Utilizing nerve conduction studies to identify very early Guillain-Barré syndrome and distinguish it from mimics in emergency settings.

INTRODUCTION/AIMS: Accurately diagnosing Guillain-Barré syndrome (GBS) in its early stages and distinguishing it from mimics poses challenges. This study aimed to evaluate the utility of an existing electrodiagnostic criterion in very early GBS (VEGBS) for discerning mimics. Additionally, we explored specific electrophysiological abnormalities in VEGBS to design a new diagnostic criterion for more accurate VEGBS diagnosis. METHODS: We retrospectively identified all patients with flaccid quadriparesis initially suspected of GBS who underwent nerve conduction studies (NCS) ≤4 days from symptom

The application of nerve conduction and clinical studies to genetic counseling in hereditary motor and sensory neuropathy type I.

One hundred and thirty two individuals at risk for hereditary motor and sensory neuropathy (HMSN) type I from 11 unrelated families were evaluated by physical examination. Motor conduction velocity (MCV) studies of median and/or peroneal nerves were performed on 99 of them. Seventy-three subjects were found to be affected. In all age categories including the first decade of life, the ratio of affected individuals at risk did not significantly differ from the expected 1:1 ratio; that is, penetrance of the gene was complete. The majority of affected members in the first decade had no clinical fe

Radial motor nerve conduction study in posterior interosseous nerve syndrome and multifocal motor neuropathy.

BACKGROUND: Finger extension weakness could be a presentation of either posterior interosseous nerve (PIN) syndrome or multifocal motor neuropathy (MMN). However, there is a delay in the diagnosis of MMN in cases with a selective radial weakness, as they are frequently misdiagnosed as PIN. OBJECTIVE: To analyze which variables in nerve conduction studies could aid in the early diagnosis of MMN. METHODS: We reviewed charts of patients with diagnoses of MMN or PIN syndrome, from 2014 to 2022, in a single Brazilian reference center. Electrophysiological parameters included in the analysis were

Gender Differences in Nerve Conduction Parameters of Upper Limb Nerves in Healthy Adults: A Comparative Study.

BACKGROUND: Nerve conduction studies (NCSs) are essential diagnostic tools for evaluating peripheral nerve function. The influence of gender on nerve conduction parameters, such as conduction velocity, latency, and amplitude, remains a topic of interest. Previous studies have suggested gender differences in nerve conduction but have not provided definitive conclusions. OBJECTIVE: This study aims to compare the nerve conduction parameters of the upper limb nerves (median, ulnar, and radial) in healthy male and female adults. MATERIAL AND METHODS: A total of 100 healthy adults (50 males and 50

Avoiding false-negative nerve conduction study in ulnar neuropathy at the elbow.

INTRODUCTION: Ulnar nerve displacement at the elbow causes overestimation of nerve conduction velocity. We hypothesized that this overestimation may cause false-negative results when the American Association of Neuromuscular and Electrodiagnostic Medicine (AANEM) diagnostic criteria are used. METHODS: We investigated 9 cases of ulnar neuropathy at the elbow, all having ulnar nerve displacement confirmed by ultrasonography and meeting none or just one of the AANEM criteria with distance measurement by anatomic landmarks. First, the distance across the elbow segment was measured by anatomic lan

Nerve conduction in relation to vibration exposure - a non-positive cohort study.

BACKGROUND: Peripheral neuropathy is one of the principal clinical disorders in workers with hand-arm vibration syndrome. Electrophysiological studies aimed at defining the nature of the injury have provided conflicting results. One reason for this lack of consistency might be the sparsity of published longitudinal etiological studies with both good assessment of exposure and a well-defined measure of disease. Against this background we measured conduction velocities in the hand after having assessed vibration exposure over 21 years in a cohort of manual workers. METHODS: The study group cons

Sensitivities of sensory nerve conduction study parameters in carpal tunnel syndrome.

It is generally accepted that median sensory nerve conduction studies are more sensitive than motor nerve conduction studies in the electrodiagnostic evidence of carpal tunnel syndrome (CTS). This study was conducted to compare the sensitivities of various parameters of sensory nerve conduction studies in the diagnosis of CTS. This prospective study included 88 consecutive patients (151 hands) with CTS and 106 control subjects. CTS was diagnosed clinically by two neurologists. Median sensory nerve responses with wrist stimulation were determined. The onset and peak latencies, peak-to-peak ampl

Which motor nerve conduction study is best in ulnar neuropathy at the elbow?

There is debate regarding how best to utilize ulnar motor nerve conduction velocity (MNCV) to identify ulnar neuropathy at the elbow (UNE). We used receiver operator characteristic (ROC) curves to compare absolute across-elbow MNCV with MNCV difference between elbow and forearm segments (VDIF) when recording from abductor digiti minimi (ADM) and first dorsal interosseous (FDI) muscles. Also, we determined how their utility was impacted by low amplitudes of compound muscle action potentials (CMAPs). We studied 85 subjects with UNE and 77 subjects with carpal tunnel syndrome but without clinical

Efficient electrodiagnosis of polyneuropathy-a targeted three-nerve conduction study protocol.

INTRODUCTION: Electrodiagnostic testing is central to the evaluation of polyneuropathy, yet substantial variability exists in nerve selection and testing protocols. This study aimed to identify the minimum nerve set that provides high sensitivity and overall diagnostic accuracy while retaining adequate specificity. Testing two motor and two sensory nerves was hypothesized to be diagnostically sufficient. METHODS: We conducted a retrospective cross-sectional cohort study using a deidentified database from the electrophysiology laboratory at Sheba Medical Center. Patients diagnosed with polyneu

Nerve Conduction Study/Electromyography: Common Neuropathies and Considerations for Patients with Polio.

Acute poliomyelitis is now extremely rare in the United States. Worldwide there are still sporadic outbreaks, which are typically treated with acute inoculation programs. Although polio has effectively been eradicated, the full scope of the disease and its myriad manifestations both in the acute phase and in the postpolio syndrome phase, remain areas of fertile research, debate, and stimulating topics.

Carpal tunnel sonography by the rheumatologist versus nerve conduction study by the neurologist.

OBJECTIVE: To determine the value of sonography (SG) performed by the rheumatologist to diagnose carpal tunnel syndrome (CTS). METHODS: Sixty-three patients with clinical signs of CTS according to the neurologist, based on patient history and clinical examination, were studied. In the 6 weeks prior to surgery, SG was performed by a rheumatologist and nerve conduction study (NCS) was assessed. Improvement of initial complaints of 90% or more 3 months after surgery was considered to be the post-hoc gold standard for the diagnosis of CTS. RESULTS: After surgery, 47 patients (75%) experienced >

Hypoglossal nerve conduction study by transcranial magnetic stimulation in normal subjects.

The introduction of transcranial magnetic stimulation has allowed the study of conduction in the proximal portions and central pathways of the cranial nerves. A study is made of cranial nerve XII with transcranial magnetic stimulation at two levels, cortical and cisternal, registering the motor evoked potential by means of surface electrodes in contact with the upper face of the tongue. Motor evoked potentials were constantly observed on cortical stimulation, in a painless, easy, and reproducible way, with mean values of 10.84 +/- 1.14 milliseconds (latency) and 7.81 +/- 1.14 mV (amplitude).
