Odontoid screw placement for Anderson type II odontoid fractures: how do duration from injury to surgery and clinical and radiological factors influence the union rate? A multicenter retrospective study.

OBJECTIVE: Anderson type II odontoid fractures are severe conditions, mostly affecting elderly people (≥ 70 years old). Surgery can be performed as a primary treatment or in cases of failed conservative management. This study aimed to investigate how duration from injury to surgery, as well as clinical, radiological, and surgical risk factors, may influence the union rate after anterior odontoid screw placement for Anderson type II odontoid fractures. METHODS: The authors conducted a retrospective multicenter study. Demographic, clinical, surgical, and radiological data of patients who underw

[Odontoid fracture after high impact trauma : Assess instability correctly].

Odontoid process fractures are frequent injuries of the cervical spine and usually occur at an advanced age and often due to minor trauma. In younger patients they are mainly observed in the context of high impact trauma. Odontoid fractures are classified according to Anderson and D'Alonzo. In rare cases the fracture results in cervical myelopathy, which can be life-threatening.In this article, two cases of patients with odontoid fractures and traumatic myelopathy are presented. The first case concerns a type III fracture, the second a type II fracture. In both cases the instability present wa

Diagnostic Challenge of a Tumor-mimicking Spinal Pseudogout Lesion: Case Report and Literature Review.

BACKGROUND/AIM: Retro-odontoid pseudotumor (ROP) is a non-neoplastic soft-tissue mass occurring around the C2 odontoid process and is most often associated with rheumatoid atlantoaxial instability. In rare instances, calcium pyrophosphate dihydrate (CPPD) crystal deposition (pseudogout) in the C1-C2 ligaments - a presentation known as "crowned dens syndrome" - can produce a periodontoid mass. Such CPPD-induced lesions usually present with acute neck pain and stiffness, with calcifications visible around the odontoid process on imaging. However, these masses can occasionally enlarge and compres

Stress Maneuver for Basilar Invagination Reduction: A Technical Report.

BACKGROUND: Basilar invagination (BI) is a prolapse of the odontoid process cranially and posteriorly towards the foramen magnum. Several surgical treatment options are available for this condition. Herein, we describe a stress maneuver technique for BI reduction using a single-stage posterior approach. METHODS: We described the case of a 56-year-old male diagnosed with type A BI and brainstem compression, who was treated with posterior fossa decompression and occipitocervical fusion. The BI was reduced intraoperatively using a stress maneuver, and cranial traction or atlantoaxial joint distr

Complete Spontaneous Regression of the Intraspinal Synovial Cyst at the C1-C2 Junction Following with Atlantoaxial Fusion of Non-Union Odontoid Fracture: A Case Report.

The author reported an extremely rare case of intraspinal synovial cyst at C1-C2 junction, located posteriorly to the odontoid process, compressing the upper cervical spinal cord. The patient had a history of spinal trauma 40 years ago without prior definitive treatment, presenting with two months of left leg and arm numbness with neck pain. Imaging studies including plain radiographs, computed tomography (CT), and magnetic resonance imaging (MRI) of the cervical spine showed a large synovial cyst associated with atlantoaxial instability from non-union fracture of the odontoid process. The atl

Traumatic rotary posterior dislocation of the atlas on the axis without fracture. Report of a case and review of literature.

BACKGROUND: Traumatic rotary posterior dislocation of C1 on C2 without fracture (TRPDAAWF) is a very rare complication in severe accidents, and its pathophysiology is still under debate. CASE DESCRIPTION: Although 6 similar cases have been reported previously, we report the lessons learned during handling of the patient. A young man who is a victim of severe car crash injury, with neglected displacement of the intact odontoid process to the front of the anterior arch of C1, is reported. Some problematic and life-threatening events in the management of the case are presented. CONCLUSIONS: We

Prevalence of os odontoideum: a systematic review, meta-analysis and meta-regression with implications for vertebral artery and osseous anomalies.

PURPOSE: Os Odontoideum (OO) is an anatomical anomaly of the craniocervical junction, which may lead to severe instability, resulting in potentially fatal spinal cord compression. Considering clinical improtance of OO, the aim of this study was to provide a thorough synthesis of data on its epidemiology, specifically regarding its prevalence with neurosurgical considerations. METHODS: The major electronic databases (Pubmed/Medline, Embase, Scopus, ScienceDirect) were thoroughly searched up to July 18th, 2025. Studies reporting original data on the prevalence of OO were deemed eligible for inc

[Post-traumatic instability between the atlas and the axis in children. Apropos of 5 cases].

The authors have observed 5 cases of atlanto-axial instability in children aged from 18 months to 8 years. All the cases were seen after severe trauma and presented with abnormal neurological signs. All patients were first treated conservatively and were operated on secondarily by atlanto-axial fusion using graft and wiring for persistent radiological instability between the atlas and axis. In two cases instability was due to an atlanto-axial subluxation with rupture of the transverse ligament; one of these was reduced by traction. In one case there was an anterior subluxation without fracture

Cervical 1-2 Posterior Instrumented Fusion Utilizing Computer-Assisted Navigation With Harvest of Rib Strut Autograft: 2-Dimensional Operative Video.

Nonunion of a type II odontoid fracture after the placement of an anterior odontoid screw can occur despite careful patient selection. Countervailing factors to successful fusion include the vascular watershed zone between the odontoid process and body of C2 as well as the relatively low surface area available for fusion. Patient-specific factors include osteoporosis, advanced age, and poor fracture fragment apposition. Cervical 1-2 posterior instrumented fusion is indicated for symptomatic nonunion. The technique leverages the larger posterolateral surface area for fusion and does not rely on

[Anterior stabilization of the CI-CIII vertebrae after transoral removal of an aggressive aneurysmal bone cyst of the CII vertebra (a case report and literature review)].

UNLABELLED: Treatment of patients with atlantoaxial instability caused by various diseases of the skull base and craniovertebral junction combined with ventral compression of the brainstem is a complex issue that is ambiguously resolved in different ways. We present a case of stepped treatment, the most important component of which was successful transoral removal of an aggressive aneurysmal bone cyst of the CII vertebra with anterior CI-CIII segment stabilization using an individual system, which was performed through the same approach. CLINICAL CASE: A 31-year-old male patient presented wit

Staged dorsolateral decompression and posterior stabilization for severe ventral odontoid compression in basilar invagination: illustrative case.

BACKGROUND: Basilar invagination (BI) with ventral cervicomedullary compression remains surgically challenging. Although transoral and endoscopic endonasal odontoidectomy are established approaches, some anatomical variations may favor alternative surgical corridors. OBSERVATIONS: A patient with BI developed progressive quadriparesis after cervical trauma. Imaging demonstrated severe ventral cervicomedullary compression from a retroverted, left-lateralized odontoid process, displacing the cervicomedullary junction contralaterally. CT angiography revealed absence of the left vertebral artery (

Conservative versus Surgical Treatment for Odontoid Fracture: Is the Surgical Treatment Harmful? Systematic Review and Meta-Analysis.

BACKGROUND: Odontoid fracture is a common injury especially in elderly people. Despite some recent studies arguing in favor of surgery, the best treatment is still being debated. OBJECTIVE: We systematically review and analyze the comparative literature between surgical and conservative treatments of odontoid fractures. METHODS: We systematically searched Medline and the Cochrane Library for studies reported from January 1990 to May 2019 in English. Comparative studies evaluating the results of surgical and conservative treatments for odontoid fractures were eligible for inclusion. Combined

[CME: Crowned-Dens-Syndrome - the Chameleon of Neck Pain].

CME: Crowned-Dens-Syndrome - the Chameleon of Neck Pain Abstract. Crowned Dens Syndrome (CDS) is a rare, under-diagnosed differential diagnosis of acute neck pain. Diagnosis is a challenge in clinical practice due to the similarity of symptoms to other diseases. Knowing this differential diagnosis of acute neck pain, unnecessary examinations and expensive therapy attempts can be avoided with a targeted approach. With this article we would like to sensitize colleagues accordingly.

Surgical treatment of Type II odontoid fractures: anterior odontoid screw fixation or posterior cervical instrumented fusion?

Odontoid fractures comprise as many as 20% of all cervical spine fractures. Fractures at the dens base, classified by the Anderson and D'Alonzo system as Type II injuries, are the most common pattern of all odontoid fractures and are also the most common cervical injuries in patients older than 70 years of age. Surgical treatment is recommended for patients older than 50 years with Type II odontoid fractures, as well as in patients at a high risk for nonunion. Anterior odontoid screw fixation (AOSF) and posterior cervical instrumented fusion (PCIF) are both well-accepted techniques for surgica

Cervicocranium and the aviator's protective helmet.

An analogy based on the likenesses of the APH-6 aviator's protective helmet and the hangman's noose has some interesting relationships to aircraft accident investigation and to the associated traumatic pathology. A superficial examination of the similarities might propose that the inferior edge of the helmet, when considered part of the continuous circle completed by the nape strap and the chin strap, forms a loop which can be likened to a hangman's noose. The analogy might be extended to the comparison of lesions made about the neck by the straps or the edge of the helmet compared with abrasi

Crowned Dens Syndrome Presenting as Failure to Thrive in a Patient With Moderate Cognitive Impairment.

Crowned dens syndrome (CDS) is a rare condition characterized by the deposition of calcium pyrophosphate (CPP) or hydroxyapatite crystals around the odontoid process of the C2 vertebra, often presenting with acute neck pain, reduced cervical mobility, and systemic inflammatory response. This case describes a 68-year-old male patient with a history of type 2 diabetes, hypertension, and benign prostatic hyperplasia who presented with generalized weakness and failure to thrive. This atypical presentation initially masked symptoms of CDS, delaying the diagnosis until the patient developed acute ne

Minimally invasive anterior vertebroplasty for C-2 metastatic lesions.

The authors describe a technique for minimally invasive anterior vertebroplasty for treating metastatic disease of the C-2 vertebra and discuss its application in 2 cases. After a 2-cm lateral neck incision is made, blunt dissection is performed toward the anterior inferior endplate of the C-2 vertebra. An 11-gauge needle is introduced through a tubular sheath and tapped into the inferior endplate of C-2, with biplanar fluoroscopy being performed to confirm position. The needle is subsequently advanced across the fracture line and into the odontoid process. Under fluoroscopic guidance, 2 ml of
