Evaluation of performance characteristics of the medicinal leech (Hirudo medicinalis) for the treatment of venous congestion.

Medicinal leeches (Hirudo medicinalis) are a standard treatment for venous congestion, a complication that can occur after reconstructive surgery. If the cause of venous congestion cannot be surgically corrected, then medicinal leeches are used to temporarily increase perfusion levels and maintain physiologic requirements within the congested tissue. Leeches increase perfusion within congested tissue by actively drawing off blood as a bloodmeal. Furthermore, the leech bite continues to bleed and relieve congestion after detachment because of the anticoagulation effects of leech saliva left beh

Calcium transients in subcompartments of the leech Retzius neuron as induced by single action potentials.

Regional Ca(2+) influx into neurons plays an essential role for fast signal processing, yet it is little understood. We have investigated intracellular Ca(2+) transients induced by a single action potential (AP) in Retzius neurons in situ of isolated ganglia of the leech Hirudo medicinalis using confocal laser scanning microscopy in the cell body, in different axonal branches, and in dendrites. In the cell body, a single AP induced a Ca(2+) transient in submembrane regions, while in central regions no fluorescence change was detected. Burst activity evoked a much larger Ca(2+) influx, which el

Nitric oxide influences injury-induced microglial migration and accumulation in the leech CNS.

Damage to the leech or mammalian CNS increases nitric oxide (NO) production and causes accumulation of phagocytic microglial cells at the injury site. The aim of this study was to determine whether NO plays a role in microglial migration and accumulation at lesions in which NO is generated by a rapidly appearing endothelial nitric oxide synthase (eNOS) in leeches. Immunohistochemistry and cytochemistry demonstrated active eNOS before and throughout the period of microglial accumulation at the lesion. Decreasing NO synthesis by application of the NOS inhibitor N(w)-nitro-L-arginine methyl ester

Chloride-dependent pH regulation in connective glial cells of the leech nervous system.

We used double-barreled pH-sensitive microelectrodes to study the mechanisms by which the intracellular pH is regulated in the connective glial cells of the medicinal leech. The experiments indicate that a Cl(-)- and HCO3(-)-dependent mechanism mediates some recovery from intracellular alkalosis even in the absence of external Na+. This suggests the presence of Na(+)-independent Cl-/HCO3- exchange in the connective glial membrane. At alkaline pHi, this exchange most likely operates in the direction of net acid loading (i.e. HCO3- efflux).

[The blocking action of snake venom polypeptides on the cholinergic mechanisms of leech dorsal muscle].

The blocking action of alpha-polypeptides from the venoms of elapid snakes (Bungarus multicinctus and Naja naja siamensis) was studied on leech dorsal muscle. It was shown that these neurotoxins failed to inhibit the responses to mononitrogen cholinomimetics (acetylcholine, nicotine, carbamylcholine, etc.). The capacity of neurotoxins to inhibit the responses to the bisquaternary cholinommimetics depended upon the length of the molecule. The neurotoxins were active against the "shorter" cholinomimetics, such as decamethonium, and bischolinic esters of malonic, succinic and glutaric acids. The

A Disaster and Salvage Revisited: Chest Wall Reconstruction (5-Rib Defect) With Gore-Tex, TRAM Flap, and Salvage With Leeches.

INTRODUCTION: A chest wall defect after resection of a recurrent desmoid tumor of the right breast was reconstructed with a TRAM flap over a Gore-Tex graft. The tumor had involved 2 ribs, and the wide resection included 5 ribs with a portion of the lateral sternum. METHODS: After the wide resection to effectively manage this aggressive, invasive lesion from the anterior chest of a 42-year-old woman, a 2-mm Gore-Tex graft was directly sewn in place to bridge the defect. A TRAM flap based on the left rectus abdominis muscle was delivered to the right chest to close the defect. RESULTS: The TRA

Investigation of the therapeutic effects of medicinal leech treatment on lower extremity ischemia-reperfusion injury.

OBJECTIVES: This study aims to investigate the therapeutic effects of medical leech treatment (MLT) on ischemia/reperfusion (I/R) injuries in the lower extremities of rats. METHODS: A total of 18 Wistar Albino rats were used and randomly assigned to three groups: Sham (n=6), I/R (n=6), and MLT (n=6). The I/R group rats underwent I/R induction in the femoral artery using an atraumatic vascular clamp. The same was done for the MLT group, after which MLT was carried out on the gastrocnemius muscle region of each rat. Histopathological analysis of the tissue and biochemical analysis was performed

Negative Pressure Wound Therapy as an Artificial Leech to Save a Congestive Flap: Case Report.

Historically, patients with lower limb defects have represented a challenge for plastic surgeons because of their higher rate of complications. One of the main complications is venous congestion. Recently, various studies have suggested the use of negative pressure wound therapy as a salvage flap therapy, showing promising results. In this case report, we will outline the case of an elderly patient with different comorbidities in whom we used negative pressure wound therapy (as an artificial leech) to reverse venous congestion in the flap, with a satisfactory clinical outcome and without any m

A Case Report on Prepubertal Vaginal Bleeding due to Leech Bite: History Is Very Important.

Though prepubertal vaginal bleeding is rare, it can create a dilemma for both physicians and parents. Prepubertal vaginal bleeding due to leech bite is even rarer and very difficult to diagnose without proper history. We report a case of a six-year-old girl presenting with vaginal bleeding and ultimately diagnosed as a case of vaginal bleeding due to leech bite. We described how focused history helped us in diagnosis and management of such case in rural areas with limited healthcare resources. A detailed history with a high index of suspicion and a thorough examination is essential to diagnose

Leech Infestation in the Vulvar Region: Causes of Vaginal Bleeding in a Six years old Child.

Leeches are hermaphroditic rare blood-sucking human endoparasitesof phylum Annelida and class Hirudinea. Leech infestation is a zoonotic disease acquired by drinking contaminated water, swimming in ponds and streams.1 Epidemiology of leech bites in literature is limited and the majority of existing data are case reports from the tropics or subtropics.2-4 Leech bites can occur on various orifices of the body including internal body cavities and orifices, such as the uterus, rectum, urinary bladder, vulva, nasal cavity, peritoneal cavity, nasopharynx, oropharynx, oesophagus, trachea, bronchi and

Genetic assessment of leech species from yak (Bos grunniens) in the tract of Northeast India.

Yak is an iconic symbol of Tibet and high altitudes of Northeast India. It is highly cherished for milk, meat, and skin. However, yaks suffer drastic change in milk production, weight loss, etc, when infested by parasites. Among them, infestation by leeches is a serious problem in the Himalayan belt of Northeast India. The parasite feeds on blood externally or from body orifices, like nasopharynx, oral, rectum, etc. But there has been limited data about the leech species infesting the yak in that region because of the difficulties in morphological identification due to plasticity of the body,

A classic model animal in the 21st century: recent lessons from the leech nervous system.

The medicinal leech (genus Hirudo) is a classic model animal in systems neuroscience. The leech has been central to many integrative studies that establish how properties of neurons and their interconnections give rise to the functioning of the animal at the behavioral level. Leeches exhibit several discrete behaviors (such as crawling, swimming and feeding) that are each relatively simple. Importantly, these behaviors can all be studied - at least at a basal level - in the isolated nervous system. The leech nervous system is particularly amenable to such studies because of its distributed nat

Comparisons of medicinal leech therapy with venous catheterization in the treatment of venous congestion of the sural flap.

INTRODUCTION: The originally described distally based sural flap technique has a risk of partial or total flap necrosis as high as 25%. The purpose of this study was to compare the medicinal leech therapy (MLT) with venous catheterization (VC) for blood volume removal, infection, wound dehiscence, and flap necrosis in the distally based sural flap with venous congestion. PATIENTS AND METHODS: Fifty-six conventional distally based sural flaps with venous congestion during reconstructive surgeries were randomly divided into two groups, MLT group and VC group. The results of comparisons were ana

Characterization of a catalase gene from Aeromonas veronii, the digestive-tract symbiont of the medicinal leech.

The catalase gene katA of the medicinal leech symbiont Aeromonas veronii bv. sobria was cloned, sequenced, and functionally characterized. Southern hybridization, using an A. veronii katA-specific hybridization probe, suggested the presence of a single gene copy in many Aeromonas species. A. veronii katA consisted of 1446 nt encoding a protein with a high degree of similarity to the small-subunit group III bacterial catalases. A catalase-null mutant (JG186) was constructed through gene-replacement mutagenesis. In the parent strain (HM21R), catalase activity was only detected in extracts of cel

Switching in the activity state of an interneuron that controls coordination of the hearts in the medicinal leech (Hirudo medicinalis).

The rhythmically active heart interneuron HN(5) in the medicinal leech exhibits two distinct activity states, which have been associated with different coordination states of the two hearts. During the active state, it discharges high-frequency bursts of action potentials interrupted by rhythmic inhibitory input from other interneurons. In the inactive state, the same cell receives rhythmic inhibition but the membrane potential remains subthreshold between these volleys, producing few or no action potentials. We investigated differences in the membrane properties of the cell during the active

Ascending urethrography using the Leech-Wilkinson intra-uterine cannula in adult male patients.

The Leech-Wilkinson Intra-Uterine Cannula used in hysterosalpingography has proved useful in ascending urethrography in the adult male patients. Lesions of the urethra, e.g., strictures, calculi, sinuses, diverticula and urethral tear are best demonstrated by ascending urethrography. This method does not replace voiding cysto-urethrography under physiological conditions.

Intersegmental coordination of the leech swimming rhythm. II. Comparison of long and short chains of ganglia.

Preparations of the nearly isolated leech nerve cord containing as few as two ganglia are sufficient to generate intersegmentally coordinated swim oscillations, provided that they receive tonic excitation from other segments via the median connective (Faivre's nerve). Due to their greatly reduced complexity, these preparations should provide useful experimental models of neuronal coordination. As a step in the development of such models, we have characterized the intersegmental coordination of nerve-cord chains ranging from 2 to 18 ganglia in length. We found that increases in swim-cycle perio
