observational series of knee ligament trauma cases. MATERIAL AND METHODS: Twelve out of 67 knees treated for dislocation or bicruciate lesion presented associated CPN palsy: two females, 10 males; mean age, 32 years. Four sports injuries, three traf

Total knee arthroplasty in a patient with neglected congenital patellar dislocation.

Late presentation of congenital patellar dislocation with advanced osteoarthritis is rare. This article presents a case of 56-year-old woman with advanced osteoarthritis due to right neglected congenital patellar dislocation treated with total knee arthroplasty (TKA) with release of the lateral retinaculum and proximal extensor mechanism realignment. One year later, the patient had improvement of her Knee Society scores and painless function, stability, and better extensor strength. A literature search revealed a limited number of similar cases in which congenital patellar dislocation was trea

Isolated complete popliteal artery rupture associated with knee dislocation. Case reports.

Complete dislocation of the knee is a rare injury, and is frequently associated with injuries to other structures in the popliteal fossa. Prompt recognition of associated popliteal artery disruption and early revascularization is paramount for successful and functional results. The necessity for emergency vascular reconstruction markedly complicates an already difficult orthopaedic problem. Arteriography is recommended in all cases of complete dislocation of the knee.

Peroneal nerve palsy following knee dislocation: pathoanatomy and implications for treatment.

Peroneal nerve palsy following knee dislocation is a serious problem, and neurolysis at the time of knee reconstruction does not always result in return of peroneal nerve function. We describe peroneal nerve pathoanatomy in three patients in whom late exploration of the peroneal nerve was performed because of ongoing absence of ankle dorsiflexion. We identified frank nerve rupture in two patients and a lengthy neuroma in continuity in one which extended far proximal to the fibular head and well above the previous surgical incision used for peroneal nerve neurolysis at the time of knee reconstr

Complete dislocation of the knee without disruption of both cruciate ligaments.

Complete dislocation of the knee is recognized to be an extremely severe injury, representing one of the few true orthopedic emergencies. There is major trauma to the ligaments, capsule, and tendons, with a high incidence of associated damage to the popliteal artery and peroneal nerve. Resulting disability can be great, reflected by pain, stiffness, or instability of the knee; neurologic deficits in the foot and ankle; and even amputation if the vascular injury is not addressed promptly. Most accounts of knee dislocations in the literature have suggested that disruption of both cruciate ligame

Idiopathic Congenital Dislocation of the Knee: Case Series of Spontaneous Reduction.

INTRODUCTION: Idiopathic congenital dislocation of the knee (CDK) represents a distinct clinical category. Grade III CDK, defined as knee flexion <30° has a poor prognosis. The standard of care is "as soon as possible" manipulation and casting. Waiting for spontaneous reduction is not regarded as an accepted treatment strategy. The purpose of this study is to report on the occurrence of spontaneous reduction in idiopathic grade III CDK. MATERIALS AND METHODS: This prospective cohort study included consecutive cases of idiopathic CDK treated from April 2020 to August 2025. Inclusion criteria w

Tibial shaft fracture with knee and proximal tibiofibular dislocation: Case report.

Tibial shaft fractures are the most common type of long bone fracture, frequently associated with high-energy traumas such as traffic accidents, which carry a high mortality rate in Colombia. The combination of an open tibial shaft fracture with knee and proximal tibiofibular joint dislocation is extremely rare. The presented case involves these injuries, which were managed surgically in a single-stage with a follow-up of six months and good functional outcomes. Intramedullary nailing of the tibia was performed using an extra-articular, extra-synovial, semi-extended technique, which optimized

Spontaneous knee dislocation: a rare and dreadful complication of septic arthritis.

Spontaneous knee dislocation without a history of trauma is a rare entity to witness. Herein, we report a case of a patient who presented to the emergency department (ED) with a history of fever, chills and vomiting associated with progressive right knee swelling, pain and impaired range of motion (ROM). Physical exam of her right knee showed symmetrical swelling with diffuse tenderness and limited ROM due to pain. Joint aspirate and full septic workup confirmed the diagnosis of septic arthritis. Following her management and two events of irrigation and debridement of the septic knee, the pati

Bilateral Congenital Knee Dislocation in Colombia: Case Report and Literature Review.

Congenital knee dislocation (CKD) is a rare disease with an estimated incidence of 1 per 100,000 live births, characterized by a rare musculoskeletal malformation in genu recurvatum deformity present at birth, affecting one or both lower limbs. The diagnosis may be suspected during ultrasound when observing that the situation of the extremities is not correct, and is confirmed by physical examination at birth, with plain radiography being helpful to establish the degree of severity. At present there are controversies regarding treatment and there is no definitive protocol. We present a new cas

Congenital Knee Dislocation at Birth - An Extraordinary Case of Spontaneous Reduction.

Congenital knee dislocation (CKD) is a rare malformation characterized by hyperextension deformity of the knee with anterior tibia displacement, present at birth. Rarely reported, CKD might occur as an isolated deformity or commonly associated with musculoskeletal abnormalities, with the most common ones being developmental dysplasia of the hip (DDH) and clubfoot. The etiology is unknown, but CKD has been associated with certain intrinsic and extrinsic factors. Treatment with conservative methods at an early stage is most likely to yield successful results. We report here a rare case of succes

Surgical management of posterior knee dislocation associated with extensor apparatus rupture.

OBJECTIVE: The purpose of this study was to report the clinical and functional results of patients who underwent surgical management for posterior knee dislocation associated with extensor apparatus rupture. INTRODUCTION: Posterior knee dislocations associated with extensor apparatus ruptures are defined as rare but complicated injuries, which are difficult to return to the level of activity prior to the injury. The study demonstrated a hypothesis that good knee stability and early gain of range of motion could be achieved with deliberate design of the treatment protocol and proper applicatio

Neglected surgically intervened bilateral congenital dislocation of knee in an adolescent.

Neglected bilateral congenital dislocation of knee is unusual. A 12 year old boy presented with inability to walk due to buckling of the knee. The symptoms were present since the child learnt walking. He preferred not to walk. Bilateral supracondylar femoral osteotomy was done at the age of 6 years. Patient had a fixed flexion deformity of both knees, 30° in the right (range of flexion from 30° to 45°) and 45° fixed flexion deformity in left knee respectively (range of flexion from 45° to 65°) when presented to us. The radiological examination revealed bilateral congenital dislocation of knee

Surgical technique: articulated external fixator for treatment of complex knee dislocation.

BACKGROUND: Knee dislocation is a severe but relatively uncommon injury caused by violent trauma that can result in long-term complications, such as arthrofibrosis, stiffness, instability, and pain. Perhaps owing in part to its rarity, treatment of this injury is controversial. We therefore describe a treatment approach for these complex cases involving a novel dynamic knee external fixator. DESCRIPTION OF TECHNIQUE: We performed open PCL reconstruction when possible and/or repair of other associated lesions. At the end of the surgical procedure, the surgeon applied an external fixator that r

Non-reducible knee dislocation with interposition of the vastus medialis muscle.

Irreducibility of the knee following complete dislocation is a rare event determined by the interposition of various capsulo-ligamentous structures in the joint space. Such cases often require urgent surgical treatment. We report the case of a healthy 70-year-old man with a sprain of the left knee that occurred after a sports trauma. The patient showed knee dislocation with multiple ligamentous injuries and articular block due to interposition of a portion of the vastus medialis muscle. After arthroscopic evaluation, we performed surgical treatment to free the muscle, regularize the medial men

Vascular injury associated with blunt trauma without dislocation of the knee.

Failure to recognize popliteal artery injury and restore vessel continuity of flow after blunt trauma is a major cause of lower extremity amputation and morbidity. A high index of suspicion and early recognition of the injury are paramount for limb salvage. We experienced a rare case of poplitial artery occlusion with the presence of arterial pulses due to collateral circulation after blunt trauma. Expeditious revascularization was achieved by using posterior approach, allowing two surgical teams to work simultaneously. This case illustrates that, even in the absence of knee dislocation, surge

Fulkerson procedure for chronic patella component dislocation after total knee arthroplasty.

Chronic patella component dislocation after primary knee arthroplasty can be a challenging problem. The purpose of this case series was to review our results of a chronic patella dislocation after total knee arthroplasty treated with a Fulkerson procedure (anteromedialization of the tibial tubercle). Five patients with an average age of 68.8 years (range, 60-76 years) underwent a Fulkerson procedure to restore extensor mechanism alignment. The tibial tubercle osteotomy was secured with lag screws with an average medialization of approximately 1.5 cm. The average preoperative Knee Society score

Traumatic anterior knee dislocation and tibial shaft fracture: a case report.

We report a case of traumatic anterior dislocation of the left knee with an ipsilateral tibial shaft fracture in association with popliteal artery and common peroneal nerve injuries. To our knowledge, such a combination of injuries has not been reported before. All components of this injury were recognised and treated promptly with rehabilitation commencing early, resulting in a good functional outcome. We discuss the possible injury mechanism and management of this unusual case.

[Atraumatic dislocation in mobile-bearing total knee arthroplasty: two case reports].

INTRODUCTION: Atraumatic dislocation following total knee arthroplasty (TKA) is a rare condition. Severe complications after dislocation are lesion of the vascular-nerve bundle, compartment syndrome or amputation. The benefit of TKA with mobile-bearing are an improvement of the range of motion and better articulation. In comparison to fixed-bearing TKA there is the risk of dislocation or breakage of the polyethylene insert. PATIENTS AND METHOD: We present two cases with dislocation following TKA with mobile-bearing. In both cases preoperatively there was a significant weakening of the functio

Noncontact knee dislocation in a female basketball player: a case report.

Knee ligamentous injuries occur during sport, and when there is extensive injury, they can be associated with subluxation or dislocation. We present the case of a female basketball player who sustained a knee dislocation during noncontact play. An immediate angiogram confirmed vascular integrity, and surgical treatment with ligamentous reconstruction was performed. Peroneal nerve injury was present but resolved in 42 weeks. A review of the literature discussing knee dislocation is presented.
