Vertigo as One of the Symptoms of Lyme Disease.

OBJECTIVES: The aim of the study was to evaluate the frequency of vertigo symptoms and potential labyrinth damage in patients with diagnosed Lyme disease (LD). LD can affect the vestibulocochlear nerve, leading to hearing loss and vertigo/dizziness. MATERIAL AND METHODS: The study included a group of 38 patients between the ages of 20 and 77, who were hospitalized due to vertigo/dizziness between 2018 and 2019. All of the patients underwent a detailed medical interview and an otolaryngological and neurological examination, including video electronystagmography (VENG), in addition to audiologi

Tinnitus suppression by electrical promontory stimulation in sensorineural deaf patients.

In 62 patients, whose hearing acuity was normal or profoundly impaired, tinnitus suppression was attempted through electrical promontory stimulation (EPS) with a Promontory Stimulator (Cochlear Co.). The causes of tinnitus were idiopathic sudden deafness, ototoxicity, noise induced hearing loss, labyrinthitis, Meniere's disease, acoustic neuroma and unknown origin. Reduction of tinnitus was achieved in 46 of 68 ears (67.6%). Of these, EPS was most effective in cases of noise induced hearing loss (100.0%; 3/3), followed by those of idiopathic sudden deafness (87.5%; 14/16), Meniere's disease (8

Prevalence of Vestibulocochlear Diseases in 4,825 Patients.

A study of the prevalence of vestibulocochlear disorders in 4,825 patients showed that these disorders occurred during middle age and mostly affected women. The most frequent pathologies are vascular labyrinthitis, emotional disturbance, and the cervical syndrome. The study also demonstrated that each disease has a preferential age group, which varies according to the gender.

MRI of the inner ear.

The recent advances in MRI of the temporal bone brought about by the availability of intravenous contrast agents sensitive to the disruption of blood-brain barrier have further expanded the role of MRI in the evaluation of patients with vestibulocochlear symptoms. Not only can MRI be used to diagnose vestibular schwannomas (and other intracanalicular and cerebellopontine angle lesions mimicking it) with a high degree of accuracy, but it can now identify a variety of inflammatory and neoplastic processes of the membranous labyrinth which, in the past, were only made presumptively and occasional

Novel role for the Golgi membrane protein TMEM165 in control of migration and invasion for breast carcinoma.

In various patterns of sensorineural hearing loss including Ménière's disease, which may show improvement in auditory function following immunosuppressive therapy, an isolated autoimmune disease of the inner ear has been postulated. Because of the lack of well-defined diagnostic criteria to identify autoimmune processes within the inner ear and the fact that the human inner ear is one of the few organs of the body not amenable to diagnostic biopsy, there has been great interest in developing animal models that mimic these clinical entities. Previous studies have found evidence that this proces

Garnett Passe and Rodney Williams Memorial Lecture: New clinical tests of unilateral vestibular dysfunction.

Vertigo is caused by unilateral vestibular dysfunction. Unilateral vestibular dysfunction represents either vestibular overactivity as benign positional vertigo or underactivity as in labyrinthitis or both, at different times, as in a Ménière's attack. Unilateral dysfunction could also be central rather than peripheral, such as in a lateral medullary syndrome. Unilateral vestibular dysfunction could affect any of the five different sensory areas in the labyrinth, the three semicircular canals and the two otoliths, or their brainstem connections. For rigorous diagnosis of the cause of vertigo,

Peripheral vestibular disorder of cervical origin.

The provoking mechanism of peripheral vestibular disorder in the cervicovestibular syndrome is discussed on the basis of the results of perivascular sympathectomy of the vertebral artery. The peripheral type of this syndrome can be differentiated from the central type in that it shows fatigue due to positioning nystagmus and the reversion phenomenon in the self-recording cupulogram. 42 cases of the peripheral type could be completely cured of episodes of vertigo by the unilateral operation, but the other 6 cases required bilateral operations to prevent such episodes.

Labyrinthitis With Negative MRI As a Precursor to Rapidly Developing Primary CNS Lymphoma of the Cerebellopontine Angle.

BACKGROUND: Few case reports have described primary central nervous system lymphoma (PCNSL) presenting as a cerebellopontine angle (CPA) lesion in HIV-positive patients. We describe a rare presentation of rapidly progressing PCNSL of the CPA/internal auditory canal (IAC) as labyrinthitis with initial negative MRI in an HIV-positive patient. CASE: A 58-year-old male with well-controlled HIV presented with sudden left sensorineural hearing loss, tinnitus, and imbalance. Vestibular testing suggested an uncompensated left peripheral vestibular weakness. MRI demonstrated facial and cochleovestibul

Fully automated segmentation in temporal bone CT with neural network: a preliminary assessment study.

BACKGROUND: Segmentation of important structures in temporal bone CT is the basis of image-guided otologic surgery. Manual segmentation of temporal bone CT is time- consuming and laborious. We assessed the feasibility and generalization ability of a proposed deep learning model for automated segmentation of critical structures in temporal bone CT scans. METHODS: Thirty-nine temporal bone CT volumes including 58 ears were divided into normal (n = 20) and abnormal groups (n = 38). Ossicular chain disruption (n = 10), facial nerve covering vestibular window (n = 10), and Mondini dysplasia (n = 1

Audiovestibular disorders as autoimmune reaction in patients with melanoma.

Melanoma is an aggressive form of cancer derived from neuroectodermal melanocytes. Melanocytes are present in the skin and hair follicles, as well as in the eye (iris and choroids), the leptomeninges, the anal canal and the inner ear. In the inner ear melanocytes are found both in the intermediate layer of the stria vascularis of the cochlea and in the dark cells of the vestibular organs. They are believed to play an important role in the production of endolymphatic potentials and in the maintenance of normal volumes of the inner ear fluids. Recently, audiovestibular dysfunctions have been dem

[Labyrinthectomy with preservation of the cochlear duct and a reconstruction of the lateral wall in the vestibule].

The authors described a technique and presented the results of the modified labyrinthectomy in 9 cases. A labyrinthectomy was performed during the removal of neuromas by translabirynthine approach, in advanced unilateral Meniere disease, in a huge perylimphatic fistula and in a case of labyrinthitis latens. In 3 cases of the neuromas removal the residual hearing was preserved. In remained cases of labyrinthectomy hearing was preserved partially.

Do measures of gain asymmetry and catch-up saccades improve video head impulse test agreement with caloric results?

OBJECTIVE: To investigate the relative sensitivity and agreement of caloric testing and video head impulse test (vHIT) across four groups of vestibular disorders. METHODS: Caloric and vHIT results of 118 patients with either Ménière's disease, vestibular neuritis/labyrinthitis, vestibular migraine, or vestibular schwannoma were retrospectively analyzed. vHIT gain, gain asymmetry, and catch-up-saccades (≥100°/sec) were compared with reference limits of 91 controls. RESULTS: Abnormal caloric results and vHIT gain were recorded in 57.6 % and 33.1 % of patients, respectively. Consideration of al

Staging Meniere's disease and reporting results in Meniere's disease: symposium panel discussion.

At its recent spring meeting, the American Neurotologic Society devoted a portion of its program to a symposium on staging Meniere's disease and reporting results. One of the problems discussed, was which preoperative audiogram should be used for reporting results of any treatment. I think that it is very important that we, as otologists, try to look at Meniere's disease or any inner ear dysfunction in a more scientific way. Some of you may feel, on initial evaluation of this material, that it is too cumbersome to be practical in your daily clinical work. However, we feel we have put forth a r

A case of neurosarcoidosis with labyrinthine involvement.

Sarcoidosis is a chronic granulomatous disease of unknown aetiology, which may involve any organ system. It most commonly occurs in adults with childhood involvement being rare. Central nervous system involvement is seen in up to 25% and typically involves meningeal disease resulting in multiple cranial neuropathies. Other common clinical findings include seizures, headache, dementia, and pituitary dysfunction. Imaging plays a central role in the diagnosis with typical findings including pachymeningeal and leptomeningeal enhancing lesions. Other imaging findings include lacunar and major terri
