Intramedullary lengthening nails for lower limb reconstruction in children are a rapidly growing field and represent an alternative to external fixators. Today, two types of intramedullary lengthening nails are used: motorized (FITBONE) and magnetically driven (PRECISE) bone lengthening nails. Indications for limb lengthening nails are: 1) Significant lower limb length discrepancy; 2) Short stature and deformities due to musculoskeletal or dysplastic syndromes; 3) Constitutional short stature. Intramedullary lengthening surgery is challenging and represents a safe and reliable mode of treatmen

CASE: A 43-year-old insulin-dependent man with a 4.4-cm posttraumatic femoral limb length discrepancy sustained a subtrochanteric femur fracture associated with failure of a motorized intramedullary limb lengthening nail during distraction osteogenesis. The patient requested a single-stage salvage operation. After implant removal, the femur was stabilized and compressed with a plate-tensioned-nail construct consisting of 4.5-mm locking compression plate tensioned and linked to a femoral reconstruction nail through interlocking screws with immediate weight-bearing.
CONCLUSION: A plate-tensione

INTRODUCTION: Progressive bone lengthening in children can be done using an external fixator, a lengthening nail, or plate with screws. The TrueLok Hexapod System™ (TL-HEX™) is the newest hexapod external fixator on the market. We hypothesized that the TL-HEX™ can accurately correct lower limb deformities in children. The goal of this study was to evaluate the clinical and radiographic outcomes after correcting lower limb deformities in children using the TL-HEX™ system.
MATERIAL AND METHODS: Data from 58 limbs that underwent bone lengthening with the TL-HEX™ were analyzed for this retrospect

A 58-year-old man developed a 14 cm limb length discrepancy with a shortened and ankylosed right hip as a sequelae of multiple surgeries for recurrent dislocation and periprosthetic joint infection. He was successfully treated by limb elongation with external fixation and subsequent revision total hip arthroplasty. We discuss the efficacy of the two-staged reconstruction with initial limb elongation.

We report a three-stage approach in managing an 18-year-old boy with osteogenesis imperfecta who had pseudarthrosis of the left femur with 150° anterior angulation and 9.5 cm shortening. Severe long bone deformity and pseudarthrosis in osteogenesis imperfecta are difficult to manage because of the relative shortening of the surrounding soft tissue and bone fragility. Our staged approach included excision of the pseudarthrosis, gradual soft tissue lengthening by external fixator, and a modified Sofield-Millar operation, to achieve union, deformity, and limb length correction without compromisin

Rexford L. Diveley was born in Bazine, Kansas, in 1892, to the owner of a variety store. The family moved to the Oklahoma Territory (which had been opened for settlement in 1890). He returned to his home state, graduated from Hutchinson High School, and obtained his undergraduate and medical degrees (1917) from the University of Kansas. During WW I he served as head of a radiology unit in Limoges, France. After the armistice, he spent a year at Johns Hopkins in medicine, and then briefly went into private practice in St. Joseph’s, Missouri, north of Kansas City. He then established a radiology

Correction of varus/valgus deformity via soft tissue balancing is the accepted technique in knee arthroplasty surgery. However, in rare instances, excessive ligamentotaxis can result in overly stretched periarticular structures leading to pain and impaired function. This has been called the "joint jack phenomenon." We report a case in which a very thick polyethylene insert resulted in a painful knee and leg length discrepancy. Our solution was to use a salvage hinge knee arthroplasty to achieve joint space reduction, restore equal leg lengths, and avoid global instability. We believe this is t

Femoral length symmetry is assumed when assessing a patient with a dysplastic acetabulum. An unexpected long femur has been observed in some adults with acetabular dysplasia. We undertook a retrospective observation study of 18 adults with unilateral and 11 adults with bilateral acetabular dysplasia. Femoral lengths were assessed using CT measurements. The ipsilateral femur was found to be 5-10 mm longer in 66% (12 patients) with unilateral dysplastic acetabulae. Femoral lengths varied greatly in the presence of bilateral dysplastic acetabulae. In the presence of a acetabular dysplasia, asymme

To investigate the developmental pattern of femoral shortening in Legg-Calve-Perthes disease, the authors made an experimental model of the disease in piglets by devascularizing the capital femoral epiphysis and following the piglets to skeletal maturity. The discrepancy first increased in the postoperative 0 to 8 weeks (1.2-1.9 mm of shortening per week), then decelerated for a considerable period during the postoperative 8 to 16 weeks (0.2-0.6 mm per week), and then increased again toward the end of the growth period of the postoperative 20 weeks (1.2 mm per week). The developmental pattern

Growth of the limb in a child can be impaired, with the coexistence of a vascular malformation. In these vascular bone syndromes, altered growth is manifest as overgrowth or hypotrophy. The vascular malformation is usually complex and gets progressively worse with time. The two types of vascular anomalies in limbs, fast-flow and slow-flow, can be associated with limb length discrepancies. The fast-flow vascular malformations together with arteriovenous fistulae are part of Parkes Weber syndrome, characterized by congenital red cutaneous staining, hypertrophy in girth and increasing of limb len

A case of a 24-week-old fetus of non consanguineous parents with an ultrasonographic diagnosis of a short right femur, ipsilateral agenesia of the fibula and a twisted right foot is described. Cordocentesis revealed a normal 46XY karyotype. The parents were informed on treatment options and after psychological counselling they decided to undergo an abortion. Post mortem examination confirmed the diagnosis of a severe right limb malformation.

We describe a new technique of percutaneous epiphysiodesis using transphyseal screws (PETS) and our experience with it in 32 cases of limb-length inequality and nine angular knee deformities. A subgroup of 18 patients with postfracture limb overgrowth formed a clinical model for study of the real efficacy of PETS. It proved a reliable method with few complications and many advantages such as simplicity of technique, short operating time, rapid postoperative rehabilitation, and reversibility. Bone-length inequality decreased from a preepiphysiodesis average of 2.47 cm (range, 1.5-4.6) to 0.51 c

In a series of 43 patients diagnosed with congenital anterolateral bowing of the tibia or congenital pseudarthrosis of the tibia treated between 1980 and 1992, the deformities corrected themselves spontaneously in five patients who had no other congenital abnormalities or evidence of neurofibromatosis. At an average follow-up of 58 months (average age at follow-up 80 months), the only residual deformity seen was a limb-length discrepancy in patients with unilateral involvement. In patients with subperiosteal callus formation on the posteromedial concavity of this deformity and an uninvolved fi

Limb lengthening in nine patients was monitored by radiographs and by ultrasound scans. The distraction gap appeared as a sonolucent area within which echogenic foci developed soon after distraction commenced. By seven weeks a new cortex was detected, and medullary canal began to develop between seven and eight weeks. Ultrasound scanning can be used to measure distraction, but it was not as useful as radiographs in detecting angulation. Its use in patients undergoing limb lengthening could reduce their exposure to radiation.

The understanding of the conditions under which bone and soft tissue can be regenerated by distraction has opened a new era in the field of limb reconstruction. Success in lengthening for posttraumatic limb length discrepancy has led to different applications of the distraction techniques. We present an overview of the clinical new possibilities, the strategy, the techniques and the complications using the distraction technique in the trauma patient. The optimal biologic method (limb distraction after resection or callus distraction by a transport technique) and the optimal fixation devices de

The lower limb length measurement is an important element for the diagnosis of mechanical or structural lumbar pain. Also it has been used for referral pain associated with hip or knee osteoarthritis or the groin and suprapubic areas. The aims of the present study were: 1) to measure the intra and inter observers variation; 2) to measure the intra-method variation using two different techniques for lower limb length measurement, one called the "apparent measure" (9) and comparing both with the radiological measurement technique. Two medical doctors, training on the techniques for lower limb me

This case report presents the rare combination of circumstances of a child with (a) dyschondroplasia (enchondromatosis) limited to all the major epiphyseal plates of one lower extremity, (b) unusual capacity to cooperate, and (c) parents who refused permission to treat the limb by amputation but allowed the child to participate in a long-term clinical experiment. In dyschondroplasia (Ollier's disease), the long bones grow in diameter but not in length. In this condition, it was possible (by fasciotomy, osteotomy, and interposition of wedge blocks or hemicylindrical bone implants (stabilized by
