Medication-related osteonecrosis of the jaw (MRONJ) has emerged as a complication of anti-resorptive medications. Despite its low incidence rate, this problem has gained attention in recent years due to its devastating consequences and lack of preventive strategy. The fact that MRONJ incidence has been exclusive to the jawbones, despite the systemic effect of anti-resorptive medications, could be a starting point to unravel the multifactorial pathogenesis of this condition. Multifocal osteonecrosis is defined as the presence of osteonecrosis in three or more osseous sites. It is an infrequent entity representing less than 3% of cases among osteonecrosis patients. Vascularized bone grafting for talar avascular osteonecrosis is indicated for patients with modified Ficat and Arlet stage I to III disease with minimal subchondral collapse. Outcomes may be more durable than core decompression alone, especially in patients with more advanced disease. Our preferred method, described in this article, involves core decompression followed by use of a vascularized cuboid pedicle graft placed in the defect. Outcomes reported in a small case series have been encouraging, with more than 80% of patients requiring no further surgery.

This is a case report of an 81-year-old male with severe headaches caused by osteonecrotic lesions in the frontotemporal part of the skull. Initially, metastatic lesions were suspected but other causes had to be identified because of chronic inflammation evidence in all biopsies. In the diagnostic process a discussion of missed malignancy, morbus Paget and steroid induced avascular necrosis are presented.

Many risk factors of aseptic osteonecrosis (AO) are well-known, even if 40% of events are idiopathic. Intravascular thrombosis is one of the physiopathological mechanisms of AO. When osteonecrosis is located in the mid- to posterior region, we generally perform a transtrochanteric posterior rotational osteotomy. We retrospectively reviewed the clinical and radiographic results in 47 consecutive patients (51 hips) in whom we performed posterior rotational osteotomies. The average age was 37 years at the time of surgery. There were 30 male and 17 female patients. Thirty-six hips were ARCO Stage III, and 15 were Stage IV. Conversion to THA was defined as the failure end point. Three patients died and one was lost to followup. We were therefore able to follow 43 patients

Skeletal radiographs were performed to determine the prevalence of dysbaric osteonecrosis (DON) in 51 Turkish sponge divers. DON was correlated with the diver's age and experience, maximum diving depth, and decompression sickness (DCS).
