[Takayasu disease--case report].

Chronic vasculitis which determines the thickness of the affected arteries, Takayasu disease is an uncommon finding. It is characterized by narrowing of the big arteries beginning from the aortic arch. The result is arterial hypotension, cerebral and retinal ischemic signs, the absence of pulse at the upper limbs. Manifestations includes: vascular narrowing, microaneurysms, hemorrhages, cotton wool spots, vascular occlusions. The authors present the case of a 20-years old female patient with retinal vascular signs, admitted for ocular pain, upper limbs and mastication pains. The clinical exami

[Erectile dysfunction arising from polyarteritis nodosa: a case report].

A 30-year-old male was referred to our department during the examination and treatment for hypertension, because he had not achieved complete erection since the age of 26 years. After detailed examination, he was diagnosed to be suffering from nodal polyarteritis. Since erectile dysfunction persisted, the patient was hospitalized to investigate the cause of erectile dysfunction. Measurement of the penile brachial index (PBI) and cavernous infusion of papaverine hydrochloride strongly suggested the presence of arterial erectile dysfunction. Internal-pudendal angiography revealed multiple microa

Concurrent sickle cell hemoglobin C disease and diabetes mellitus: no added risk of proliferative retinopathy?

Of 148 adults with sickle cell hemoglobin C (SC) disease seen at Cook County Hospital and Clinics, Chicago, Illinois, eight patients had coexistent noninsulin-dependent diabetes mellitus. The clinical findings were notable for the paucity of retinal vascular changes. No patient showed proliferative diabetic retinopathy; one patient showed background diabetic retinopathy consisting of a few microaneurysms. Six of the eight patients showed no lesions of proliferative sickle retinopathy. Coexistence of noninsulin-dependent diabetes mellitus and SC disease does not appear to have an additive adver

Kappa light chain nephropathy without evidence of myeloma cells. Response to chemotherapy with cessation of maintenance hemodialysis.

A rarely diagnosed nodular glomerulopathy is presented arising secondary to kappa light chain deposition and clinically characterized by hypertension, congestive heart failure, massive proteinuria and slowly progressive azotemia. Kappa light chains were detected in the urine, the glomerular nodules, and the basement membranes of both glomeruli and tubules. A malignant proliferation of plasma cells could not be detected. Two morphologic features were unusual: the presence of microaneurysms, and the deposition of immunoglobulin and complement in a similar pattern to the kappa light chains. Notew

Hypertension, hyperreninemia, and secondary hyperaldosteronism in systemic necrotizing vasculitis.

We studied the pathogenesis of hypertension in two patients with hepatitis-B surface antigen-positive systemic necrotizing vasculitis. Both presented with hypertension, hypokalemia, and renal potassium wasting. Plasma renin activity and urinary aldosterone levels were markedly elevated. Renal arteriograms showed widespread microaneurysms, and necrotizing vasculitis involving renal arteries was confirmed histologically. Hypertension was refractory to conventional treatment in both patients. In one patient, hypertension was easily controlled with the angiotension-converting enzyme inhibitor capt

The value of fluorescein fundus angiography in evaluating optic disc oedema.

43 patients with blurred disc margins were studied by means of fluorescein fundus angiography. The differentiation between very early disc oedema and a normal disc was possible, due to an increased capillary network, some microaneurysms and the late fluorescence located in a special circular manner at disc margin. It was not possible to differentiate between optic neuritis and papilloedema, although minor differences may exist. Ischaemic optic neuropathy is a type of optic disc oedema which clearly differs from other causes of oedema when studied by fluorescein fundus angiography. The method i

Peripheral retinal evaluation comparing fundus photographs with fluorescein angiograms in patients with diabetes mellitus.

PURPOSE: To evaluate peripheral nonproliferative retinopathy by comparing fundus photograph changes with angiographic findings in type 1 diabetic patients with a long duration of diabetes. METHODS: Thirty-eight patients with type 1 diabetes, without proliferative retinopathy and with at least 30 years of diabetes duration, were selected to be examined with fundus photography and fluorescein angiography. Two levels of retinopathy degree were identified. In fundus photographs, level 1 represented eyes with no or mild peripheral background retinopathy (i.e., five or fewer pinpoint hemorrhages or

Retinopathy in a patient with thrombotic thrombocytopenic purpura complicated by polymyositis.

BACKGROUND: Cotton wool spots are known to be a complication in patients with thrombotic thrombocytopenic purpura or with polymyositis. CASE: A 53-year-old woman developed numerous cotton wool spots around the optic disc of both eyes. OBSERVATIONS: Fluorescein angiography disclosed capillary obstruction and microaneurysms in the early phase, followed by dye leakage in the late phase. Systemically, she showed fever, hemolytic anemia, fragmented red blood cells, thrombocytopenia, proximal muscle weakness, and elevation of serum muscle enzymes. This led to a diagnosis of thrombotic thrombocytop

Central retinal vein occlusion in two patients with immunoglobulin G multiple myeloma associated with blood hyperviscosity.

A 69-year-old man (Patient 1) complained of anorexia, lumbago, and seeing floaters OS. The results of laboratory tests showed immunoglobulin (Ig) G lambda-type multiple myeloma associated with blood hyperviscosity. Retinal hemorrhages OD and central retinal vein occlusion OS were found. A 71-year-old woman (Patient 2) with IgG kappa-type multiple myeloma had blood hyperviscosity. Retinal microaneurysms OD and central retinal vein occlusion OS were found. We believe that our patients represent rare cases of IgG multiple myeloma associated with blood hyperviscosity and central retinal vein occlu

Effect of treatment with an inhibitor of platelet aggregation on the evolution of background retinopathy: 2 years of follow-up.

Ophthalmic evolution was studied for 2 years in 17 patients with insulin-dependent diabetes mellitus and background diabetic retinopathy. Nine patients were treated with triflusal, a new platelet antiaggregant drug, and the eight remaining patients, with similar clinical and biological characteristics, were considered the control group. At the end of the study the ophthalmic evolution was different in the two groups. In the control group the degree of fluorescein leakage and the number of microaneurysms increased, while in the triflusal-treated group both parameters were reduced. There were no

Multisystem vasculopathy in deficiency of adenosine deaminase 2.

Deficiency of adenosine deaminase 2 (DADA2) is a rare monogenic autoinflammatory vasculopathy characterised by systemic vasculitis, immune dysregulation and haematological abnormalities. We report a man in his 30s who presented with recurrent ischaemic strokes, cytopenias, renal microaneurysms, cardiomyopathy and gastrointestinal involvement. He was initially diagnosed with polyarteritis nodosa and treated with conventional immunosuppression, with poor clinical response. Genetic testing confirmed DADA2, following which infliximab therapy was initiated, resulting in clinical stabilisation and i

Intracerebral hemorrhage after prophylactic revascularization in a patient with adult moyamoya disease.

BACKGROUND: The effect of revascularization surgery for a patient with moyamoya disease remains controversial. CASE DESCRIPTION: A 60-year-old man presented with bleeding from asymptomatic moyamoya vessels 10 years after prophylactic revascularization surgery. Cerebral angiography 10 years after the surgery demonstrated that the bilateral direct anastomoses remained effective and a small aneurysm persisted in the anterior choroidal artery. The territories of perfusion through the anastomoses and the size of the aneurysm remained unchanged compared with the angiograms performed 10 years ago.

A young female case of polyarteritis nodosa strongly suspected by typical angiographic findings which improved rapidly after prednisolone and cyclophosphamide therapy.

A 16-year-old girl was admitted with the complaints of headache, chest pain, low abdominal pain and left hemi-numbness. Her blood pressure was high and plasma renin activity and aldosterone levels were elevated. Renal angiography revealed vascular stenoses and microaneurysms although the renal artery and its main branches were not involved. Polyarteritis nodosa (PN) was strongly suspected and oral prednisolone and intravenous pulse therapy of cyclophosphamide were started. The second renal angiography which was performed 11 days after the therapy was started, showed marked improvement of vascu

Pattern electroretinograms become abnormal when background diabetic retinopathy deteriorates to a preproliferative stage: possible use as a screening test.

The pattern electroretinogram (PERG) and other tests were given to a selected group of patients with diabetes, ranging from those who had no retinopathy or funduscopic changes to those in the preproliferative state. None had visual symptoms. The PERG was found to be normal in patients with microaneurysms and a few blot haemorrhages. However, when cotton-wool spots and angiographic evidence of areas of capillary non-perfusion were present, the PERG was reduced below the normal value. Similar changes occurred with the oscillatory potentials of the ERG evoked by an intense flash, but the results

The pathology of visible blood vessels on the nasal septum in children with epistaxis.

OBJECTIVE: Epistaxis is common in children, but its cause remains unknown. About half the children who present with epistaxis have prominent vessels on the nasal septum. The aim of this study was to determine the pathological nature of the prominent septal vessels in children with recurrent epistaxis. METHODS: 4mm punch biopsies of the nasal septal mucosa were taken from 5 children undergoing nasal cautery under general anaesthesia. RESULTS: Histology showed that the prominent vessels were thin-walled arterioles and capillaries with a surrounding inflammatory infiltrate. There was no evidenc

Segmental arterial mediolysis: report of 2 cases and review of the literature.

Segmental arterial mediolysis (SAM) is an idiopathic noninflammatory vasculopathy involving small to medium arteries, usually in the abdomen, although arteries in the cerebral and coronary circulations also may be affected. Some cases present as abdominal apoplexy due to aneurysmal rupture, but ischemia and infarction also occur. Not uncommonly, SAM may be misdiagnosed as a systemic necrotizing vasculitis. We present 2 patients with bilateral renal infarctions, cerebral arterial dissections, and visceral artery microaneurysms. Both were diagnosed initially as polyarteritis nodosa. The diagnosi

INTERFERON-β-ASSOCIATED RETINOPATHY.

BACKGROUND: Interferon-associated retinopathy is a rare complication of interferon treatment. It has been well described with the use of interferon-α. METHODS: This study reports a retrospective case series of interferon-β-associated retinopathy in three patients that were being treated for multiple sclerosis. RESULTS: All cases were characterized by cotton-wool spots or peripheral microaneurysms. One central retinal artery occlusion was observed associated with interferon-β treatment. Findings improved spontaneously in two of three cases; all three patients elected to continue interferon tr

[Isolated renal involvement in periarteritis nodosa].

Periarteritis nodosa (PAN) is a disease involving the kidneys in 70% of cases but always affecting other organs. Initial presentation with acute renal failure (ARF) and high blood pressure (HBP) is unusual. In two patients presenting those two features without any clinical or biochemical signs of other organ involvements, diagnosis of PAN was based on the presence of microaneurysms on renal angiograms. Combined immunosuppressive and steroid treatment resulted in control of blood pressure of both patients and improvement of renal function in one. ARF associated with HBP without evidence of othe

Juvenile temporal arteritis. Biopsy study of four cases.

Two young adults (aged 21 and 22 years) and two children (aged 7 and 8 years) complained of an unsightly, soft, painless unilateral nodule in the temporal region, ranging from 0.5 to 1.5 cm in diameter, clinically diagnosed as lipoma, sebaceous cyst, or dermoid cyst. In each instance, the patient had no evidence of systemic disease or history of trauma, and the nodule was excised for cosmetic reasons. Histologic examination of the lesions showed non-giant-cell granulomatous inflammation of the temporal arteries with intimal proliferation and microaneurysmal disruption of the media. Whether the
