Liraglutide in the management of obesity: real world data (Portugal).

BACKGROUND: Overweight and obesity are major public health issues with increasing incidence and prevalence, affecting more than 50% of the population in developed countries. Due to its complex pathophysiology and multifactorial etiology, disease understanding, diagnostic approach and management remain suboptimal. Together with a structured nutritional intervention and physical activity plan, pharmacological treatment has the potential to magnify weight loss and health related benefits. Liraglutide is one of the most effective and frequently prescribed weight loss medication. Its efficacy and s

Surgical management of obesity using a soft adjustable gastric band.

BACKGROUND: Laparoscopically inserted adjustable gastric bands are an effective means of weight reduction for obese patients. METHODS: The Swedish Adjustable Gastric Band was evaluated prospectively in a consecutive series of 273 patients from August 1996 to March 2003. RESULTS: The first 58 patients had their band sited by the 'peri-gastric' technique, with the subsequent 215 using the 'pars flaccida' technique, which provides better posterior fixation of the band. Results from these two groups were considered separately. Prolapse of the stomach through the band was the major complication o

Modern medical management of obesity: the role of pharmaceutical intervention.

The medical model of obesity treatment--combining diet, exercise, and behavior modification with antiobesity agents--suffered a setback when fenfluramine and dexfenfluramine were withdrawn from the market because of an association between these medications and valvular regurgitation. The Food and Drug Administration has recently approved sibutramine (Meridia), a norepinephrine and serotonin reuptake inhibitor that was originally developed as an antidepressant, but which has also been shown to reduce weight. In a 1-year placebo-controlled trial, 65% of patients receiving 15 mg sibutramine daily

Obesity: a move from traditional to more patient-oriented management.

BACKGROUND: Family physicians have been encouraged to be aggressive in their treatment of obesity in their overweight patients. This article examines our traditional management of obesity and presents a management approach that is relatively safe, healthy, and patient-oriented. METHODS: A review of the literature on obesity and its treatment was performed by searching MEDLINE, PsychINFO, ERIC, and SOCA data bases from 1980 to 1993. Additional references were accessed by cross-referencing the bibliographies of the articles obtained through this search. RESULTS AND CONCLUSIONS: Both physicians

Reappraising obesity management in the GLP-1 receptor agonist era.

Incretin-based therapies are increasingly used for obesity management, raising debate about whether they could challenge or replace metabolic bariatric surgery (MBS). Here, we compare evidence for both treatments, focusing on effectiveness, limitations and costs. We also explore how obesity care pathways may evolve and highlight key evidence gaps that must be addressed to define the future roles of incretin-based therapies and MBS in obesity management.

Obesity in Classic Congenital Adrenal Hyperplasia: Mechanisms, Complications and Management.

Classic congenital adrenal hyperplasia (CCAH) is an autosomal recessive genetic disorder primarily caused by 21-hydroxylase deficiency. Although the survival rate of patients has significantly improved with glucocorticoid replacement therapy, long-term use of supraphysiological doses and multiple factors inherent to the disease itself have led to obesity becoming a common metabolic complication in CCAH patients. The mechanism of obesity in CCAH patients is complex and involves multiple factors related to both treatment and the disease itself. This article provides a narrative review of the inc

Current and future state of pharmacological management of pediatric obesity.

Pediatric obesity is a highly prevalent chronic disease, which has traditionally been treated with lifestyle therapy alone. Yet for many youth, lifestyle intervention as a monotherapy is often insufficient for achieving clinically significant and durable BMI reduction. While metabolic/bariatric surgery achieves robust and long-lasting outcomes, it is neither widely accessible nor wanted by most pediatric patients and families. In the past 3 years, this treatment gap between lifestyle therapy and metabolic/bariatric surgery has been filled with a number of landmark clinical trials examining the

Personalized optimal nutrition lifestyle for self obesity management using metaalgorithms.

Precision medicine applies machine learning methods to estimate the personalized optimal treatment decision based on individual information, such as genetic data and medical history. The main purpose of self obesity management is to develop a personalized optimal life plan that is easy to implement and adhere to, thereby reducing the incidence of obesity and obesity-related diseases. The methodology comprises three components. First, we apply catboost, random forest and lasso covariance test to evaluate the importance of individual features in forecasting body mass index. Second, we apply meta

Approaches to Obesity Management in Dialysis Settings: Renal Dietitian Perspectives.

OBJECTIVES: Over 40% of individuals in the United States with end-stage kidney disease have obesity. Little is known about renal dietitian perspectives on obesity management in the setting of dialysis dependence. DESIGN AND METHODS: An online 21-item survey was distributed to 118 renal dietitians via individual outreach and a professional organization e-mail listserv. Four themes were explored: the burden of obesity among dialysis patients, concepts of healthy weight loss, weight loss approaches, and challenges of obesity management in dialysis settings. Respondents were asked to rank approac

Epigenetics and obesity cardiomyopathy: From pathophysiology to prevention and management.

Uncorrected obesity has been associated with cardiac hypertrophy and contractile dysfunction. Several mechanisms for this cardiomyopathy have been identified, including oxidative stress, autophagy, adrenergic and renin-angiotensin aldosterone overflow. Another process that may regulate effects of obesity is epigenetics, which refers to the heritable alterations in gene expression or cellular phenotype that are not encoded on the DNA sequence. Advances in epigenome profiling have greatly improved the understanding of the epigenome in obesity, where environmental exposures during early life resu

Lercanidipine/enalapril combination in the management of obesity-related hypertension.

Obesity-related hypertension represents a condition frequently observed in current clinical practice characterized by a complex pathophysiological background and a very high cardiovascular risk profile, particularly in severely obese individuals. This explains, on the one hand, the difficulty in reducing elevated blood pressure values in this pathological state and, on the other, the need to achieve this goal in a relatively short-time period to prevent the occurrence of fatal and nonfatal cardiovascular events. Both nonpharmacological and pharmacological measures are available in the therapeu

[Difficult airway management due to obesity and lingual tonsillar hyperplasia].

We experienced anesthesia for a female patient of BMI 39.2 with unanticipated lingual tonsillar hyperplasia. At the induction of anesthesia, both mask ventilation and tracheal intubation were difficult due to the lingual tonsillar hyperplasia. However, tracheal intubation was performed by Intubating LMA with the fiberoptic bronchoscopy. After the placing of TE (the catheter for tracheal tube exchanger), the tracheal tube was extubated without any airway trouble. Following the extubation, the opening of the respiratory tract was evaluated by TE and the fiberoptic bronchoscopy. In this case we c

Molecular and genetic mechanisms of obesity: implications for future management.

Obesity has become a worldwide public health problem affecting millions of people. A disruption of the balance between energy intake and energy expenditure is believed to be the major cause of obesity. Substantial progress has been made in deciphering the pathogenesis of energy homeostasis over the past few years. The fact that obesity is under strong genetic control has been well established. Human monogenic obesity is rare in large populations, the most common form of obesity is considered to be a polygenic disorder arising from the interaction of multiple genetic and environmental factors.

Nonoperative management of stomal stenosis after gastroplasty for morbid obesity.

Over a period of six years, 33 of 172 (19 per cent) patients who had gastric partitioning had stomal stenosis develop which was defined as an inability to drink fluids or swallow saliva, or both. All were managed conservatively in the hospital or on an outpatient basis. This consisted of total parenteral nutrition and endoscopy to evaluate the stoma with or without dilation; Eder-Puestow dilaters and long term jejunostomy feeding were used. Thirteen of these patients were dilated a total of 36 times. Three required three to six dilations each, up to the maximum size (45F). There were no compli

Hyperuricemia, gout and obesity: should weight management be more of a focus in gout management?

AIMS: Hyperuricemia-elevated serum uric acid (SUA)-is the pathogenic driver of gout and is recognized as a systemic condition associated with cardiometabolic risk, including cardiovascular disease and type 2 diabetes, although causality remains debated. Its global prevalence has risen substantially over decades, paralleling the growing epidemic of obesity and increasing life expectancy. This narrative review examines the relationship between adiposity and hyperuricemia and summarises the knowledge about the impact of weight-loss interventions on SUA levels and gout risk.
