Low-Grade and Intermediate-Grade Apocrine Intraductal Carcinoma Arising in a Nevus Sebaceous of Jadassohn.

Nevus sebaceous (NS) is a congenital hamartoma that may give rise to a wide spectrum of benign and malignant adnexal neoplasms, although malignant transformation is uncommon. We report a case of low- and intermediate-grade apocrine intraductal carcinoma arising in a NS of Jadassohn in a 93-year-old woman. Histopathologic examination demonstrated an intraductal apocrine neoplasm with solid and cribriform architecture, focal comedonecrosis, and an intact surrounding myoepithelial cell layer highlighted by p63 and CK5/6 immunostaining. The neoplastic cells expressed CK7, epithelial membrane antig

Xeroderma pigmentosum associated with recurrent squamous and basal cell carcinoma: a case report from Nepal.

INTRODUCTION: Xeroderma pigmentosum (XP) significantly increases the risk of skin malignancies due to impaired DNA repair mechanisms. The estimated incidence is 1 in 250 000 in the USA. Patients with XP are more likely to develop cutaneous neoplasms, primarily melanoma, squamous cell carcinoma (SCC), and basal cell carcinoma (BCC), and the most prevalent cutaneous and ocular tumor is SCC. CASE PRESENTATION: We present a case of a 21-year-old male from western Nepal who presented to Tribhuvan University Teaching Hospital with multiple lesions on the face and was diagnosed with SCC and BCC. We

Cutaneous neoplasms in a military population of HIV-1-positive patients. Military Medical Consortium for the Advancement of Retroviral Research.

BACKGROUND: In HIV-1-positive patients there have been no prospective studies that show an increase in cutaneous neoplasms. OBJECTIVE: We observed HIV-1-positive patients to determine whether or not there was an increased incidence of cutaneous malignancies. METHODS: A total of 724 HIV-1-positive patients were examined during a 36-month period for the development of cutaneous malignancies. RESULTS: The most common cutaneous neoplasm found was Kaposi's sarcoma, especially in patients with late-stage disease. Basal cell carcinomas were the next most frequent tumor. We have also seen three mal

Sebaceous carcinoma of scalp with proliferating trichilemmal cyst.

Sebaceous carcinoma is a rare neoplasm of the sebaceous gland. It is diagnosed mainly on histopathology and, clinically, it may mimic other neoplasms like squamous or basal cell carcinoma. We came across a patient presenting with a non-healing ulcer over the left temporo-parietal region of the scalp since 4 years and a single asymptomatic nodule over the occipital region since 3 years. Histopathology from an ulcer was pathognomonic of sebaceous carcinoma and that from a nodule was suggestive of proliferating trichilemmal cyst. The patient was screened for Muir Torre syndrome. We referred the p

Expression of S100A6 protein in a broad spectrum of cutaneous tumors using tissue microarrays.

BACKGROUND: S100A6, a calcium-binding protein in the S100 family, has been observed in melanocytic nevi, neural tumors, fibrohistiocytic tumors and is overexpressed in melanoma. Previous studies reported S100A6 expression in atypical fibroxanthomas (AFX) but not in a small number of desmoplastic melanomas (DM). Limited data on S100A6 expression in cutaneous epithelial tumors exists in the literature. The goal of this study was to determine the specificity and sensitivity of S100A6 protein in a spectrum of cutaneous mesenchymal or epithelial tumors. METHODS: Tissue microarrays of cutaneous epi

A possible chemopreventive role for photodynamic therapy in Gorlin syndrome: a report of basal cell carcinoma reduction and review of literature.

Multiple basal cell carcinomas (BCC) are a significant cause of morbidity and disfigurement in patients with naevoid basal cell carcinoma syndrome (NBCCS). Chemopreventive strategies are needed to reduce the formation of new BCC in these patients. Appropriate evidence-based guidelines for photodynamic therapy chemoprevention in NBCCS do not exist. We herein report one patient treated with methyl aminolevulinate PDT with red light (630 nm) activation to continue sustained chemoprevention following other BCC chemopreventive modalities and a relevant literature review.

Pulmonary metastasis in nevoid basal cell carcinoma syndrome.

Nevoid basal cell carcinoma syndrome is a relatively uncommon clinical entity. It is essentially characterized by the early onset of multiple basal cell carcinomas of the skin generally above wrist-line level, by the presence of jaw cysts, musculoskeletal and other anomalies. Metastatic disease secondary to ordinary basal cell carcinoma is indeed rare. What is even more rare is pulmonary metastases from basal cell carcinoma. Very few cases have been reported in which pulmonary resection was performed for metastatic basal cell carcinoma. We were unable to find a reported case in which pulmonary

Malignant change in seborrhoeic keratoses in a region with high solar ultraviolet levels.

Regions with high solar UV levels and high skin cancer rates may experience a greater incidence of malignancy in association with seborrhoeic keratoses (SebK) than in low UV regions. Previous reports have indicated that basal cell carcinoma is the most common neoplasm with reported rates of up to 4 per cent of excised SebK. The rates of such compound lesions occurring in our practice were reviewed, indicating that Bowen's disease was the most frequently observed neoplasm with a rate of 7 per cent. In total, 10 per cent of all excised specimens showed either frank malignancy or some degree of a

Reflectance confocal microscopy: Diagnostic criteria of common benign and malignant neoplasms, dermoscopic and histopathologic correlates of key confocal criteria, and diagnostic algorithms.

Reflectance confocal microscopy (RCM) is a high-resolution, noninvasive tool that is currently approved by the US Food and Drug Administration for obtaining and interpreting images of the skin and cutaneous neoplasms with the goal of decreasing unnecessary biopsy procedures in patients with benign lesions. The second article in this continuing medical education series focuses on identifying key criteria for the diagnosis of common skin cancers-melanoma, basal cell carcinoma, and squamous cell carcinoma. We contrast these findings with RCM features of common benign lesions-melanocytic nevi, sol

Care of the skin at midlife: diagnosis of pigmented lesions.

Intrinsic skin changes with advancing years include dryness, decreasing elasticity, increasing skin fragility, and more prominent vasculature. Extrinsic skin aging, caused primarily by cigarette smoking and exposure to sunlight, includes mottled pigmentation and yellow discoloration, rough leathery textural changes, and wrinkling. Major premalignant and malignant neoplasms in photodamaged skin are actinic keratosis, basal cell carcinoma, squamous cell carcinoma, and melanoma. Nonmalignant lesions include solar lentigines and seborrheic keratoses. The A, B, C, D criteria can assist in the evalu

NUT Is a Specific Immunohistochemical Marker for the Diagnosis of YAP1-NUTM1-rearranged Cutaneous Poroid Neoplasms.

YAP1-NUTM1 fusion transcripts have been recently reported in poroma and porocarcinoma. NUTM1 translocation can be screened by nuclear protein in testis (NUT) immunohistochemistry in various malignancies, but its diagnostic performance has not been thoroughly validated on a large cohort of cutaneous epithelial neoplasms. We have evaluated NUT immunohistochemical expression in a large cohort encompassing 835 cases of various cutaneous epidermal or adnexal epithelial neoplasms. NUT expression was specific to eccrine poromas and porocarcinoma, with 32% of cases showing NUT expression. All other cu

Ameloblastoma: a neglected criterion for nevoid basal cell carcinoma (Gorlin) syndrome.

Ameloblastomas are considered to be aggressive and locally invasive neoplasms derived from odontogenic epithelium with a tendency for recurrence and bone destruction. Although the relationship between nevoid basal cell carcinoma syndrome (NBCCS) and ameloblastoma is less frequent, it might constitute a peculiar stigmata of this hereditary disorder. The objective of the current study was to evaluate whether a combined clinical and biomolecular approach could be useful for the identification of NBCCS among patients with a diagnosis of ameloblastoma. The authors collected ameloblastoma tumors rec

Trichoblastic carcinosarcoma: A case report and literature review.

Trichoblastic carcinosarcoma is a rare biphasic adnexal neoplasm. This case report chronicals the eighth occurrence of this tumor published in the English literature and provides a review of the prior publications. Clinically, this tumor presents as an isolated, rapidly growing lesion in elderly patients and is usually cured by complete surgical excision, with no evidence of recurrence or metastasis at follow-up (7/8 cases). Histopathologically, trichoblastic carcinosarcoma is dermal-based, with an epithelial component of basal cells and a mesenchymal component of spindle cells, both of which

Reduced levels of UV-induced unscheduled DNA synthesis in epidermal keratinocytes of patients with xeroderma pigmentosum and correlation with development of skin neoplasms.

Primary epidermal keratinocytes obtained from 25 patients with xeroderma pigmentosum (XP) (nine with XP-A, one with XP-C, two with XP-D, five with XP-E, and eight with XP-variant) exhibited less UV-induced unscheduled DNA synthesis (UDS) than did those from 34 normal subjects. Levels of UDS depended greatly on the type of XP; i.e., 3-17% of the control in XP-A, 14% in XP-C, 33-53% in XP-D, 38-77% in XP-E and 58-98% in XP-variant. The extent of UDS in epidermal keratinocytes was almost the same as that in dermal fibroblasts in XP-C, D, and E, but in three out of eight of the XP-variant the leve

Utility of optical coherence tomography in basal cell naevus syndrome: A case report.

Optical Coherence Tomography (OCT) is a useful non-invasive diagnostic tool for diagnosing and monitoring treatment of basal cell carcinomas. We describe the use of OCT in a patient with Basal Cell Naevus Syndrome. Through measuring tumour depth on OCT, management of individual tumours was triaged accordingly using 0.4 mm tumour depth as a cut-off for surgical and non-surgical management. OCT has potential to reduce unnecessary excisions and associated morbidity in this population of patients.

Dermatoscopic characteristics of acrochordon-like basal cell carcinomas in Gorlin-Goltz syndrome.

BACKGROUND: Gorlin-Goltz syndrome (GGS) is an autosomal-dominant disease characterized by the early onset of multiple basal cell carcinomas (BCCs), among other findings. Clinically, the BCCs may appear as soft pedunculated neoplasms that can be mistaken for true acrochordons. OBJECTIVE: We sought to describe the dermatoscopic characteristics of small acrochordon-like or polypoid BCCs in a child with GGS, and to perform histopathologic correlation. METHODS: Acrochordon-like growths from a child with GGS were studied. Clinical records and digital dermatoscopic images were collected, and excisi

The nevoid basal cell carcinoma syndrome.

The nevoid basal cell carcinoma syndrome is a rare and complex disease with multiple manifestations. As an example, we present the case of an eight-year-old girl with this syndrome whose initial complaint was acute abdominal pain secondary to torsion of an ovarian fibroma. Because of its multisystem effects, patients with this syndrome can present to any of a number of medical or surgical specialists. Close attention to the family and past medical history and physical examination will alert the clinician to its presence, allowing for appropriate genetic counseling and serial screening for the

Nevoid basal cell carcinoma syndrome. A review and case report of a patient with unilateral basal cell nevus syndrome.

An exceptionally rare example of nevoid basal cell carcinoma syndrome occurring unilaterally is presented, along with a review of the syndrome. We ruled out the possible association of this case with the linear unilateral basal cell nevus with comedones because of several incongruities, including the absence of epidermal cysts and strialike atrophy and the presence of pigmented basal cell carcinomas. Linear unilateral basal cell nevus with comedones is not a variant of the nevoid basal cell carcinoma syndrome and the tumors follow a benign course. The aggressive, destructive behavior of numero

Basal cell adenoma with extensive squamous metaplasia and cellular atypia: a case report with cytohistopathological correlation and review of the literature.

Basal cell adenoma (BCA) is a rare benign basaloid neoplasm of the salivary gland. There are four histopathological types of BCA: solid, tubular, trabecular, and membranous. It is known that focal squamous metaplasia may be seen in some BCAs, but it is rare to see extensive squamous metaplasia, especially with cellular atypia. Here, a 25-year-old male with right parotid swelling is presented. Ultrasonography revealed a 2-cm well defined mass in his parotid gland. Fine-needle aspiration (FNA), performed prior to surgical excision, showed a highly cellular tumor composed of basaloid cells, formi
