Methicillin-resistant Staphylococcus aureus (MRSA) has emerged to be one of the most important pathogens both in health care and in community-onset infections. Daptomycin (DAP) is a cyclic anionic lipopeptide recommended for treatment of skin infections, bacteremia, and right-sided endocarditis caused by MRSA. Inadvertent leakage of medications with vesicant properties can cause severe tissue necrosis, which can lead to devastating long-term consequences. To determine whether recommendations of infectious diseases specialists affect outcome for patients, we evaluated 244 hospitalized patients with Staphylococcus aureus bacteremia. We offered our management recommendations to each patient's physicians and then assessed the clinical outcome for both patients for whom our consultative advice was followed and those for whom our advice was not heeded. Arterial large vessel vasculopathy is an unusual complication of bacterial meningitis in adults that may result in cerebral ischemia and severe neurological sequelae. Previous therapy has included antimicrobials with no specific vascular interventions. The authors report their experience with a patient with bacterial meningitis who developed a vasculopathy with neurological deficits. The frequency, predisposing factors, therapy, and outcome of 45 episodes of bacteremia due to Staphylococcus aureus were reviewed in adult cancer patients. A poor performance status (i.e., patients largely bedridden), progressive neoplastic disease, and compromise of the mucocutaneous defense barriers characterized the patients with S. aureus sepsis. This article demonstrates how rheumatoid arthritis can be confused with septicemic arthritis. Effective diagnosis and treatment depends on timely and effective confirmation of the cause for arthritis. In the future, if such a case were presented, actions must be taken as soon as the bacteremia has been confirmed since the organism is the primary cause of the inflammation and not rheumatoid arthritis. If this is done immediately, the patient's body will be more effective at fighting off nosocomial infections resulting in a better prognosis.
