Treatment of distal metaphyseal tibia fractures using an external fixator in children.

INTRODUCTION: Current treatment of pediatric distal metaphyseal tibial/fibular fractures is challenging due to poor skin and soft tissue coverage and limited blood supply to the distal tibia area in children. It remains unknown whether the SK combined external fixator (made by Double Medical Technology Inc., China) is effective for the treatment of distal metaphyseal tibia/fibula fractures in children. HYPOTHESIS: We hypothesized that SK combined external fixator could achieve satisfying outcomes for pediatric distal metaphyseal tibia/fibula fractures. PATIENTS AND METHODS: A total of 19 ped

The Risk of Assuming Abuse in an Infant with an Isolated Metaphyseal Lesion: A Case Report.

CASE: The classic metaphyseal lesion, or corner fracture, is considered a strong indicator of nonaccidental trauma. In the present case, the parents brought their 8-month-old boy to a community hospital after he stopped crawling on the right leg. A knee radiograph demonstrated an isolated metaphyseal lesion, which prompted concern for child abuse. The patient was transferred to the nearest academic medical center; the pediatric radiology expert in child abuse determined that the lesion was a normal variant and not the result of abuse. CONCLUSION: An isolated metaphyseal lesion is not pathogno

Embodied free will beliefs: some effects of physical states on metaphysical opinions.

The present research suggests that people's bodily states affect their beliefs about free will. People with epilepsy and people with panic disorder, which are disorders characterized by a lack of control over one's body, reported less belief in free will compared to people without such disorders (Study 1). The more intensely people felt sexual desire, physical tiredness, and the urge to urinate, the less they believed in free will (Study 2). Among non-dieters, the more intensely they felt hunger, the less they believed in free will. However, dieters showed a trend in the opposite direction (St

The early phase influence of bone marrow concentrate on metaphyseal bone healing.

Bone marrow concentrate (BMC) contains high densities of progenitor cells. Therefore, in critical size defects BMC may have the potency to support bone healing. The aim of this study was to investigate the effect of BMC in combination with calcium phosphate granules (CPG) on bone defect healing in a metaphyseal long bone defect in mini-pigs. A metaphyseal critical-size bone defect at the proximal tibia of 24 mini-pigs was filled with CPG combined with BMC, CPG solely (control group) or with an autograft. Radiological and histomorphometrical evaluations after 6 weeks (42 days) showed significan

Metaphyseal chondromatosis combined with D-2-hydroxyglutaric aciduria in four patients.

We report four patients who presented with a severe form of metaphyseal chondromatosis in association with D-2-hydroxyglutaric aciduria (D-2-HGA). All patients showed splaying columns of irregular ossification defects with bulbous metaphyses of the long tubular bones, as well as remarkable involvement of the short tubular and flat bones. The vertebral bodies revealed platyspondyly with irregular, stippled endplates. D-2-HGA has been described as a neurometabolic disorder manifesting a broad range of impairment in mental and motor development. Although hydroxyglutaric acid was excreted in high

Double-barrel fibular graft for metaphyseal areas reconstruction around the knee.

UNLABELLED: The metaphyses around the knee (distal femoral and proximal tibial) are junction areas subject to considerable stress. Double-barrel free vascular fibular graft (D-FVFG) provides good bone augmentation and restores the metaphyseal cone; the present study assessed its capacity to restore metaphyseal anatomy and axis and investigated its impact on the neighboring knee joint. MATERIAL AND METHODS: Eight D-FVFGs performed for metaphyseal segmental bone defect were followed up for a mean 6.5 years (range, 3-14 years). There were seven femoral nonunions, five of which were septic, and o

Limitations of peripheral quantitative computed tomography metaphyseal bone density measurements.

CONTEXT: Peripheral quantitative computed tomography (pQCT) measurements are frequently obtained to assess cancellous bone density in the appendicular skeleton. Large variations in bone morphology associated with skeletal development may limit the interpretation of pediatric pQCT studies based on a single slice. OBJECTIVE: The objective of the study was to characterize the variability in trabecular bone density values along the length of the metaphysis. DESIGN: The design was an analysis of pQCT bone density data. SETTING: The study was conducted at a hospital radiology department. PATIENT

Schmid type of metaphyseal chondrodysplasia and COL10A1 mutations--findings in 10 patients.

The Schmid type of metaphyseal chondrodyplasia (MCDS) is characterized by short stature, widened growth plates, and bowing of the long bones. It results from autosomal dominant mutations of COL10A1, the gene which encodes alpha1(X) chains of type X collagen. We report the clinical and radiographic findings in 10 patients with MCDS and COL10A1 mutations. Six patients had lower limb deformities, which necessitated orthopedic surgeries in all of them. One patient demonstrated no deformities and normal stature at age 11 years (height -1.2 SDS) while the others manifested severe short stature (<-3.

Use of a distraction plate for distal radial fractures with metaphyseal and diaphyseal comminution.

BACKGROUND: Distal radial fractures with extensive comminution involving the metaphyseal-diaphyseal junction present a major treatment dilemma. Of particular difficulty are those fractures involving the articular surface. One approach is to apply a dorsal 3.5-mm plate extra-articularly from the radius to the third metacarpal, stabilizing the diaphysis and maintaining distraction across the radiocarpal joint. METHODS: Twenty-two patients treated with a distraction plate for a comminuted distal radial fracture were included in the study. With use of three limited incisions, a 3.5-mm ASIF plate

Classic metaphyseal lesion following external cephalic version and cesarean section.

We report a case of an otherwise healthy neonate diagnosed at birth with a classic metaphyseal lesion of the proximal tibia following external cephalic version for frank breech presentation and a subsequent urgent cesarean section. Although the classic metaphyseal lesion is considered highly specific for infant abuse, this case demonstrates the importance of obtaining a history of obstetric trauma for neonates presenting to the imaging department for suspected non-accidental injury.

Aneurysmal bone cyst arising from a fibrous metaphyseal defect in a toddler.

This is a case of aneurysmal bone cyst arising from a fibrous metaphyseal defect of the proximal tibia in a toddler. A 32-month-old girl presented with physiologic bowing with a small metaphyseal defect at the medial aspect of the left proximal tibia. A large aneurysmal bone cyst, occupying almost the entire metaphysis of the proximal tibia, evolved from this lesion during the following 1 year 3 months. The lesion healed uneventfully after curettage and bone grafting. This case emphasizes the importance of regular followups to observe the clinical course of a fibrous metaphyseal defect during

[Treatment of distal metaphyseal fractures of the tibia with the interlocking flexible osteosynthesis system].

PURPOSE: Accurate anatomic identification of distal metaphyseal fractures of the tibia is necessary when comparing the fixation ability of internal fixation devices. First, we have set the dimensions of the metaphyseal area. The purpose of this study was to evaluate the Interlocking Flexible Osteosynthesis system in properly selected distal metaphyseal fractures, not to be confused with distal shaft fractures. PATIENTS: This study involves 28 fractures, including 10 diaphyseal-metaphyseal fractures, and 18 pure metaphyseal fractures, 9 of which were distal fractures involving the inferior hal

A mild form of pseudoachondroplasia: minimal epi-metaphyseal involvement of long bones.

A problem in diagnosing pseudoachondroplasia (PSACH) is that the pathognomonic changes of vertebral bodies invariably disappear around the age of 10, and an adult case might subsequently be misdiagnosed as MED, Fairbank type, though the latter is less severe than PSACH. We present a Japanese girl (follow-up, 3-10 years of age) whose epi-metaphyseal involvement of long bones was minimal and whose height was +2 S.D. of standard for PSACH in spite of the typical changes of the vertebral bodies. She might not have been diagnosed only as PSACH but also as MED, Fairbank type, after disappearance of

Esophageal atresia with distal tracheoesophageal fistula in a patient with fronto-metaphyseal dysplasia.

Fronto-metaphyseal dysplasia (FMD) is an uncommon but clinically striking condition affecting bone and connective tissue. The terms used to define this syndrome fail to cover all the reported findings, the abnormalities not being confined to the metaphyses and to the frontal bones. We report on a patient who, in addition to the clinical manifestations characteristic of the syndrome, showed esophageal atresia with distal tracheoesophageal fistula. Particular emphasis is given to the extraskeletal manifestations of the syndrome reported in the literature.

Metaphyseal histology and magnetic resonance imaging in Legg-Calvé-Perthes disease.

Cylindrical biopsy specimens from the proximal femoral metaphysis were obtained in conjunction with surgery in 22 patients with Legg-Calvé-Perthes disease. The histopathological examination revealed fat necrosis, vascular proliferation, and focal fibrosis indicating previous episodes of ischemia. There was no correlation between the histopathological findings and the corresponding magnetic resonance images.

Femoral head avascular necrosis associated with metaphyseal aneurysmal bone cyst.

A 12-year-old girl had an aneurysmal bone cyst in the proximal femoral metaphysis simultaneously with an avascular necrosis of the femoral head, as seen on radiological examination. No extension of the lesion across the growth plate into the femoral epiphysis was observed. The expansive behavior of this lesion (ABC) could have compromised the posterosuperior vascular supply of the femoral epiphysis, since no radiographic signs of fractures could be found. Vascular anomalies within the lesion that increase venous pressure could be another factor responsible for the decrease in the blood supply

Magnetic resonance evaluation of "metaphyseal" changes in Legg-Calvé-Perthes disease.

Twenty involved hips in 16 patients with Legg-Calvé-Perthes disease (LCP) were studied with both plain radiographs and magnetic resonance imaging (MRI) scans to better evaluate the existence of "metaphyseal" changes. Thirty-four sets of radiographs and MRI scans were reviewed in a blinded fashion and compared for the presence and location of these changes. Of 23 hips with plain radiographic metaphyseal changes, 11 showed no such changes on MRI scans (48%). Twelve hips did show MRI changes located in the anterior metaphysis. One hip studied three times had a discrete cystic change located in th
