Soft-tissue infection in lower-extremity trauma.

Soft-tissue infection after lower-extremity trauma has not been studied in detail in light of recent data on the biology of infection. This article examines specific problems in lower-extremity trauma that allow the wound to become susceptible to wound infection. It also illustrates the various principles of wound management in lower-extremity trauma that serve to prevent infection. Two case examples are used to illustrate principles of management. Other wound problems in lower-extremity trauma are also discussed, such as rabies, necrotizing soft-tissue infection, tetanus, and diabetic foot in

Nerve compression syndromes of the lower extremity.

Nerve compression syndromes of the lower extremity present a challenge in differential diagnosis. Compression of the common peroneal nerve occurs relatively frequently; compression of the sciatic nerve occurs infrequently. The pattern of weakness helps distinguish lumbar root entrapment from peripheral compression syndromes. Compression syndromes must also be differentiated from diabetic, alcoholic or vasculitic neuropathy. Recovery correlates with the degree and duration of nerve injury. Thus, early diagnosis and treatment are important.

Free tissue transfer to the lower extremity.

The results of 100 free microvascular transfers to the lower limb are presented. These show a high success rate with successful salvage of 95 limbs. The major complications were infection and delayed healing, osteomyelitis, and non-union. Early skin cover, within 14 days of injury, reduced the incidence of complications by half. Aggressive debridement and provision of early skin cover should be the aim of management of tibial trauma with tissue loss.

Scurvy: bilateral lower extremity ecchymoses and paraparesis.

A 46-year-old man presented with two weeks of progressive paraparesis and large confluent ecchymoses of both thighs. There was a history of poor dietary intake and daily alcohol use. He had had similar problems each of the past two winters. In 1979 he required hospitalization for "sciatica." In 1980 he was bedridden for two months because of lower extremity weakness. Examination revealed poorly fitting dentures, large confluent ecchymoses of both thighs, perifollicular hemorrhages, and a low serum ascorbic acid concentration. Oral ascorbic acid was begun in the hospital and all clinical sympto

Assessment of a lower extremity training program.

The response to a lower extremity program has been studied in workmen undergoing progressive rehabilitation following injury to the lower leg or ankle region. When first seen, the isometric strength of the injured limb was substantially reduced relative to its counterpart, and aerobic power was also poor, relative to normal standards. Substantial gains of isometric strength occurred over a four- to six-week period of therapy. Measurements above the site of injury suggested a 13% gain of strength, and measurements below the injury indicated a gain of 34%. On the other hand, there were minimal c

Sequential Free Flaps in Lower Extremity Reconstruction.

INTRODUCTION: Sequential free flaps are often utilized for complex defects, particularly for recurrent head and neck tumors. However, their application in lower extremity (LE) reconstruction following trauma or oncology is less common. This study evaluated the indications, flap survival rates, and complications of sequential free flaps utilized in LE reconstruction. METHODS: Data from our multicenter database spanning from 2002 to 2020 were analyzed retrospectively through chart review. Adult patients who underwent sequential free flaps to the LE without complete initial flap loss were includ

Predicting wound complications following lower extremity revascularization.

OBJECTIVE: The aim of this study was to create a simple risk score to identify factors associated with wound complications after infrainguinal revascularization. METHODS: The Veterans Affairs Surgical Quality Improvement Program national data set was queried from 2005 to 2021 to identify 22,114 patients undergoing elective open revascularization for peripheral arterial disease (claudication, rest pain, tissue loss) or peripheral aneurysm. Emergency and trauma cases were excluded. The data set was divided into a two-thirds derivation set and one-third validation set to create a risk prediction

Lower extremity lymphoedema: some recent literature highlights.

Lower extremity lymphoedema is common after many cancers. Nevertheless, epidemiological estimates of the prevalence of lower extremity lymphoedema vary widely. A recent study, however, clarified the rates in women with colorectal, endometrial and ovarian malignancies: about one third self-reported lower extremity lymphoedema. The study also confirmed that lower extremity lymphoedema can markedly undermine physical functioning. Some patients, however, seem to be at especially high-risk of developing lower extremity lymphoedema, such as those who undergo extensive lymphadenectomy. Moreover, unti

Analysis of Early Lower Extremity Re-amputation.

BACKGROUND: Data is scarce regarding the need for early re-amputation to a higher anatomic level. This study seeks to define outcomes and risk factors for re-amputation. METHODS: Patients undergoing primary major lower extremity amputation were identified within the 2012-2016 ACS-NSQIP database. Demographics, outcomes, and peri-operative characteristics were compared, and multivariable logistic regression model was used to determine association with early re-amputation. RESULTS: Over a 4-year period, 8306 below knee amputations and 6367 above knee amputations were identified. Thirty-day re-a

Difficult life events affect lower extremity illness.

BACKGROUND: Given the relationship between psychological distress and activity tolerance (capability), a stressful life event might diminish accommodation, increase symptoms, and induce a person to seek specialty care. As a first step to investigate this possibility, this study addressed whether difficult life events are associated with greater activity intolerance and pain intensity. METHODS: A cohort of 127 patients seeking specialty care for lower extremity symptoms completed questionnaires that inquired about difficult life events within the last 12 months as derived from the Holmes Rahe

Antithrombotic Therapy in Lower Extremity Artery Disease.

Lower extremity artery disease (LEAD) is a marker of a more advanced atherosclerotic process often affecting multiple vascular beds beyond the lower limbs, with a consequent increased risk for all-cause and cardiovascular mortality. Antithrombotic therapy is the cornerstone of management of these patients to prevent ischaemic cardiovascular and limb events and death. In patients with symptomatic LEAD, the efficacy of aspirin has been established long ago for the prevention of cardiovascular events. In the current guidelines, clopidogrel may be preferred over aspirin following its incremental a

The Gastrocnemius Flap for Lower Extremity Reconstruction.

The gastrocnemius flap is a versatile flap when muscle, or, muscle and skin is desired for coverage of bone of the proximal one-third of the leg, and about the knee. Both the medial and lateral heads, or both combined, may be used as flaps. Typically, the reach of only the muscle will restrict reach to just below the knee. Release off the medial femoral condyle permits increased reach, and a bit more is obtained by scoring the deep fascia and gently slow expansion of the muscle.

Nationwide Analysis of Lower Extremity Periprosthetic Fractures.

BACKGROUND: Although the annual incidence of primary total joint arthroplasty is increasing, trends in the annual incidence of periprosthetic fractures have not been established. This study aimed to define the annual incidence of periprosthetic fractures in the United States. METHODS: Inpatient admission data for 60,887 surgically treated lower extremity periprosthetic fractures between 2006 and 2015 were obtained from the National Inpatient Sample database. The annual incidence of periprosthetic fractures was defined as the number of new cases per year and presented as a population-adjusted
