Appropriateness of proton pump inhibitors prescription in patient admitted to hospital: An observational study.

OBJECTIVES: The objective of this study was to assess the relevance of proton pump inhibitors prescribing in patients entering a ward with a clinical pharmacist and therefore identifying inappropriate prescribing. METHODS: A prospective study was conducted for 4 months. Patients admitted to the hospital for elective or emergency surgery, who had medication reconciliation performed by the clinical pharmacy team and who were prescribed proton pump inhibitors before admission were included. The indication for the proton pump inhibitors was collected from the patient or inferred from the medical

The 3 D's of geriatric psychiatry: depression, delirium, and dementia.

A Caucasian female octogenarian with multiple medical problems was admitted to the inpatient geriatric psychiatry unit with intermittent altered mental status and decline in memory. She had been hospitalized four times in the previous three months. She was admitted on more than 10 medications and received more than 20 different medications in this time period. It was determined that she had delirium concurrent with dementia and/or depression. During her hospital stay a urinary tract infection (UTI) was treated, her anticholinergic medications were minimized, and her digoxin dose was adjusted.

Ciprofloxacin-Induced Unilateral Tremor and Weakness Mimicking Acute Ischemic Stroke: A Case Report.

Fluoroquinolones are widely prescribed antibiotics and are generally well-tolerated; however, they are associated with rare but clinically significant neurological adverse effects that may mimic acute cerebrovascular events. We describe the case of a 50-year-old man who developed intermittent unilateral coarse tremors and weakness of the left upper limb approximately two weeks after initiating ciprofloxacin therapy following endoscopic retrograde cholangiopancreatography (ERCP) for suspected choledocholithiasis. The acute presentation prompted a Code Stroke activation. Initial computed tomogra

Management of Urinary Retention in Older Women.

Urinary retention in older women is a clinically significant and often underdiagnosed condition with complex and multifactorial etiology. Age-related changes in bladder contractile function and bladder sensation, combined with common comorbidities such as neurologic disease, diabetic bladder dysfunction, and pelvic organ prolapse, substantially increase the risk of retention in this population. Medications commonly prescribed to older adults, including anticholinergics and antidepressants, are an often overlooked cause of urinary retention. Evaluation includes a thorough history, physical exam

Sodium-Glucose Co-Transporter-2 (SGLT2) Inhibitor-Related Euglycemic Diabetic Ketoacidosis: A Case Series.

Objectives: Sodium-glucose transporter-2 inhibitors (SGLT2i) are commonly used for the treatment of Type 2 Diabetes Mellitus, offering additional benefits in non-diabetic patients with conditions such as chronic kidney disease and heart failure. However, SGLT2i have been associated with an increased risk of euglycemic diabetic ketoacidosis (DKA). This case series describes three cases of patients who developed euglycemic DKA while taking SGLT2i. Key Findings: Each of the three patients with euglycemic DKA were taking SGLT2i for the treatment of diabetes and all had additional risk factors for

Angiotensin-Converting Enzyme (ACE) Inhibitor-Induced Angioedema of the Small Bowel: A Diagnostic Dilemma.

BACKGROUND: Angioedema of the tongue, oral mucosa, and pharynx is a highly visible and easily diagnoseable side effect of Angiotensin-converting enzyme inhibitors (ACEI). Angioedema of the small bowel is a rarer, and underrecognized, adverse event that may present as a diagnostic challenge due to its nonspecific symptoms and lack of visibility, and because of a general lack of awareness of it among physicians. Failure to consider ACEI-induced angioedema of the small bowel in differential diagnoses may result in unnecessary interventions and delay of treatment. METHODS: We describe the case of

House Calls.

The demand for house calls is increasing because of the aging U.S. population, an increase in patients who are homebound, and the acknowledgment of the value of house calls by the public and health care industry. Literature from current U.S. home-based primary care programs describes health care cost savings and improved patient outcomes for older adults and other vulnerable populations. Common indications for house calls are management of acute or chronic illnesses, coordination of a post-hospitalization transition of care, health assessments, and end-of-life care. House calls may also includ

Management of anxiety and depression syndromes in the elderly.

Elderly patients are susceptible to disruptions in mood from a variety of sources: primary anxiety or mood disorders, medical illnesses and treatments, and psychosocial stressors. The goals of intervention are to discover reversible etiologies, treat in the least intrusive manner, and improve or restore quality of life. The clinical presentation may vary from typical anxiety or depression, to mixed symptoms, to a highly somatized or dementia-like picture. The most common disturbance is mixed anxiety/depression. Once the threshold is reached for drug therapy, there are safe and effective remedi

Development and evaluation of a clinical pharmacist-led full-course management model for venous thromboembolism prophylaxis in orthopedic trauma patients: study protocol for a randomized controlled trial.

BACKGROUND: Venous thromboembolism (VTE) is a common and potentially fatal complication in orthopedic trauma patients, particularly following lower limb and hip-related fractures. While pharmacological prophylaxis is the cornerstone of prevention for patients at moderate to high risk, its use is a balance between efficacy and bleeding risk. Clinical pharmacists play an increasingly important role in optimizing pharmacotherapy, but evidence for the effectiveness of a comprehensive, pharmacist-led management model spanning both inpatient and outpatient care for VTE prophylaxis in this population

A systematic review investigating the role and impact of pharmacist interventions in cardiac rehabilitation.

BACKGROUND: Cardiovascular disease (CVD) is a predominant cause of mortality. Pharmacists play an important role in secondary prevention of CVD, however, their role in cardiac rehabilitation is under-reported and services are under-utilised. AIM: To explore the role of pharmacists in cardiac rehabilitation, the impact of their interventions on patient outcomes, and prospects of future role development. METHOD: Databases searched were PubMed, Embase, Medline, Cochrane Library, Cumulative Index to Nursing and Allied Health Literature (CINAHL), and PsycINFO from January 2006 to October 2021. Ra

Integrated drug management for elderly patients with congestive heart failure: enhancing medication accuracy using the MINE (Medicine IN gEriatric) application.

BACKGROUND: This study evaluated the impact of the Medicine IN Geriatric (MINE) application, which integrates the Integrated Medicine Management (IMM) model and STOPP/START criteria, on improving prescribing practices in elderly patients with congestive heart failure (CHF). METHODS: A two-phase study was conducted: validation of the IMM model and its implementation via the MINE app. A quasi-experimental pretest-posttest control group design was used in a hospital in East Kalimantan, Indonesia. Patients aged 60-79 years were randomly assigned to the intervention or control group. The intervent

Greater burden of chronic comorbidities and co-medications among people living with HIV versus people without HIV in Japan: A hospital claims database study.

OBJECTIVE: This study examined the prevalence of chronic comorbidities and the use of co-medications among people living with HIV (PLWH) on antiretrovirals in Japan, compared with age-matched controls without HIV. METHODS: This was an observational, retrospective, cross-sectional study using a hospital claims database of Japanese hospitals with advanced medical capabilities (i.e., advanced treatment hospitals, general hospitals, acute care hospitals). We extracted data for PLWH aged ≥18 years with a prescription record of antiretrovirals between January 2010 and December 2015, and for age-, s

Assessing the implementability of clinical pharmacist interventions in patients with chronic kidney disease: an analysis of systematic reviews.

BACKGROUND: Pharmacy practice studies have been criticized for presenting poor intervention description, which has serious implications when attempting to replicate interventions elsewhere. Evidence of improved outcomes for patients with chronic kidney disease (CKD) following pharmacist intervention exist in the literature, but similar concerns with intervention description have been raised. OBJECTIVE: To assess the implementability of evidence-based clinical pharmacist interventions in patients with CKD, based on the information contained in the published manuscripts. METHODS: PubMed was se

Ethical Issues Surrounding Facial Feminization Surgery: Five Major Considerations.

Facial feminization surgery (FFS) is a form of gender-affirming care for the transgender population that is currently a highly debated topic both inside and outside of the medical community. Currently, a paucity of information is available in plastic surgery literature on ethical issues surrounding FFS. In this paper, we discuss 5 major ethical considerations for plastic surgeons with regard to FFS: (1) how society's changing view of gender has impacted the importance of FFS; (2) whether FFS is medically necessary and should be covered by insurance; (3) to what extent resources should be inves

Effect of house officer clerking on short-term outcomes in paediatric orthopaedic patients.

AIM: Children who are admitted with acute orthopaedic problems are often fit and healthy. The house officer (ie, intern) is required to carry out the formal clerking/admission of inpatients. However, they are often also busy with sick ward patients, which leads to delays in formal clerking of these children until after they have been to the operating theatre (OT). The aim of this study was to assess what information, if any, is missed in children who were seen by a house officer prior to going to OT, compared to those who were not. METHODS: All patients admitted to the paediatric surgical war

Home Parenteral Nutrition and Intravenous Fluid Errors Discovered Through Novel Clinical Practice of Reconciling Compounding Records: A Case Series.

Home parenteral nutrition (HPN) and home intravenous (IV) hydration are complex, high-risk life-sustaining therapies for children and adults with severe digestive disorders. HPN compounding errors have the potential to cause serious patient harm. Here we present a retrospective case series at an interdisciplinary pediatric HPN program that includes specialized pharmacists reconciling prescriptions against commercial compounding records. Seven HPN or IV hydration patients were affected by significant errors in anion balance, copper, potassium, sodium, and infusion volume. Outpatient pharmacists

Codesigning a pharmacist-led transition of care program in the emergency department: a qualitative participatory study.

INTRODUCTION: Transitions from the emergency department (ED) to home are high-risk periods for medication-related harm. Pharmacist-led interventions may improve medication safety and care continuity, however, co-designed approaches to developing such interventions remain limited. AIM: This study aimed to co-design a pharmacist-led transition-of-care (ToC) program to improve medication management for adults discharged from the ED, with the intention of subsequently evaluating it in a pilot randomized controlled trial. METHOD: A two-phase qualitative study was undertaken at a general teaching

Is higher psychotropic medication burden associated with involuntary treatment under the Mental Health Act? A four-year Australian cohort study.

BACKGROUND: Involuntary treatment for individuals who lack sufficient capacity to make informed decisions regarding treatment has been associated with increased rates of injectable antipsychotics, antipsychotic polytherapy, and/or high doses. However, little is known about non-antipsychotic psychotropic prescription, or psychotropic medication burden as a more encompassing approach for people treated involuntarily. The aim of this study was to examine the relationship between Mental Health Act (MHA) status and psychotropic polypharmacy and/or high-dose medication prescribing practices in an Au
