Beyond voice: perceptions of gender in male-to-female transsexuals.

PURPOSE OF REVIEW: This review covers recent research findings that demonstrate the effectiveness of speech pathology intervention in enabling male-to-female transsexual individuals to portray their gender identity through speech. Research reports that extend our understanding of communication characteristics that contribute to perceptions of sex in male-to-female transsexuals are identified. Current issues related to the measurement of therapy effectiveness are also presented. RECENT FINDINGS: Earlier studies confirmed that an increase in voice pitch is achievable for many transsexuals. Aerodynamic Parameters in Byzantine Chant Voices: Comparisons Across Pitch and Loudness.

OBJECTIVE: This study was designed to assess the impact of phonation frequency and loudness increase on aerodynamic parameters of the singing voice in Byzantine chant (BC). DESIGN: Aerodynamic measurements in BC were obtained and statistically analyzed. METHOD: Fifteen experienced BC chanters, all baritones, performed the ascending notes G2, C3, E3, G3, C4, E4, and G4, at normal and high levels of loudness within a mask, while repeating strings of /pi/ syllables. The parameters of airflow (FR), subglottal pressure (Psub), and sound pressure level (SPL) were directly measured, and from them,

Estimating occupant satisfaction of HVAC system noise using quality assessment index.

Noise may be defined as any unwanted sound. Sound becomes noise when it is too loud, unexpected, uncontrolled, happens at the wrong time, contains unwanted pure tones or unpleasant. In addition to being annoying, loud noise can cause hearing loss, and, depending on other factors, can affect stress level, sleep patterns and heart rate. The primary object for determining subjective estimations of loudness is to present sounds to a sample of listeners under controlled conditions. In heating, ventilation and air conditioning (HVAC) systems only the ventilation fan industry (e.g., bathroom exhaust

The comparison of the comodulation masking release (CMR) in individuals with and without chronic tinnitus.

In comodulation masking release (CMR), the auditory signal threshold improves when maskers far from the frequency of the target signal are coherently amplitude-modulated. Studies show that inhibitory function may play a role in CMR. In individuals who have tinnitus, decreasing inhibitory functioning in auditory central nervous system has been documented. Therefore, in this study, the purpose was to evaluate CMR in individuals suffering from tinnitus and compare CMR findings with those who do not have tinnitus. Seventeen normal-hearing individuals (without tinnitus) and 13 individuals having un

Differences in performance between Oticon MultiFocus Compact and ReSound BT2-E hearing aids.

Differences in performance were evaluated between binaural fittings of the Oticon MultiFocus (MF) and ReSound BT2-E on 25 hearing-impaired subjects across two sites. Subjects were initially fit using each manufacturer's algorithm and adjustments were made at 1 week based on subjects' responses to diary questions. Performance was assessed after a 4- to 6-week trial period with each hearing aid set using the Speech Perception in Noise (SPIN) test administered at 50, 65, and 80 dB SPL, the Abbreviated Profile of Hearing Aid Benefit (APHAB) questionnaire, loudness judgments of female connected dis

Persistent auditory hallucinations and their relationship to delusions and mood.

Twelve patients who met DSM-III-R criteria for schizophrenia and had persistent auditory hallucinations completed a diary three times daily for 2 to 3 weeks. Using 5-point equal interval rating scales, nearly all patients were able to record consistently the nature of their hallucinations, the intensity of their delusional beliefs, and the mood and clarity of their thinking. For at least half the patients, there was a statistically significant relationship between the loudness and intrusiveness of hallucinations and the intensity of delusional beliefs; the more intrusive and distressing were t

An evaluation of the Reltus ear massager for short-term tinnitus relief.

OBJECTIVE: Auditory stimulation has been shown to suppress the loudness of tinnitus (residual inhibition, RI). Somatosensory manipulations have also been shown to sometimes decrease tinnitus perception. An 'ear-massaging' device, the 'Reltus' ( www.reltus.com ), has been marketed as a tinnitus treatment device. This study was undertaken to evaluate its short-term effectiveness and mode of effect. DESIGN: The research was undertaken in two phases. Phase 1 measured the change in tinnitus perception after one minute of auditory stimulation through headphones and after application of a vibrating

Listener Factors Explain Little Variability in Self-Adjusted Hearing Aid Gain.

Self-adjustment of hearing aid gain can provide valuable information about the gain preferences of individual listeners, but these preferences are not well understood. Listeners with mild-to-moderate hearing loss used self-adjustment to select amplification gain and compression parameters in real time on a portable touch screen device while listening in quiet and noisy backgrounds. Adjustments to gain prescribed by the National Acoustics Laboratories' non-linear fitting procedure (NAL-NL2) showed large between-subject variability. Known listener characteristics (age, gender, hearing thresholds

Simulation of the effects of loudness recruitment and threshold elevation on the intelligibility of speech in quiet and in a background of speech.

These experiments simulated the threshold elevation and loudness recruitment associated with three different types of cochlear hearing loss: Moderate flat (condition R2), severe flat (condition R3), and moderate-to-severe sloping (condition RX). This was done to allow an examination of the effects of these factors on the intelligibility of speech, in isolation from other factors that are normally associated with cochlear hearing loss, such as reduced frequency selectivity. The simulation was performed by splitting the input signal into 13 frequency bands, and processing the envelope in each ba

Manifestations of intense noise stimulation on spontaneous otoacoustic emission and threshold microstructure: experiment and model.

Comparison between changes that occur simultaneously on spontaneous otoacoustic emissions (SOAEs) and on other cochlear origin phenomena can contribute to the understanding of cochlear micromechanical activity. The temporary changes that arise after short noise exposure are investigated in the following paper. The effects of noise exposure on the threshold microstructure near an SOAE and on the amplitude and frequency of the SOAE were measured. These experimental results indicate the following: (1) exposure to wideband noise for a short time causes a temporary reduction in the SOAE frequency a

Are the non-classical auditory pathways involved in autism and PDD?

OBJECTIVE: To test the hypothesis that some of the abnormal sensory perceptions that characterize autism may be explained by an abnormal activation of non-classical (extra-lemniscal) sensory pathways. METHODS: Twenty-one individuals, 18--45 years of age who were diagnosed with autism participated in the study. Sounds (clicks presented at a rate of 40 per second and 65 dB above the normal threshold) were applied through earphones. Electrical stimulation (100 microS rectangular impulses at a rate of 4 per second) was applied through electrodes placed on the skin over the median nerve at the wri

Bimodal Stimulation of the Auditory-Somatosensory System Improves Auditory Sensory Gating Function in Patients with Tinnitus.

BACKGROUND: Tinnitus perception may be associated with impairments in the mechanisms of sensory gating (SG), which is a preattentive process that filters redundant auditory stimuli to prevent sensory overload. Acoustic stimulation of the auditory system alone (unimodal intervention), electrical stimulation of the vagus nerve alone, or a combination of both (bimodal intervention) can effectively treat tinnitus by influencing SG. PURPOSE: This study aimed to examine the impact of unimodal and bimodal interventions on auditory SG (ASG). RESEARCH DESIGN: Participants were assigned to unimodal or

Measuring the Moment-to-Moment Variability of Tinnitus: The TrackYourTinnitus Smart Phone App.

Tinnitus, the phantom perception of sound without a corresponding external sound, is a frequent disorder which causes significant morbidity. So far there is no treatment available that reliably reduces the tinnitus perception. The research is hampered by the large heterogeneity of tinnitus and the fact that the tinnitus perception fluctuates over time. It is therefore necessary to develop tools for measuring fluctuations of tinnitus perception over time and for analyzing data on single subject basis. However, this type of longitudinal measurement is difficult to perform using the traditional r

Ventral cochlear nucleus bushy cells encode hyperacusis in guinea pigs.

Psychophysical studies characterize hyperacusis as increased loudness growth over a wide-frequency range, decreased tolerance to loud sounds and reduced behavioral reaction time latencies to high-intensity sounds. While commonly associated with hearing loss, hyperacusis can also occur without hearing loss, implicating the central nervous system in the generation of hyperacusis. Previous studies suggest that ventral cochlear nucleus bushy cells may be putative neural contributors to hyperacusis. Compared to other ventral cochlear nucleus output neurons, bushy cells show high firing rates as wel

Aerosol emission and superemission during human speech increase with voice loudness.

Mechanistic hypotheses about airborne infectious disease transmission have traditionally emphasized the role of coughing and sneezing, which are dramatic expiratory events that yield both easily visible droplets and large quantities of particles too small to see by eye. Nonetheless, it has long been known that normal speech also yields large quantities of particles that are too small to see by eye, but are large enough to carry a variety of communicable respiratory pathogens. Here we show that the rate of particle emission during normal human speech is positively correlated with the loudness (

Acoustic-reflex growth and loudness.

Acoustic-reflex growth functions and loudness-balance judgments were obtained for three normal-hearing subjects with normal middle-ear function. The hypothesis that acoustic reflex-activating signals producing proportionately equal acoustic-impedance changes are judged equal in loudness was evaluated. The mean acoustic impedance and associated standard deviations were computed for the baseline (static) and activator (reflex) portions of each reflex event. An acoustic-impedance change exceeding two standard deviations of baseline was defined as the criterion acoustic-reflex response. Acoustic i

Auditory Brainstem Changes in Timing may Underlie Hyperacusis in a Salicylate-induced Acute Rat Model.

Hyperacusis, an exaggerated, sometimes painful perception of loudness even for soft sounds, is a poorly understood distressing condition. While the involvement of modified gain of central auditory neurons and the influence of nonauditory brain regions are well-documented, the issue of where in the auditory system these abnormalities arise remains open, particularly when hyperacusis comes without sensorineural hearing loss. Here we used acute intraperitoneal administration of sodium salicylate (150 mg/kg) in rats, enough to produce > 10-dB decrease in acoustic startle threshold with mild hearin

Loud Music Exposure and Cochlear Synaptopathy in Young Adults: Isolated Auditory Brainstem Response Effects but No Perceptual Consequences.

The purpose of this study was to test the hypothesis that listeners with frequent exposure to loud music exhibit deficits in suprathreshold auditory performance consistent with cochlear synaptopathy. Young adults with normal audiograms were recruited who either did ( n = 31) or did not ( n = 30) have a history of frequent attendance at loud music venues where the typical sound levels could be expected to result in temporary threshold shifts. A test battery was administered that comprised three sets of procedures: (a) electrophysiological tests including distortion product otoacoustic emissions

Hidden Electrode Failure in a Cochlear Implant User.

When the impedance of an electrode contact is highly increased in a cochlear implant, a failure of the appropriate electrode seems obvious. We present a case where impedances of some electrodes were at the lower edge of the normal range and not regarded as suspicious neither by the clinical fitting software nor by in-vivo tests conducted by the implant manufacturer. However, speech comprehension was substantially degraded and sound perception distorted. Also, on the affected electrodes, loudness perception was compromised and responses of the electrically evoked compound action potential were
