Anatomical Features in Lower-Lip Depressor Muscles for Optimization of Myectomies in Marginal Mandibular Nerve Palsy.

INTRODUCTION: Myectomies of the lower-lip depressor muscles, with the aim to improve facial balance in unilateral facial paralysis, have unexplained high recurrence rates. A potential explanation is that these recurrences are due to inadequate resection through the muscle width, leaving lateral muscle fibers intact. AIM: Revisit the anatomy of the lower-lip depressor muscles and suggest an optimization of the surgical technique. Perform a literature review to identify recurrence rates and surgical technique of the procedure. MATERIALS AND METHODS: Ten fresh hemifaces were dissected. The foll

FREQUENCY OF LINGUAL NERVE INJURY IN MANDIBULAR THIRD MOLAR EXTRACTION: A COMPARISON OF TWO SURGICAL TECHNIQUES.

BACKGROUND: Surgical removal of impacted mandibular third molar is associated with a number of complications including postoperative bleeding, dry socket, postoperative infection, and injury to regional nerves. Lingual nerve damage is one of the main complications. To prevent this complication different techniques had been used. Lingual flap reflection is one of these procedures in which lingual soft tissue is reflected and retracted deliberately, the nerve is identified and is kept out of the surgical field. The objective of this study was to evaluate a surgical technique for third molar remo

Technical communication: percutaneous radiofrequency mandibular nerve rhizotomy guided by high-speed real-time computed tomography fluoroscopy.

We present a new method of percutaneous radiofrequency mandibular nerve rhizotomy for pain relief in the mandibular region, in which needle placement is guided by high-speed real-time computed tomography (CT) fluoroscopy. Eleven patients (13 procedures) with idiopathic trigeminal neuralgia underwent the procedure. CT fluoroscopy simultaneously provided 3 slices (1-mm interval series, craniocaudally) in 1 fluoroscopic view, allowing for accurate needle placement. Trigeminal neuralgia improved in all patients without severe complications. The mean numerical rating scales of pain intensity (+ or

Paresthesia of the mental nerve stem from periapical infection of mandibular canine tooth: a case report.

Sensory disturbances such as paresthesia, anesthesia, hypoesthesia, and hyperesthesia may be present in the oral cavity. Paresthesia is defined as a burning or prickling sensation or partial numbness caused by neural injury. Paresthesia in dentistry can be caused by local or systemic factors. Local factors include traumatic injuries such as mandibular fractures, expanding compressive lesions (benign or malignant neoplasia and cysts), impacted teeth, local infections (osteomyelitis, periapical, and peri-implant infections), iatrogenic lesions after tooth extractions, anesthetic injection, endod

Incidence of cervical branch injury with "marginal mandibular nerve pseudo-paralysis" in patients undergoing face lift.

The anatomy of the cervical and marginal mandibular branches of the facial nerve is reviewed. In the senior author's practice, "pseudoparalysis of the marginal mandibular nerve" due to cervical branch injury occurred in 34 of 2002 superficial musculoaponeurotic system-platysma face lifts (1.7 percent) and was associated with a full recovery in 100 percent of cases within a time period ranging from 3 weeks to 6 months. Cervical branch injury can be distinguished from marginal mandibular nerve injury by the fact that the patient will be able to evert the lower lip because of a functioning mental

Mechanosensory and thermosensory changes across the border of impaired sensitivity to pinprick after mandibular nerve injury.

PURPOSE: The study goal was to determine how sensory function varies across the border of impaired sensitivity to pinprick in patients with mandibular nerve injuries. PATIENTS AND METHODS: Borders of decreased sensitivity to pinprick were mapped in 15 patients who reported altered sensation. Four mechanoreceptive, 2 thermoreceptive, and 2 thermonociceptive functions were studied at 5 sites separated by 0.6 cm across the border. The tests were repeated to evaluate day-to-day consistency in the pattern of variation for each sensory measure. RESULTS: The estimates of sensory function were not f

Ultrasound-Guided Mandibular Nerve Block in the Infratemporal Fossa for Awake Craniotomy: A Cadaveric Feasibility Study.

Awake craniotomies are an essential neurosurgical technique, allowing surgical intervention in adjacent areas while preserving eloquent brain areas such as Brodmann areas 1-4, Broca's area, and Wernicke's area. Central to the success of awake craniotomy is the provision of safe and effective analgesia throughout the procedure. The traditional scalp block for an awake craniotomy involves anatomical landmark localization of six nerves: supraorbital, supratrochlear, zygomaticotemporal, auriculotemporal, and greater and lesser occipital nerves. Employing adjunctive techniques via trigeminal nerve

Postoperative Rigid Cervical Collar Causing a Marginal Mandibular Branch of Facial Nerve Palsy: A Case Report.

CASE: We present a marginal mandibular branch of the facial nerve palsy that occurred in a 55-year-old woman from external compression of a rigid cervical collar. This nerve palsy occurred after less than 24 hours of collar utilization and was treated conservatively with delayed recovery. CONCLUSION: A marginal mandibular branch of the facial nerve palsy is a rare complication of cervical collar usage with only 1 other documented case in the literature. The resemblance of this palsy to a postoperative cerebrovascular accident makes it an important complication to consider when using a collar.

Operative technique for benign submandibular gland mass without identifying the mandibular branch of the facial nerve.

OBJECTIVE: The marginal mandibular branch of the facial nerve must be protected during surgery for benign diseases of submandibular gland. Methods for protecting the marginal mandibular branch include the nerve identification method and the non-identification method. METHODS: We performed submandibular gland surgery in 138 patients with benign submandibular gland diseases using the non-identification method to preserve the marginal mandibular branch. In brief, the submandibular gland capsule is incised at the inferior border of the gland and detached along the gland parenchyma. The nerve is p

Age-dependent impact of inferior alveolar nerve transection on mandibular bone metabolism and the underlying mechanisms.

Aging is associated with peripheral nerve degradation and bone destruction. The aim of the study is to elucidate the influence of sensory denervation on bone metabolism in different age groups by establishing a modified unilateral inferior alveolar nerve transection (IANT) model. The rats, divided into young, middle-aged and aged group, were sacrificed at 1, 2, 4 and 8 weeks after right IANT. The histological changes of mandibles were analyzed by fluorescent double labeling, micro-CT, HE, TRAP and anti-CGRP immunohistochemical staining. Molecular mechanisms underlying the changes were analyzed

Predictive Value of Panoramic Radiography for Injury of Inferior Alveolar Nerve After Mandibular Third Molar Surgery.

PURPOSE: The purpose of the present systematic review was to assess the added value of panoramic radiography in predicting postoperative injury of the inferior alveolar nerve (IAN) in the decision-making before mandibular third molar (MM3) surgery. MATERIALS AND METHODS: MEDLINE and EMBASE were searched electronically to identify the diagnostic accuracy of studies that had assessed the predictive value of 7 panoramic radiographic signs, including root-related signs (darkening of the root, deflection of the root, narrowing of the root, and dark and bifid apex of the root) and canal-related sig

Contralateral facial nerve palsy following mandibular second molar removal: is there co-relation or just coincidence?

Peripheral facial nerve palsy (FNP) is the most common cranial nerves neuropathy. It is very rare during dental treatment. Classically, it begins immediately after the injection of local anaesthetic into the region of inferior dental foramen and it's homolateral to the injection. Recovery takes a few hours, normally as long the anaesthetic lasts. The authors present a 44-year-old patient who presented a contralateral delayed-onset facial paralysis arising from dental procedure and discuss the plausible pathogenesis mechanism of happen and a possible relationship between dental procedure and co

Digital versus conventional panoramic radiography in predicting inferior alveolar nerve injury after mandibular third molar removal.

The aim of the study was to compare the accuracy of conventional and digital panoramic radiography (OPG) in relation to 4 specific high-risk signs (interruption of the superior cortical line, diversion, narrowing of the canal, and dark band of the root), which would indicate a close anatomic relationship between third molar roots and the inferior alveolar canal.Four hundred mandibular third molar surgical removals after conventional and 272 after digital radiographs were evaluated in the study. Submandibular approach through the submandibular gland fascia for treating mandibular fractures without identifying the facial nerve.

BACKGROUND: : The submandibular, or Risdon, approach is the classic, reliable extraoral approach for treating mandibular fractures. Although the intraoral approach has been used recently for open reduction and internal fixation, in some cases, such as comminuted fractures or fractures in an atrophic mandible, the submandibular approach is still prerequisite. Damage to the marginal mandibular branch of the facial nerve is the only concern other than skin scarring. METHODS: : To minimize the risk of neuropraxia, we prospectively assessed an approach for treating mandibular fractures at 24 surgi

An evaluation of clinical and electrophysiologic tests in nerve injury diagnosis after mandibular sagittal split osteotomy.

The yield of clinical sensory tests and electrophysiologic tests in the diagnostics of inferior alveolar nerve (IAN) damage after bilateral sagittal split osteotomy (BSSO) was studied. The diagnostic value of these tests was evaluated by comparing the test results to the degree of nerve damage at the end of the operation as documented by means of the intraoperative nerve conduction recording of the IAN. Twenty patients undergoing BSSO were analysed preoperatively and 2 weeks postoperatively. The frequency of the IAN disturbance ranged from 10% to 94% depending on the test method and the test s

Lingual nerve impairment/injury after retrieval of the displaced mandibular third molar into the floor of the mouth.

Retrieval of the displaced mandibular third molar in the floor of the mouth is challenging as the lingual nerve is always at risk of injury. However, there are no available data to show the incidence of the injury caused by the retrieval. The goal of this review article is to provide the incidence of the iatrogenic lingual nerve impairment/injury caused by the retrieval based on the review of the existing literature. The retrieval cases were collected with the search words below using PubMed, Google Scholar, and CENTRAL Cochrane Library database on October 6, 2021. A total of 38 cases of lingu

Key Anatomical Clarifications for the Marginal Mandibular Branch of the Facial Nerve: Clinical Significance for the Plastic Surgeon.

BACKGROUND: The marginal mandibular branch (MMBr) of the facial nerve is the least likely to recover from injury due to infrequent anastomosis with other branches. The MMBr has been described as coursing superior to the inferior border of the mandible. However, studies have reported variations in its location in embalmed and fresh specimens. It has been postulated that the embalming process may effect its anatomic position. OBJECTIVES: The aim of this study was to re-evalulate the location of the MMBr relative to the inferior border of the mandible in both fresh and embalmed cadavers, and inv
