[Complications of post-irradiation radiotherapy mandibular resection: problems of reconstruction (author's transl)].

One hundred and twenty-six patients underwent a mandibular resection (segmental or marginal) as a part in the treatment of a malignant lesion. The surgical morbidity is increased by the preoperative high-dose irradiation. Complications such as osteitis and/or fracture of the mandible were noted in 31 cases (a total of 80) who underwent a preoperative external irradiation with a tumor-dose of 5,500 rads or higher, whereas only in 8 cases (a total of 46) without such preoperative radiotherapy. Prophylactic antibiotics, the use of flaps to replace the buccal mucosa without any suture-tension, rep

Clinical application of the functional matrix: mandibular reconstruction and monobloc functional therapy.

A 13-year-old black girl with an extensive ameloblastoma of the mandible required subtotal mandibular resection and immediate reconstruction with use of an autogenous, ipsilateral, corticocancellous graft from the iliac crest and monobic functional therapy. Twenty-seven months later, clinical evaluation showed normal mandibular function and no facial asymmetry. Bony architectural changes after 27 months included reformation of the coronoid process, angular process, and external oblique ridge; preservation of mandibular length and thickness; and maintenance of the alveolar ridge height. Immediate Transoral Allogeneic Bone Grafting for Large Mandibular Defects. Less Morbidity, More Bone. A Paradigm in Benign Tumor Mandibular Reconstruction?

PURPOSE: Reconstruction of hard tissue continuity defects caused by ablative tumor surgery has been traditionally reconstructed with autogenous bone grafts or microvascular free flaps. Although results have been predictable from these 2 methods of reconstruction, the morbidity associated with bone harvest is quite serious for the patient. Predictable results have been obtained with using a combination of 100% cadaver bone, bone marrow aspirate concentrate (BMAC), and recombinant human bone morphogenic protein in immediate reconstruction for benign tumor extirpations through the extraoral appro

Reconstruction of lateral mandibular defect: a comparison of functional and aesthetic outcomes of bony reconstruction vs soft tissue reconstruction - long-term follow-up.

CONCLUSION: Reconstruction with a vascularized bone flap provided superior postoperative outcomes compared with reconstruction with a soft tissue free flap. However, patients obtained acceptable long-term functional outcomes even with a soft tissue free flap and this method is an option in selected patients. A multidisciplinary approach involving dental treatments and nutrition education is important to improve postoperative function. OBJECTIVE: To assess the postoperative outcomes of two different options for reconstruction of the lateral mandible using bony or soft tissue reconstruction. M

Nonvascularized bone grafts: how successful are they in reconstruction of segmental mandibular defects?

OBJECTIVE: Segmental mandibular defects can occur due to various etiologies, including trauma and tumor resection. Reconstruction should provide adequate support for subsequent dental rehabilitation and allow for proper occlusion. Nonvascularized bone grafts have been used for reconstructing mandibular defects in cases where vascularized grafts were not feasible. The objective of this study was to assess the success rate of these grafts in reconstruction of segmental defects of various sizes in the mandible. STUDY DESIGN: Fifty patients were included in this retrospective chart review. Length

Reconstruction of a maxillary defect using the remnant bone of mandibular angle ostectomy.

The buttress is the cornerstone of the midface both functionally and aesthetically. Therefore, fracture of the buttress requires surgery. A patient wanted cosmetic surgery simultaneously with a reduction of facial bone fracture. To achieve this, we reconstructed the defect of the maxillary buttress using the remnant bone of the mandible angle ostectomy. A 27-year-old man presented with a left maxilla fracture and defect at the maxillary buttress. Since the patient was considering cosmetic surgery to alter his square face, we planned a mandibular angle ostectomy and used the remnant bone as an

Experience with the prefabricated free fibula flap for reconstruction of maxillary and mandibular defects.

PURPOSE: We report our 11-year experience with a new technique to prefabricate the osteocutaneous free fibula flap to reconstruct defects of the maxilla and mandible not amenable to conventional methods of treatment. MATERIAL AND METHODS: We treated 11 patients aged 17 to 47 years with jaw defects using prefabricated free fibula grafts from 1994 to 2005. We prepared the fibula on the leg with a 6- to 8-mm muscle cuff; next we transferred the bone flap to the surface of the leg without severing the pedicle, and then covered the muscle almost circumferentially by partial thickness skin graft. The anatomical basis of the osteo-musculo-cutaneous trapezius flap in mandibular reconstruction.

The osteo-musculo-cutaneous trapezius flap has seen its indications in mandibular reconstruction reduced since the appearance of micro-anastomotic flaps. However, it may still be useful when the patient has marked atheroma contraindicating a free flap. While the arterial supply of this flap is well known, the same is not true for its venous drainage. The preservation of the accessory nerve and its limits in mandibular reconstruction has been little studied. We carried out a study on 30 subjects (60 cadaveric dissections of trapezius flaps) in order to address these questions. The study has bee
