Noninvasive and invasive ventilation and enteral nutrition for ALS in Italy.

INTRODUCTION: We performed a population-based study to assess amyotrophic lateral sclerosis (ALS) survival after noninvasive ventilation (NIV), invasive ventilation (IV), and enteral nutrition (EN). METHODS: We included patients diagnosed from 2000 to 2009 in Modena, where a prospective registry and a Motor Neuron Diseases Centre have been active since 2000. RESULTS: Of the 193 incident cases, 47.7% received NIV, 24.3% received tracheostomy, and 49.2% received EN. A total of 10.4% of the patients refused NIV, 31.6% refused IV, and 8.7% refused EN. The median survival times after NIV, IV, and

History and epidemiology of noninvasive ventilation in the acute-care setting.

Although noninvasive ventilation (NIV) was first used to treat patients with acute respiratory failure in the 1940s, the history of this mainstay of today's respiratory care armamentarium has mainly been written in the last 20 years. There is now a robust evidence base documenting the efficacy of NIV in exacerbations of chronic obstructive pulmonary disease, cardiogenic pulmonary edema, and acute respiratory failure in immunocompromised patients, and evidence in support of NIV in other settings, such as hypoxemic acute respiratory failure and the management of patients who decline endotracheal

Should noninvasive ventilation be used with the do-not-intubate patient?

Most of the large quantity of data on noninvasive ventilation (NIV) in acute respiratory failure is from patients who want all possible treatments and life-support. Few data are available on NIV in patients who have elected specific limits on life support and treatments (eg, patients with do-not-intubate [DNI] orders) and patients who are near the end of life and will receive comfort measures only (CMO). The most critical issue regarding NIV in DNI and CMO patients is informed consent. The patient must be informed of the risks and potential benefits of NIV, and must consent to NIV. We have few

[Noninvasive ventilation in palliative care and near the end of life].

INTRODUCTION: The occurrence of life threatening severe respiratory failure in patients with an incurable illness may be an indication for the use of noninvasive ventilation (NIV). STATE OF THE ART: Two approaches are associated with the use of NIV in palliative care settings. In the "palliative approach", NIV is proposed for patients with end stage of chronic respiratory failure and do-not-tracheostomize orders as a ceiling of care. In the "palliative and probably curative" approach, NIV may help patients with do-not-intubate orders or to forego endotracheal intubation. This review provides

[Noninvasive ventilation in the acute care hospital--a cost factor?].

BACKGROUND: Noninvasive ventilation as well established in treatment of chronic respiratory failure. Many announcements and our own experience give evidence that this method of treatment is useful for patients with acute respiratory failure too. Also the actual situation of our health system requires increasing attention to financial points of view. PATIENTS AND METHOD: We analyzed a number of 185 patients who needed mechanical ventilation in our intensive care unit in 1995. 80 of these 185 needed mechanical ventilation due to pulmonary and cardiopulmonary diseases (e. g. cardiac failure, exa

Noninvasive Positive Pressure Ventilation in the Adult Acute Care Setting.

BACKGROUND: Noninvasive positive pressure ventilation (NIPPV) is a method of mechanical ventilation that provides respiratory support for patients with varying acute or chronic respiratory conditions and can be an effective intervention that may result in the avoidance of endotracheal intubation. NIPPV relies on the combination of a patient's ability to spontaneously breathe and the assistance of a ventilator to produce an inspiratory positive airway pressure, resulting in increased tidal volumes as well as improved ventilation and gas exchange. OBJECTIVES: This manuscript aims to provide a c

Long-term noninvasive ventilation in children with fibrodysplasia ossificans progressiva.

Fibrodysplasia ossificans progressiva (FOP) is an autosomal dominant rare disease characterized by progressive episodic multi-focal heterotopic ossification of skeletal muscle, ligaments, tendons, and fascia, associated with congenital malformation of the great toes. Progressive thoracic deformity with scoliosis is present in approximately 65% of patients, causing restrictive lung disease, respiratory failure, and premature death. We report 3 children who developed severe scoliosis and sleep-disordered breathing with nocturnal hypoventilation, and were treated with long-term noninvasive ventil

High-Flow Nasal Oxygen and Noninvasive Ventilation for COVID-19.

High-flow nasal oxygen (HFNO) and noninvasive ventilation (NIV) via facemask or helmet have been increasingly used in managing acute hypoxemic respiratory failure (AHRF) owing to COVID-19 with the premise of reducing the need for invasive mechanical ventilation and possibly mortality. Their use carries the risk of delaying intubation and nosocomial infection transmission. To date, most studies on the effectiveness of these modalities are observational and suggest that HFNO and NIV have a role in the management of AHRF owing to COVID-19. Trials are ongoing and are evaluating different aspects o

Delirium in Pediatric Patients With Respiratory Insufficiency Requiring Noninvasive Ventilation.

BACKGROUND: Delirium is associated with increased length of stay, duration of mechanical ventilation, in-hospital mortality, and cost. Independent predictors of delirium include age < 2 years, developmental delay, severity of illness, mechanical ventilation, and administration of benzodiazepines and anticholinergic medications. Although patients receiving noninvasive ventilation (NIV) may have been included in prior studies, there are no data specifically focusing on delirium in children receiving NIV. Our primary aim was to investigate the prevalence of delirium in patients on NIV in the pedi

Man on Noninvasive Positive Airway Pressure Ventilation With Throat Pain.

Dentures dislodged into throat on bilevel positive airway pressure (BiPAP) ventilation can be overlooked and potentially compromise airway patency. An 81-year-old man with a history of chronic obstructive pulmonary disease (COPD) presented with increased shortness of breath and productive cough for 1 week. Inhaled bronchodilators, parenteral steroids, and BiPAP ventilation were administered for acute exacerbation of COPD complicated with acute hypercapnic respiratory failure. Fifty minutes after receiving BiPAP ventilation, his respiratory condition improved; however, he started to complain of

Noninvasive ventilation in acute respiratory failure: which recipe for success?

Noninvasive positive-pressure ventilation (NPPV) to treat acute respiratory failure has expanded tremendously over the world in terms of the spectrum of diseases that can be successfully managed, the locations of its application and achievable goals. The turning point for the successful expansion of NPPV is its ability to achieve the same physiological effects as invasive mechanical ventilation with the avoidance of the life-threatening risks correlated with the use of an artificial airway.Cardiorespiratory arrest, extreme psychomotor agitation, severe haemodynamic instability, nonhypercapnic c

Noninvasive ventilation in difficult endotracheal intubation: systematic and review analysis.

Noninvasive ventilation has been widely used in the management of acute respiratory failure in appropriate clinical settings. In addition to known benefit of alleviating the need for invasive mechanical ventilation, recent literature suggested its beneficial use in the process of endotracheal intubation. Search of the PubMed database and manual review of selected articles investigating the methods and outcomes of endotracheal intubation in difficult airway due to hypoxemic respiratory failure and the role of noninvasive ventilation in this process. Large randomized controlled studies focused o

Aerosol Therapy During Noninvasive Ventilation or High-Flow Nasal Cannula.

Noninvasive ventilation (NIV) and high-flow nasal cannula (HFNC) are increasingly used for patients with acute respiratory failure. Some patients receiving these therapies might also benefit from inhaled drug delivery. Thus, it is attractive to combine aerosol therapy with NIV or HFNC. The purpose of this paper is to review the available evidence related to the use of inhaled aerosols with NIV or HFNC. Available evidence supports the delivery of aerosols during NIV. Inhaled bronchodilator response might be improved with the use of NIV in acute asthma, but the evidence is not sufficiently matur

Where is Noninvasive Ventilation Actually Delivered for Acute Respiratory Failure?

PURPOSE: Few studies have examined locations of noninvasive ventilation (NIV) application for acute respiratory failure (ARF). We aimed to track actual locations of NIV delivery and related outcomes. METHODS: Observational cohort study based at 8 acute care hospitals in Massachusetts on adult patients admitted for ARF requiring ventilatory support during pre-determined time intervals. RESULTS: Of 1225 ventilator starts, 499 were NIV; 209 (42%) in intensive care units (ICU), 185 (37%) in emergency departments (ED), 91 (18%) on general wards, and 14 (3%) in other units. The importance of controlling PaCO₂ throughout long-term noninvasive ventilation.

BACKGROUND: The significance of changes in P(aCO2) during long-term noninvasive ventilation (NIV) on prognosis remains unclear. We aimed to clarify whether stabilizing P(aCO2) during NIV had a favorable prognostic effect. METHODS: Data from 190 subjects with restrictive thoracic disease and who received long-term NIV were studied retrospectively. The annual change in P(aCO2) during NIV was determined using a simple linear regression method for each subject who had at least 4 6-month intervals of P(aCO2) data. Annual changes in P(aCO2) during long-term NIV and possible confounders were analyze

Use of neonatal chest ultrasound to predict noninvasive ventilation failure.

BACKGROUND: Noninvasive ventilation is the treatment of choice for neonatal moderate respiratory distress (RD). Predictors of nasal ventilation failure are helpful in preventing clinical deterioration. Work on neonatal lung ultrasound has shown that the persistence of a hyperechogenic, "white lung" image correlates with severe distress in the preterm infant. We investigate the persistent white lung ultrasound image as a marker of noninvasive ventilation failure. METHODS: Newborns admitted to the NICU with moderate RD and stabilized on nasal continuous positive airway pressure for 120 minutes
