The nurse's role in the prevention of cervical cancer among underserved and minority populations.

Since the implementation of the Papanicolaou test, there has been a significant decline in the incidence of cervical cancer over the last 50 years. Despite this reduction, each year there are approximately 11,000 women in the United States diagnosed with cervical cancer, the second most common type of cancer in women worldwide. Infection with oncogenic human papillomavirus (HPV) is necessary for the development of precancerous lesions and the progression to cervical cancer. For those diagnosed with an HPV infection or cervical cancer, a considerable personal and financial burden often results.

Imaging of intracranial aneurysms causing isolated third cranial nerve palsy.

Isolated third cranial nerve palsies may be caused by compressive intracranial aneurysms located at the junction of the internal carotid and posterior communicating arteries or, less commonly, at the apex of the basilar artery or its junction with the superior cerebellar or posterior cerebral arteries. Such aneurysms typically measure at least 4 mm in diameter. Technical improvements in noninvasive techniques, including CT and MRA, have yielded a detection rate of such aneurysms that approaches that of catheter cerebral angiography (CCA), which itself carries a small but serious risk. Controlling postpartum hemorrhage after home births in Tanzania.

OBJECTIVES: Determine safety of household management of postpartum hemorrhage (PPH) with 1000 microg of rectal misoprostol, and assess possible reduction in referrals and the need for additional interventions. METHODS: Traditional birth attendants (TBAs) in Kigoma, Tanzania were trained to recognize PPH (500 ml of blood loss). Blood loss measurement was standardized by using a local garment, the "kanga". TBAs in the intervention area gave 1000 microg of misoprostol rectally when PPH occurred. Those in the non-intervention area referred the women to the nearest facility. RESULTS: 454 women in

The Effects of Environmental Health Literacy and Climate Change-Health Perception on Climate Change-Health Nursing Practice Among Korean Community Health Practitioners: A Descriptive Correlational Study.

BACKGROUND: The climate crisis is among the most pressing public health threats of the 21st century, and the climate-health competencies of primary care nursing personnel serving medically underserved areas are a critical determinant of community resilience. Korean Community Health Practitioners (CHPs), established in 1980 under the Special Measures Act for Public Health and Medical Services in Rural Areas, have provided primary health care (PHC) in medically underserved rural regions for over 40 years; however, their environmental and climate-health competencies have rarely been studied. A Contract-based System for Physician Practice in Their Home Region in Japan: Follow-up Data of Graduates from Jichi Medical University (1978-2021).

INTRODUCTION: Jichi Medical University (JMU) was founded in Japan in 1972 to provide tuition-free medical education in exchange for a nine-year service commitment in graduates' home prefectures, with the mission of training physicians for rural practice. The long-term outcomes of this model remain incompletely characterized. We aimed to examine the long-term outcomes of JMU graduates. METHODS: This descriptive, retrospective, repeated cross-sectional study followed JMU graduates from 1978 onward, with assessments in 2011, 2016, and 2021. The primary outcome was the proportion of graduates pra

Beyond the surface: a collaborative effort for free skin cancer screenings in an underserved community.

BACKGROUND: Skin cancer rates continue to rise in underserved populations, particularly among Hispanics who often present with later-stage diagnoses. While photoprotective practices and regular skin examinations can mitigate risks, barriers such as limited dermatologist access and insufficient awareness persist. This study aimed to evaluate the effectiveness of a community-based educational intervention in improving skin cancer awareness and prevention behaviors in an underserved U.S.-Mexico border community. METHODS: We conducted a community-based educational intervention combining bilingual

Cost-effectiveness analysis of a virtually administered pain coping skills training intervention in women with breast cancer in underserved areas.

OBJECTIVES: Women with cancer who live in medically underserved areas could benefit from behavioral pain interventions, but access is limited. A randomized trial reported that a 4-session virtual program incorporating pain coping skills training (mPCST) was effective in improving pain outcomes compared to an attention-control condition. We performed a cost-effectiveness analysis of mPCST vs. control. METHODS: Data on medical resource use, therapist time, and participants' attendance at intervention sessions and time associated with travel and using a mobile app were collected. The 5-level Eur

Healthcare contact and treatment uptake following hepatitis C virus screening and counseling among rural Appalachian people who use drugs.

BACKGROUND: Hepatitis C virus (HCV) remains a major contributor to morbidity and mortality worldwide. Since 2009, Kentucky has led the United States in cases of acute HCV, driven largely by injection drug use in rural areas. Improved treatment regimens hold promise of mitigating the impact and transmission of HCV, but numerous barriers obstruct people who inject drugs (PWID) from receiving care, particularly in medically underserved settings. METHODS: 503 rural people who use drugs were recruited using respondent-driven sampling and received HCV screening and post-test counseling. Presence of

Scaling up proven innovative cervical cancer screening strategies: Challenges and opportunities in implementation at the population level in low- and lower-middle-income countries.

The problem of cervical cancer in low- and lower-middle-income countries (LLMICs) is both urgent and important, and calls for governments to move beyond pilot testing to population-based screening approaches as quickly as possible. Experiences from Zambia, Bangladesh, Guatemala, Honduras, and Nicaragua, where scale-up of evidence-based screening strategies is taking place, may help other countries plan for large-scale implementation. These countries selected screening modalities recommended by the WHO that are within budgetary constraints, improve access for women, and reduce health system bot

Cardiovascular Health Practices Among Black Patients in an Urban Underserved Clinic.

BACKGROUND: An understanding of cardiovascular health practices among black patients can provide more comprehensive patient-centered care and reduce health disparities. We studied home blood pressure monitoring in our underserved patient population to determine feasibility of providing automatic blood pressure monitors to our hypertensive patients. We believed that if we provided blood pressure monitors for home use, our patients would increase home blood pressure assessments, frequency of blood pressure assessments, and provide more data on managing their hypertension. METHODS: Forty patient

Who prescribes buprenorphine for rural patients? The impact of specialty, location and practice type in Washington State.

We determined the specialty, geographic location, practice type and treatment capacity of waivered clinicians in Washington State. We utilized the April 2011 Drug Enforcement Agency roster of all waivered buprenorphine prescribers and cross-referenced the data with information from the American Medical Association and online resources. Waivered physicians, as compared to Washington State physicians overall, are more likely to be primary care providers, be older, less likely to be younger than 35 years, and more likely to be female. Isolated rural areas have the lowest provider to population ra

Breast cancer in limited-resource countries: an overview of the Breast Health Global Initiative 2005 guidelines.

Breast cancer is the most common cause of cancer-related death among women worldwide, with case fatality rates highest in low-resource countries. Despite significant scientific advances in its management, most of the world faces resource constraints that limit the capacity to improve early detection, diagnosis, and treatment of the disease. The Breast Health Global Initiative (BHGI) strives to develop evidence-based, economically feasible, and culturally appropriate guidelines that can be used in nations with limited health care resources to improve breast cancer outcomes.
