Clinicopathological Factors Predicting Melanoma Recurrence: A Study From South East Ireland.

INTRODUCTION: Cutaneous malignant melanoma is a neoplastic transformation of melanocytes and a major cause of skin cancer-related mortality. Ireland, with its fair-skinned population, continues to see rising incidence rates, highlighting the importance of identifying clinicopathological predictors of recurrence. OBJECTIVES: To assess the relationship between excision margins and melanoma recurrence, with emphasis on anatomical distribution and clinicopathological features. METHODS: This retrospective observational study analyzed 565 patient records from University Hospital Waterford, Ireland

Interpretability of excisional biopsies of the cervix: cone biopsy and loop excision.

OBJECTIVE: To compare the results of cold knife conization (CKC) and loop electrosurgical excision procedure (LEEP) for cervical intraepithelial neoplasia to determine if excisional method has effects on pathologic interpretation. METHODS: Retrospective review of the perioperative medical records of patients who had a CKC and electrosurgical loop excision of cervix. Patients selected had either primary treatment for cervical intraepithelial neoplasia, suspected invasion, glandular abnormalities or discordant cytology. RESULTS: Among the eligible patients, 61 had CKC and 96 had LEEP. Overall,

The use of Mohs micrographic surgery (MMS) for melanoma in situ (MIS) of the trunk and proximal extremities.

BACKGROUND: Evaluation of the entire surgical margin results in high rates of complete excision, low local recurrence rates, and maximal tissue conservation. Although well recognized for melanoma of the head and neck, few studies have focused exclusively on the trunk and proximal extremities. OBJECTIVE: We sought to evaluate the efficacy of Mohs micrographic surgery for melanoma in situ (MIS) of the trunk and proximal extremities, and determine adequate excision margins for MIS when total margin evaluation is not used. METHODS: Long-term outcomes in 882 cases of MIS treated with Mohs microgr

"Personalized Excision" of Malignant Melanoma-Need for a Paradigm Shift in the Beginning Era of Personalized Medicine.

The premises on which guidelines for the excision of primary cutaneous melanoma are based are illogical and fail to take into account peculiarities of the individual lesion. The horizontal margins of excision continue to be adjusted to the vertical thickness of the neoplasm, and recommended clinical margins do not reflect the histopathologic borders of melanoma. Micrographically controlled surgery has become accepted for acral melanomas and melanomas of the face and neck but not for melanomas on the trunk, arms, and legs, although the latter tend to be more sharply confined. Extending margins

Impact of cavity shave margins in patients with ductal carcinoma in situ undergoing conserving breast surgery.

AIM: The main challenge during breast-conserving surgery (BCS) is to obtain clear margins, especially in patients with ductal carcinoma in situ (DCIS) due to the absence of well-defined nodules. Many surgical approaches have been used in an attempt to reduce the positive margin rate. The aim of this retrospective study is to compare the cavity shave margin technique with standard surgery and the intraoperative evaluation of surgical margins. METHODS: This is a single-center retrospective study analyzing margin status, need for re-excision, and surgical time in a cohort of 227 patients who und

Optimizing breast conservative surgery for ductal carcinoma in situ: Comparative outcomes of intraoperative ultrasound and wire localization in achieving negative margins.

BACKGROUND: Intraoperative ultrasound-guided surgery (IOUS) is an effective technique for invasive breast cancer, offering advantages over wire localization (WL), such as smaller resection volumes, lower rates of involved margins, and better patient satisfaction. However, evidence for ductal carcinoma in situ (DCIS) is limited. This study aimed to compare specimen margins and excision volume in DCIS treated with IOUS versus WL. MATERIAL AND METHODS: From February 2018 to December 2023, women diagnosed with DCIS and eligible for breast-conserving surgery guided by IOUS or WL were prospectively

Dermatofibrosarcoma Protuberans: A Rare Anatomical Location of Dermal Sarcoma.

Dermatofibrosarcoma protuberans (DFSP) is a rare, slow-growing cutaneous sarcoma that presents diagnostic and therapeutic challenges. This case report refers to a 47-year-old female patient who presented to the dermatologist with a stable, asymptomatic, firm lesion located on the proximal lower extremity. Local surgical excision was performed, and the specimen was sent for biopsy, where histopathological analysis, along with immunohistochemistry, confirmed the diagnosis of DFSP. Fluorescence in situ hybridization (FISH) analysis was also conducted, serving a supportive role in the detection of

Vulvar granular cell tumor.

Granular cell tumors are rare, usually benign, soft tissue neoplasms of neural origin. They occur more often in females than males, the peak age incidence is in the fourth through fifth decades. They can occur anywhere in the body with up to 15% situated in the vulva. The commonest presentation is as an asymptomatic mass. Microscopic findings are usually sufficient, but immunohistochemistry can also be helpful in confirming the diagnosis. The vulvar tumors are benign in 98% of cases with 2% reported as malignant. In this case report we describe a woman with a granular cell tumor confirmed by b

Skin Cancer: Management of Cutaneous Melanoma.

Melanoma is the fifth most common cancer in the United States and one of the deadliest. Tumor depth (Breslow depth) is the most important prognostic factor. Wide local excision is used to manage melanoma stage 0 (in situ) with 0.5- to 1-cm margins, as well as stage IA with 1-cm margins. For lentigo maligna (melanoma in situ), surgical margins wider than 0.5 cm are often necessary to achieve histologically negative margins, and comprehensive margin assessment techniques (Mohs surgery or staged excision) are associated with lower recurrence rates. Localized melanoma with a Breslow depth of less

Minimally Invasive Intact Excision of High-Risk Breast Lesions and Small Breast Cancers: The Intact Percutaneous Excision (IPEX) Registry.

BACKGROUND: Aiming to minimize overtreatment of high-risk breast lesions (HRLs), including atypical ductal hyperplasia, and small breast cancers, including ductal carcinoma in situ (DCIS), we investigated a minimally invasive (MI) approach to definitive diagnosis and management of these conditions. METHODS: In the prospective Intact Percutaneous Excision registry study, women aged 31-86 years had removal of small invasive cancers, DCIS, or HRLs using image-guided 12-20 mm radiofrequency basket capture (MI excision). Second-pass 20 mm basket capture obtained shaved margins in cancer patients.

Rate of residual tumor after repeat surgery for positive margins in ductal carcinoma in Situ, and predictive factors.

AIMS: To evaluate the rate of residual tumor in re-excision specimen of patients with positive margins in ductal carcinoma in situ (DCIS) following breast-conservative surgery, and to identify predictive factors of residual tumor. MATERIAL AND METHODS: We conducted a monocentric, retrospective study, from January 2010 to December 2020. All 103 patients who underwent re-excision for positive margins in DCIS following breast-conservative surgery for in situ or invasive breast carcinoma were included. Positive margins were defined as inferior to 2 mm from the DCIS component. Two groups were defi

Pleomorphic Lobular Carcinoma In Situ: Radiologic-Pathologic Features and Clinical Management.

BACKGROUND: Pleomorphic lobular carcinoma in situ (PLCIS) is an unusual variant of LCIS for which optimal management remains unclear. METHODS: We conducted a 15-year (2000-2014) retrospective chart review of the radiologic, pathologic, clinical management, and recurrence rates of patients with PLCIS on diagnostic biopsy. Fifty-one patients were found to have PLCIS either alone or with concomitant breast cancer. Of these, 23 were found to have pure PLCIS on diagnostic biopsy. Rates of upstaging after local excision, positive or close margins, mastectomy, and recurrence associated with pure ple

Does reduction of the oncologic safety margin for facial basal cell carcinoma result in higher recurrence rates?

BACKGROUND: Wide surgical excision is the gold standard for basal cell carcinoma (BCC) treatment. Typically, resection requires a safety margin ≥ 4 mm. We aimed to confirm BCC excisions' cancer recurrence rate and safety on the facial region with new safety margins. METHODS: We included patients with primary BCC on the facial region who underwent wide excision with 2- or 3-mm safety margins at our institution between January 2010 and December 2018. Medical records were reviewed to confirm the epidemiology and surgical information. Recurrence was confirmed by physical examination through regul

Prognostic impact of close margins on Loco-regional recurrence in ductal carcinoma in situ treated with breast-conserving surgery and radiotherapy.

INTRODUCTION: Current guidelines recommend achieving negative margins ≥2 mm for ductal carcinoma in situ (DCIS) treated with breast-conserving surgery (BCS) and radiotherapy (RT). However, the need for re-excision in cases with close (<2 mm), but negative margins remain controversial. We evaluated the prognostic impact of close margins on locoregional recurrence (LRR) and identified factors associated with recurrence. MATERIAL AND METHODS: We retrospectively analyzed 672 patients with pure DCIS who underwent BCS and adjuvant RT between 2004 and 2020. Patients were categorized into negative- a

Cutaneous leiomyosarcoma: a retrospective review of 45 cases.

Primary cutaneous leiomyosarcoma (LMS) is a rare soft tissue tumour type with two subtypes, dermal and subcutaneous. As deeper tumours confer a worse prognosis, they require a more aggressive approach. Conversely, a more conservative surgical approach for dermal LMS has been suggested. Few studies have comprehensively reported both clinical surgical and histological excision margins. Therefore, we sought to provide margin recommendations based on our experience and review of the existing literature. We undertook a retrospective case-note review (1998-2019) of cutaneous LMS management to establ

Single Institution Review of Patients With Prior Breast Augmentation Undergoing Breast Conservation Therapy for Breast Cancer.

BACKGROUND: Increasing number of patients with preexisting breast implants desire breast conservation therapy for breast cancer. There is paucity of comparative data on tumor margins and re-excisions in these patients. High re-excision rates up to 25% have been reported in breast conservation therapy patients; efforts to obtain cosmesis and avoid implant rupture might increase this further. We analyzed tumor margins, re-excision rates, and recurrence in previously augmented versus non-augmented patients undergoing lumpectomy for breast cancer. We preserved preexisting implants if feasible with

Immediate, optimal reconstruction of facial lentigo maligna and melanoma following total peripheral margin control.

BACKGROUND: Peripheral margin control of lentigo maligna and melanoma on the head and neck can be problematic. Frozen sections are unreliable, and conventional histopathology cannot examine the entire margin. Customary treatment involves wide excision and dressing care or skin graft coverage until histopathologic evaluation is complete, as reexcision is frequently required because of positive margins. Wound contraction, donor-site morbidity, and additional procedures before reconstruction are inherent disadvantages to this approach. METHODS: After excisional biopsy of facial lentigo maligna a

Mohs Micrographic Surgery for the Treatment of Cutaneous Melanomas of the Head and Neck.

Surgical excision achieving clear histologic margins remains the mainstay treatment for primary cutaneous melanoma. Tumors of the head and neck, particularly those arising in chronically sun-damaged skin, often demonstrate extensive and asymmetric subclinical extension. Over the decades, this has proven to be a significant problem for tumors arising on the head and neck, as anatomic and functional complexities of these areas have led to suboptimal surgical treatment, yielding unacceptably high rates of local recurrence and persistently positive margins with traditional wide local excision. Pat

Utilization of Tumor Markers for Intraoperative Specimen Radiography Does Not Decrease the Rate of Margin Positivity in Breast Conservation Surgery (BCS).

IntroductionMargin status after lumpectomy is crucial in determining risk of local recurrence. Current guidelines recommend "no ink on tumor" as adequate negative margins for invasive breast carcinoma. Still, up to 20% of patients may have positive margins and need re-excision. This study explored the use of a commercially available tumor marking system during specimen radiography and its effect on re-excision rates in breast conservation surgery.MethodsWith IRRB approval, a review was conducted of a prospectively collected cohort of 105 study patients and a retrospectively collected cohort of
