Rapid response to meningococcal disease cluster in Foya district, Lofa County, Liberia January to February 2018.

INTRODUCTION: Early detection of disease outbreaks is paramount to averting associated morbidity and mortality. In January 2018, nine cases including four deaths associated with meningococcal disease were reported in three communities of Foya district, Lofa County, Liberia. Due to the porous borders between Lofa County and communities in neighboring Sierra Leone and Guinea, the possibility of epidemic spread of meningococcal disease could not be underestimated. METHODS: The county incidence management system (IMS) was activated that coordinated the response activities. Daily meetings were con

Evaluation of Ebola virus disease surveillance system capability to promptly detect a new outbreak in Liberia.

INTRODUCTION: Liberia was heavily affected by the 2014-2016 Ebola virus disease (EVD) outbreak. With substantial investments in interventions to combat future outbreaks, it is hoped that Liberia is well prepared for a new incursion. We assessed the performance of the current EVD surveillance system in Liberia, focusing on its ability to promptly detect a new EVD outbreak. METHODS: We integrated WHO and US Centers for Disease Control and Prevention guidelines for public health surveillance system evaluation and used standardised indicators to measure system performance. We conducted 23 key inf

Effects of socioeconomic factors and booking time on the WHO recommended eight antenatal care contacts in Liberia.

Antenatal care (ANC) is an important intervention that has been linked to reduce maternal and newborn adverse outcomes. However, the long years of war in Liberia may have contributed to the poor health indices including the uptake of maternal health care services. The objective of this study was to determine the marginal interaction effects between booking time and socioeconomic factors in eight or more ANC contacts. A total sample of 4,185 women who had given birth were included in this study. The 2020 Liberia Demographic and Health Survey (LDHS) dataset was analyzed. The outcome variable was

Epidemiological characteristics of Lassa fever cases in Liberia: a retrospective analysis of surveillance data, 2019-2020.

OBJECTIVES: Liberia is endemic to Lassa fever (LF) and has the largest reported per capita incidence of LF patients in the West African region. Cases of the disease increased unprecedentedly in 2019 and 2020, characterized by a geographical drift in epidemiology and seasonal variation of occurrence. This study aims to describe the epidemiological and clinical characteristics of LF in Liberia from 2019 to 2020. METHODS: A retrospective study was conducted on cases of LF confirmed at the National Public Health Reference Laboratory from January 2019 to December 2020. Medical records were reviewe

Prevalence of Hepatitis B and C Among Healthcare Workers in a Tertiary Care Center in Monrovia Liberia.

BACKGROUND: Viral hepatitis is a leading cause of death worldwide, with a higher mortality rate than that from human immunodeficiency virus (HIV), tuberculosis (TB) or malaria. In order to meet the WHO's goal of eliminating hepatitis B and C by 2030, there is a dire need to establish baseline prevalence rates and increase public health awareness of this detrimental disease, especially in low and middle-income countries (LMICs) where establishing prevalence rates of hepatitis B (HBV) and hepatitis (HCV) continues to be a problem. OBJECTIVE: To establish baseline rates of hepatitis B and C amon

Clinical Presentation and Care of Patients with Ebola Virus Disease in the China Ebola Treatment Unit, Liberia.

In order to evaluate the clinical characteristics of confirmed Ebola Virus Disease (EVD) patients admitted to the China Ebola Treatment Unit (China ETU) between January 2015 and March 2015, we retrospectively analyzed clinical symptoms, treatment, and epidemiologic features of 5 patients with confirmed EVD, and reviewed the relevant medical literature. Of these, 3 patients survived, and 2 died. The time interval from the onset of symptoms to the negative PCR test for Ebola virus in the 3 survivors was 14-18 days. All survivors reported direct contact with confirmed EVD patients up to 21 days p

Implementation of Ebola case-finding using a village chieftaincy taskforce in a remote outbreak - Liberia, 2014.

On October 16, 2014, a woman aged 48 years traveled from Monrovia, Liberia, to the Kayah region of Rivercess County, a remote, resource-poor, and sparsely populated region of Liberia, and died on October 21 with symptoms compatible with Ebola virus disease (Ebola). She was buried in accordance with local tradition, which included grooming, touching, and kissing the body by family and other community members while it was being prepared for burial. During October 24-November 12, eight persons with probable and 13 with confirmed Ebola epidemiologically linked to the deceased woman had onset of sy

A case series study of perinatal deaths at one referral center in rural post-conflict Liberia.

The overall objective of this study was to further our understanding of the factors contributing to the high perinatal mortality rates at a busy rural, referral hospital in Liberia. The specific aims were to: (1) analyze the records of women who experienced a perinatal loss for both medical and nonmedical contributing factors; (2) describe the timing and causes of all documented stillbirths and early neonatal deaths; and (3) understand the factors surrounding stillbirth and early neonatal death in this context. This case series study was conducted through a retrospective hospital-based record

Adult and paediatric mortality patterns in a referral hospital in Liberia 1 year after the end of the war.

The aim of this study was to describe and analyse hospital mortality patterns after the Liberian war. Data were collected retrospectively from January to July 2005 in a referral hospital in Monrovia, Liberia. The overall fatality rate was 17.2% (438/2543) of medical admissions. One-third of deaths occurred in the first 24h. The adult fatality rate was 23.3% (241/1034). Non-infectious diseases accounted for 56% of the adult deaths. The main causes of death were meningitis (16%), stroke (14%) and heart failure (10%). Associated fatality rates were 48%, 54% and 31% respectively. The paediatric fa

Ultrastructure of infective larvae (L3) of Onchocerca volvulus (Nematoda: Filarioidea) developed in Simulium yahense in Liberia.

Third-stage infective larvae of Onchocerca volvulus were examined to elucidate the ultrastructure and the interrelations of the stoma, esophagus, intestine, and nervous system. The alimentary canal involves a cuticularized stoma with a triradiate lumen that is continuous with a similar triradiate lumen in the muscular region of the esophagus. The lumen wall may be laterally appressed or opened into a stellate form in the glandular region. Posteriad from the esophagointestinal valve, the cylindroid lumen becomes partially occluded with microvilli formed by the evaginations of the apical membran

Endemic Lassa fever in Liberia. IV. Selection of optimally effective plasma for treatment by passive immunization.

The efficacy of passive immunization for treatment of Lassa Fever (LF) is believed to depend on the titre of the neutralizing antibody infused. For the purpose of identifying optimal donors of LV-immune plasma, a population of LF-convalescent patients in Liberia was tested for prevalence of neutralizing antibody. Minimally protective titres, expressed as a log10 neutralization index, (LNI), were established in animal models as LNI greater than 2. LNI titres for 26 donors, tested eight or more months after illness, were modest: 16 titred 1 less than LNI less than 2, 4 titred 2 greater than LNI

Morphological identification of simulium sanctipauli and S. yahense in Liberia and comparison of results with those of enzyme electrophoresis.

Simulium sanctipauli and S. yahense have been incriminated as the main vectors of Onchocerca volvulus in the rain-forest zone of Liberia. An accurate identification of the females of the two species which breed in different types of water-courses but are morphologically similar is essential for a proper study of their epidemiological importance and the planning of control measures. Various external characters were examined for their diagnostic value by the comparison of identifications using these characters and those based on enzyme electrophoresis (phosphoglucomutase, trehalase). It was show

Enhancing laboratory capacity during Ebola virus disease (EVD) heightened surveillance in Liberia: lessons learned and recommendations.

INTRODUCTION: Following a declaration by the World Health Organization that Liberia had successfully interrupted Ebola virus transmission on May 9th, 2015; the country entered a period of enhanced surveillance. The number of cases had significantly reduced prior to the declaration, leading to closure of eight out of eleven Ebola testing laboratories. Enhanced surveillance led to an abrupt increase in demand for laboratory services. We report interventions, achievements, lessons learned and recommendations drawn from enhancing laboratory capacity. METHODS: Using archived data, we reported befo

Community-Led Total Sanitation, Open Defecation Free Status, and Ebola Virus Disease in Lofa County, Liberia.

The Ebola virus disease (EVD) epidemic entered Liberia through Lofa County in February 2014 and spread to two health districts where the nongovernmental organization Global Communities had been implementing community-led total sanitation (CLTS) since 2012. By December 2014 the county had 928 Ebola cases (422 of them confirmed) and 648 deaths. Before the epidemic, CLTS was triggered in 155 communities, and 98 communities were certified as Open Defecation Free (ODF). Using mixed quantitative and qualitative methods, we determined that no cases of EVD were found in ODF communities and in only one

Assessment of ebola virus disease, health care infrastructure, and preparedness - four counties,Southeastern Liberia, august 2014.

Ebola virus disease (Ebola) is a multisystem disease caused by a virus of the genus Ebolavirus. In late March 2014, Ebola cases were described in Liberia, with epicenters in Lofa County and later in Montserrado County. While information about case burden and health care infrastructure was available for the two epicenters, little information was available about remote counties in southeastern Liberia. Over 9 days, August 6-14, 2014, Ebola case burden, health care infrastructure, and emergency preparedness were assessed in collaboration with the Liberian Ministry of Health and Social Welfare in

Lassa fever symptomatology, viral dynamics, and host immune response (PREPARE): a prospective, observational cohort study in Liberia.

BACKGROUND: Lassa virus (LASV) is a persistent threat to public health in west Africa and beyond. LASV is endemic in west Africa and each year it is responsible for an estimated 2·7 million infections, 23 700 hospitalisations, and 5000 deaths. With over 32 reported cases of Lassa fever imported into non-endemic countries-one-third of which were fatal-the importance of enhanced detection and management of Lassa fever extends beyond west Africa. METHODS: The prevalence, pathogenesis, and persistence (PREPARE) study was a prospective cohort study among patients admitted to two hospitals in a hyp

Assessing induced abortion underreporting in restrictive settings using prospective morbidity surveys from Kenya, Liberia, and Sierra Leone.

Accurate estimation of induced abortion rates in legally restrictive contexts remains a challenge because fear of prosecution and stigma hinder truthful reporting among women. We examined induced abortion reporting accuracy by assessing the agreement between women's self-reports of induced abortion and provider diagnosis of women's abortions. We used data from three prospective morbidity surveys conducted in health facilities in Kenya (surveyed in 2022), Liberia (surveyed in 2021), and Sierra Leone (surveyed in 2021). The data were collected from post-abortion care (PAC) patients and healthcar

Syndromic surveillance using monthly aggregate health systems information data: methods with application to COVID-19 in Liberia.

BACKGROUND: Early detection of SARS-CoV-2 circulation is imperative to inform local public health response. However, it has been hindered by limited access to SARS-CoV-2 diagnostic tests and testing infrastructure. In regions with limited testing capacity, routinely collected health data might be leveraged to identify geographical locales experiencing higher than expected rates of COVID-19-associated symptoms for more specific testing activities. METHODS: We developed syndromic surveillance tools to analyse aggregated health facility data on COVID-19-related indicators in seven low- and middl
