The comparison between vocal facilitating techniques, manual circumlaryngeal therapy, and combined voice therapy in teachers with muscle tension dysphonia: a randomized clinical trial.

PURPOSE: Although voice therapy (VT) has been known effective in muscle tension dysphonia (MTD), it is not obviously clear which VT approach is more effective. This study aimed to compare the effectiveness of Vocal Facilitating Techniques (VFTs), Manual Circumlaryngeal Therapy (MCT), and combined VT in teachers with MTD. METHODS: This study was a double-blind parallel randomized clinical trial. Thirty elementary female teachers with MTD were divided into three treatment groups including VFTs; MCT, and combined VT. In addition, vocal hygiene was presented to all groups. All participants receiv

Efficient Actor-Critic Reinforcement Learning With Embodiment of Muscle Tone for Posture Stabilization of the Human Arm.

This letter proposes a new idea to improve learning efficiency in reinforcement learning (RL) with the actor-critic method used as a muscle controller for posture stabilization of the human arm. Actor-critic RL (ACRL) is used for simulations to realize posture controls in humans or robots using muscle tension control. However, it requires very high computational costs to acquire a better muscle control policy for desirable postures. For efficient ACRL, we focused on embodiment that is supposed to potentially achieve efficient controls in research fields of artificial intelligence or robotics.

Current evidence on the unit equivalence of different botulinum neurotoxin A formulations and recommendations for clinical practice in dermatology.

BACKGROUND: The unit equivalence between the two main Botulinum neurotoxin A (BoNTA) preparations, Dysport (Ipsen Ltd., Slough, Berkshire, UK) and BOTOX (Allergan Inc., Irvine, CA), is a matter of discussion. The UK assay used to test Dysport is more sensitive than the U.S. assay used for BOTOX, resulting in a different efficacy per unit in both formulations. Ratios ranging from 6:1 to 1:1 can be found in the literature, but the more recently published literature suggests that 1 unit of BOTOX is equivalent to approximately 2 to 4 units of Dysport (ratio 2:1-4:1). OBJECTIVE: Because the number

Dynamic Stretching Has Sustained Effects on Range of Motion and Passive Stiffness of the Hamstring Muscles.

Dynamic stretching (DS) is often performed during warm-up to help avoid hamstring muscle injuries, increase joint flexibility, and optimize performance. We examined the effects of DS of the hamstring muscles on passive knee extension range of motion (ROM), passive torque (PT) at the onset of pain (as a measure of stretch tolerance), and passive stiffness of the muscle-tendon unit over an extended period after stretching. Twenty-four healthy subjects participated, with 12 each in the experimental and control groups. Stretching was performed, and measurements were recorded using an isokinetic dy

Unstable rocker shoes promote recovery from marathon-induced muscle damage in novice runners.

We recently reported that wearing unstable rocker shoes (Masai Barefoot Technology: MBT) may enhance recovery from marathon race-induced fatigue. However, this earlier study only utilized a questionnaire. In this study, we evaluated MBT utilizing objective physiological measures of recovery from marathon-induced muscle damages. Twenty-five university student novice runners were divided into two groups. After running a full marathon, one group wore MBT shoes (MBT group), and the control group (CON) wore ordinary shoes daily for 1 week following the race. We measured maximal isometric joint torq

Gastric sensitivity and reflexes: basic mechanisms underlying clinical problems.

Both reflex and sensory mechanisms control the function of the stomach, and disturbances in these mechanisms may explain the pathophysiology of disorders of gastric function. The objective of this report is to perform a literature-based critical analysis of new, relevant or conflicting information on gastric sensitivity and reflexes, with particular emphasis on the comprehensive integration of basic and clinical research data. The stomach exerts both phasic and tonic muscular (contractile and relaxatory) activity. Gastric tone determines the capacity of the stomach and mediates both gastric ac

Reflex effects of vibration in patients with spinal cord lesions.

The vibration reflex was studied in 49 patients with traumatic spinal cord lesions. It was elicited in all patients, even after presumably complete division of the cord. The vibration relfex consisted of a short-latency, brief outburst of phasic activity of motor units, followed by rapidly decreasing phasic component and a later slowly declining tonic component. When periods of vibration were repeated at short intervals of 2 to 10 seconds, the responses showed an approximately exponential decline, although the beginning of each subsequent response was always larger than the end of the precedin

[Effect of stimulation of the spinal cord on phasic and tonic reflexes in patients with central motor neuron damage].

Spinal cord stimulation (SCS) is a method enabling the control of increased muscle tonus to be achieved in various spinal cord injuries. Polyelectromyographic (PEMG) methods were used for neurophysiological assessment of the degree of cord damage and persistent spinal reflexes as well as supramedullary influences. The analysed material comprised 40 PEMG records in 19 patients with spastic paraparesis or paraplegia after cord injury, cord tumour or multiple sclerosis. In 15 cases tentative epidural cord stimulation was done and 11 patients received implantation of a system for long-term stimula

Neurobiology of Polyorchis. I. Function of effector systems.

The electrical correlates of activity in the effector systems responsible for swimming, crumpling and postural changes have been recorded in the anthomedusan Polyorchis penicillatus. Motor spikes (pre-swim pulses), that initiate swimming contractions, appear without delay at distant sites on the inner nerve-ring in unstimulated preparations. Levels of Mg++ anaesthesia which block the neuromuscular junctions between PSP giant neurons and swimming muscle do not affect PSP activity. Swimming muscle potentials can be recorded from subumbrella and velar muscle sheets using extra- and intracellular

Development of low postural tone compensatory patterns - predicted dysfunction patterns in lower part of the body.

Lower postural tone is not always associated with central nervous system structural damage. There is such kind of tone that stays within the broadly defined normal range, but is characterized by distinct decrease of tone of the deep muscles responsible for stabilization. External syndromes are features of active or passive compensation observed in the postural and motor patterns. Active compensation of the lower muscle tone is associated with excessive use of the superficial muscles for stabilization that leads to limitation of motion in the joints and to functional shortening of some muscles.

Understanding the floppy baby.

Muscle tone develops in a orderly sequence through gestation and continues to change after birth. Hypotonia is frequently found in infants, and pathological degrees must be differentiated from normal variations. This distinction is possible if the clinician understands how muscle tone is regulated and modifies his examination to include some special clinical signs. A variety of pathological conditions can influence muscle tone, and hypotonia can be an early and valuable clue to recognizing neuromuscular, CNS, metabolic, and other disease states in this age group.

Vibration-induced changes in vascular tone.

Longitudinal mechanical vibrations in sonic range (10-100 Hz) and of moderate amplitude (50-500) considerably reduce the contractile force of smooth muscle as well as striated muscle. Experiments elucidating the mechanism behind this effect are described. The inhibition of the contractile force is probably due to a direct interference with the action of the contractile proteins. Vibrations are supposed to increase the rate with which the interfilamentary crosslinks are broken. Thereby the number of established crosslinks is reduced and thus the force. Finally a hypothesis concerning the possib

In-vitro characterization of the pharmacological effects induced by (-)-α-bisabolol in rat smooth muscle preparations.

The present study deals with the pharmacological effects of the sesquiterpene alcohol (-)-α-bisabolol on various smooth-muscle preparations from rats. Under resting tonus, (-)-α-bisabolol (30-300 µmol/L) relaxed duodenal strips, whereas it showed biphasic effects in other preparations, contracting endothelium-intact aortic rings and urinary bladder strips, and relaxing these tissues at higher concentrations (600-1000 µmol/L). In preparations precontracted either electromechanically (by 60 mmol/L K(+)) or pharmacomechanically (by phenylephrine or carbachol), (-)-α-bisabolol showed only relaxing
