Initiation and Receipt of Medication for Opioid Use Disorder Among Adolescents and Young Adults in 4 State Medicaid Programs in 2018: Improving Medicaid Quality Metrics.

BACKGROUND: Medications for opioid use disorder (MOUD) in youth can reduce harms but many youths do not receive MOUD. Improving quality metrics of MOUD among youth can advance interventions for youth with opioid use disorder (OUD). METHODS: We relied on 2018 Medicaid claims data from California, Colorado, Massachusetts, and New Mexico. We calculated the percentage of youth with OUD included in the quality metric for initiation, and the percentage who initiated by state. We also calculated the percentage excluded from the quality metric for initiation because they have an existing episode of O

Impact of Medicaid Expansion on Pancreatic Adenocarcinoma Survival in the Southern US.

BACKGROUND: The Affordable Care Act and the Recalcitrant Cancer Research Act are health policies that can impact cancer survival. Of the 10 states that did not adopt Medicaid expansion, 7 are in the South. Here, we examine these policies' impact on pancreatic cancer treatment and survival. STUDY DESIGN: The National Cancer Database was used, identifying patients with pancreatic adenocarcinoma between 2008 and 2022. Trend analysis was conducted to evaluate changes in treatment over time. Survival analysis and logistic regression were used. RESULTS: The rate of pancreatectomy remained stable:

Evaluating Medicaid Coverage for Over-the-Counter Treatments for Diaper Dermatitis.

Diaper dermatitis is a common dermatologic condition in infancy, frequently managed with over-the-counter (OTC) products. We conducted a cross-sectional review of Medicaid Preferred Drug Lists across 49 states and the District of Columbia to evaluate coverage of 14 commonly used OTC diaper dermatitis treatments. Coverage varied widely by state and product class, with therapeutic agents, such as topical antifungals, low potency corticosteroids, and antibiotics, more frequently covered than preventive barrier products, such as petrolatum and zinc oxide. These findings reveal significant state-le

Enrolled and unaware: characteristics of Medicaid enrollees misreporting their coverage Status.

INTRODUCTION: While surveys suggest that many Medicaid enrollees mistakenly report that they do not have Medicaid coverage, little is known about this population. Enrollment misreporting could be associated with poorer access if enrollees delay or avoid needed care because they incorrectly believe they cannot afford it. METHODS: We exploited the COVID-19 Medicaid continuous coverage provision to identify sociodemographic and health characteristics of enrollees who initially reported Medicaid but subsequently misreported their coverage, and to characterize their patterns of health care access,

Medicaid Expansion and Optimal Starts of Treatment for Incident Kidney Failure.

RATIONALE & OBJECTIVE: Beginning in 2014, some states expanded Medicaid eligibility to include additional low-income adults under the age of 65 years. Examining whether the expansion led to improvements in optimal treatment at the start of kidney failure has been understudied and was the focus of this investigation. STUDY DESIGN: Observational cohort study. SETTING & PARTICIPANTS: Patients aged 26-64 years represented in the US Renal Data System initiating treatment for kidney failure between 2008 and 2019. EXPOSURE: Residence in states that did or did not expand Medicaid in 2014. OUTCOME:

Impact of Place on Dental Use by Medicaid-Enrolled Adults in Massachusetts.

INTRODUCTION: Place-level factors influence oral health disparities, yet research predominantly focuses on individual-level determinants. This study examines how neighborhood characteristics influence dental service utilization among Medicaid-enrolled adults in Massachusetts. METHODS: This retrospective study analyzed data from adult Medicaid beneficiaries obtained from the 2015 Massachusetts All-Payer Claims Database, which includes dental, medical, and member eligibility information. ZIP Code Tabulation Area (ZCTA)-level Social Vulnerability Index (SVI) scores for 4 themes (socioeconomic st

Adult Medicaid Coverage for Periodontal Treatment: A State-to-State Comparison.

Dental care remains an optional benefit for adults with Medicaid, with coverage varying across states. Recent expansions increased periodontal service coverage, but the scope of coverage and the policies that govern that scope remain unknown. This study analyzed coverage policies and fees for four periodontal services across 43 Medicaid programs between January and March 2024. Frequency limitations were the most common coverage policies identified, followed by prior authorization requirements, clinical requirements, and quadrant limitations. Veteran Health Administration fees were roughly thre

Association of Medicaid expansion with children's insurance coverage and healthcare utilization.

INTRODUCTION: In 2014, multiple states expanded Medicaid coverage to low-income adults under the Affordable Care Act. We evaluated the association of Medicaid expansion (ME) with children's insurance coverage and health service utilization. METHODS: We compared changes in children's insurance coverage and health service utilization between 2010-2013 and 2014-2016 in expansion and non-expansion states. Repeated cross-sectional analysis with linear difference-in-differences (DID) models was conducted using the Medical Expenditure Panel Survey. RESULTS: Medicaid expansion was not associated wit

Medicaid Reimbursement for Maternal Depression Screening and Care for Postpartum Depression.

IMPORTANCE: Clinical guidelines recommend routine screening for postpartum depression (PPD) during well-child visits. All but 5 US states provide Medicaid reimbursement to pediatric practitioners for screening mothers for PPD during well-child visits, but little is known about how reimbursement affects screening, diagnosis, and treatment. OBJECTIVE: To evaluate the association between Medicaid reimbursement for PPD screening during well-child visits and screening, diagnosis, and treatment for mood or anxiety disorders in the postpartum year. DESIGN, SETTING, AND PARTICIPANTS: This cohort stu

Pronounced State-Level Disparities in Prescription of Cannabinoids to Medicaid Patients.

INTRODUCTION: Dronabinol is approved in the USA for chemotherapy-induced nausea as well as vomiting and HIV-induced anorexia, while cannabidiol is primarily approved for childhood epileptic disorders Lennox-Gastaut and Dravet syndrome. The use pattern for these prescription cannabinoids in the USA is unknown. This study examined Medicaid claims for two FDA-approved prescription cannabinoids, dronabinol and cannabidiol, approved in 1985 and 2018, respectively, from 2016-2020 to better understand the pharmacoepidemiologic trends and distribution of these drugs in US Medicaid amidst the increasin

Sociodemographic differences in quality of treatment to Medicaid enrollees receiving buprenorphine.

Background: Buprenorphine is a key medication to treat opioid use disorder, but little is known about how treatment quality varies across sociodemographic groups. Objective: We examined measures of treatment quality and explored variation by sociodemographic factors. Methods: We used Medicaid MAX data from 50 states from 2006 to 2014 to identify buprenorphine treatment episodes (N = 317,494). We used multivariable logistic regression to examine the quality of buprenorphine treatment along four dimensions: (1) sufficient duration, (2) effective dosage, and concurrent prescribing of (3) opioid a

Impact of Medicaid Expansion on Rhinologic Cancer Presentation, Treatment, and Outcomes.

OBJECTIVE: The United States Patient Protection and Affordable Care Act allocated funds for states to expand Medicaid coverage. However, several states declined expansion. We aim to determine whether Medicaid expansion is associated with healthcare coverage, cancer stage at diagnosis, treatment, and survival among patients with rhinologic cancer. Rhinologic cancer was defined to include cancer of the nasal cavity, paranasal sinus, nasopharynx, or olfactory nerve. STUDY DESIGN: Cohort study. METHODS: Patients diagnosed with primary rhinologic malignancies between 2007 to 2016 were extracted f

Adherence to Telemonitoring Therapy for Medicaid Patients With Hypertension: Case Study.

BACKGROUND: Almost 50% of the adults in the United States have hypertension. Although clinical trials indicate that home blood pressure monitoring can be effective in managing hypertension, the reported results might not materialize in practice because of patient adherence problems. OBJECTIVE: The aims of this study are to characterize the adherence of Medicaid patients with hypertension to daily telemonitoring, identify the impacts of adherence reminder calls, and investigate associations with blood pressure control. METHODS: This study targeted Medicaid patients with hypertension from the

Opioid Prescribing Safety Measures in Medicaid Enrollees With and Without Cancer.

INTRODUCTION: Opioid prescribing safety among individuals with cancer is poorly understood. This study estimates the prevalence of Pharmacy Quality Alliance opioid measures among individuals with cancer undergoing or not undergoing active treatment versus those without cancer. METHODS: Pennsylvania Medicaid data (2016) were analyzed in 2018 to identify adults aged 18-64 years with and without cancer diagnoses who had 2 or more opioid prescriptions. Active cancer treatment, defined as having chemotherapy, radiotherapy, cancer surgery, or hospitalization with a primary diagnosis of cancer, was

The Complex Needs of Medicaid Expansion Enrollees with Very Low Incomes.

ISSUE: Access to health care, use of services, and patient outcomes can be complicated by many medical and nonmedical factors. People facing complex challenges such as mental illness, housing insecurity, or substance use, however, are not a homogeneous group; different individuals have different needs. GOALS: To understand the needs of people with very low income--no more than 75 percent of the federal poverty level--who enrolled in Medicaid under Minnesota’s expansion of the program prior to the Affordable Care Act. METHODS: The authors analyzed data on nondisabled, childless adults in the

Prevalence and management of patent ductus arteriosus in a pediatric medicaid cohort.

BACKGROUND: Widespread use of echocardiography has made earlier diagnosis of patent ductus arteriosus (PDA) possible, but pharmacological or surgical intervention is highly variable. Herein, we investigate the prevalence of PDA and its management in a routine care system. METHODS: A 15-year retrospective dataset (1996-2010) was analyzed. Selection criteria included age ≤17 years, enrollees in South Carolina State Medicaid, and diagnosed as having PDA on 1 or more service visits to a pediatrician or pediatric cardiologist. RESULTS: The 15-year treated prevalence rate of PDA was 0.25/1000 pedi

Effect of the Women's Health Initiative on osteoporosis therapy and expenditure in Medicaid.

UNLABELLED: Decreasing HRT use among postmenopausal women may have a reciprocal impact on other osteoporosis therapy. Time series analysis of prescribing trends for millions of Medicaid beneficiaries revealed a 57% decline in HRT without augmenting the pace of bisphosphonate use. Prescribing changes dramatically increased Medicaid spending on osteoporosis therapy over the last decade and requires further evaluation of cost effectiveness. INTRODUCTION: Hormone replacement therapy (HRT) has been commonly prescribed to postmenopausal women, but its use is decreasing because adverse cardiac outco
