Computational design and experimental performance testing of an axial-flow pediatric ventricular assist device.

The Virginia Artificial Heart Institute continues to design and develop an axial-flow pediatric ventricular assist device (PVAD) for infants and children in the United States. Our research team has created a database to track potential PVAD candidates at the University of Virginia Children's Hospital. The findings of this database aided with need assessment and design optimization of the PVAD. A numerical analysis of the optimized PVAD1 design (PVAD2 model) was also completed using computational fluid dynamics (CFD) to predict pressure-flow performance, fluid force estimations, and blood damag

Computer-assisted system for diagnosis of Alzheimer disease using data base- independent estimation and fluorodeoxyglucose- positron-emission tomography and 3D-stereotactic surface projection.

BACKGROUND AND PURPOSE: Recently, voxel-based statistical parametric images have been developed as additional diagnostic tools for AD. However these methods require the generation of a data base of healthy brain images. The purpose of this study was to produce and evaluate an automatic method using a data base-independent estimation system for the diagnosis of mild AD. MATERIALS AND METHODS: We retrospectively selected 66 subjects, including 33 patients with early AD and 33 age-matched healthy volunteers. Individual brain metabolic images were obtained by using FDG-PET. These were transformed

The venous return simulator: an effective tool for investigating the effects of external compression on the venous hemodynamics--first results after thigh compression.

BACKGROUND: To present a virtual model, the venous return simulator (VRS), designed to compute venous hemodynamic variations when compression is applied to the leg. METHODS: The VRS defines a numerical network of the lower extremity and computes the dynamic variables (flow rate, venous diameter and internal pressure) for a defined external pressure. The VRS was based on physiological data from the literature and clinical studies on healthy subjects. Clinical correlations were required to confirm its validity; for this purpose, we carried out experiments simulating the conditions of a clinical

Infant skull and suture properties: measurements and implications for mechanisms of pediatric brain injury.

The mechanical properties of the adult human skull are well documented, but little information is available for the infant skull. To determine the age-dependent changes in skull properties, we tested human and porcine infant cranial bone in three-point bending. The measurement of elastic modulus in the human and porcine infant cranial bone agrees with and extends previous published data [McPherson, G. K., and Kriewall, T. J. (1980), J. Biomech., 13, pp. 9-16] for human infant cranial bone. After confirming that the porcine and human cranial bone properties were comparable, additional tensile a

Computed tomography liver volumetry using 3-dimensional image data in living donor liver transplantation: effects of the slice thickness on the volume calculation.

The purpose of this study was to evaluate the relationship between the slice thickness and the calculated volume in computed tomography (CT) liver volumetry through the comparison of the results from images [including 3-dimensional (3D) images] with various slice thicknesses. Twenty potential adult liver donors (12 men and 8 women) with a mean age of 39 years (range = 24-64 years) underwent CT with a 64-section multidetector row CT scanner after the intravenous injection of a contrast material. Four image sets with slice thicknesses of 0.625, 2.5, 5, and 10 mm were used. First, a program devel

A Bayesian semiparametric multivariate joint model for multiple longitudinal outcomes and a time-to-event.

Motivated by a real data example on renal graft failure, we propose a new semiparametric multivariate joint model that relates multiple longitudinal outcomes to a time-to-event. To allow for greater flexibility, key components of the model are modelled nonparametrically. In particular, for the subject-specific longitudinal evolutions we use a spline-based approach, the baseline risk function is assumed piecewise constant, and the distribution of the latent terms is modelled using a Dirichlet Process prior formulation. Additionally, we discuss the choice of a suitable parameterization, from a p

Faces of emotion in Parkinsons disease: micro-expressivity and bradykinesia during voluntary facial expressions.

In humans, the neural circuitry underlying facial expressions differs, depending on whether facial expressions are spontaneously (i.e., limbic, subcortical) or voluntarily initiated (i.e., frontal cortex). Previous investigators have suggested that the "masked face" of Parkinson's disease involves spontaneous, but not intentional, facial expressions. In contrast, we hypothesized that intentional facial expressions may be slowed (bradykinetic) and involve less movement, in much the same way that other intentional movements are affected by Parkinson's disease. To test this hypothesis, we used so

Multi-layer microfluidic glass chips for microanalytical applications.

A new, versatile architecture is presented for microfluidic devices made entirely from glass, for use with reagents which would prove highly corrosive for silicon. Chips consist of three layers of glass wafers bonded together by fusion bonding. On the inside wafer faces a network of microfluidic channels is created by photolithography and wet chemical etching. Low dead-volume fluidic connections between the layers are fabricated by spark-assisted etching (SAE), a computer numerical controlled (CNC)-like machining technique new to microfluidic system fabrication. This method is also used to for

Ribosomal RNA gene restriction patterns of Helicobacter pylori: analysis and appraisal of Hae III digests as a molecular typing system.

Ribosomal RNA gene restriction patterns (ribopatterns) of 162 strains of Helicobacter pylori from 93 patients were studied to assess their suitability for use as the basis of a molecular typing system. Computer-assisted numerical analysis of Hae III ribopatterns of 122 strains from 9 countries in 4 continents showed that almost every strain had a distinct and unique ribopattern and only strains from the same individual were genomically matched in all bands or with minor (1-2 band) differences. Hae III ribopatterns offered high typability and reproducibility but were too discriminatory for larg

Mathematical model to assess vaccination and effective contact rate impact in the spread of tuberculosis.

The long and binding treatment of tuberculosis (TB) at least 6-8 months for the new cases, the partial immunity given by BCG vaccine, the loss of immunity after a few years doing that strategy of TB control via vaccination and treatment of infectious are not sufficient to eradicate TB. TB is an infectious disease caused by the bacillus Mycobacterium tuberculosis. Adults are principally attacked. In this work, we assess the impact of vaccination in the spread of TB via a deterministic epidemic model (SVELI) (Susceptible, Vaccinated, Early latent, Late latent, Infectious). A novel pattern recognition algorithm to classify membrane protein unfolding pathways with high-throughput single-molecule force spectroscopy.

MOTIVATION: Misfolding of membrane proteins plays an important role in many human diseases such as retinitis pigmentosa, hereditary deafness and diabetes insipidus. Little is known about membrane proteins as there are only very few high-resolution structures. Single-molecule force spectroscopy is a novel technique, which measures the force necessary to pull a protein out of a membrane. Such force curves contain valuable information on the protein structure, conformation, and inter- and intra-molecular forces. High-throughput force spectroscopy experiments generate hundreds of force curves incl

Computational Investigation of a Self-Powered Fontan Circulation.

Children born with anatomic or functional "single ventricle" must progress through two or more major operations to sustain life. This management sequence culminates in the total cavopulmonary connection, or "Fontan" operation. A consequence of the "Fontan circulation", however, is elevated central venous pressure and inadequate ventricular preload, which contribute to continued morbidity. We propose a solution to these problems by increasing pulmonary blood flow using an "injection jet" (IJS) in which the source of blood flow and energy is the ventricle itself. The IJS has the unique property

Coronary Stent Fracture: Clinical Evidence Vs. the Testing Paradigm.

PURPOSE: The United States' Food and Drug Administration (FDA) recommends that device manufacturers demonstrate 10 years of equivalent life duration for endovascular stents. Yet since the early 2000s clinical evidence of stent strut fracture defies the recommendations for these FDA approved devices. Stent strut fracture has been correlated with a higher incidence of adverse clinical events, such as in-stent thrombosis and in-stent restenosis. METHODS: This paper reviews the current clinical evidence, computational modelling relating to fatigue lifetimes, experimental testing of coronary stent

Role of residual mandibular teeth after computer-assisted mandibular reconstruction using a fibular flap.

BACKGROUND: Computer-assisted mandibular reconstruction after mandibulectomy enables accurate reconstruction of the occlusal positions between the maxilla and mandible. Understanding the remaining teeth condition is essential for sensory mastication in patients with numerous tooth loss. However, no studies have examined the dental status of the remaining mandible after computer-assisted mandibular reconstruction using a fibular flap. This study evaluated the role of residual teeth and other factors in effective acquisition of oral intake after computer-assisted mandibular reconstruction using

The subthalamic nucleus in primary dystonia: single-unit discharge characteristics.

Most models of dystonia pathophysiology predict alterations of activity in the basal ganglia thalamocortical motor circuit. The globus pallidus interna (GPi) shows bursting and oscillatory neuronal discharge in both human dystonia and in animal models, but it is not clear which intrinsic basal ganglia pathways are implicated in this abnormal output. The subthalamic nucleus (STN) receives prominent excitatory input directly from cortical areas implicated in dystonia pathogenesis and inhibitory input from the external globus pallidus. The goal of this study was to elucidate the role of the STN i

Aortic hemodynamics after thoracic endovascular aortic repair, with particular attention to the bird-beak configuration.

PURPOSE: To quantitatively evaluate the impact of thoracic endovascular aortic repair (TEVAR) on aortic hemodynamics, focusing on the implications of a bird-beak configuration. METHODS: Pre- and postoperative CTA images from a patient treated with TEVAR for post-dissecting thoracic aortic aneurysm were used to evaluate the anatomical changes induced by the stent-graft and to generate the computational network essential for computational fluid dynamics (CFD) analysis. These analyses focused on the bird-beak configuration, flow distribution into the supra-aortic branches, and narrowing of the d

Comparison of pulmonary arterial flow phenomena in spiral and Lecompte models by computational fluid dynamics.

OBJECTIVES: The transposed great arteries are simply reversed by means of a conventional arterial switch operation with the Lecompte maneuver without resumption of their spiral relationship. We seek to clarify the functional implications of the spiral relationship of the great arteries by means of mathematic modeling. METHODS: Computational fluid dynamics is used to compare flow phenomena of the spiral and Lecompte (nonspiral) models under various body surface areas. RESULTS: The velocity profile and wall-shear stress distribution are more uniform for the spiral than for the Lecompte model.

Formant frequency estimation of high-pitched vowels using weighted linear prediction.

All-pole modeling is a widely used formant estimation method, but its performance is known to deteriorate for high-pitched voices. In order to address this problem, several all-pole modeling methods robust to fundamental frequency have been proposed. This study compares five such previously known methods and introduces a technique, Weighted Linear Prediction with Attenuated Main Excitation (WLP-AME). WLP-AME utilizes temporally weighted linear prediction (LP) in which the square of the prediction error is multiplied by a given parametric weighting function. The weighting downgrades the contrib

[Sector-related grey scale analysis of video ultrasound recorded tongue movements in swallowing].

Diseases of the oral cavity, floor of the mouth, and nervous system can be accompanied by disturbances in tongue movement during swallowing. These disturbances can be diagnosed by videosonography whereby the examiner has to evaluate extensive video documentation of lingual motion. It was the aim of this study to facilitate this evaluation by the application of a reproducible computer-assisted quantitative analysis procedure. Video sequences of 56 healthy adults and 19 patients with dysphagias of different etiologies were analysed. A numerical estimation of swallowing movements was carried out
