Management of comminuted scapular fracture by open reduction and internal fixation: A case report.

INTRODUCTION: Scapular fracture is rare fracture as it comprises of only 3 % to 5 % of total shoulder fractures also in that, comminuted type is most uncommon type. High velocity force and direct blow are the most common cause of scapular fracture. We report a case of 60 years male with comminuted scapular fracture following physical assault by stick over back. CASE PRESENTATION: The case study presents a sixty years old male who sustained comminuted scapular fracture following physical assault by stick in the back. The patient presented to Emergency Department with complains of pain and swel

Open reduction and C1C3 posterior Harms-Goel fixation for unstable Hangman's fracture: technical note.

Hangman's fracture occurs in the elderly following low kinetic energy fall from their height, or in the young during traffic accident. Classically described as bipedicular C2 fracture, Hangman's fracture results from oblique hyperextension-flexion vector forces which can lead to anterior dislocation of C1C2 complex over C3 vertebral body in case of associated damage to C2C3 disc and ligaments. Even though most cases of Hangman's fracture are not displaced (grade 1) and can be managed with orthopaedic treatment using cervical brace, highly displaced cases of Hangman's fractures (some grade 2, a

Impetigo Surgical Site Infection Following Open Reduction and Internal Fixation of a Patella Fracture.

Impetigo is a pediatric skin infection characterized by the presence of pathognomonic "honey-crusted" lesions caused by either Staphylococcus aureus or Streptococcus pyogenes. The diagnosis of impetigo is largely based on clinical judgment, confirmatory skin cultures, and Gram staining. Surgical site infections following patellar surgery are a relatively uncommon occurrence, with the most common causative organisms being Staphylococcus aureus, Streptococcus pyogenes, Pseudomonas aeruginosa, and other gram-negative bacilli. Surgical site infections have a range of risk factors that largely depe

Open reduction and internal fixation for posterior pilon fracture: Transfibular approach versus posterior approach.

BACKGROUND: For posterior pilon fractures, the posterior approach (PA) is widely used but has its limitations. The transfibular approach (TFA) has been adopted to treat posterior malleolar fractures for the advantage of direct visual confirmation of the reduction status intraoperatively, yet the report of its application on posterior pilon fractures is rare. This study aims to compare TFA with PA in terms of their corresponding reduction quality. METHODS: Clinical data of 85 posterior pilon fracture patients treated via PA (n = 62) or TFA (n = 23) were retrospectively reviewed. Radiographic a

Functional Outcomes of Symptomatic Implant Removal Following Ankle Fracture Open Reduction and Internal Fixation.

BACKGROUND: Orthopedic implant removal following open reduction internal fixation of a fracture is a common procedure, especially in the foot and ankle. The purpose of this study was to evaluate functional change after the removal of symptomatic implants following ankle fracture open reduction internal fixation (ORIF) using the Short Musculoskeletal Function Assessment (SMFA) outcome score. We hypothesized that implant removal after ankle fracture would result in improved functional outcomes. METHODS: Retrospective review of prospectively collected data on patients with a closed ankle fractur

Clinical effect of distal radius fracture treated with open reduction and internal plate fixation.

BACKGROUND: For some specific comminuted unstable intra-articular fracture, the plaster cast can not maintain the alignment of the articular surface effectively. The aim of this study was to evaluate the clinical effects of distal radius fracture treated with open reduction and internal plate fixation retrospectively. METHODS: From January 2002 to March 2010, 539 cases of distal radius fracture were treated with open reduction and internal fixation, including 184 males and 355 females aging 21 - 72 years (mean 57 years). Fractures were caused by falling to the ground in 459 cases, by traffic

External iliac artery thrombosis after open reduction of an acetabular fracture: a case report.

Major arterial injury from open reduction and internal fixation of an acetabular fracture through an ilioinguinal approach is rare. Detection of a major intraoperative arterial injury is simple. Detection of a more occult injury leading to thrombosis is not. We report the third known case of thrombosis of the external iliac artery following open reduction and internal fixation of an anterior column fracture. This case is of particular interest because it highlights the ability of young patients to compensate for major vessel occlusion and still maintain a well-perfused foot. Heightened awarene

Osteochondral fracture of the fourth metatarsal head treated by open reduction and internal fixation.

Fracture of the metatarsal head is uncommon, and reports of isolated osteochondral fracture of the metatarsal head are rare. Because of the distal location of the fracture, it is difficult to achieve and maintain reduction, and potential complications include avascular necrosis and subchondral fatigue fracture. The authors present a case of an osteochondral fracture in a 40-year-old man, which was treated by open reduction and internal fixation with a single twist-off screw, with good results 12 months postoperatively.

Ultrasound-guided lumbar plexus block for open reduction and internal fixation of hip fracture.

PURPOSE: Ultrasound technology has been applied to increase both efficacy and safety of certain peripheral nerve blocks. This case report describes the first successful ultrasound-guided lumber plexus block. CLINICAL FEATURES: We describe a 91-year-old woman with aortic stenosis who successfully underwent open reduction and internal fixation of a fractured right hip with a lumbar plexus block. Ultrasound provided direct visualization to help identify the anatomical structures and guide the block needle during performance of the block. Complete block of the lumbar plexus was attained within 15

Open reduction and cementation for femoral head fracture secondary to avascular necrosis: preliminary report.

Current treatment for femoral head avascular necrosis has shown good results in early stages of disease, but are not as impressive after progression to collapse. We treated 19 patients (20 hips) with Stage III avascular necrosis (AVN) by open reduction augmented by methylmethacrylate cementation. Follow up ranges from 6 months to 2 years (average = 8.7 months). We followed patient progress with pre- and post-operative Harris Hip Scores, Womac Osteoarthritis Index and a Health Status Questionnaire (SF36). All patients realized immediate post-operative pain relief and improvement in function. Ha

Perilunate dislocations and fracture-dislocations. Closed and early open reduction compared in 28 cases.

27 patients (28 cases), were treated for perilunate dislocations in 20 and fracture-dislocations in 8. Capitate displacement was dorsal in 27 cases and palmar in 1 case, and in 16, the scaphoid was fractured. There was no substantial delay of treatment. In 8 patients treated with closed reduction, follow-up averaged 6 (1-15) years. 19 patients (20 cases) who underwent early open reduction with K-wire stabilization and ligamentous repair, had an average follow-up of 2 (0.5-7) years. The clinical outcome was evaluated using a scoring system based on pain, occupation, ROM and grip strength, while

Percutaneous reduction with double screwdrivers versus limited open reduction in the treatment of irreducible extracapsular hip fractures.

BACKGROUND: The reduction in irreducible extracapsular hip fractures has always been controversial. Here, we present a new minimally invasive reduction technique and compare it with limited open reduction (LOR) to treat irreducible extracapsular hip fractures. METHODS: From January 2016 to January 2018, our institution treated 653 patients with extracapsular hip fractures by intramedullary fixation. Among them, 525 cases got a successful closed reduction. The other 128 were irreducible and reduced by percutaneous reduction with double screwdrivers (PRDS) or LOR. There were 66 cases in the PRD

Risk Factors for Loss of Reduction After Open Reduction and Internal Fixation of Isolated Olecranon Fractures in Adults.

BACKGROUND: We sought to determine whether any relevant patient, fracture, surgical, or postoperative characteristics are associated with loss of reduction after plate fixation of isolated olecranon fractures in adults. METHODS: Patients who underwent open reduction and internal fixation of an olecranon fracture at our institution over an 11-year period were analyzed. Electronic patient charts and radiographic images were reviewed to gather patient, fracture, surgical, and postoperative data. Statistical analysis to explore the differences between groups was performed. RESULTS: Seven of 96 p

Extra-articular Metacarpal Fractures: Closed Reduction and Percutaneous Pinning Versus Open Reduction and Internal Fixation.

BACKGROUND: There is no consensus on the optimal operative treatment of isolated closed metacarpal fractures as every technique is associated with advantages and shortcomings. This retrospective study aims to compare the outcomes of single metacarpal, extra-articular fractures treated with closed reduction and percutaneous pinning (CRPP) versus open reduction and internal fixation (ORIF). METHODS: The charts of all patients who underwent surgical repair of closed metacarpal fractures at our institutions from 2009 to 2016 were reviewed. 70 patients met the inclusion criteria, 44 had undergone
