Upadacitinib Therapy in Adolescent Severe Alopecia Areata: A Case Series and Narrative Review.

PURPOSE: Alopecia areata (AA) is a common, immune-mediated, non-scarring form of hair loss. Janus kinase inhibitors provide considerable insight into the treatment of severe AA. However, the efficacy and safety of upadacitinib treatment of adolescents and pediatric patients with severe AA is unclear, especially in those without concomitant atopic diseases. PATIENTS AND METHODS: Adolescents with severe AA receiving upadacitinib were recruited from the Dermatology Outpatient department. This is a retrospective case series. Clinical characteristics, hair regeneration, and adverse events were ana

Targeting neutrophilic inflammation in obstructive airway disease - A narrative review of brensocatib therapy.

Brensocatib is an oral inhibitor of dipeptidyl peptidase 1, an enzyme that activates neutrophil serine proteases. Its potential to reduce neutrophil-driven inflammation has generated interest across a range of chronic inflammatory and respiratory conditions, particularly non-cystic fibrosis (CF) bronchiectasis. As the body of evidence supporting brensocatib continues to expand, there is a clear need for a comprehensive, rigorous, and practical narrative review to consolidate current knowledge and highlight gaps for future research. The aim of this narrative review was to systematically examine

Narrative reconstruction therapy for prolonged grief disorder: Basic interventions and mechanisms of change.

Narrative reconstruction is a time-limited integrative therapy, originally developed in the context of post-traumatic stress disorder and adjusted for the treatment of prolonged grief disorder (PGD). It consists of exposure to the loss memory and narrating a detailed written reconstruction of it. In this paper, we describe narrative reconstruction interventions and modifications for the treatment of PGD as well as the underlying mechanisms of change including integration of the loss episode into the life narrative. A case demonstration of narrative reconstruction with a patient with PGD presen

No-reflow phenomenon following stroke recanalization therapy: Clinical assessment advances: A narrative review.

The no-reflow phenomenon (NRP) after successful vascular recanalization in acute ischemic stroke (AIS) has become a major cause of poor clinical prognosis and ineffective recanalization. However, there is currently no clear definition or unified clinical assessment method for the NRP. Therefore, it is urgent to clarify the clinical evaluation criteria for the NRP and develop new no-reflow evaluation techniques so that remedial treatment can be applied to AIS patients suffering from the NRP. In this brief review, a variety of NRP assessment methods and defining criteria for clinical practice ar

Surfactant replacement therapy as promising treatment for COVID-19: an updated narrative review.

Patients with COVID-19 exhibit similar symptoms to neonatal respiratory distress syndrome. SARS-CoV-2 spike protein has been shown to target alveolar type 2 lung cells which synthesize and secrete endogenous surfactants leading to acute respiratory distress syndrome in some patients. This was proven by post-mortem histopathological findings revealing desquamated alveolar type 2 cells. Surfactant use in patients with COVID-19 respiratory distress syndrome results in marked improvement in respiratory parameters but not mortality which needs further clinical trials comparing surfactant formulas a

Acne Vulgaris-Novel Treatment Options and Factors Affecting Therapy Adherence: A Narrative Review.

Acne vulgaris is an extremely common skin condition, affecting a large population of adolescents, but at the same time, remaining a quite common issue in the group of adult patients. Its complex pathogenesis includes increased sebum secretion, impaired follicular keratinization, colonization of sebaceous glands with Cutibacterium acne bacteria, and the development of inflammation in pilosebaceous units. Although there are many methods of treatment available targeting the mechanisms mentioned above, a large percentage of patients remain undertreated or non-compliant with treatment. Ineffective

Updates on androgen replacement therapy and lower urinary tract symptoms: a narrative review.

Lower urinary tract symptoms (LUTS) are caused by higher tension at the bladder neck level (due to fibrosis or stiffness) or benign prostatic hyperplasia, which causes static obstruction of the bladder outlet. Both forms cause a group of symptoms such as hesitancy, intermittency, weak stream, nocturia, urine frequency, and urgency. Additionally, LUTS (obstructive or irritative symptoms) are common in elderly men with hypogonadism, identified as the reduced testes capability in producing sex steroids and sperm, and are categorized as testosterone deficiency. Even though the mode of action (MoA)

Hydroxyethyl starch for perioperative goal-directed fluid therapy in 2020: a narrative review.

BACKGROUND: Perioperative fluid management - including the type, dose, and timing of administration -directly affects patient outcome after major surgery. The objective of fluid administration is to optimize intravascular fluid status to maintain adequate tissue perfusion. There is continuing controversy around the perioperative use of crystalloid versus colloid fluids. Unfortunately, the importance of fluid volume, which significantly influences the benefit-to-risk ratio of each chosen solution, has often been overlooked in this debate. MAIN TEXT: The volume of fluid administered during the

Hyperbaric oxygen therapy in patients suffering from wounds in calciphylaxis: a narrative review.

BACKGROUND: Calcific uremic arteriolopathy (calciphylaxis) is a rare and highly lethal vascular disease. Vascular calcification with calcium depositions lead to ischemic ulcers associated with gangrene, severe pain and poor healing. Although hyperbaric oxygen (HBO2) therapy has been used in the treatment of calciphylaxis, evidence of its effectiveness is limited. OBJECTIVE: To determine whether HBO2 therapy has a beneficial effect in the healing of calciphylaxis ulcers. METHODS: A search was made in PubMed using a comprehensive strategy to identify the effect of HBO2 on calciphylaxis wounds.

The Role of Body Composition on Drug Tolerance in Cancer Therapy: A Structured Narrative Review.

PURPOSE OF REVIEW: The aim of the present review is to analyse the current knowledge on how body composition parameters, principally low muscle mass (frequently reported as "sarcopenia") and myosteatosis (low muscle quality), relate to drug tolerance and the occurrence of adverse events during cancer therapy across different malignancies. RECENT FINDINGS: Because dosing based on weight or body surface area does not capture body composition, its potential limitations have increasingly been questioned. The available research spans a highly heterogeneous set of studies with inconsistent results.

Obesity treatment for postmenopausal women: Established therapies and emerging approaches - a narrative review.

Obesity has become one of the most prevalent chronic conditions of the twenty-first century, affecting women's health throughout their lifespan. It is associated with many chronic diseases and malignancies. It also worsens vasomotor symptoms. It is estimated that 50-80% of menopausal women have vasomotor symptoms and may need menopausal hormonal therapy. Although data indicate that it can reduce abdominal fat, obesity treatment strategies based solely on menopausal hormone therapy are not currently recommended by any gynecological or endocrinological society. Based predominantly on general pop

An update on current challenges of extracellular vesicle therapy in reproductive complications: A narrative review.

In recent decades, cell-free therapies, such as extracellular vesicle (EV) applications, have emerged as effective alternatives for treating various pathologies that lead to male and female infertility. Among different types of EVs, exosomes, which range from 30 to 150 nm, are released by nearly all cell types under both physiological and pathological conditions. The exchange of exosomes facilitates active paracrine communication between donor and recipient cells by conveying signaling biomolecules and altering specific signaling pathways in target cell populations. Despite the numerous benefi

Aortic response to cancer therapy: a narrative review of the radiotherapy paradox and clinical implications.

BACKGROUND: Cancer treatments such as chemotherapy and radiotherapy significantly impact the cardiovascular system. While cardiac toxicity is well characterized, the effects on the aorta, the body's largest artery, remain less understood. Given that aortic weakening may lead to life-threatening conditions such as aneurysm formation and rupture, understanding these effects is crucial for improving long-term vascular health in cancer survivors. METHODS: This narrative review synthesizes findings from six selected studies comprising clinical investigations and reviews. It examines the impact of
