Lidocaine versus diphenhydramine for anesthesia in the repair of minor lacerations.

This prospective study compared the effectiveness of 1% diphenhydramine with 1% lidocaine for local anesthesia in repair of minor skin lacerations in adults with simple linear lacerations treated at our Emergency Department. Wounds were anesthetized with either lidocaine or diphenhydramine according to a random numbers table. Ninety-nine patients were included in the final analysis (51 received lidocaine; 48 diphenhydramine). Patients and physicians rated the pain of injection and suturing using a standard visual analog scale. Univariate and multivariate linear models were used on rank transfo

Bilateral Open Achilles Tendon Complete Lacerations Following Interpersonal Violence: A Case Report.

Introduction: Interpersonal violence victims account for a sizable proportion of trauma cases encountered in South African trauma centers. Open Achilles tendon injuries are more prevalent in male patients, accounting for a high number of all cases. Case Report: A 45-year-old male was brought in a trauma casualty by a paramedic after being referred by a local clinic in severe pain on both ankles, unable to walk or stand, and dressed bilaterally with blood stains. On examination, he demonstrated the triad of Achilles tendon injuries, including Matle's sign, Thompson's test, and a gap on palpatio

Epidemiology, Incidence of Infection, and Antibiotic Use in Pediatric Traumatic Ear Lacerations.

BACKGROUND: The management and use of antibiotics in pediatric ear lacerations lacks standardization. This study sought to provide a comprehensive review of these injuries and assess the impact of administering perioperative antibiotics on the incidence of infections after repair. METHODS: Retrospective review of all pediatric ear lacerations treated at our institution's emergency department (ED) between 1998 and 2024. RESULTS: A total of 246 patients (177 boys) were included. Patients aged 0-6 years were most affected (n = 117, 47.6%), with the peak between 2-5 years old (n = 78, 31.7%). Mo

The Puck Stops Here: Head and Neck Ice Hockey Lacerations in Adults.

OBJECTIVE: The goal of this study was to describe lacerations of the head and neck sustained among ice hockey players in the US. METHODS: Data on adult (20-65 years) ice hockey injuries were collected from the 2003-2022 National Electronic Injury Surveillance System (NEISS) database. Injuries of the head, neck, face, mouth, and ears were included, while those sustained as a spectator, coach, or on a non-ice surface were excluded. Frequencies and means were calculated. A one-way ANOVA and chi-squared tests were performed, with p < 0.05 indicating statistical significance. RESULTS: Five hundre

Canalicular lacerations in a tertiary eye hospital: our experience with monocanalicular stents.

Introduction: Canalicular injury is commonly encountered in lid trauma. A multitude of techniques and stents are available to manage canalicular lacerations. Monocanalicular stents offer a simple, technically easy and cost-effective solution for managing such cases. Objective: This is a retrospective review of the patients presenting with canalicular lacerations to a tertiary eye hospital from January 2014 to September 2017. We evaluated factors like time of surgery, cause of injury, time of stent removal and their association with the surgical outcome. Additionally, we also reviewed the curre

Practical Management of Common Skin Injuries, Lacerations, Wounds, Trigger Fingers, and Burns.

Primary care clinicians encounter many conditions during their day-to-day visits with patients. A few of these common presentations include burns, lacerations, trauma to the hand, and wounds, some of which do not require an evaluation by a specialist and can be managed outpatient by primary care clinicians. In this article, we share evidence-based tips to avoid common pitfalls in primary care recognition and management of such presentations as well as guide them to manage many of these conditions themselves. We also provide guidance in the decision to refer the patient to a plastic surgeon or

Virtual monoenergetic dual-energy CT for evaluation of hepatic and splenic lacerations.

PURPOSE: To evaluate the utility of virtual monoenergetic imaging in assessing hepatic and splenic lacerations and to determine the optimal energy level to maximize injury contrast-to-noise ratio. METHODS: We retrospectively examined 49 contrast-enhanced abdominal CT studies performed on a dual-source dual-energy CT (DECT) scanner with reported liver and/or splenic lacerations. All studies included portal venous phase imaging acquired simultaneously at low (80 or 100 kVp) and high (140 kVp with tin filtration) energy levels. Conventional 120 kVp-equivalent images were generated for routine re

Accuracy of High-Resolution Ultrasonography in the Detection of Extensor Tendon Lacerations.

BACKGROUND: Lacerations to the extensor mechanism are usually diagnosed clinically. Ultrasound (US) has been a growing diagnostic tool for tendon injuries since the 1990s. To date, there has been no publication establishing the accuracy and reliability of US in the evaluation of extensor mechanism lacerations in the hand. The purpose of this study is to determine the accuracy of US to detect extensor tendon injuries in the hand. METHODS: Sixteen fingers and 4 thumbs in 4 fresh-frozen and thawed cadaveric hands were used. Sixty-eight 0.5-cm transverse skin lacerations were created. Twenty-seve

Use of silicone tubes to repair canalicular lacerations via a novel method.

PURPOSE: To develop a novel method to repair canalicular lacerations using silicone tubes. METHODS: A total of 47 adult patients (47 eyes) with canalicular lacerations were collected from the outpatient department from November 2010 to December 2012. The age ranged from 16 to 53 years. Among the 47 eyes, 37 had lower canalicular lacerations, 6 had upper canalicular lacerations, and 4 had bicanalicular lacerations. A soft probe was made using a stainless steel acupuncture needle, which was inserted into the lumen of the proximal part of the catheter to increase its rigidity. The probe was then

Extensor tendon lacerations from arthroscopic excision of dorsal wrist ganglion: case report.

Arthroscopy is an accepted technique for the resection of wrist ganglions. The reported complication rate is comparable with open resection at 2%; however, this rate may be underestimated. Most reported complications are relatively benign and self-limited. In this case report, we detail lacerations of multiple digital extensor tendons from arthroscopic resection of a dorsal ganglion and describe our management of this complication.

The use of topical anaesthesia at children's minor lacerations: an experimental study.

BACKGROUND: In a great many situations within health care and treatment, children are subjected to unnecessary pain and suffering. When local anaesthetics is to be administered the child can experience this as incomprehensible especially when the nursing staff assures the child that no pain would be felt, only to discover soon after, that it actually did hurt at the moment of anaesthetic infiltration. The soothing of pain during the suturing of wounds in emergency wards can be reduced, ensuring that unnecessary pain in the cafe-and-treatment process is mot meted out to children. In order to pr

Closure of lacerations and incisions with octylcyanoacrylate: a multicenter randomized controlled trial.

BACKGROUND: Most lacerations and surgical incisions are closed with sutures or staples. Octylcyanoacrylate tissue adhesive (OCA) was recently approved for use in the United States. We compared the cosmetic appearance of lacerations and incisions repaired with OCA versus standard wound closure methods (SWC). METHODS: A multicenter randomized clinical trial including patients with simple lacerations or surgical incisions was conducted at 10 clinical sites. Patients were randomly assigned to treatment with OCA or SWC. Follow-up was performed at 1 week and at 3 months to determine infection rates

[Posterior laceration of the abdominal aorta on vertebral fracture].

Abdominal aortic lacerations are very rare in closed trauma and are usually caused by high energy trauma. Aortic injuries should be assessed as a priority as they present an immediate life-threatening risk in the event of major haemorrhage. However, the clinical presentations are highly variable and often misleading. We -present a case of traumatic blunt abdominal aortic laceration following a fall down the stairs. The laceration involved the posterior part of the abdominal aorta secondary to a lumbar fracture with L1-L2 disc tear.
