Chronic Environmental and Occupational Lead Exposure and Kidney Function among African Americans: Dallas Lead Project II.

Background: We examined the effects of lead on kidney function in occupationally and environmentally exposed adults from a Dallas lead smelter community that was the site of an Environmental Protection Agency (EPA) Superfund clean-up. All subjects were African Americans-a racial group that bears a disproportionate burden of kidney disease. Methods: A two-phase health screening was conducted. Phase II included a physical examination and laboratory tests. Study subjects were African Americans residents, aged ≥19 years to ≤89 years. Of 778 subjects, 726 were environmentally exposed and 52 were bo

Soil Lead and Children's Blood Lead Disparities in Pre- and Post-Hurricane Katrina New Orleans (USA).

This study appraises New Orleans soil lead and children's lead exposure before and ten years after Hurricane Katrina flooded the city. Introduction: Early childhood exposure to lead is associated with lifelong and multiple health, learning, and behavioral disorders. Lead exposure is an important factor hindering the long-term resilience and sustainability of communities. Lead exposure disproportionately affects low socioeconomic status of communities. No safe lead exposure is known and the common intervention is not effective. An essential responsibility of health practitioners is to develop a

Abdominal pain due to lead-contaminated opium: a new source of inorganic lead poisoning in Iran.

Although the incidence of occupational and adult lead poisoning has declined, the problem still exists. We encountered three patients with lead poisoning in Iran, all of whom associated with presented with diffuse abdominal pain, which was at times colicky in nature, anemia, constipation, nausea, vomiting, and slightly abnormal liver biochemistries. A history of opium ingestion was present in each of these patients. None of the patients reported known occupational exposure to toxins. Diagnoses of lead poisoning were confirmed through the detection of elevated blood lead levels. The cause of le

Reuse of occluded veins during permanent pacemaker lead extraction: a new indication for femoral lead extraction.

OBJECTIVES: This study examined the utility of a novel technique for reuse of thrombosed veins when extracting permanent pacemaker leads via a femoral vein approach. BACKGROUND: Although lead extraction permanent pacemaker using a femoral approach has advantages over the subclavian approach, it cannot be used to provide access for a new lead using currently employed techniques. This is important because up to 23% of patients have occluded veins after permanent pacemaker implantation. METHODS: The pacemaker lead to be extracted was released from the generator and retaining sutures at the impl

Acute elevation of blood lead levels within hours of ingestion of large quantities of lead shot.

BACKGROUND: Ingestion of elemental lead foreign bodies is felt to have a low risk of clinically significant lead absorption unless gastrointestinal pathology and/or prolonged transit time are present. We present a case of ingestion of a large quantity of small diameter lead shot accompanied by rapid elevation of blood lead levels. CASE REPORT: A 5 1/2-year-old previously healthy girl was found eating the pellets from an ankle weight. She vomited and complained of abdominal pain. In the emergency department, she had no complaints and normal vital signs. An abdominal X-ray showed thousands of s

Clinical surveillance of an active fixation, bipolar, polyurethane insulated pacing lead, Part II: The ventricular lead.

Since 1989, 72 Telectronics 330-201 active fixation, polyurethane insulated ventricular leads (Accufix) have been implanted at the Mayo Clinic. There were four (5.6%) acute lead related complications (perforation, microdislodgment, and macrodislodgment), three of which led to early reoperation. Over a follow-up time of up to 2.7 years (median 9.4 months), there were six (8.3%) chronic lead related complications but no failures of lead material. Most of these complications developed during the first month, and half of them were transient, with documented improvement later. Two patients (2.8%) r

Inhibition of the pentose phosphate shunt by lead: a potential mechanism for hemolysis in lead poisoning.

Recent investigations have disclosed a decrease in pentose phosphate shunt activity in hereditary pyrimidine 5'-nucleotidase deficiency. Clinical lead poisoning is associated with an acquired decrease in pyrimidine 5'-nucleotidase activity. The current investigations were undertaken (1) to determine if pentose shunt activity was decreased in erythrocytes exposed to lead, and (2) to compare the mechanism of inhibition to that seen in hereditary pyrimidine 5'-nucleotidase deficiency. Normal erythrocytes incubated with lead acetate in vitro demonstrated increased Heinz body formation, decreased r

Occupational lead poisoning in the United States: clinical and biochemical findings related to blood lead levels.

Dose-response relationships between blood lead levels and toxic effects have been evaluated in 160 lead workers in two smelters and a chemicals plant. Blood lead levels ranged from 0.77 to 13.51 mumol/litre (16-280 microgram/dl). Clinical evidence of toxic exposure was found in 70 workers (44%), including colic in 33, wrist or ankle extensor muscle weakness in 12, anaemia (Hgb less than 8.69 mumol/litre (Hb/4) or 14.0 gm/dl) in 27, elevated blood urea nitrogen (greater than or equal to 7.14 mmol/litre or 20 mg/dl) in 28, and possible encephalopathy in two. No toxicity was detected at blood lea

Anti-aliasing-enhanced WaveUNet for clinically reliable 12-lead ECG reconstruction from limited 3-lead input.

Standard 12-lead electrocardiography (ECG) remains the clinical gold standard for diagnosing cardiac disorders, particularly myocardial infarction (MI). However, electrode-count constraints in portable and wearable ECG systems make simultaneous acquisition of all 12 leads impractical, thereby challenging the preservation of diagnostic fidelity. This study aims to reconstruct physiologically coherent and clinically meaningful 12-lead ECGs from a limited 3-lead input configuration. To this end, we developed a WaveUNet-based encoder-decoder architecture and systematically extended it with alterna

Quantitative spatiotemporal evolution of deposited lead sources in Tibetan Plateau glaciers: Insights from lead isotopic reanalysis.

Lead (Pb), owing to its high toxicity and low-level environmental exposure, is internationally regulated. Nevertheless, anthropogenic Pb signals have been detected in remote glaciers of the Tibetan Plateau (TP). In this review, we compile available Pb isotope data (n = 439) from multiple glacial archives (ice cores, snowpits, cryoconite, and precipitation) and employ the concentration-dependent MixSIAR isotope mixing model to quantitatively reconstruct the spatiotemporal evolution of Pb sources in TP glaciers. First, the multiple media distribution and spatiotemporal patterns of Pb isotopes ar

Childhood Lead Exposure and the Risk of Dental Caries in Permanent Teeth: The Cincinnati Lead Study.

INTRODUCTION: Dental caries is a significant public health problem in the United States. The evidence on the association between lead exposure and dental caries is conflicting because the impact of childhood lead exposure on the prevalence of caries in young adults has not been evaluated in prospective cohort studies. OBJECTIVE: To assess the association of prenatal and postnatal childhood lead exposure with dental caries in young adults living in Cincinnati, Ohio, United States. METHODS: We examined 206 predominately African American participants aged 16 to 22 y in the Cincinnati Lead Study

Lead tolerance and siderophore production by native Pseudomonas spp. isolated from lead-contaminated environments in Colombia.

Siderophores are organic compounds known for their chelating properties and specific biochemical characteristics, with potential applications in biotechnology, especially in bioremediation. This study evaluated lead tolerance and pyoverdine-type siderophore production in native Pseudomonas spp. isolates from lead-contaminated water. Lead tolerance was assessed using different lead concentrations (5, 10, 25, 50 mg/L) through the minimum inhibitory concentration. Siderophore production was estimated based on the percentage of siderophore units (PSU) in the presence and absence of lead. Among the

Peppered with lead: An environmental forensics approach to identify the source of rising blood lead levels.

Despite the phase-out of lead-based products, lead contamination can still present a contemporary risk to public health. In situations where elevated blood lead cannot be attributed to common sources, detailed environmental investigation is needed to identify more elusive sources and manage harmful exposure pathways. We apply a forensics approach to assess common and elusive sources of lead in the home environment of two individuals with fluctuating blood lead levels in Sydney, Australia. Using multiple analytical lines of evidence (portable X-Ray Fluorescence spectrometry (pXRF), inductively

Lead speciation in body tissues, gastrointestinal contents, and feces of lead-exposed wild rats (Rattus rattus).

The toxic effects of lead (Pb) are an ongoing concern for which research continues to seek a solution. In Pb-contaminated areas, Pb concentrations in the environment and organisms are quantified to assess the degree of contamination. Understanding organismal uptake of Pb and its behavior in the body requires distinguishing Pb chemical species. We used Pb L3-edge X-ray absorption near-edge structure (XANES) to study the distribution of Pb species in body tissues, digestive tract contents, and feces of wild rats (Rattus rattus) collected from a heavily Pb-contaminated mining area in Zambia. Transvenous lead extraction of lumenless 3830 pacing lead in conduction system pacing: a single-center experience.

BACKGROUND: The Medtronic SelectSecure Model 3830 lumenless lead (Medtronic, Inc., Minneapolis, MN) is commonly used for conduction system pacing (CSP). However, with this increased use, the potential need for transvenous lead extraction (TLE) also will increase. While extraction of endocardial 3830 leads is rather well described especially in pediatric and adult congenital heart disease population, there is very limited data on extraction of CSP leads. In the present study, we reported our preliminary experience on TLE of CSP leads and provided technical considerations. METHODS: The study po

Lead migration rate in dorsal root ganglion stimulation using lead anchoring techniques: A follow-up study.

BACKGROUND: Lead anchoring has previously been shown to reduce the rate of dorsal root ganglion stimulation (DRG-S) lead migration. The aim of this study was to assess longer-term follow-up and consistency of lead migration prevention with lead anchoring in a new cohort of patients. METHODS: We performed a retrospective chart review from September 2017 to November 2022 of all patients who had DRG-S implants at our institute to identify the number of lead migrations that occurred over this period. The first cohort consisted of patients reported on in a previous publication (implanted from Sept
