Cervical dystonia after ear surgery.

BACKGROUND: The pathophysiology of primary focal dystonia remains insufficiently understood, but may be explained by a 'double-lesion' model, in which a particular trigger on top of an intrinsic susceptibility due to a certain genetic predisposition can induce dystonia. CASE-REPORT: Here, we describe a patient who developed cervical dystonia soon after ear surgery (revision stapedectomy), which had caused vestibular hypofunction. DISCUSSION: We also discuss other cases of dystonia associated with vestibular lesions and with other reported triggers, and put these into the context of the possi

The middle cranial fossa approach: an anatomical study.

Hearing preservation surgery has become an option for an increasing number of patients with vestibular schwannomas due to diagnosis at an earlier stage. The middle cranial fossa approach represents one such surgical approach for resection of vestibular schwannomas with hearing preservation. We have undertaken an anatomical study of the middle cranial fossa approach to the internal auditory meatus using 20 fresh temporal bones. By simulating the surgical approach it was possible to analyze critically two of the main recognized subapproaches to the internal acoustic meatus. The results confirmed

Postauricular myocutaneous flap in otologic surgery.

Management of a large mastoid defect resulting from skull base operations or extensive surgical procedures because of chronic ear disease continues to challenge the otologic surgeon. Various local muscle or periosteal rotation flaps have been used to help reduce the size of the postoperative mastoid cavity. With these techniques there are problems with flap retraction and epithelization that may result in delayed healing or chronic drainage. Closure of the ear canal and tissue obliteration of the mastoid results in a maximal conductive hearing loss. A postauricular myocutaneous flap based on t

A prospective multicentre otology database.

UNLABELLED: WHY IS AN INTERNATIONAL OTOLOGY DATABASE IMPORTANT? There are many reports on the outcome of otological interventions in the literature. However, it is difficult to make direct comparisons between these reports because of the lack of uniformity in reporting outcomes and patient selection. The aim is to create an interactive otology database for surgeons in the UK and Europe. METHODOLOGY: The proposed project is the creation of a common database on a website. Two levels of data entry are available: (1) level I: minimum database (only main outcomes are included; for any otolaryngolo

Incidence of dehiscence of the facial nerve at surgery for middle ear cholesteatoma.

OBJECTIVE: To identify the incidence of dehiscence of the facial nerve in patients undergoing surgery for cholesteatoma and to describe its relevance with iatrogenic facial nerve injury. STUDY DESIGN AND SETTING: This was a retrospective study including 117 tympanoplasties with or without mastoidectomy performed in 49 men and 66 women. RESULTS: The presences of FND after exenteration of disease are 33.3% of total surgical procedures, 33% of the initial procedures, and 37.5% of the revision procedures. The locations of facial nerve dehiscence were 87.2% in only the tympanic segment, 7.7% in o

Evaluation of Artificial Fixation of the Incus and Malleus With Minimally Invasive Intraoperative Laser Vibrometry (MIVIB) in a Temporal Bone Model.

BACKGROUND: A significant number of adults suffer from conductive hearing loss due to chronic otitis media, otosclerosis, or other pathologies. An objective measurement of ossicular mobility is needed to avoid unnecessarily invasive middle ear surgery and to improve hearing outcomes. METHODS: Minimally invasive intraoperative laser vibrometry provides a method that is compatible with middle ear surgery, where the tympanic membrane is elevated. The ossicles were driven by a floating mass transducer and their mobility was measured using a laser Doppler vibrometer. Utilising this method, we asse

Prospective study of resident-performed stapedectomy.

Because of the diminishing number of patients diagnosed with otosclerosis, the adequacy of residency training in stapedectomy techniques remains controversial. A prospective study, conducted from 1986 to 1991, assessed whether or not surgical outcome obtained by residents, under close supervision by otologic faculty, could be improved using a single stapedectomy technique. Comparison of pre- and postoperative hearing results (pure-tone averages of 500, 1000, and 2000 Hz) from 49 cases revealed closure of the air-bone gap to within 10 dB in 68 percent of procedures. Complications included failu

Vestibular nerve dysfunction and decompression in Engelmann's disease.

Vestibular nerve dysfunction as the major neuro-otological symptom in one of the osteopetrosis group of bone disorders is unusual. We describe a patient with Engelmann's disease who presented in this manner and who benefited from an eighth nerve decompression procedure. Surgical decompression of the internal auditory meatus to relieve vertigo has to our knowledge not been previously reported in this condition.

Cerebrospinal fluid otorrhoea and otorhinorrhoea following closed head injury.

A discussion of acute traumatic CSF otorrhoea and otorhinorrhoea is presented. The incidence of meningeal infection is reviewed and the management of these cases is outlined with special emphasis on the indications for surgical intervention and the otological procedures available. Two patients who sustained transverse fractures of the petrous temporal bone are described in whom subsequent attacks of meningitis were prevented by a new, simple yet effective operation.

Quinine in Otology and Neurotology: Ototoxicity and Historic Role in Therapy.

OBJECTIVES/HYPOTHESIS: Quinine, a cinchona bark-derived antimalarial alkaloid, is a known ototoxic. Isolated and named in 1820 by the French scientists Pierre-Joseph Pelletier and Joseph-Bienaimé Caventou, it has since been employed in the treatment of different maladies. Quinine was also recommended as a local anesthetic in surgical procedures in the early 20th century. This article aims to identify early ototoxicity reports regarding quinine and to investigate if quinine was previously used in otology as an anesthetic agent or as an actual therapy. METHOD: Historical review of medical and p

Ossiculoplasty in congenital hearing loss.

We have presented an extensive review of congenital ossicular anomalies. These deformities occur in combination with one another or as isolated problems with equal frequency. They are so rare that even the busiest otologists have limited clinical experience with them. The astute otologist will keep these facts in mind when assessing congenital middle ear problems in the office and in the operating room. Careful, detailed history taking may reveal the presence of familial conductive hearing loss or other congenital syndromes. Complete head and neck examination as well as observation of the extr

Perioperative management of intracranial aneurysms and otologic disease.

OBJECTIVE: Diagnosis and treatment of asymptomatic, unruptured intracranial aneurysms (UIA) are areas of longstanding controversy in the neurologic and neurosurgical literature. Treatment decisions require careful consideration of both the potential for aneurysm rupture as well as the risks associated with repair. Studies examining the natural history of UIA, the morbidity and mortality associated with subarachnoid hemorrhage (SAH), and the various treatment options have led to general guidelines for UIA management. In the literature, the simultaneous presence of otologic disease and UIA has n

Postoperative recurrence of traumatic acquired external auditory canal atresia: a case report.

Acquired external auditory canal stenosis (EACS) is a rare otologic disorder with diverse etiologies, including infection, trauma, neoplasm, and inflammation. Post-traumatic stenosis is particularly uncommon, accounting for only 10% of cases. Patients typically present progressive conductive hearing loss and notable narrowing of the external auditory canal (EAC). Careful physical examination is essential to identify local inflammatory or fibrotic changes within the EAC. A history of recent trauma, recurrent external ear infections, or previous otologic surgery often suggests the diagnosis and
