Prophylactic mastectomy and reconstruction: clinical outcomes and patient satisfaction.

BACKGROUND: The purpose of this study was to evaluate both clinical outcomes and satisfaction in patients who have undergone prophylactic mastectomy and breast reconstruction. METHODS: A 5-year retrospective analysis of the senior author's (S.L.S.) experience with breast reconstruction following prophylactic mastectomy was performed. Timing, type of mastectomy and reconstruction, complications, and cancer occurrence/recurrence were examined. Patients reported their level of satisfaction and willingness to undergo the procedure again. Aesthetic outcomes were graded by an independent and blinde

Surgical resection for persistent seroma, following modified radical mastectomy.

BACKGROUND: Seroma formation following modified radical mastectomy with axillary lymph node dissection for breast cancer is a most common wound complication. In our experience seroma occurs in approximately 50% of patients undergoing mastectomy. Postmastectomy seromas usually vanishes within a few weeks after operation. CASE PRESENTATION: In this report we present the case of a 73 year old woman who had undergone mastectomy with axillary lymph node dissection for breast cancer, complicated by lymphorrhea and formation fibrous encapsulated seroma resistant to conservative treatment which requi

Report of simultaneous off-pump CABG and modified mastectomy.

The patient was a 75-year-old female who had received medical treatment for effort angina. Recently, she noticed a left breast tumor, which was diagnosed as breast cancer with axillary lymph node swelling. Coronary angiography performed prior to the surgery for the breast cancer showed 90% stenosis in segment 6 of the left anterior descending artery (LAD). Coronary intervention was not possible due to anatomical reasons, so she was admitted for simultaneous surgery for the breast cancer and angina. The chest was opened through a median sternotomy and the full-skeletonized right internal thorac

Delayed nipple-sparing modified subcutaneous mastectomy: rationale and technique.

The problem of nipple-areola complex (NAC) preservation during mastectomy is a very intriguing and stimulating issue. In fact, in order to perform an oncologically safe operation, no mammary tissue (enclosed in the main galactophoric ducts) should remain; on the other hand, without the blood supply coming from the breast gland, NAC viability is greatly impaired because the surrounding vascular dermal network is not developed enough to support its metabolic requirements. We suggest therefore a two-step surgical procedure. The first step, on an outpatient basis with local tumescent anesthesia, i

Utility of four-quadrant random sections in mastectomy specimens.

Standard practice in surgical pathology dictates that random sections from the four quadrants of the breast be taken in mastectomy specimens. These sections are obtained in addition to sampling of any grossly visible lesions within the breast specimen. While tradition dictates the submission of these sections, we are unaware of any study supporting their efficacy. We have investigated the utility and significance of these random sections in a series of 78 mastectomy specimens. This retrospective study identified mastectomy specimens from pathology files of Magee Woman's Hospital, the Universit

Surgical treatment of gynecomastia: liposuction combined with subcutaneous mastectomy.

The purpose of the present work has been to evaluate surgical treatment of gynecomastia performed by liposuction combined with subcutaneous mastectomy. It was designed as a prospective consecutive registration of 21 patients (28 breasts) operated in a four month period. Treatment was done in local anaesthesia in the out-patient clinic. Treatment was in one patient complicated with a haematoma. In 86% of cases the patients were satisfied with the postoperative result. Liposuction combined with surgical excision of the gland performed as an out-patient treatment in local anaesthesia is followed

Factors influencing local recurrence of cancer after partial mastectomy.

From 1957 to 1975, 291 patients underwent partial mastectomy for carcinoma of the breast. In a follow-up period of 6 to 25 years, 32 patients had local recurrences. These 32 were matched with 32 patients without local recurrence but who had had the same treatment during the same periods of time for the same clinical stage of disease. Comparisons were made for age of the patient, family history, size of tumor, proximity of tumor to line of resection, involvement of lymph nodes, and prophylactic radiation. The average age of the patients with recurrence was 7 years less than those without, and t

[Hemostatic potential during the development of post-mastectomy edema].

The work deals with the study of the mosaic character of the hemostatic potential, connected with the development of a localized pathological process, taking postmastectomy edema in illustration. It is shown that this condition is an example of the manifestation of the RASK system mosaics in one organism in pathology. It should be pointed out that the degree of the development of the local DVS syndrome changes significantly some of the RASK system parameters at the level of the whole organism. This is evidence that normalization of the microcirculatory processes in the edematous extremity will

Post-mastectomy breast reconstruction using the inflatable tissue expander.

Breast reconstruction following mastectomy has previously relied on the insertion of a silicone gel implant or the use of a myocutaneous flap. We report the use of an innovation, the inflatable tissue expander, for both immediate and delayed breast reconstruction in 26 patients where soft tissue cover was inadequate to permit the use of the silicone implant. By serial inflation of the tissue expander with saline, sufficient tissue cover was achieved for a second operative placement of a silicone prosthesis of appropriate volume to match the normal breast. To date, 10 patients undergoing delaye

Breast carcinoma: pattern of recurrence and metastasis after mastectomy.

This is a retrospective review of 556 patients who had carcinoma of the breast (1971-1980). Among the 476 patients who had mastectomy, 38% have died and the median follow-up time of those who were alive was 53 months. When the axillary nodes were free of metastasis initially, 18% of the patients had relapse. Among the patients who had axillary nodal metastasis, 24% of those with 1-3 positive nodes, and 69% of those with greater than or equal to 4 positive nodes relapsed. Regarding specific type of first recurrence, about 10-30% had loco-regional relapse, 60-70% had distant metastasis and 10-30

Subclavian artery occlusion 42 years after mastectomy and radiotherapy.

Left subclavian artery occlusion developed in a patient 42 years after a left radical mastectomy and radiotherapy. The vascular supply was successfully reconstructed by a graft from the right subclavian artery to the left brachial artery. Clinical and experimental evidence has demonstrated that radiotherapy can damage large vessels [2,4,5]. It is our contention that radiotherapy was the causative factor in the case of subclavian artery occlusion reported herein.

[Comparison of quandrantectomy and Halsted's radial mastectomy. Personal cases].

The authors review all the breast diseases and tumours observed from 1972 through 1979 at the Division of Surgery, Hospital of Rapallo. On the basis of personal experience they discuss the possibility that, in the treatment of early breast cancer, a relatively conservative surgery (i.e. quadrantectomy), followed by postoperative radiotherapy and adjuvant chemotherapy in the N+ cases, give no worse results than standard radical mastectomy, in preventing local relapses and distant metastases, and therefore in conditioning both free-of-disease interval and survival of the patients, as many Author

Chronic pain after mastectomy: current knowledge and knowledge gaps.

Post-mastectomy pain syndrome (PMPS) is a frequent and burdensome complication of breast cancer surgery, manifesting as persistent pain following mastectomy, typically with neuropathic characteristics. This review synthesizes current knowledge on the epidemiology, prevention, and treatment of PMPS, and highlight knowledge gaps to inform future research. PMPS occurs in 28-52% of patients. It is associated with impairment in quality of life, and reductions in physical and psychosocial functioning. Consistently identified risk factors include younger age, extensive axillary surgery, total mastect

Impairment of upper extremity function following mastectomy and lumpectomy.

INTRODUCTION: Breast cancer surgery may lead to impairments in upper body function, which can negatively affect quality of life. The aim of this study is to evaluate the long-term impact of breast cancer surgery on arm and shoulder function. MATERIALS AND METHODS: This prospective multicentre study included patients who underwent mastectomy or lumpectomy between 2016 and 2019. Arm and shoulder function were assessed preoperatively, and at three and six months, one and two years postoperatively using the Disabilities of Arm, Shoulder and Hand (DASH) questionnaire, grip strength and range of mo

Awake endoscopic mastectomy in one-day-surgery: Preliminary results.

BACKGROUND: While enhanced recovery after surgery (ERAS) and awake monitored anaesthesia care (MAC) have been described to promote same-day discharge (SDD) in conventional mastectomy, their application in reverse-sequence endoscopic nipple-sparing mastectomy (R-E-NSM) has not yet been established. We present our protocol and early experience with awake R-E-NSM. METHODS: In this retrospective pilot case series, from February 2026 to April 2026, all consecutive awake R-E-NSM performed at Tor Vergata University Hospital were retrieved from a prospectively maintained database (Approval number 62.

Acute Ocular Pain Following Endoscopic Mastectomy: A Case Report.

Endoscopic breast surgery (EBS) is a widely adopted minimally invasive technique that offers improved cosmetic outcomes and faster recovery. However, the use of carbon dioxide insufflation can occasionally result in unexpected complications. We report a rare case of acute postoperative ocular pain with suspected transient intraocular pressure elevation following EBS in a healthy 36-year-old female. The symptoms were likely due to CO₂ diffusion into subcutaneous tissue and Trendelenburg positioning, which contributed to venous congestion and elevated episcleral venous pressure. Conservative man

Predictive models for breast cancer-related lymphedema after mastectomy.

OBJECTIVES: To establish a nomogram incorporating clinical characteristics to predict the risk of breast cancer-related lymphedema (BCRL). METHODS: In this retrospective study, we included 200 consecutive patients with breast cancer undergoing radical mastectomy from January 2022 to December 2023. Of these, 98 patients diagnosed with BCRL were designated as the experimental group, while 102 patients served as the control group. Logistic regression analyses were conducted to explore factors associated with clinical prognosis and to construct and validate a nomogram for predicting the risk of B
